ZusammenfassungDie Nützlichkeit von Binswangers „Sexualität per se und in actu“ sowie der Unterscheidung von Sexuellem und Nicht-Sexuellem für therapeutische Zwecke wird in diesem Kommentar bezweifelt. Entgegengehalten wird ein in der psychodynamischen Psychotherapie bewährtes Begriffsinventarium: Erregung und Hemmung, Lust und Unlust, Angst, Schuld, Wiederholung und Wiederholungszwang. Nicht vergessen werden sollte, dass wir es in der Psychotherapie mit Psychosexualität und nicht mit dem rein physiologisch definierten Begriff der Sexualität zu tun haben. Bei Nutzung dieser beobachtungsnahen und leicht handhabbaren Begriffe bleibt man dem Erleben der Patient_innen deutlich näher verbunden als mit den abstrakteren Einordnungen in Libido, Aggression oder gar Perversion oder die medizinischen Termini gesund und krank.

Die Zunahme psychosozialer Probleme mit Personen, die mit fundamentalistisch oder radikal ideologisch orientierten Gruppen zu tun haben, lassen uns noch einmal einen neuen Blick auf die kulturelle Bedeutung von Religion und Mythen (als Vorlaufer von Religionen) werfen. Die massive Religionskritik durch die vernunftorientierte Philosophie der Aufklarung sah im Religiosen Irrationales, das einem naiven Rationalismus weichen sollte. Erst nach dem Zweiten Weltkrieg wurden die Auswuchse eines sich auf Rationalitat berufenden Materialismus im Kommunismus auf der einen Seite und eines primitiven Sozialdarwinismus als Grundlage des Faschismus auf der anderen Seite kritisch gesehen. So hat sich die gegen gesellschaftliche Totalitat gerichtete Kritische Theorie der Frankfurter Schule auch gegen einen pseudo-objektiven Positivismus gewandt, der ubersieht in wessen Diensten er steht. Deren Arbeiten zum »autoritaren Charakter« haben in neuester Zeit bei der Auseinandersetzung mit dem Rechtsradikalismus wieder Anwendung gefunden. In der vorliegenden Arbeit wird Rene Girards religionsphilosophischer Ansatz vorgestellt, der das Bedurfnis des Menschen, anderen zu gleichen, thematisiert und es als Mimesis bezeichnet. Es zeigt den Menschen einerseits als soziales Wesen, aber auch als Rivalen seiner Umgebung, der von Eitelkeit und Neid getrieben wird, solange er sein mimetisches Bedurfnis nicht transzendiert und auf ein auserweltliches (gottliches) Wesen richten kann, das als Rivale gar nicht in Frage kommt. Am Beispiel der fundamentalistisch evangelikalen Religiositat der Colonia Dignidad werden die »salutogenen« Aspekte von Religion den pathogenen Folgen einer Religion mit totalitarem Anspruch gegenubergestellt. Moderne neuro-psychologische Befunde konnten fur beides Erklarung bieten.

Das Phanomen, Kinderpornographie bis zum Grad suchtiger Abhangigkeit zu konsumieren, hat besonders durch die Entwicklung des Internets massiv zugenommen und beschaftigt seither Polizei, Gerichte und therapeutische Institutionen. Die funfjahrige psychoanalytische Behandlung eines Padagogen im Kirchendienst bietet Einblicke in lebensgeschichtliche Voraussetzungen, Entwicklungsdynamik, Behandlungsmoglichkeiten und deren Grenzen bei solchen Fallen. In seinem Fall bestand eine latente padophile Neigung, die eher den hoher strukturierten Formen im Sinne Kernbergs bzw. Perdigaos angehort und die sich durch das Pornographie-Angebot im Internet massiv steigerte. Nach Abschluss der Analyse wurde der Patient noch drei Jahre in groseren Abstanden katamnestisch nachuntersucht. Es bestatigt sich, dass die Gruppe von Storungen, die den hoher strukturierten Formen perverser Symptombildung angehoren, der Methode der Psychoanalyse zuganglich ist.

This chapter studies the effects of acute and chronic substance use on sexual offending. A significant proportion of sexual offenders suffer from substance use disorders. Most of the studies that have a non–sexual violent offender control group showed that more sex offenders than non–sexual violent offenders abused alcohol. For drug abuse, mixed results were found. Substance use seems to be more prevalent in rapists than in child molesters and more frequent in nonparaphilic than in paraphilic sexual offenders. Ultimately, the nature of the relationship between substance use and sexual offending is complex and confounded with several factors. Acute and long-term effects of alcohol probably play a contributive but not a primary causal role in sexual offending. Substance abuse also plays a significant role in risk assessment and relapse prevention and should be considered in treatment options more specifically.

About 1% of sentenced sexual offenders are female. Investigations on victims of child molestation reveal at least a participation of female perpetrators in one-fourth of cases of male victims and in 13% of female victims. The spectrum of offenses of female offenders reaches from minor forms of indecency, exhibitionism, different forms of child molestation, and incest to all forms of aggressive acts, including rape and murder. Male and female children are the most prominent victims. In a high percentage of offenses, the female offender acts together with a male accomplice. Only in rare cases is the diagnosis of paraphilia reported; far more frequent are different forms of depression, alcohol and drug abuse, and personality disorders (borderline, antisocial, dependent). Most authors agree that a high percentage (more than 50%) of female sexual offenders were themselves victims of sexual and/or physical abuse. Conflicts with the mother and resulting problems with women—especially mother identity—are reported by psychodynamic-oriented authors. Only a few case reports exist on effective medical treatment with selective serotonin reuptake inhibitors and antiandrogens. A carefully differentiated psychotherapeutic treatment program, including group, individual, and social therapeutic strategies, should take into consideration the good lives model (GLM), but also specific factors of female offending: The importance of female identity problems, the higher importance of relationship aspects, and the victim–victimizer circle.

In dem Band »Angst, Lust, Zerstorung« haben Schorsch & Becker (1977) 14 Falle von sexuell konnotierten Totungsdelikten und zwei weitere schwere Sexualdelikte beschrieben. Die Tater sind von psychoanalytisch orientierten Autoren gesehen, ausfuhrlich begutachtet und theoretisch eingeordnet worden. Diese Arbeit kann als einer der wesentlichen Ausgangspunkte einer spezifisch deutschen Auseinandersetzung mit Perversionskonzepten – besonders der Perversionen, die zu schweren Delikten fuhren, aber keineswegs auf sie beschrankt – gesehen werden. Dem Einfluss dieser Arbeit bis in die Gegenwart wird nachgegangen.

Sollte Menschen, die ein sexuelles Interesse an Kindern haben, die Missbrauchsabbildungen nutzen oder befürchten, sexuelle Missbrauchshandlungen an Kindern zu begehen, ein Therapieangebot im Dunkelfeld gemacht werden? Sollte dies ein zentraler Bestandteil der Prävention sexuellen Kindesmissbrauchs sein? Wie sollte ein solches Angebot aussehen, damit es das Ziel, die Prävention von Kindesmissbrauch, erfüllen kann?

Sexual sadism is a paraphilia considered to be associated with a proneness to violence. In contrast to consensual sadomasochistic role play, forensically relevant forms of sadism include the humiliation, coercion, and possibly the injury, of involuntary victims. Consequently, the perpetrators tend to be regarded as high-risk offenders who are difficult (or maybe even impossible) to treat. The current chapter first provides an overview of past developments in treating sexual sadism within a forensic framework. Behaviour and cognitive behaviour therapy, psychodynamic treatment, and surgical and pharmacological interventions will be reviewed. Given the lack of studies with sufficiently large samples (let alone randomized controlled studies), mostly case descriptions and small-sample studies will be reviewed. Based on this overview, recommendations for an integrative treatment routine (combining psychotherapy and pharmacological treatment) will be delineated. Finally, case vignettes from our own practice will be presented in order to highlight the particular problems and pitfalls of treating sexual sadists in a correctional or forensic-psychiatric setting. In this regard, the co-occurrence of sexual preference disorders and of personality disorders will be discussed.

The use of electrical conduit seals to prevent the release of an explosion from an explosion-proof enclosure and to minimize the passage of flammable vapor and gas migration to an ignition source is as prevalent today as it was yesterday in the petrochemical industry. With the harmonization of international standards and recent equipment advancements, options are available to reduce the use of conduit seals for a more reliable, safe, and maintainable installation. The harmonization of the standards is reviewed, specifically on the International Electrotechnical Commission, National Electrical Code, and Canadian Electrical Code requirements for equipment in hazardous locations. Electrical conduit seal condition data from a 30-year-old petrochemical facility are provided, to illustrate that the threat of gas migration at a facility is real. The collection of these data was achieved by applying new devices allowing for the removal of the conduit seals. An overview of this application and how it was achieved is provided. The outlook on the future development with equipment rated for hazardous locations to reduce conduit seal requirements is explored.

Partnerschaften von aus der Haft oder dem Masregelvollzug entlassenen Sexualstraftatern wurden bisher kaum beforscht. Anliegen der vorliegenden Arbeit ist eine systematische Ubersicht zu diesem Thema. Es wurden lediglich 9 empirische Studien gefunden, die das Thema mit quantitativen oder qualitativen Methoden untersuchen. In den Arbeiten dominieren Feststellungen von diversen Problembereichen, Intimitats- und Empathiedefiziten bei den Tatern, Konflikte und Aggressionen, aber auch die soziale Isolation der Partnerinnen. Die Partnerinnen der Sexualstraftater zeigen hohe Verleugnungs- und Bagatellisierungstendenzen. Eine Interpretation ist, dass die Frauen sich in der Rolle der „Retterin“ eines in Not geratenen Mannes gefallen und betonen, sie gaben gerne eine „zweite Chance“. Vorhandene Studien haben allerdings kaum „Sexualstraftaterpaare“ untersucht und deren Partnerschaftsdynamik erhoben, um auch stabilisierende Faktoren zu erfassen. In einer eigenen Pilotstudie zum Thema „Partnerschaften von Sexualstraftatern“ wurden 17 Paare mit Interviews und Fragebogen befragt, um deren Partnerschaftsdynamik zu erfassen. Im Rahmen der quantitativen Auswertung wurde vornehmlich ein unsicherer Bindungsstil der Partnerinnen gefunden; die Sexualstraftater beschrieben sich als eher sicher gebunden. Es wird diskutiert, ob sich Tater eine unsicher-angstliche Partnerin suchen, um eigene Unsicherheitsgefuhle zu bewaltigen. Weitere Studien zu diesem Thema sind notwendig, um im Hinblick auf die Kriminalprognose vertiefte Erkenntnisse uber die gelebten Partnerschaften von entlassenen Sexualstraftatern zu gewinnen.

Research on sex offenders' relationships is scarce. The aim of this qualitative study was to investigate sex offenders' relationships as well as their female partners' adjustment strategies by means of interview analysis. Both partners profit from the relationship in terms of mutual support and acceptance. The sexual offense is a taboo subject, and the female partners were found to demonstrate cognitive distortions. The imbalance of power found in the sex offenders' relationships is discussed, as is the finding that those sexual offenders interviewed live out their need for dominance and sometimes their aggression. The women interviewed were found to cling to their partners as a result of their insecure attachment style. We discuss couples counseling and therapy as possibilities for addressing the imbalance of power and casting light upon the sexual aspect of the relationship.

A testosterone-lowering medication is relatively commonly used as a form of treatment for sexual offenders with severe paraphilic disorders in German forensic psychiatric hospitals; however, a double-blind, controlled and randomized study, which investigates the efficacy of this medication, is still lacking.This article describes the process from the planning to the rejection of a clinical trial over the period from 2009 to 2015.Despite the careful planning with an interdisciplinary team and giving special consideration to the complex legal situation, the Federal Institute for Drugs and Medical Devices (BfArM) rejected the proposed trial in a brief formal letter with reference to the German Drug Law (§ 40 para. 1 p. 3 nr. 4 AMG). The ethics committee of the Hamburg Medical Association considered that clinical research is basically not possible with patients detained in a forensic psychiatric hospital.In the opinion of the authors, the described facts illustrate how legal regulations that should protect vulnerable groups in medical research, in a specific case can lead to the fact that a therapy form relevant to the corresponding patient group cannot be scientifically investigated.

Research indicates that sex offenders' relationships are often unstable and superficial. Sex offenders are portrayed as showing little empathy toward their partners and being incapable of sharing intimacy. This study aimed to explore sex offenders' intimate relationships and identify potential stabilizing factors in an outpatient setting. In an exploratory pilot study, the authors used standardized instruments to assess 17 heterosexual sex offenders and their intimate partners. Results indicate that both partners rate high in attachment anxiety. The authors found a high level of correlation between both partners with regard to attachment avoidance. The sex offender couples demonstrate similarly low scores for "dominance, pugnaciousness, and aggression" in intimate relationships, and the female partners revealed high neuroticism and conscientiousness scores.

Little is known about personality disorders (PDs) in offenders with histories of both sexual and (nonsexual) violent offenses. This study aimed to identify possible differences of PD profiles across three different offender groups with both sexual and violent (S+V), only sexual (S), and only violent (V) offenses. Nonviolent (N) offenders were used as a comparison group. Typing of individuals according to their offensive histories was performed on the basis of 259 psychiatric court reports that included the Structured Clinical Interview (SCID)-II for PD diagnostics. Men from the S+V group committed significantly more acts of rape and sexual coercion than the mere sexual offenders. Furthermore, S+V offenders showed the highest rates of PDs overall (68.3%), with every second offender being diagnosed with an antisocial PD and every third offender with a borderline PD. In summary, the results suggest that S+V offenders form a group of individuals with remarkable differences regarding PD profiles, the relatively highest frequencies of conduct disorders, familial addictive problems, and PDs overall.

Introduction. The present study investigates the relevance of genital abnormalities (GA) like cryptorchidism, hypospadias, and phimosis usually diagnosed in early childhood for the development of psychosexual problems and deficits in a sample of N = 163 convicted sexual homicide perpetrators. Aims. The first aim was to investigate the prevalence of early childhood GA in a sample of sexual homicide perpetrators. The second was to explore differences in the psychosexual development of participants with GA in early childhood compared with those without GA. It was expected that offenders with GA show specific problems in their psychosexual development compared with offenders without GA. Methods. The data for the present study were obtained by reanalyzing an existing database derived from a large‐scale research project about sexual homicide. Using a predominantly exploratory design we, therefore, divided the total sample into two subgroups (with vs. without indicators of GA). Main Outcome Measures. Main outcome measures were the number of sexual homicide perpetrators showing GA in early childhood and the differences of subjects with and without GA with regard to their psychosexual development (i.e., according to sexual deviant interests or sexual dysfunctions). Results. The prevalence of GA is substantially higher in this sample than epidemiological studies indicated in the normal population. This result provided first support for the importance of GA in the population of sexual homicide perpetrators. Further analyses indicate significant differences between both subgroups: Offenders with GA in early childhood showed indicators for more sexual dysfunctions (e.g., erectile dysfunction) in adulthood and a distinct tendency of more masochistic sexual interests. Conclusion. Even if the exploratory design of the present investigation allows no causal conclusions between GA and sexual homicide offenses, the result provided support for the relevance of early childhood sexual diseases in the assessment (and treatment) of offenders who have committed severe sexual violence.

In modern psychiatric classifications the term paraphilia has replaced the term perversion by changing the scope of the definitions from avoided procreation to failures of relationship-aspects of sexuality. Contemporary psychoanalysts also seem less interested in pleasure seeking aspects, which were so important within original Freudian thought, and instead concentrate interpretation on hostility and the history of the representation of objects. This paper discusses the connection between distinct object representations in perversions and attachment theory and neurobiological representations. It will discuss the observation that the neglect of pleasure seeking in perversion often results in a failure to recognise the addiction-like aspects of perversion, which seem to be particularly relevant to modern psychiatric and psychological thinking. The SEEKING-system (Panksepp, 1998) is used to conceptualise a neurobiological basis for pleasure seeking. This SEEKING-system may be "hijacked" by rewards in different forms of addiction as well as in sexual obsessions. The polarity between "drive representation" and "object representation," as created by Freud (1933, 1940a), may correspond to the polarity identified in contemporary thinking between the addictive or compulsive characteristics of sexual gratification (drive representation) and the influence of early object representation on the later ability to integrate instinctual wishes into relationships (object representation).

The aims of this study were to compare the prevalence of psychiatric disorders and "psychopathy" in homicidal and nonhomicidal sexual offenders and to investigate the specificity of previous studies on psychiatric morbidity of a sample of sexual murderers. Information from court reports of 166 homicidal and 56 nonhomicidal sex offenders was evaluated using standardized instruments (SCID-II, PCL-R) and classification systems (DSM-IV). Sexual murderers were diagnosed more often with a personality disorder (80.1% vs. 50%; p0.001), especially schizoid personality disorder (16.3% vs. 5.4%; p0.05), as well as with sexual sadism (36.7% vs. 8.9%; p0.001) and sexual dysfunctions (21.7% vs. 7.1%; p0.05). Additionally, they had more often used alcohol during the offense (63.2% vs. 41%; p0.05). The results indicate that sexual murderers have more and a greater variety of psychiatric disorders when compared to nonhomicidal sex offenders.

Wir sind heute weit davon entfernt, in der Behandlung von sexuellen Praferenzstorungen nach einer einzigen Ursache zu suchen. Die bisherigen Untersuchungen an groseren Kollektiven legen die Annahme nahe, dass es ganz unterschiedliche Wege der Entstehung solcher Storungen gibt, die von korperlichen Veranlagungen uber Erfahrungen in den Primarbeziehungen, sexuell traumatischen oder Gewalterlebnissen in der Kindheit bis hin zu aktuellen Storungen der emotionellen Regulation und sozialen Anpassung reichen. Alle diese Faktoren durften in jeweils unterschiedlicher Quantitat zur Entstehung beitragen. Dementsprechend muss beim Auftreten dieser Storungen zunachst untersucht werden, welche Bereiche der Personlichkeit betroffen sind und dann welche Hilfen eingesetzt werden sollen. Die Psychotherapien fur Sexualstraftater haben in den letzten beiden Jahrzehnten auch empirisch ihre Wirksamkeit nahelegen konnen, sodass man es heute als Fehler ansehen muss, wenn man sich nicht spezifischer fur Sexualstraftater entwickelter Programme bedient. Auch die medikamentose Behandlung differenziert sich standig weiter. Zur leichteren Orientierung bei dieser Vielfalt von Moglichkeiten wurden Leitlinien entwickelt, von denen in dieser Arbeit zwei diskutiert werden.

Die von Krafft-Ebing geschaffene Kategorie des Sadismus hat sich in den letzten hundert Jahren als ein vielgestaltiges Phanomen dargestellt, das von ganz harmlosen symbolischen und ritualisierten Gesten der Unterwerfung bis zu todlich endenden Gewalthandlungen reicht, die mit groser Impulsivitat oder in lang hingezogenen, von vielen Fantasien und Ritualen begleiteten Handlungen ablaufen konnen. So vielgestaltig das Phanomen selbst ist, so unterschiedlich sind Entstehungsbedingungen und Ursachen. Wenn es auch nur wenige Uberschneidungen zwischen Vorlieben, die heute von den Betroffenen mit dem beschreibenden Akronym BDSM („bondage & discipline, dominance & submission, sadism & masochism“) bezeichnet werden, und dem forensisch bedeutsamen „schweren Sadismus“ gibt, so gibt es doch auch Brucken zumindest in den beteiligten Fantasien. Aufgrund eigener Erfahrungen mit Sadismus im forensischen Kontext treten die Autoren fur die im geplanten Diagnostic and Statistical Manual of Mental Disorders- (DSM-)V vorgeschlagenen Neuformulierungen und besonders fur eine diagnostische Trennung von Sadismus und Masochismus ein. Sadismus trifft man wesentlich haufiger im forensischen Kontext und dort nur selten in Verbindung mit Masochismus, eher mit der sadistischen Personlichkeitsstorung, der antisozialen und der Borderline-Storung. Masochistische Storungen werden zwar von Patienten im psychiatrischen Kontext berichtet, sind dort aber wieder kaum mit Sadismus, sondern eher mit Depression und dependenten Personlichkeitsauffalligkeiten verbunden. Die grostenteils befurwortete Beibehaltung des diagnostischen Begriffs des Sadismus macht nach Meinung der Autoren die Einfuhrung einer Kategorie „paraphilic coercive disorder“ unnotig. Der diagnostische Begriff ist auch fur Therapieplanung und prognostische Einschatzungen wichtig. Dazu fehlen verlassliche Studien mit groseren Gruppen und langeren Verlaufen. Es ist aber davon auszugehen, dass bei „schwerem Sadismus“ mit Progredienzzeichen eine medikamentose Behandlung, u. U. auch mit Antiandrogenen, indiziert sein kann.

Juristische, medizinische und von der Forschung benutzte Definitionen sexueller Gewalt werden einer Alltagsdefinition gegenubergestellt. Nach kritischem Vergleich von Haufigkeitsangaben juristisch definierter sexueller Gewalt und Schatzungen, die soziologischen Forschungskriterien entsprechen und damit zu viel hoheren Werten kommen, werden Bedingungen des Auftretens von sexueller Gewalt untersucht: Dabei sind die „von innen“ kommenden individuellen korperlichen und psychologischen Bedingungen, den auseren, sich aus dem Zusammenleben der Menschen ergebenden (soziobiologischen und soziokulturellen) Bedingungen gegenuberzustellen. Entsprechend den auseren und inneren Bedingungen sexueller Gewalt sind auch bei den praventiven und therapeutischen Masnahmen ausere und innere Angriffspunkte zu unterscheiden: Schaffung einer offentlichen Meinung, die Gewalt verurteilt, und die individuelle Therapie von Opfern und Tatern sexueller Ubergriffe. Die Effekte der individuellen Therapien sind besser belegbar als die Effekte von Offentlichkeitsarbeit und universeller Pravention. Das aktuelle Beispiel der Diskussion um die standige Zunahme des Angebots von Pornographie im Internet veranschaulicht die Schwierigkeit der Beurteilung und Eingrenzung sexueller Gewalt. Standig entstehen neue Manifestationen sexueller Gewalt, die neue Formen des therapeutischen Umgangs erforderlich machen.

Ziel: In einer aktuellen Definition sexuell süchtigen Verhaltens werden deviante von nicht devianten Formen sexuell exzessiven Verhaltens unterschieden. Für die nicht devianten Störungsbilder wird der Begriff der Paraphilie-verwandten Störung verwendet. Dieser Artikel soll eine Übersicht über den derzeitigen Diskussionsstand und Behandlungsmöglichkeiten Paraphilieverwandter Störungen geben. Ätiologie und Modelle: Konzeptionelle Modelle reichen von Parallelen zu stoffgebundenen Abhängigkeitserkrankungen zu stoffungebundenen Verhaltenssüchten, Zwangsspektrumsstörungen und Impulskontrollstörungen. Ätiologische Konzepte beziehen Interaktionen von biologischer Vulnerabilität, Bindungs- und daraus resultierenden Beziehungsstörungen sowie Störungen der Affektivität und der Kontrolle sexueller Erregung ein. Wichtig ist in diesem Zusammenhang auch eine Überschneidung mit sexuellem Risikoverhalten. Diagnostik und Behandlung: In der Diagnostik haben sich neben einer ausführlichen Sexualanamnese Screeninginstrumente bewährt. Neben spezifischen psychotherapeutischen Angeboten spielen Selbsthilfegruppen und medikamentöse Ansätze mit Antidepressiva in der Therapie sexuell süchtiger Syndrome eine Rolle.

OBJECTIVE: Evaluation of the relationship between antiandrogenic therapy of sexual offenders and home leave steps in a forensic psychiatric hospital. METHODS: Retrospective evaluation of all sexual offenders (n = 79) who were treated in the forensic psychiatric hospital Berlin between the 1.1.1998 and the 1.1.2005. RESULTS: There is an increase in the number of patients treated with antiandrogenic medication during the observed time period. Patients under this kind of pharmacotherapy receive higher grades of home leave steps. CONCLUSIONS: As possible reasons for the observed changes we discuss the contemporaneous implementation of a new pharmacotherapeutic treatment option (LHRH-agonists) and an aggravation of the German criminal law for sexual offenders. Sexuelle Totungsdelikte durch Jugendliche sind kriminalstatistisch sehr selten und noch weitgehend unerforscht. Mittels einer retrospektiven Gutachtenanalyse und einer Auswertung aktueller Bundeszentralregisterauszuge wurde die Delinquenz einer Stichprobe 19 jugendlicher Sexualmorder von der Kindheit bis zum durchschnittlichen Alter von 45 Jahren erfasst. Mehr als die Halfte (58%) hatte schon vor dem sexuellen Totungsdelikt sexuelle Ubergriffe begangen oder versucht. Von 13 aus dem Gewahrsam entlassenen Tatern verubten sieben (54%) ein neues Sexual- oder Gewaltdelikt. Im Langzeitverlauf waren vergleichbar viele gunstige und ungunstige Delinquenzentwicklungen unterscheidbar. Die Ergebnisse weisen darauf hin, dass Personen, die als Jugendliche ein sexuelles Totungsdelikt begehen, haufig schon vorher sexuell Ubergriffe veruben und langfristig ein hohes einschlagiges Ruckfallrisiko haben.

In einer Untersuchung an 166 in der Zeit zwischen 1963 und 1991 verurteilten Straftatern, deren Sexualdelikt fur die Opfer todlich endete, ergaben sich zunachst beim Vergleich von Einfach- mit Mehrfachtatern ahnliche Verhaltnisse, wie aus Voruntersuchungen mit kleineren Stichproben bekannt war. Bei den Mehrfachtatern wurden haufiger sexueller Sadismus und andere Storungen der Sexualpraferenz sowie antisoziale, schizoide und sadistische Personlichkeitsstorungen gefunden. Katamnestische Daten aus dem Bundeszentralregister lagen von 139 Tatern vor. Wahrend die Entlassenen (n=90) durchschnittlich 12,2 Jahre in Gewahrsam verbrachten, waren die Nicht-Entlassenen (n=49) bis zur Erstellung der Bundeszentralregister-Auszuge im Durchschnitt seit 20,6 Jahren ununterbrochen in Haft oder im Masregelvollzug. Bei den Nicht-Entlassenen waren signifikant mehr Storungen der Sexualpraferenz und mehr antisoziale und sadistische Personlichkeitsstorungen zu verzeichnen. Storungen der Sexualpraferenz und Antisozialitat stellen ein empirisch gut belegtes besonderes Risiko fur einschlagige Ruckfalligkeit bei Sexualstraftatern dar. Auserdem zeigten samtliche angewandten Prognose-Instrumente (PCL-R, HCR-20, SVR-20, Static-99) bei den Nicht-Entlassenen hohere Werte. Vor diesem Hintergrund muss es gesehen werden, dass bei den entlassenen Tatern nur 1,1% (n=1) Ruckfalligkeit in vollendeten und 2,2% (n=2) Ruckfalligkeit in versuchten Totungsdelikten gefunden wurde. Die Ruckfalligkeit mit Sexual- und Gewaltdelikten war mit den Ergebnissen der Metaanalyse von Hanson und Morton-Bourgon (4) vergleichbar. Nachdem die bekannten Risikofaktoren offensichtlich schon bei der Entscheidung fur oder gegen eine Entlassung „verbraucht“ worden waren, blieben in der Katamnese nur mehr das Alter beim Delikt und das Alter bei der Entlassung als weitere Risikofaktoren zur Erklarung von Ruckfalligkeit mit Sexual- und anderen Gewaltdelikten uber. Das heist, je junger der Tater beim sexuellen Totungsdelikt und je junger bei der Entlassung, umso wahrscheinlicher ist ein Ruckfall im Katamnese-Zeitraum.

For about 15 years selective serotonine reuptake inhibitors (SSRI) have been used in the treatment of paraphilias. In an open, uncontrolled, retrospective study, which was the first in the German speaking countries we investigated 16 male outpatients, who have been treated for different paraphilias with SSRI and psychotherapy. There was a marked reduction in paraphilic symptoms. Despite high rates of sexual side effects most patients reported a high overall treatment satisfaction. SSRI are an important addition in pharmacological treatment of paraphilic patients, especially with a risk of so called "hands-off" delinquency.

Controversies exist about the diagnostic concept of “sexual addiction.” The aim of this study was to investigate if German specialized sex therapists are confronted with patients who possess sexual addiction symptoms and to obtain information about diagnoses, comorbidity and treatment. A 12-item questionnaire was sent to all members of the German Society of Sex Research asking them about diagnoses, comorbidity and treatment of patients who experienced sexual addiction problems. Forty-three out of 149 members (28.9%) responded to the survey and reported about 97 patients with sexual addiction symptoms. Most common were pornography dependence, compulsive masturbation, and protracted promiscuity. Using ICD-10 definition “excessive sexual drive” was diagnosed more often in women while “disorder of sexual preference” was mainly diagnosed in men. Hostility towards relationships is one prominent characteristic symptom for disorders of sexual preference (ICD-10) and paraphilias (DSM-IV). Paraphilic symptoms sometimes progress to obsessive or addictive- like forms leading to a loss of self-control but can occur also as single incidents or as episodic events. Besides constitutional aspects, problems in the development of close relationships to primary caregivers (attachment) play an important role in the development of these disorders. Actual relationship- and self-confidence problems often trigger the severity of disturbance, especially in the episodic forms of paraphilia. For patients who are in conflict with the law, cognitive-behavioral therapeutic approaches with the aim to minimize self-deception regarding the effects of the paraphilic behavior have become more and more relevant. Regarding the medical treatment, anti-hormonal therapy plays an important role, but also treatment with serotonergic agents and naltrexone are used. Only little can be advised in terms of prevention; general psycho-hygiene (regarding the parent-child relationship) is recommended. Beside these general measures, institutions which offer special treatment for people in danger to become delinquents may be able to prevent serious harm for possible victims of abuse.

Internet pornography has been regarded as either stimulating sexual aggression and abuse or as serving as a safety valve. This controversy is an important issue in health, media and legal politics. According to empirical studies on pornography in general, soft-core pornography and nonviolent pornography can be regarded as harmless, whereas non-violent hard-core pornography and violent pornography may increase aggression. Individuals with a high risk for sexual aggression show more interest in violent pornography and are stimulated more strongly through such material. Two case histories illustrate the characteristics of internet pornography and "cybersex": easy access, anonymity, affordability, wide range and deviation of the material, unlimited market, blurring the borders between consumer and producer, interactive communication, space for experimenting between fantasy and in real-life behavior, virtual identities, easy contact between offender and victim or among offenders, and low risk of apprehension. The phenomenon of "sexual addiction" (or paraphilia- related disorder) is particularly relevant for the problematic use of internet pornography. Preventive measures to protect possible victims are presented as well as treatment strategies for offenders. Beside limiting access to the internet, these include therapy of comorbid psychiatric disorders and psychological problems (social isolation, bereavement, stress- and anger-management, guilt and shame, childhood traumata, cognitive distortion, victim empathy), psychopharmacotherapy and the enhancement of a more integrative and relationship-oriented sexuality.
