2. A systematic review of the efficacy and safety of turmeric in the treatment of digestive disorders.

Abstract

Turmeric has been gaining popularity as a treatment option for digestive disorders, although a rigorous synthesis of efficacy has not been conducted. This study aimed to summarize the evidence for the efficacy and safety of turmeric in the treatment of digestive disorders, including inflammatory bowel diseases (IBD), irritable bowel syndrome (IBS), dyspepsia, gastroesophageal reflux disease, and peptic ulcers. Literature searches were conducted in Medline, EMBASE, AMED, the Cochrane Central Register of Control Trials, and Dissertation Abstracts from inception to November 15, 2021. Dual independent screening of citations and full texts was conducted and studies meeting inclusion criteria were retained: randomized controlled trials (RCT) and comparative observational studies evaluating turmeric use in people of any age with one of the digestive disorders of interest. Extraction of relevant data and risk of bias assessments were performed by two reviewers independently. Meta‐analysis was not conducted due to high heterogeneity. From 1136 citations screened, 26 eligible studies were retained. Most studies were assessed to have a high risk of bias, and many had methodological limitations. Descriptive summaries suggest that turmeric is safe, with possible efficacy in patients with IBD or IBS, but its effects were inconsistent for other conditions. The efficacy of turmeric in digestive disorders remains unclear due to the high risk of bias and methodological limitations of the included studies. Future studies should be designed to include larger sample sizes, use rigorous statistical methods, employ core outcome sets, and adhere to reporting guidance for RCTs of herbal interventions to facilitate more meaningful comparisons and robust conclusions. Abstract

Breast cancer is the second most common type of cancer in Canada, and the fourth most common cause of cancer-related death. There have been significant advances in the treatment of early-stage disease through the performance of large, randomized trials. Unfortunately, it is challenging and expensive to perform a traditional, large randomised trial to compare different adjuvant interventions, with the average per-study cost for a phase III US oncology trial estimated to be $22.1 million. Novel trial designs may help reduce some of the challenges associated with performing such studies. One potential strategy to reduce costs (for outcome measurement) and patient attrition in the performance of pragmatic trials is use of routinely collected health datasets (e.g. administrative data). Routinely collected health data systematically capture health system encounters from one or more administrative sources. Indeed, there are examples from the cardiology literature where researchers have integrated data from administrative data and electronic medical records to create a cardiovascular-specific database to support data analytics in their field [3]. A similar approach of harmonized data sets, such as the Canadian Cancer Trials Group (trial) and the administrative data sets housed at the Institute for Clinical Evaluative Sciences (ICES), were used to demonstrate economical data collection that had improved the quality of data collected in clinical trials alone, and demonstrated potential to minimize data collection burdens on patients [4, 5]. Furthermore, prospectively linking these was both feasible and accurate if patients give consent to link their personal data with health administrative records [5,6]. Our overarching goal is to determine whether routinely collected data can be effectively used in trials of interventions in early breast cancer to assess the important outcomes of; disease-free survival (DFS) invasive disease-free survival (iDFS) and overall survival (OS). Similarly for metastatic disease, we wish to evaluate the endpoints of progression free survival (PFS) and OS. This scoping review will be conducted to summarize past studies that have looked at this issue. If feasible, use of such data sources can be valuable in notably reducing trial-related costs of lengthy patient follow-up, increasing the chances of funding for more high-quality research studies in the field.

Abstract

Before the COVID-19 pandemic, substance use health services to treat alcohol use disorder and problematic alcohol use (AUD/PAU) were fragmented and difficult for clients to access. The pandemic further elucidated system weaknesses, often leading to disruptions of treatment as alcohol use increased during the pandemic. When services for treatment were available, use was often low for several rationale. Virtual care was used to counteract the drop-off of in-person services, however accessibility to virtual care was not universal. Identification of the characteristics of treatment services for AUD/PAU that impact accessibility, as perceived by the individuals accessing or providing the services, will provide insights to enable improved access. We will conduct a scoping review of the literature to identify traits of services for treatment of clients with AUD/PAU that have been identified as barriers to or facilitators of service access from the perspectives of this population.
