Abstract

Tuberculosis knowledge, practices, and perceived stigma and discrimination among patients with tuberculosis are key factors for the management of the disease. The objectives of the study were to assess knowledge, practices, perceived stigma and discrimination, perceived family and health workers support, perceived level of satisfaction with healthcare services, delay in diagnosis/treatment and reasons for delay among patients with tuberculosis in Jordan. A cross-sectional study was conducted among patients who were under treatment for tuberculosis in Jordan in 2021. This study included a total of 452 patients with tuberculosis. About 91.4% of patients had low to moderate overall tuberculosis-related knowledge score and 8.6% had high knowledge score. Almost two-thirds of patients (67.5%) had perceived a low level of stigma, 61 (13.5%) perceived a moderate level of stigma, and 86 (19.0%) perceived a high level of stigma. The majority (84.5%) of patients with tuberculosis thought that there was a delay in diagnosis and/or treatment of tuberculosis. Our study showed gaps in tuberculosis knowledge and practices, high perceived stigma and discrimination, and perceived delay in diagnosis and treatment initiation,. Efforts within the national tuberculosis control program should be made to increase public awareness about the symptoms of tuberculosis and the importance of seeking early care. Abstract

Infection is a common complication of severe trauma and is a major cause of extra hospitalization and death. Patients who have sustained major blunt trauma have been found to have a suppressed immune system, which contributes to the occurrence of secondary infection. Blunt trauma is frequent in children, and while the immune system of children differs from that of adults, changes in its function following severe injury have not been well-studied. Immune system function was studied in 25 children, aged 2 to 14, who were hospitalized in intensive care for major trauma. Infections occurred in 14 children, and three patients died. Skin tests of delayed-type hypersensitivity (DTH) reactions to common bacterial antigens (molecules provoking immune responses) were used as initial indicators of immune function; 14 patients were anergic (absence of immune system reactivity), and these children were significantly more likely to develop infections. Non-anergic trauma patients had significantly fewer positive skin tests than did 10 control children with minor disorders. Specific changes in patient subgroups of lymphocytes, important infection-fighting white blood cells, were also studied. Among another group of 10 children, aged 2 months to 14 years and hospitalized with major head trauma, there was a significant decrease in levels of B lymphocytes, but not T lymphocytes, in the first week following injury. The study indicates that unlike adults with trauma injuries, children similarly injured sustain decreases in B lymphocytes, but not T lymphocytes. Use of the DTH skin test should be helpful in identifying patients at risk for infection and allow treatment focused at preventing infections in these patients. Abstract

Abstract Introduction Tuberculosis remains a major public health problem in India with the country accounting for one-fifth or 21% of all tuberculosis cases reported globally. The purpose of the study was to obtain an understanding on pro-poor initiatives within the framework of tuberculosis control programme in India and to identify mechanisms to improve the uptake and access to TB services among the poor. Methodology A national level workshop was held with participation from all relevant stakeholder groups. This study conducted during the stakeholder workshop adopted participatory research methods. The data was elicited through consultative and collegiate processes. The research study also factored information from primary and secondary sources that included literature review examining poverty headcount ratios and below poverty line population in the country; and quasi-profiling assessments to identify poor, backward and tribal districts as defined by the TB programme in India. Results Results revealed that current pro-poor initiatives in TB control included collaboration with private providers and engaging community to improve access among the poor to TB diagnostic and treatment services. The participants identified gaps in existing pro-poor strategies that related to implementation of advocacy, communication and social mobilisation; decentralisation of DOT; and incentives for the poor through the available schemes for public-private partnerships and provided key recommendations for action. Synergies between TB control programme and centrally sponsored social welfare schemes and state specific social welfare programmes aimed at benefitting the poor were unclear. Conclusion Further in-depth analysis and systems/policy/operations research exploring pro-poor initiatives, in particular examining service delivery synergies between existing poverty alleviation schemes and TB control programme is essential. The understanding, reflection and knowledge of the key stakeholders during this participatory workshop provides recommendations for action, further planning and research on pro-poor TB centric interventions in the country.

38. A rapid assessment and response approach to review and enhance Advocacy, Communication and Social Mobilisation for Tuberculosis control in Odisha state, India

Abstract

Abstract Background Tuberculosis remains a major public health problem in India with the country accounting for 1 in 5 of all TB cases reported globally. An advocacy, communication and social mobilisation project for Tuberculosis control was implemented and evaluated in Odisha state of India. The purpose of the study was to identify the impact of project interventions including the use of 'Interface NGOs' and involvement of community groups such as women's self-help groups, local government bodies, village health sanitation committees, and general health staff in promoting TB control efforts. Methods The study utilized a rapid assessment and response (RAR) methodology. The approach combined both qualitative field work approaches, including semi-structured interviews and focus group discussions with empirical data collection and desk research. Results Results revealed that a combination of factors including the involvement of Interface NGOs, coupled with increased training and engagement of front line health workers and community groups, and dissemination of community based resources, contributed to improved awareness and knowledge about TB in the targeted districts. Project activities also contributed towards improving health worker and community effectiveness to raise the TB agenda, and improved TB literacy and treatment adherence. Engagement of successfully treated patients also assisted in reducing community stigma and discrimination. Conclusion The expanded use of advocacy, communication and social mobilisation activities in TB control has resulted in a number of benefits. These include bridging pre-existing gaps between the health system and the community through support and coordination of general health services stakeholders, NGOs and the community. The strategic use of 'tailored messages' to address specific TB problems in low performing areas also led to more positive behavioural outcomes and improved efficiencies in service delivery. Implications for future studies are that a comprehensive and well planned range of ACSM activities can enhance TB knowledge, attitudes and behaviours while also mobilising specific community groups to build community efficacy to combat TB. The use of rapid assessments combined with other complementary evaluation approaches can be effective when reviewing the impact of TB advocacy, communication and social mobilisation activities.

41. Severe cow's milk induced colitis in an exclusively breast-fed neonate: case report and clinical review of cow's milk allergy

Abstract

Colitis, an inflammation of the colon, can be caused by allergy to cow's milk and usually occurs in late infancy. Colitis due to cow's milk allergy is rare in infants less than one week old. An unusual case describes a four-day-old healthy female infant who suddenly developed rectal bleeding, which became severe. She had no other gastrointestinal symptoms, such as vomiting or diarrhea. The baby had been fed only breast milk since birth, but tested positive for exposure to cow's milk protein, and, based on the results of skin tests, was clearly allergic to it. Her mother had milk intolerance, but had been drinking milk in large amounts since she began breast-feeding. Apparently the baby had been sensitized to cow's milk protein before birth. Consumption of cow's milk by the mother while breast-feeding caused the cow's milk proteins to enter the breast milk and be transmitted to the baby, who reacted with colitis. The treatment was to prevent exposing the baby to cow's milk. The mother was offered the option of following a milk-free diet or feeding the baby a hypoallergenic formula; she chose the latter and the infant tolerated this well. The rectal bleeding stopped and tests showed that the colon was healing well. It was noted that, in addition to the mother's allergy to milk, there was a family history of environmental allergies. Cow's milk allergy develops in 1 to 3 percent of infants, but almost always at a later age. Formulas that may be tolerated well by babies with cow's milk allergy include those based on soy protein, hydrolyzed whey or hydrolyzed casein. However, one in four infants who is allergic to cow's milk is also allergic to soy protein.
