The purpose of the randomized Phase II study by RTOG 0020 was to evaluate the addition of weekly low-dose gemcitabine with concurrent paclitaxel/RT and to evaluate the efficacy and safety of the farnesyl transferase inhibitor R115777 following chemoradiation.Patients and methods: Patients with unresectable, nonmetastatic adenocarcinoma of the pancreas were eligible. Patients in Arm 1 received gemcitabine, 75 mg/m2/week, and paclitaxel, 40 mg/m2/week, for 6 weeks, with 50.4 Gy radiation (CXRT). Patients in Arm 2 received an identical chemoradiation regimen but then received maintenance R115777, 300 mg twice a day for 21 days every 28 days (CXRT+R115777), until disease progression or unacceptable toxicity.Results: One hundred ninety-five patients were entered into this study, and 184 were analyzable. Grade 4 nonhematologic toxicities occurred in less than 5% of CXRT patients. The most common grade 3/4 toxicity from R115777 was myelosuppression; however, grade 3/4 hepatic, metabolic, musculoskeletal, and neurologic toxicities were also reported. The median survival time was 11.5 months and 8.9 months for the CXRT and CXRT+R115777 arms, respectively.Conclusions: The CXRT arm achieved a median survival of almost 1-year, supporting chemoradiation as an important therapeutic modality for locally advanced pancreatic cancer. Maintenance R115777 is not effective and is associated with a broad range of toxicities. These findings provide clinical evidence that inhibition of farnesylation affects many metabolic pathways, underscoring the challenge of developing an effective K-ras inhibitor.Keywords: pancreas cancer, paclitaxel, gemcitabine, irradiation

Abstract

Elevated CO2 (eCO2) experiments provide critical information to quantify the effects of rising CO2 on vegetation1–6. Many eCO2 experiments suggest that nutrient limitations modulate the local magnitude of the eCO2 effect on plant biomass1,3,5, but the global extent of these limitations has not been empirically quantified, complicating projections of the capacity of plants to take up CO27,8. Here, we present a data-driven global quantification of the eCO2 effect on biomass based on 138 eCO2 experiments. The strength of CO2 fertilization is primarily driven by nitrogen (N) in ~65% of global vegetation and by phosphorus (P) in ~25% of global vegetation, with N- or P-limitation modulated by mycorrhizal association. Our approach suggests that CO2 levels expected by 2100 can potentially enhance plant biomass by 12 ± 3% above current values, equivalent to 59 ± 13 PgC. The global-scale response to eCO2 we derive from experiments is similar to past changes in greenness9 and biomass10 with rising CO2, suggesting that CO2 will continue to stimulate plant biomass in the future despite the constraining effect of soil nutrients. Our research reconciles conflicting evidence on CO2 fertilization across scales and provides an empirical estimate of the biomass sensitivity to eCO2 that may help to constrain climate projections.

Abstract

Purpose: The German Retina.net ROP registry and its Europe‐wide successor, the EU‐ROP registry, collect data from patients treated for ROP. This analysis compares input parameters of these two registries to establish a procedure for joint analyses of different registry data using exemplary datasets from the two registries. Methods: Exemplary datasets from the two databases over a 1‐year period each (German Retina.net ROP Registry, 2011, 22 infants; EU‐ROP Registry, 2021, 44 infants) were compared. The parameters documented in the two databases were aligned and analysed regarding demographic parameters, treatment modalities, complications within first 24 h and retreatments. Results: The current analysis showed that data can be aligned for joint analyses with some adjustments within the data structure. The registry with more detailed data collection (EU‐ROP) needs to be reduced regarding granularity in order to align the different registries, as the registry with lower granularity determines the level of analyses that can be performed in a comparative approach. In the exemplary datasets, we observed that the overall most common ROP severity in both registries was zone II, 3+ (2011: 70.5%; 2021: 65%), with decreasing numbers of clock hours showing preretinal neovascularisations (2011: 10–12 clock hours in 29% of cases, 2021: 4–6 clock hours in 38%). The most prevalent treatment method was laser coagulation in 2011 (75%) and anti‐VEGF therapy in 2021 (86.1%). Within the anti‐VEGF group, all patients were treated with bevacizumab in 2011 and with ranibizumab in 2021. Retreatment rates were comparable in 2011 and 2021. Conclusion: Data from two different ROP registries can be aligned and jointly analysed. The analysis reveals a paradigm shift in treatment modalities, from predominantly laser to anti‐VEGF, and within the anti‐VEGF group from bevacizumab to ranibizumab in Germany. Abstract

Urban and peri-urban lands can be an important source of food production for localised and sustainable food systems, however, their soils can be of poor quality, degraded or damaged by anthropic activities, and little is known about their suitability or safety. This paper aims to contribute to this knowledge gap by assessing the soil remediation capacity and qualities of different types of compost made from urban and peri-urban organic wastes for agroecological food production. Prepared over the course of 2021, and used in 2022 for food growing, five different composts were observed and analysed, in two different farms in the city of Rosario, Argentina. Four raw materials generated largely by local industries were used to make the composts: chicken manure, rumen (cow's stomachs), brewer's bagasse (byproducts of the beer industry) and urban leaves collected from the municipality waste collection. These were mixed in different proportions (all reaching the 20-30 C/N ratio, typical of quality compost) to produce viable growing substrates where radishes and lettuces were grown. The aim of the study was to assess the possibility, quality and limitations to use locally available organic inputs for soil fertility management in agroecological farming, in the context of urbanisation and to assess pathways to develop closedcycle agroecological agriculture at metropolitan level. Natural manure substrates (raw and composted) were analysed, as well as crops grown and fertilised with each of the substrates. The attributes and limiting factors of each substrate and their response to local soil conditions were compared and physicochemical, biochemical, and microbiological analyses were performed, including among others, the study of microbial biomass, biological activity, biophytotoxicity, pH, aerobic heterotrophs, nitrogen fixation, and the presence of antibiotics, agrochemicals and heavy metals. The results of the analyses show that all the composted materials improved the physical, chemical and biological properties. However, in some cases, pollutants were present even after composting. Analysis carried out on the vegetables generally indicate undetectable levels or levels below the admissible limits, demonstrating the filtering capacity of the different composts and the soil. Abstract

The nuclear emulsion target of the CHORUS detector was exposed to the wide-band neutrino beam of the CERN SPS of 27 GeV average neutrino energy from 1994 to 1997. In total about 100000 charged-current neutrino interactions with at least one identified muon were located in the emulsion target and fully reconstructed, using newly developed automated scanning systems. Charmed particles were searched for by a program recognizing particle decays. The observation of the decay in nuclear emulsion makes it possible to select a sample with very low background and minimal kinematical bias. 2013 charged-current interactions with a charmed hadron candidate in the final state were selected and confirmed through visual inspection. The charm production rate induced by neutrinos relative to the charged-current cross-section is measured to be sigma(nu_mu N -> mu- C X)/sigma(CC) = (5.75 +-0.32 stat +-0.30 syst)%. The charm production cross-section as a function of the neutrino energy is also obtained. The results are in good agreement with previous measurements. The charm-quark hadronization produces the following charmed hadrons with relative fractions (in %): f_Dzero = 43.7+-4.5, f_Lambda_c^plus = 19.2+-4.2, f_Dplus = 25.3+-4.2, and f_D_splus = 11.8+-4.7., Comment: 16 pages, 5 figures

Abstract

A new ephemeris has been determined for the supersoft X-ray binary CAL 83 using MACHO photometry. With an improved orbital period of 1.047568 days, it is now possible to phase together photometric and spectroscopic data obtained over the past two decades with new far ultraviolet spectra taken with FUSE. We discuss the properties of the orbital and longterm optical light curves as well as the colors of CAL 83. In the far ultraviolet the only well-detected stellar feature is emission from the O VI resonance doublet. The radial velocity of this emission appears to differ from that of HeII in the optical region, although we only have partial phase coverage for the O VI line. The FUSE continuum variations are similar to the optical light curve in phase and amplitude., Comment: 21 pages and 7 figures

Abstract

Far-ultraviolet spectra of LMC X-3 were taken covering photometric phases 0.47 to 0.74 in the 1.7-day orbital period of the black-hole binary (phase zero being superior conjunction of the X-ray source). The continuum is faint and flat, but appears to vary significantly during the observations. Concurrent RXTE/ASM observations show the system was in its most luminous X-ray state during the FUSE observations. The FUV spectrum contains strong terrestrial airglow emission lines, while the only stellar lines clearly present are emissions from the O VI resonance doublet. Their flux does not change significantly during the FUSE observations. These lines are modelled as two asymmetrical profiles, including the local ISM absorptions due to C II and possibly O VI. We have obtained spectroscopy with the Far Ultraviolet Spectroscopic Explorer (FUSE) of the supersoft X-ray binary RX J0513.9-6951 over a complete binary orbital cycle. The spectra show a hot continuum with extremely broad O VI emission and weak Lyman absorptions. He II emission is weak and narrow, while N III and C III emissions are undetected, although lines from these ions are prominent at optical wavelengths. The broad O VI emission and Lyman absorption show radial velocity curves that are approximately antiphased and have semiamplitudes of ~117 +- 40 and 54 +- 10 km/s, respectively. Narrow emissions from He II and O VI show small velocity variations with phasing different from the broad O VI, but consistent with the optical line peaks. We also measure considerable changes in the FUV continuum and O VI emission line flux. Abstract

Pseudoaneurysms of the posterior circulation pose a unique management challenge. The fragile nature of the pseudoaneurysm wall presents a high risk of rupture and demands treatment. Small vasculature, particularly distal in the posterior circulation, can preclude management with traditional flow diverters, where the alternative of vessel sacrifice is unacceptable. Small stents can have flow-diversion properties and can be used in these high-risk, difficult-to-access aneurysms. We describe a 40-year-old woman presenting with a ruptured dissecting right superior cerebellar artery pseudoaneurysm after minor trauma. Given the aneurysm's small size and morphology, it was not amenable to coiling and parent vessel sacrifice was potentially morbid. The pseudoaneurysm was initially stabilized with a Low-Profile Visualized Intraluminal Support Junior (LVIS Jr.) stent due to its reported flow-diverting properties. At six-month follow-up the pseudoaneurysm was stable and the vasospasm had resolved. At this point, definitive treatment with a "FRED Jr." (Flow Re-Direction Endoluminal Device Junior) flow diverter was pursued. Complete obliteration of the pseudoaneurysm was seen at 12 months' follow-up after staged treatment. Due to the unique challenges associated with ruptured pseudoaneurysms located on small-caliber vessels, the options for definitive treatment are limited. The small size of the LVIS Jr. stent and its flow-diverting properties make it a practical treatment option in a difficult situation. This case report provides further support for the flow-diverting properties of the LVIS Jr. and its potential application in the treatment of ruptured pseudoaneurysms in small-caliber intracranial vessels. Abstract

Background Clinical complexity is increasingly prevalent among patients with atrial fibrillation (AF). The ‘Atrial fibrillation Better Care’ (ABC) pathway approach has been proposed to streamline a more holistic and integrated approach to AF care; however, there are limited data on its usefulness among clinically complex patients. We aim to determine the impact of ABC pathway in a contemporary cohort of clinically complex AF patients. Methods From the ESC-EHRA EORP-AF General Long-Term Registry, we analysed clinically complex AF patients, defined as the presence of frailty, multimorbidity and/or polypharmacy. A K-medoids cluster analysis was performed to identify different groups of clinical complexity. The impact of an ABC-adherent approach on major outcomes was analysed through Cox-regression analyses and delay of event (DoE) analyses. Results Among 9966 AF patients included, 8289 (83.1%) were clinically complex. Adherence to the ABC pathway in the clinically complex group reduced the risk of all-cause death (adjusted HR [aHR]: 0.72, 95%CI 0.58–0.91), major adverse cardiovascular events (MACEs; aHR: 0.68, 95%CI 0.52–0.87) and composite outcome (aHR: 0.70, 95%CI: 0.58–0.85). Adherence to the ABC pathway was associated with a significant reduction in the risk of death (aHR: 0.74, 95%CI 0.56–0.98) and composite outcome (aHR: 0.76, 95%CI 0.60–0.96) also in the high-complexity cluster; similar trends were observed for MACEs. In DoE analyses, an ABC-adherent approach resulted in significant gains in event-free survival for all the outcomes investigated in clinically complex patients. Based on absolute risk reduction at 1 year of follow-up, the number needed to treat for ABC pathway adherence was 24 for all-cause death, 31 for MACEs and 20 for the composite outcome. Conclusions An ABC-adherent approach reduces the risk of major outcomes in clinically complex AF patients. Ensuring adherence to the ABC pathway is essential to improve clinical outcomes among clinically complex AF patients.

Abstract

Background: Atrial fibrillation (AF) and renal impairment share a bidirectional relationship with important pathophysiological interactions. We evaluated the impact of renal impairment in a contemporary cohort of patients with AF. Methods: We utilised the ESC-EHRA EORP-AF Long-Term General Registry. Outcomes were analysed according to renal function by CKD-EPI equation. The primary endpoint was a composite of thromboembolism, major bleeding, acute coronary syndrome and all-cause death. Secondary endpoints were each of these separately including ischaemic stroke, haemorrhagic event, intracranial haemorrhage, cardiovascular death and hospital admission. Results: A total of 9306 patients were included. The distribution of patients with no, mild, moderate and severe renal impairment at baseline were 16.9%, 49.3%, 30% and 3.8%, respectively. AF patients with impaired renal function were older, more likely to be females, had worse cardiac imaging parameters and multiple comorbidities. Among patients with an indication for anticoagulation, prescription of these agents was reduced in those with severe renal impairment, p 2 decrease), that was most notable in patients with eGFR 2 (HR 2.21 [95% CI, 1.23–3.99] compared to eGFR ≥90 ml/min/1.73 m 2). Conclusion: A significant proportion of patients with AF suffer from concomitant renal impairment which impacts their overall management. Furthermore, renal impairment is an independent predictor of major adverse events including thromboembolism, major bleeding, acute coronary syndrome and all-cause death in patients with AF.

Abstract

Background Frailty is a medical syndrome characterised by reduced physiological reserve and increased vulnerability to stressors. Data regarding the relationship between frailty and atrial fibrillation (AF) are still inconsistent. Objectives We aim to perform a comprehensive evaluation of frailty in a large European cohort of AF patients. Methods A 40-item frailty index (FI) was built according to the accumulation of deficits model in the AF patients enrolled in the ESC-EHRA EORP-AF General Long-Term Registry. Association of baseline characteristics, clinical management, quality of life, healthcare resources use and risk of outcomes with frailty was examined. Results Among 10,177 patients [mean age (standard deviation) 69.0 (11.4) years, 4,103 (40.3%) females], 6,066 (59.6%) were pre-frail and 2,172 (21.3%) were frail, whereas only 1,939 (19.1%) were considered robust. Baseline thromboembolic and bleeding risks were independently associated with increasing FI. Frail patients with AF were less likely to be treated with oral anticoagulants (OACs) (odds ratio 0.70, 95% confidence interval 0.55–0.89), especially with non-vitamin K antagonist OACs and managed with a rhythm control strategy, compared with robust patients. Increasing frailty was associated with a higher risk for all outcomes examined, with a non-linear exponential relationship. The use of OAC was associated with a lower risk of outcomes, except in patients with very/extremely high frailty. Conclusions In this large cohort of AF patients, there was a high burden of frailty, influencing clinical management and risk of adverse outcomes. The clinical benefit of OAC is maintained in patients with high frailty, but not in very high/extremely frail ones.
