Abstract

The NA48/2 experiment at CERN reports the first observation of the $K^{\pm} \rightarrow \pi^{0} \pi^{0} \mu^{\pm} \nu$ decay based on a sample of 2437 candidates with 15% background contamination collected in 2003--2004. The decay branching ratio in the kinematic region of the squared dilepton mass above $0.03$~GeV$^2/c^4$ is measured to be $(0.65 \pm 0.03) \times 10^{-6}$. The extrapolation to the full kinematic space, using a specific model, is found to be $(3.45 \pm 0.16) \times 10^{-6}$, in agreement with chiral perturbation theory predictions., Comment: 16 pages, 9 figures

3. Examining Linguistic Differences in Electronic Health Records for Diverse Patients With Diabetes: Natural Language Processing Analysis

Abstract

Abstract BackgroundIndividuals from minoritized racial and ethnic backgrounds experience pernicious and pervasive health disparities that have emerged, in part, from clinician bias. ObjectiveWe used a natural language processing approach to examine whether linguistic markers in electronic health record (EHR) notes differ based on the race and ethnicity of the patient. To validate this methodological approach, we also assessed the extent to which clinicians perceive linguistic markers to be indicative of bias. MethodsIn this cross-sectional study, we extracted EHR notes for patients who were aged 18 years or older; had more than 5 years of diabetes diagnosis codes; and received care between 2006 and 2014 from family physicians, general internists, or endocrinologists practicing in an urban, academic network of clinics. The race and ethnicity of patients were defined as White non-HispanicBlack non-HispanicHispanic or Latino ResultsWe examined EHR notes (n=12,905) of Black non-Hispanic, White non-Hispanic, and Hispanic or Latino patients (n=1562), who were seen by 281 physicians. A total of 27 clinicians participated in the validation study. In terms of bias, participants rated negative adjectives as 8.63 (SD 2.06), fear and disgust words as 8.11 (SD 2.15), and positive adjectives as 7.93 (SD 2.46) on a scale of 1 to 10, with 10 being extremely indicative of bias. Notes for Black non-Hispanic patients contained significantly more negative adjectives (coefficient 0.07, SE 0.02) and significantly more fear and disgust words (coefficient 0.007, SE 0.002) than those for White non-Hispanic patients. The notes for Hispanic or Latino patients included significantly fewer positive adjectives (coefficient −0.02, SE 0.007), trust verbs (coefficient −0.009, SE 0.004), and joy words (coefficient −0.03, SE 0.01) than those for White non-Hispanic patients. ConclusionsThis approach may enable physicians and researchers to identify and mitigate bias in medical interactions, with the goal of reducing health disparities stemming from bias.

Abstract

The RNA-guided nuclease Cas9 has unlocked powerful methods for perturbing both the genome through targeted DNA cleavage and the regulome through targeted DNA binding, but limited biochemical data have hampered efforts to quantitatively model sequence perturbation of target binding and cleavage across diverse guide sequences. We present scalable, sequencing-based platforms for high-throughput filter binding and cleavage and then perform 62,444 quantitative binding and cleavage assays on 35,047 on- and off-target DNA sequences across 90 Cas9 ribonucleoproteins (RNPs) loaded with distinct guide RNAs. We observe that binding and cleavage efficacy, as well as specificity, vary substantially across RNPs; canonically studied guides often have atypically high specificity; sequence context surrounding the target modulates Cas9 on-rate; and Cas9 RNPs may sequester targets in nonproductive states that contribute to "proofreading" capability. Lastly, we distill our findings into an interpretable biophysical model that predicts changes in binding and cleavage for diverse target sequence perturbations.

Abstract

We present a measurement of $B(\pi^0 \rightarrow e^+e^- \gamma)/B(\pi^0 \rightarrow \gamma\gamma)$, the Dalitz branching ratio, using data taken in 1999 by the E832 KTeV experiment at Fermi National Accelerator Laboratory. We use neutral pions from fully reconstructed $K_L$ decays in flight; the measurement is based on about 60 thousand $K_L \rightarrow \pi^0\pi^0\pi^0 \rightarrow \gamma\gamma~\gamma\gamma~e^+e^-\gamma$ decays. We normalize to $K_L \rightarrow \pi^0\pi^0\pi^0 \rightarrow 6\gamma$ decays. We find $B(\pi^0 \rightarrow e^+e^- \gamma)/B(\pi^0 \rightarrow \gamma\gamma)$ $(m_{e^+e^-}$ > 15 MeV/$c^2)$ = $[3.920 \pm 0.016(stat) \pm 0.036 (syst)] \times 10^{-3}$. Using the Mikaelian and Smith prediction for the $e^+e^-$ mass spectrum, we correct the result to the full $e^+e^-$ mass range. The corrected result is $B(\pi^0 \rightarrow e^+e^- \gamma)/B(\pi^0 \rightarrow \gamma\gamma) = [1.1559 \pm 0.0047(stat) \pm 0.0106 (syst)]$%. 10. An Assessment of How Clinicians and Staff Members Use a Diabetes Artificial Intelligence Prediction Tool: Mixed Methods Study

Abstract

BackgroundNearly one-third of patients with diabetes are poorly controlled (hemoglobin A1c≥9%). Identifying at-risk individuals and providing them with effective treatment is an important strategy for preventing poor control. ObjectiveThis study aims to assess how clinicians and staff members would use a clinical decision support tool based on artificial intelligence (AI) and identify factors that affect adoption. MethodsThis was a mixed methods study that combined semistructured interviews and surveys to assess the perceived usefulness and ease of use, intent to use, and factors affecting tool adoption. We recruited clinicians and staff members from practices that manage diabetes. During the interviews, participants reviewed a sample electronic health record alert and were informed that the tool uses AI to identify those at high risk for poor control. Participants discussed how they would use the tool, whether it would contribute to care, and the factors affecting its implementation. In a survey, participants reported their demographics; rank-ordered factors influencing the adoption of the tool; and reported their perception of the tool’s usefulness as well as their intent to use, ease of use, and organizational support for use. Qualitative data were analyzed using a thematic content analysis approach. We used descriptive statistics to report demographics and analyze the findings of the survey. ResultsIn total, 22 individuals participated in the study. Two-thirds (14/22, 63%) of respondents were physicians. Overall, 36% (8/22) of respondents worked in academic health centers, whereas 27% (6/22) of respondents worked in federally qualified health centers. The interviews identified several themes: this tool has the potential to be useful because it provides information that is not currently available and can make care more efficient and effective; clinicians and staff members were concerned about how the tool affects patient-oriented outcomes and clinical workflows; adoption of the tool is dependent on its validation, transparency, actionability, and design and could be increased with changes to the interface and usability; and implementation would require buy-in and need to be tailored to the demands and resources of clinics and communities. Survey findings supported these themes, as 77% (17/22) of participants somewhat, moderately, or strongly agreed that they would use the tool, whereas these figures were 82% (18/22) for usefulness, 82% (18/22) for ease of use, and 68% (15/22) for clinic support. The 2 highest ranked factors affecting adoption were whether the tool improves health and the accuracy of the tool. ConclusionsMost participants found the tool to be easy to use and useful, although they had concerns about alert fatigue, bias, and transparency. These data will be used to enhance the design of an AI tool.

Abstract

$K^{+}\rightarrow\pi^{+}\nu\overline{\nu}$ is one of the theoretically cleanest meson decay where to look for indirect effects of new physics complementary to LHC searches. The NA62 experiment at CERN SPS is designed to measure the branching ratio of this decay with 10\% precision. NA62 took data in pilot runs in 2014 and 2015 reaching the final designed beam intensity. Abstract

Endmember Extraction is a critical step in hyperspectral image analysis and classification. It is an useful method to decompose a mixed spectrum into a collection of spectra and their corresponding proportions. In this paper, we solve a linear endmember extraction problem as an evolutionary optimization task, maximizing the Simplex Volume in the endmember space. We propose a standard genetic algorithm and a variation with In Vitro Fertilization module (IVFm) to find the best solutions and compare the results with the state-of-art Vertex Component Analysis (VCA) method and the traditional algorithms Pixel Purity Index (PPI) and N-FINDR. Abstract

The NA48/2 Collaboration at CERN has accumulated unprecedented statistics of rare kaon decays in the Ke4 modes: Ke4(+-) ($K^\pm \to \pi^+ \pi^- e^\pm \nu$) and Ke4(00) ($K^\pm \to \pi^0 \pi^0 e^\pm \nu$) with nearly one percent background contamination. The detailed study of form factors and branching rates, based on these data, has been completed recently. The results brings new inputs to low energy strong interactions description and tests of Chiral Perturbation Theory (ChPT) and lattice QCD calculations. In particular, new data support the ChPT prediction for a cusp in the $\pi^0\pi^0$ invariant mass spectrum at the two charged pions threshold for Ke4(00) decay. New final results from an analysis of about 400 $K^\pm \to \pi^\pm \gamma \gamma$ rare decay candidates collected by the NA48/2 and NA62 experiments at CERN during low intensity runs with minimum bias trigger configurations are presented. Abstract

With conversational agents triaging symptoms, cameras aiding diagnoses, and remote sensors monitoring vital signs, the use of artificial intelligence (AI) outside of hospitals has the potential to improve health, according to a recently released report from the National Academy of Medicine. Despite this promise, the success of AI is not guaranteed, and stakeholders need to be involved with its development to ensure that the resulting tools can be easily used by clinicians, protect patient privacy, and enhance the value of the care delivered. A crucial stakeholder group missing from the conversation is primary care. As the nation’s largest delivery platform, primary care will have a powerful impact on whether AI is adopted and subsequently exacerbates health disparities. To leverage these benefits, primary care needs to serve as a medical home for AI, broaden its teams and training, and build on government initiatives and funding.

Abstract

Background. The risk of donor-derived severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection in solid organ (heart, lung, liver, kidney, pancreas, and intestine) transplant recipients is poorly understood. Since hematogenous transmission of SARS-CoV-2 has not been documented to date, nonlung solid organs might be suitable for transplantation since they likely portend a low risk of viral transmission. Methods. Abdominal solid organs from SARS-CoV-2–infected donors were transplanted into uninfected recipients. Results. Between April 18, 2021, and October 30, 2021, we performed transplants of 2 livers, 1 simultaneous liver and kidney, 1 kidney, and 1 simultaneous kidney and pancreas from SARS-CoV-2–infected donors into 5 uninfected recipients. None of the recipients developed SARS-CoV-2 infection or coronavirus disease 2019, and when tested, allograft biopsies showed no evidence of SARS-CoV-2 RNA. Conclusions. Transplanting nonlung organs from SARS-CoV-2–infected donors into uninfected recipients demonstrated no evidence of virus transmission.

Abstract

Abstract Background Many patients with poorly controlled multiple chronic conditions (MCC) also have unhealthy behaviors, mental health challenges, and unmet social needs. Medical management of MCC may have limited benefit if patients are struggling to address their basic life needs. Health systems and communities increasingly recognize the need to address these issues and are experimenting with and investing in new models for connecting patients with needed services. Yet primary care clinicians, whose regular contact with patients makes them more familiar with patients’ needs, are often not included in these systems. Methods We are starting a clinician-level cluster-randomized controlled trial to evaluate how primary care clinicians can participate in these community and hospital solutions and whether doing so is effective in controlling MCC. Sixty clinicians in the Virginia Ambulatory Care Outcomes Research Network will be matched by age and sex and randomized to usual care (control condition) or enhanced care planning with clinical-community linkage support (intervention). From the electronic health record we will identify all patients with MCC, including cardiovascular disease or risks, diabetes, obesity, or depression. A baseline assessment will be mailed to up to 50 randomly selected patients for each clinician (3000 total). Ten respondents per clinician (600 patients total) with uncontrolled MCC will be randomly selected for study inclusion, with oversampling of minorities. The intervention includes two components. First, we will use an enhanced care planning tool, My Own Health Report (MOHR), to screen patients for health behavior, mental health, and social needs. Patients will be supported by a patient navigator, who will help patients prioritize needs, create care plans, and write a personal narrative to guide the care team. Patients will update care plans every 1 to 2 weeks. Second, we will create community-clinical linkage to help address patients’ needs. The linkage will include community resource registries, personnel to span settings (patient navigators and a community health worker), and care team coordination across team members through MOHR. Discussion This study will help inform efforts by primary care clinicians to help address unhealthy behaviors, mental health needs, and social risks as a strategy to better control MCC. Abstract

From the 2002 data taking with a neutral kaon beam extracted from the CERN-SPS, the NA48/1 experiment observed 97 $\Xi^{0}\rightarrow \Sigma^{+} \mu^{-} \bar{\nu}_{\mu}$ candidates with a background contamination of $30.8 \pm 4.2$ events. From this sample, the BR($\Xi^{0}\rightarrow \Sigma^{+} \mu^{-} \bar{\nu}_{\mu}$) is measured to be $(2.17 \pm 0.32_{\mathrm{stat}}\pm 0.17_{\mathrm{syst}})\times10^{-6}$.

Abstract

The KTeV E799 experiment has conducted a search for the rare decays KL->pi0pi0mu+mu- and KL->pi0pi0X0->pi0pi0mu+mu-, where the X0 is a possible new neutral boson that was reported by the HyperCP experiment with a mass of (214.3 pm 0.5) MeV/c^{2}. We find no evidence for either decay. We obtain upper limits of Br(KL->pi0pi0X0->pi0pi0mu+mu-) < 1.0 x 10^{-10} and Br(KL->pi0pi0mu+mu-) < 9.2 x 10^{-11} at the 90% confidence level. This result rules out the pseudoscalar X0 as an explanation of the HyperCP result under the scenario that the \bar{d}sX0 coupling is completely real.

Introduction and objectives: Acute on chronic liver failure (ACLF), leads to high mortality. These patients are at risk of being delisted for liver transplantation (LT). Emerging data shows 1y post-transplant survival of 80–92%. The Share 35 (S35) policy was implemented to prioritize patients with MELD ≥35 on the LT waitlist. Our aim was to compare the LT outcomes of ACLF patients as a result of S35. Materials and methods: Data from the UNOS scientific registry were used to classify ACLF patients using the NACSELD criteria. For the analyses, data were divided into two eras; 2 years before S35 (Era 1) and 2 years after S35 (Era 2). Waitlist status was classified into categories: Transplanted, Death or Too Sick to Transplant and Still Waiting/Other. LT cumulative incidence between the populations in the eras was calculated using Fine and Gray's method. A proportional hazards model was used to investigate the era effect on cumulative incidence of LT. Results: 46,861 patients were reviewed, of which 817 had ACLF. 366 patients (mean MELD: 37.1) were identified in Era 1 and 451 patients (mean MELD: 37.3) in Era 2. We found that ACLF patients were more likely to receive a liver transplant in Era 2 (p = 0.0074). Abstract

We present precise tests of CP and CPT symmetry based on the full dataset of K to pipi decays collected by the KTeV experiment at Fermi National Accelerator Laboratory during 1996, 1997, and 1999. This dataset contains 16 million K to 2pi0 and 69 million K to pi+pi- decays. We measure the direct CP violation parameter Re(epsilon'/epsilon) = (19.2 pm 2.1)x10-4. We find the KL-KS mass difference Deltam = (5270 pm 12)x10^6 hbar/s and the KS lifetime tauS = (89.62 pm 0.05)x10-12 s. We also measure several parameters that test CPT invariance. We find the difference between the phase of the indirect CP violation parameter, epsilon, and the superweak phase, phi_epsilon - phi_SW = (0.40 pm 0.56) degrees. We measure the difference of the relative phases between the CP violating and CP conserving decay amplitudes for K to pi+pi- (phi+-) and for K to 2pi0 (phi00), Delta phi = (0.30 pm 0.35) degrees. From these phase measurements, we place a limit on the mass difference between K0 and K0bar, DeltaM < 4.8 x 10-19 GeV/c^2 at 95% C.L. Abstract

As first observed by the NA48/2 experiment at the CERN SPS, the $\p0p0$ invariant mass (M00) distribution from $\kcnn$ decay shows a cusp-like anomaly at M00=2m+, where m+ is the charged pion mass. An analysis to extract the pi pi scattering lengths in the isospin I=0 and I=2 states, a0 and a2, respectively, has been recently reported. In the present work the Dalitz plot of this decay is fitted to a new empirical parameterization suitable for practical purposes, such as Monte Carlo simulations of K+- -> pi+- pi0 pi0 decays., Comment: 9 pages, 3 figures.

Abstract

We report on a new measurement of the branching ratio B(KL->pi0 gamma gamma)using the KTeV detector. We reconstruct 1982 events with an estimated background of 608, that results in B(KL -> pi0 gamma gamma) = (1.29 +/- 0.03(stat) +/- 0.05(syst)) x 10**-6. We also measure the parameter, aV, which characterizes the strength of vector meson exchange terms in this decay. We find aV = -0.31 +/- 0.05(stat) +/- 0.07(syst). These results utilize the full KTeV data set collected from 1997 to 2000 and supersede earlier KTeV measurements of the branching ratio and aV., Comment: 8 pages, 8 figures

Abstract

We present a new determination of the parity of the neutral pion via the double Dalitz decay pi^0 -> e+ e- e+ e-. Our sample, which consists of 30511 candidate decays, was collected from K_L -> pi0 pi0 pi0 decays in flight at the KTeV-E799 experiment at Fermi National Accelerator Laboratory. We confirm the negative pi^0 parity, and place a limit on scalar contributions to the pi^0 -> e+ e- e+ e- decay amplitude of less than 3.3% assuming CPT conservation. The pi^0 gamma* gamma* form factor is well described by a momentum-dependent model with a slope parameter fit to the final state phase space distribution. Additionally, we have measured the branching ratio of this mode to be B(pi^0 -> e+ e- e+ e-) = (3.26 +- 0.18) x 10^(-5)., Comment: 5 pages, 4 figures. Abstract

The KTeV E799 experiment has conducted a search for the rare decay $K_{L}\to\pi^{0}\pi^{0}\gamma$ via the topology $K_{L}\to\pi^{0}\pi^{0}_D\gamma$ (where $\pi^0_D\to\gamma e^+e^-$). Due to Bose statistics of the $\pi^0$ pair and the real nature of the photon, the $K_{L}\to\pi^{0}\pi^{0}\gamma$ decay is restricted to proceed at lowest order by the CP conserving direct emission (DE) of an E2 electric quadrupole photon. The rate of this decay is interesting theoretically since chiral perturbation theory predicts that this process vanishes at level $O(p^4)$. Therefore, this mode probes chiral perturbation theory at $O(p^6)$. In this paper we report a determination of an upper limit of $2.43\times 10^{-7}$ (90% CL) for $K_{L}\to\pi^{0}\pi^{0}\gamma$. Abstract

Abstract

The Xi^0 muon semi-leptonic decay has been observed for the first time with nine identified events using the KTeV beam line and detector at Fermilab. Abstract

We present measurements of R_Kl3rad = Gamma(KL->\pi\ell\nu\gamma; Egcm > 10 MeV)/Gamma(KL->\pi\ell\nu), where ell = mu or e, and Egcm is the photon energy in the kaon rest frame. These measurements are based on KL decays collected in 1997 by the KTeV (E832) experiment at Fermilab. With samples of 1385 KL->\pi\mu\nu\gamma and 14221 KL->\pi\e\nu\gamma candidates, we find R_Km3rad = (0.530 +- 0.019)% and R_Ke3rad = (4.942 +- 0.062)%. Abstract

In this study, we evaluated family physicians' ability to estimate the service area of their patient panel--a critical first step in contextual population-based primary care. We surveyed 14 clinicians and administrators from 6 practices. Participants circled their estimated service area on county maps that were compared with the actual service area containing 70% of the practice's patients. Accuracy was ascertained from overlap and the amount of estimated census tracts that were not part of the actual service area. Average overlap was 75%, but participants overestimated their service area by an average of 166 square miles. Service area overestimation impedes implementation of targeted community interventions by practices. Abstract

PURPOSE Loneliness has important health consequences. Little is known, however, about loneliness in primary care patient populations. This study describes the prevalence of loneliness in patients presenting for primary care and associations with self-reported demographic factors, health care utilization, and health-related quality of life. METHODS We conducted cross-sectional surveys of adults presenting for routine care to outpatient primary care practices in 2 diverse practice-based research networks. The 3-item University of California, Los Angeles Loneliness Scale was utilized to determine loneliness. RESULTS The prevalence of loneliness was 20% (246/1,235). Loneliness prevalence was inversely associated with age (P CONCLUSIONS Our findings demonstrate that loneliness is common in primary care patients and is associated with adverse health consequences including poorer health status and greater health care utilization. Further work is needed to understand the value of screening for and using interventions to treat loneliness in primary care.
