2. Integrating multimodal information for intraoperative assistance in neurosurgery

Abstract

Computer-assisted planning of complex neurosurgical interventions benefits from a variety of specific functions and tools. However, commercial planning- and neuronavigation systems are rather restrictive concerning the availability of innovative methods such as novel imaging modalities, fiber tracking algorithms or electrical dipole mapping. In this respect there is a demand for modular neurosurgical planning systems offering flexible interfaces for easy enhancement. Furthermore all relevant planning information should be available within neuron-avigation. In this work we present a planning system providing these capabilities and its suitability and application in a clinical setting. Our Multimodal Planning System (MOPS 3D) offers a variety of tools such as definition of trajectories for minimally invasive surgery, segmentation of ROIs, integration of functional information from atlas maps or magnetoencephalography. It also supplies plugin interfaces for future extensions. For intraoperative application MOPS is coupled with the neuronavigation system Brainlab Vector Vision Cranial/ENT (VVC). We evaluated MOPS in the Department of Neurosurgery at the University Hospital Heidelberg. Surgical planning and navigation was performed in 5 frequently occurring clinical cases. The time necessary for planning was between 5 and 15 minutes including data import, segmentation and planning tasks. The additional information intraoperatively provided by MOPS 3D was highly appreciated by the neurosurgeons and the performance was satisfactory.

Abstract

Three-dimensional overlay of objects onto the operating field has big advantages compared to current commercial approaches. There are several problems which can be solved by providing a full color stereoscopic overlay to the surgeon, orienting himself by looking away from the operating field onto a navigation monitor as well as merging the overlaid two-dimensional contours of an object correctly to a three-dimensional structure. This paper introduces a conceptual approach for a full color, stereoscopic overlay of objects onto the operating field using the optics of the operating microscope. Models segmented from preoperatively acquired image data are inserted into the optics of the operating microscope. We use the virtual camera approach to provide the collimation. In the scope of developing a prototype for image overlay, we analyzed current display technologies to determine the best display technique to use. We will present the general process model for our approach, the visualization concept using the virtual camera model, the current workflow, the prototype and a brain phantom which we developed for our needs. 35. Preoperative planning and intraoperative navigation in skull base surgery

Abstract

Experience with the commercially available, 3-D navigation systems Viewing Wand (ISG, Mississauga, Ontario, Canada) and SPOCS (Aesculap, Germany) in skull base surgery is presented. Having meanwhile been tested in over 60 clinical trials, the systems achieved an accuracy of @?2.7 mm which, at the moment, we deem sufficiently acceptable to proceed with their clinical evaluation. There was no difference in intraoperative accuracy between the mechanical and the optical navigation systems. The systems proved to be very helpful in identifying the extent of the tumours and in visualizing the proximity of vital structures. 3-D-planning, simulation and intraoperative navigation especially facilitates surgery in anatomically complicated situations, without risk of damaging neighbouring structures. The SPOCS (Surgical Planning and Orientation Computer System) revealed a considerably improved flexibility in handling and a better integration into the surgical procedure in comparison with the relatively inflexible and space-demanding Viewing Wand arm. Especially, the 'offset' function of the SPOCS offers the possibility of a virtual elongation of the instrument and thus, in combination with the on-line visualization of the corresponding images, of a 'look ahead' operation. By using computer-assisted simulation and navigation systems, we can expect quality improvement and risk reduction. More extensive and radical interventions seem possible.

Abstract

Excision of a ruptured aneurysm located at a lenticulostriate feeding artery associated with an arteriovenous malformation was performed using image guided neuronavigation in an 8-year-old child. The management of this lesion, which is rare in childhood, demonstrates the potential of combining frameless stereotaxy for precise target approach with conventional open microneurosurgery to minimize morbidity. Because of the unavoidable shifting of the brain that occurs during surgery, a catheter pointing towards the dome of the aneurysm was placed using image guidance prior to insertion of spatulae.
