Abstract

We investigated the cortical bone changes in 35 patients with total hip arthroplasty operated on only for osteoarthritis with more than 10 years of follow-up and with nonrevised femoral components and without radiologic signs of loosening. The mean follow-up was 16 ± 5 years. The thicknesses of femoral cortices were measured medially and laterally at 6 levels from the first postoperative and the last follow-up x-rays. A comparison with 10 patients who had a nonoperated contralateral hip was performed. We found a significant decrease in cortical thicknesses in total hip arthroplasty. The cortical thinning was significant at all periprosthetic levels but less expressed distally. Prosthetic femora were associated with greater cortical thinning as compared with the contralateral nonoperated femora, exceeding that caused by natural aging. [ABSTRACT FROM AUTHOR]

30. Femoral head diameter affects the revision rate in total hip arthroplasty: an analysis of 1,720 hip replacements with 9-21 years of follow-up.

Abstract

Background In a previous study concerning 1,660 ScanHip THAs that were followed for up to 12 years, the cumulative revision rate was not found to be dependent on whether a 22-mm or a 32-mm head size had been used. We have re-examined these patients to see whether a longer follow-up time (9-21 years) would disclose an effect of head size on the revision rate. Patients and methods We analyzed the cumulative revision rate for 1,720 Scan Hip arthroplasties with either 22-mm or 32-mm femoral heads. The patients were followed for 9-21 years. Results Arthroplasties with 32-mm head had 2.8-times higher cumulative revision rate than those with a 22-mm head. Older age reduced the risk of revision while male sex increased the risk. Interpretation We found that head size affects revision risk, but that even in a reasonably large material a long follow-up time is required to disclose the effects of head size-and thus wear-on survival. 33. Functional outcome after displaced femoral neck fractures treated with osteosynthesis or hemiarthroplasty: a matched-pair study of 714 patients.

Abstract

Osteosynthesis (OS) and hemiarthroplasty (HA) are the commonest treatments for displaced cervical hip fractures in the elderly, but there is no consensus as to which is better. In this prospective matched-pair study we compared these methods as regards functional outcome. In 1989-1996, using the same standardized forms, all displaced cervical hip fractures were prospectively registered in the university hospitals of Oulu in Finland and Lund in Sweden. Osteosynthesis was performed in Lund and hemiarthroplasty in Oulu. Cross-matching, done for age, sex, preoperative residence, and ambulatory ability, resulted in 357 pairs of displaced fractures. More OS than HA patients could manage in their own homes or live semi-independently at 4 months after the fracture. OS patients had better ambulatory ability (p = 0.001) and used walking aids less than HA ones (p = 0.001). The reoperation rates at 4 months were the same among HA and OS patients, but at 1 year, OS patients had a higher reoperation rate (17%) than HA ones (9.5%). [ABSTRACT FROM AUTHOR]

34. The ScanHip[sup (R)] total hip arthroplasty: Radiographic assessment of 72 hips after 10 years.

Abstract

We analyzed the radiographic and clinical outcome of the ScanHip total hip arthroplasty in 70 patients after 10 years. The Swedish National Hip Register, in which the end-point of the survival analysis is defined as revisions, reported a 10-year survival rate of 94% with the ScanHip, but in the present series 13% of the femoral stems and 29% of the sockets met the criteria for aseptic loosening. Focal osteolysis was found around 8 sockets (11%) and 23 stems (32%) and had occurred significantly oftener around loose sockets, but not around femoral stems. Linear polyethylene wear was significantly increased in loose sockets, but no relationship was noted between polyethylene wear and stem loosening or was there a correlation between clinical symptoms or patients' satisfaction and component loosening. Therefore precise serial long-term radiographic follow-up is the only satisfactory method for detecting aseptic loosening of total hip arthroplasty. 44. Pitfalls in Ultrasonography of Hip Joint Synovitis in the Child

Abstract

The normal and abnormal anatomy of the hip joint in childhood was evaluated by ultrasonography (US) with reference to examination technique and errors. When compared with computed tomography (CT), it was confirmed that the two sonographic echoes from the hip joint represent the femoral neck and the external surface of the capsule and that the thickness of the echo received from the capsule depends on the frequency of the transducer. Partial volume effects may simulate or increase the sonographic appearance of hip joint effusion. The normal relationship between joint capsule and femoral neck was established.

46. Measurement accuracy in acetabular cup wear

Abstract

The accuracy of three methods (the simple and noncomputerized Scheier-Sandel and Charnley-Duo methods and the computerized Ein Bild Roentgen Analyse [EBRA] method) for retrospective wear measurements of the acetabular cup from standard pelvis radiographs was studied. Measurements on 13 hip prostheses were compared with those obtained by roentgen stereophotogrammetry analysis. The Scheier Sandel method had the lowest accuracy and the EBRA method had the best accuracy. The Charnley-Duo method was almost as good when starting analysis 3 months after surgery and is easier to use. The EBRA method is useful for accurate measurements on a small number of patients; the Charnley-Duo method is recommended for clinical wear studies on a larger number of patients.

47. Intracapsular pressure and pain in coxarthrosis

Abstract

Intracapsular hip pressure was measured before surgery in 17 patients (18 hips) with coxarthrosis. The results were correlated to pain as registered on visual analog scale. With the hip in extension, the pressure was 49.5 mmHg (SD, 40.2); in 45^o of flexion, it was 21.3 mmHg (SD, 14.6); in extension and inward rotation, it was 105.7 mmHg (SD, 76.0); and in extension and outward rotation, it was 40.8 mmHg (SD, 32.0). A small amount, 0.7 mL (SD, 1.14), of joint fluid was aspirated following pressure registration. Pressure correlated significantly to pain at night, when starting to walk, and on walking. It is suggested that the increase in intracapsular hip pressure is a cause of pain in coxarthrosis, with subsequent limitation of movement and joint contracture

48. Conventional Radiography in Transient Synovitis of the Hip in Children

Abstract

Conventional radiography was performed at diagnosis and at follow-up 5 to 9 months later in 70 children with transient synovitis of the hip. Twenty-four of the patients also had CT examination at diagnosis. The cartilaginous and osseous reaction in the conventional antero-posterior radiographs was studied as was the relation between the radiographic and CT findings concerning fat planes, joint effusion, and position of the hip joint. At diagnosis there was significant increase in the medial joint space and at follow-up examination there was significant increase in medial and cranial joint space, metaphyseal width and acetabular roof width. There was a correlation of a bulging lateral ‘capsular fat plane’ to the position of abduction in the affected hip which explains the asymmetry in the lateral fat plane observed in these patients.
