Background/Objectives: Falls and related injuries are a frequent and serious health problem in older persons. Among the various strategies, different forms of active physical training, in particular, have demonstrated success in reducing fall risk. A task-specific training approach is perturbation-based training of reactive balance. Performing this training modality on a perturbation treadmill, secured with a safety harness, is an innovative new approach facilitating task-specific training with unannounced perturbations in a safe environment. The aim of this study was to investigate the feasibility and effectiveness of this specific training in multimorbid older hospitalized patients with prefrailty and frailty. Methods: The trial was conducted as a prospective single-center, assessor-blinded randomized controlled trial. A total of 127 acute-care geriatric hospitalized patients were enrolled in a program either involving a minimum of 60 min perturbation-based treadmill training or treadmill training without perturbations on the identical device and for a comparable training period. Results: Participants were 81 ± 6 years old (64% female) with a baseline FRAIL Scale, SPPB, and MoCA scores of 3.5 ± 1.6, 8.3 ± 2.6, and 21 ± 5 points, respectively. The training was performed on six occasions with an average total training period of 89 min during a mean hospital stay of 17 ± 3 days. Between the baseline and up to 2 days after the last training, the Short Physical Performance Battery score, which was considered the primary endpoint, improved by 1.4 ± 2.1 points in the intervention group compared to 0.5 ± 1.7 in the control group, with a 0.9-point difference between the groups (p < 0.001). Conclusions: a relatively short training period of approximately 90 min on a perturbation treadmill led to a significant and clinically meaningful increase in the physical performance of frail and prefrail hospitalized geriatric patients. However, its effectiveness in reducing fall risk is yet to be proven in this population. [ABSTRACT FROM AUTHOR]

Background/Objectives Iron deficiency is common in older patients. We investigated whether iron deficiency is an independent risk factor for functional impairment, low muscle function, fatigue, and rehabilitation progress in older hospitalized patients. Subjects/Methods Two hundred twenty-four patients (age range 65-95 years; 67% females) who were consecutively admitted to a geriatric acute care ward participated in this prospective longitudinal observational study. Ferritin, iron, transferrin in serum, and blood hemoglobin were measured and current iron supplementation was recorded. Fatigue and comorbidity were measured using the fatigue severity scale and Charlson Comorbidity Index, respectively. Barthel Index, handgrip strength, and isometric knee extension strength were conducted at the time of hospital admission and before discharge. Results Ninety-one (41%) patients had iron deficiency in which the majority had functional iron deficiency (78/91, 86%). Absolute iron deficiency with and without anemia was diagnosed in 12 (13%) and one patients, respectively. Barthel Index and handgrip and knee extension strength significantly improved during hospitalization in iron deficiency and non-iron deficiency groups. Knee extension strength showed better improvement in iron-deficient patients receiving iron supplementation and iron supplementation during hospital stay was the main predictor for improvement in knee extension strength. Comorbidity, iron deficiency, and changes in handgrip strength were the major independent risk factors for poor improvement in Barthel Index during hospitalization. There were significant associations between patients' fatigue and iron deficiency, comorbidity, and female gender. Conclusion Iron deficiency is an independent risk factor for fatigue and poor functional recovery among older hospitalized patients. Plant reproduction is influenced by resource availability, environment and interactions with pollinators, pathogens and herbivores. In the Neotropical region, the leaf‐cutting ants are voracious herbivores and soil engineers, potentially affecting plant performance. This study investigated the effects of herbivory and soil changes via nest construction and maintenance linked to Atta opaciceps (Hymenoptera: Formicidae) (Bormeier, 1939) on the reproductive success of two Leguminosae species (Cenostigma pyramidale (Tul.) E. Gagnon & G.P. Lewis and Indigofera suffruticosa Mill. Mill.) widely distributed in the Caatinga dry forest.We selected eight nests from A. opaciceps colonies, with both plant species nearby. For each colony, we selected similar‐sized adult plants from foraging trail ends, where ants concentrate foraging efforts and herbivory is pronounced. We also selected plants located 20 m away from the foraging trail ends, representing herbivory‐protected scenarios, secured by tanglefoot barriers. Additionally, to assess ant‐related soil effects, we selected individuals from different nest‐associated soils (i.e., nest mounds and external refuse dumps) and from locations 20 m away from the aforementioned locations. All these individuals were shielded with tanglefoot barriers.Herbivory and soil changes from A. opaciceps did not affect any metric evaluated of C. pyramidale. Yet, for I. suffruticosa, ant herbivory boosted fruit set, and nest‐related soil changes, notably from refuse dumps, increased the number of flowers and fruits.Our findings indicate that leaf‐cutting ant effects on plant performance are species‐specific and may eventually benefit plant reproduction. Given the proliferation of leaf‐cutting ants in human‐modified landscapes of the Caatinga dry forest, positive interactions with plants might be vital to improving plant recruitment and forest regeneration dynamics.
