Objective: To compare different techniques for carotid imaging including contrast-enhanced, unenhanced and dynamic techniques to find an alternative to contrast-enhanced MRA.Methods: 43 patients referred for imaging of the carotids were enrolled in this IRB-approved study. Imaging included dark-blood, time-of-flight, ECG-gated SSFP and dynamic and static contrast-enhanced MRA. Two radiologists evaluated all datasets in terms of image quality (vessel lumen, signal homogeneity, diagnostic confidence, preferred technique) on a four-point Likert-scale and in measuring the vessel area.Results: Of the 43 included patients the first 8 subjects served for protocol optimisation and 4 individuals discontinued the examination. Thus 31 datasets served for evaluation. CE-MRA revealed best results for delineation of vessel lumen, signal homogeneity and diagnostic confidence with values of 3.61, 3.42 and 3.77. It was also rated as the most preferred technique. SSFP-MRA was rated second in all categories with values of 3.1, 2.9 and 3.11. This unenhanced technique was the only one showing non-significantly different results in quantitative analysis.Conclusion: SSFP-MRA, an unenhanced form of MRA, represents an alternative to CE-MRA, particularly in patients where administration of gadolinium for CE-MRA may be contraindicated. OBJECTIVE: Compare white and African American caregivers of people with moderate to severe traumatic brain injury (TBI) regarding caregiving patterns, emotional function and life satisfaction, and preferred supports. DESIGN: Prospective, observational study; 1, 2, or 5 years post-TBI. SETTING: Six TBI model systems. Participants: Two hundred fifty-six caregivers (195 white and 61 African American). MEASURES: Brief Symptom Inventory-18, Satisfaction With Life Scale. RESULTS: Races differed as to kinship patterns, with more white caregivers including spouses and more African Americans including 'other relatives.' African Americans spent significantly more time in direct caregiving, and reported more depression. African American TBI survivors were significantly more disabled than whites, which appeared to account for emotional function differences. Whites were more likely to use professional services for emotional support. CONCLUSION: Across races, TBI caregiver emotional health is affected by the functional level of the survivor. African American caregivers may be at risk for worse emotional consequences due to worse survivor outcomes, yet may underutilize professional services. [ABSTRACT FROM AUTHOR]

In this review, I examine the social psychological research on ostracism, social exclusion, and rejection. Being ignored, excluded, and/or rejected signals a threat for which reflexive detection in the form of pain and distress is adaptive for survival. Brief ostracism episodes result in sadness and anger and threaten fundamental needs. Individuals then act to fortify or replenish their thwarted need or needs. Behavioral consequences appear to be split into two general categories: attempts to fortify relational needs (belonging, self-esteem, shared understanding, and trust), which lead generally to prosocial thoughts and behaviors, or attempts to fortify efficacy/existence needs of control and recognition that may be dealt with most efficiently through antisocial thoughts and behaviors. Available research on chronic exposure to ostracism appears to deplete coping resources, resulting in depression and helplessness. [ABSTRACT FROM AUTHOR]

The Central Arkansas Veterans Healthcare System (CAVHS) spends $1 million annually on occupational illnesses and injuries. To address the problem of injuries among hospital employees, a retrospective case-control study was conducted to examine select risk factors for work-related injuries (WRI) among CAVHS employees. Study methods included a review of employee health charts and computer and manual databases from 1997 to 2002 (N = 2,050). The researchers found that WRI increased with age; WRI occurred more often in women than in men; WRI was greater among maintenance and custodial staff compared to direct caregivers, and less among clerical staff; WRI occurred less often in part-time than full-time staff; and WRI increased with increasing body mass index. Developing standards, guidelines, and policies for preplacement screening, preventive measures, training, and education may help to minimize WRI and associated costs. [ABSTRACT FROM AUTHOR]

Ostracism-being ignored and excluded-is part of many individuals' daily lives. Yet, ostracism is often studied in laboratory settings and rarely in natural settings. Here, we report one of the first investigations into ostracism in everyday life by documenting how often and where ostracism occurs; who the sources of ostracism are; and how ostracism affects targets' feelings and behaviors. Two experience sampling studies using event-contingent ( N = 323, k = 1,107 ostracism experiences in 14 days) and time-signaling sampling approaches ( N = 272, k = 7,943 assessments including 767 ostracism experiences in 7 days) show that ostracism is an aversive experience that takes place in a range of contexts and relationships, as often as two to three times per week on average. Reconciling previously mixed findings regarding ostracism's effects on behavior and extending existing theory, we propose a novel framework of behavioral reactions based on need-threat levels: When psychological needs are severely threatened, individuals react to everyday ostracism with avoidance (i.e., withdrawal) and antisocial inclinations (i.e., they exhibit significantly stronger antisocial intentions, although they do not engage in antisocial behavior more frequently). Conversely, when psychological needs are threatened to a lesser extent, individuals are more likely to adopt approach behaviors (i.e., prosocial behavior, talking to others, or connecting with them on social media). Our findings considerably extend present theorizing in ostracism research as they allow to understand when and how individuals experience everyday ostracism and how behavioral reactions after ostracism form in real life. Although ghosting (i.e., unilaterally ending a relationship by ceasing communication) has only recently entered the lexicon, it is a regularly used form of relationship dissolution. However, little research has examined the emotional experiences of ghosting, particularly the experiences of those on both sides of the ghosting process. In a multi-method study, participants who had both ghosted and been ghosted in previous romantic relationships ( N = 80) provided narratives of their experiences and completed questionnaires. The narrative responses were analyzed by coders and by using LIWC. Ghosters and ghostees used similar overall levels of positively and negatively valenced words to describe their experiences, but ghosters were more likely to express guilt and relief, whereas ghostees were more likely to express sadness and hurt feelings. Ghostees also experienced more of a threat to their fundamental needs - control, self-esteem, belongingness, meaningful existence - than ghosters.

How pervasive is the power of human touch? Physical touch by other humans shows social support and alleviates stress - stress that may otherwise interfere with performance in athletic tasks. We argue that physical touch improves performance in highly stressful situations: free throws in basketball. In two studies (N total  = 60 NCAAW games, k total  = 835 free throw tandems), we assessed how often teammates touched the shooting player (e.g., a tap on the shoulder) in between shooting two free throws. We find that the extent of touch (i.e., being touched by 0, 1, 2, 3, or all 4 teammates) after the first free throw predicted success with the second free throw, but only when players missed the first free throw (integrated data analysis: main effect hand taps: b = 0.47, p = .021; main effect first free throw: b = 1.36, p = .019; interaction effect: b = -0.55, p = .024). We argue that this means that teammates' support expressed in physical touch helps particularly when stress levels are already high. Results are robust when controlling for players' skill level, home versus away games, point difference, and remaining playing time. More frequent touch was also tendentially associated with teams' season success (ACC-teams only, main effect of hand taps: b = -0.42, p = .062). Unlike one-time lab manipulations of exclusion, in real life, many people experience exclusion, from others and from groups, over extended periods, raising the question of whether individuals could, over time, develop hypo- or hypersensitive responses to chronic exclusion. In Study 1, we subjected participants to repeated experiences of inclusion or exclusion (three Cyberball games, time lag of three days, N = 194; 659 observations). We find that repeatedly excluded individuals become hypersensitive to inclusion, but not to exclusion. Study 2 ( N  = 183) tested whether individuals with chronic experiences of real-world exclusion show hypo- or hypersensitive responses to a novel episode of exclusion. In line with Study 1, exclusion hurt to the same extent regardless of baseline levels of chronic exclusion in daily life. However, chronically excluded individuals show more psychological distress in general. Thyrotropin-releasing hormone (TRH) and TRH-like peptides carry a therapeutic potential for neurological conditions. Nanoparticles (NP) made of the biodegradable polymer, Poly(Sebacic Anhydride) (PSA), have been developed to carry TRH, intended for intranasal administration to patients. There is limited information on the safety of biodegradable polymers when given intranasally, and therefore, we have performed two preclinical safety and toxicity studies in cynomolgus monkeys and rats using TRH-PSA nanoparticles. The rats and monkeys were dosed intranasally for 42 days or 28 days, respectively, and several animals were followed for additional 14 days. Animals received either placebo, vehicle (PSA), or different concentrations of TRH-PSA. No systemic adverse effects were seen. Changes in T3 or T4 concentrations were observed in some TRH-PSA-treated animals, which did not have clinical or microscopic correlates. No effect was seen on TSH or prolactin concentrations. In the monkey study, microscopic changes in the nasal turbinates were observed, which were attributed to incidental mechanical trauma caused during administration. Taken together, the TRH-loaded PSA NPs have proven to be safe, with no local or systemic adverse effects attributed to the drug loaded nanoparticles. These findings provide additional support to the growing evidence of the safety of peptide-loaded NPs for intranasal delivery and pave the way for future clinical trials in humans.

We used Cerner Real-World Data™, representing hospital admission records from 2020, to examine patients with co-occurring sickle cell disease and COVID-19 by discharge disposition grouped as death/hospice versus transfers to other facilities, returned home, or left against medical advice. Among the death/hospice group, we found older age and higher rates of congestive heart failure and diabetes. There were also significant differences in tachypnea, mechanical ventilation, minimum O 2 saturation, and length of stay with higher rates in the death/hospice group. In the United States, preparing researchers and practitioners for careers in cancer requires multiple components for success. In this reflection article, we discuss our approach to designing a comprehensive research training program in cancer disparities. We focused on elements that provide students and early career scientists a deep understanding of disparities through first-hand experiences and skills training necessary to build a research career in the area. Our Educational Program sits within the framework of an NCI P20 program, "UHAND (University of Houston/MD Anderson Cancer Center)", jointly established by an NCI-designated comprehensive cancer center and a minority-serving university as a collaborative partnership devoted to the elimination of cancer inequities among disproportionately affected racial and ethnic groups (UHAND Program to Reduce Cancer Disparities; NCI P20CA221696/ P20CA221697). The Education Program was designed to build on and enhance skills that are critical to pursuing a career in cancer disparities research at the undergraduate, doctoral, and post-doctoral levels-such as scientific communication, career planning and development, professional and community-based collaboration, and resilience in addition to solid scientific training. As such, our program integrates (1) opportunities for learning through service to community organizations providing resources to populations with documented cancer disparities, (2) a tailored curriculum of learning activities with program leadership and mentored research with scientists focused on cancer disparities and cancer prevention, (3) professional development training critical to career success in disparities research, and (4) support to address unique challenges faced by trainees from backgrounds that are historically underrepresented in research., (© 2022. Polycystic ovary syndrome (PCOS) is an endocrine disorder increasingly affecting women in the reproductive age group. The women usually present with menstruation irregularities, hirsutism, weight gain, and acne. There has been ongoing research about the increased risk of gynecological cancers in women with polycystic ovary syndrome compared to those without it. This review aimed to understand the risk of gynecological cancers, endometrial, ovarian, and breast cancer in PCOS, and to study in detail the underlying mechanisms involved. We searched PubMed and Google Scholar databases for studies and selected 10 articles from a total of 19,388 relevant articles. We found an increased risk of endometrial cancer in women with PCOS whereas the risk of ovarian and breast cancer was not increased. A recent study has even reported a reduced risk of ovarian cancer in genetically predicted PCOS. In understanding various medical conditions possibly leading to cancer in these women we found that hyperandrogenism, hyperinsulinemia, unopposed estrogen action, chronic inflammation, and dyslipidemia were major contributors. There is a need for more large-scale cohort studies which will take into consideration other factors leading to cancers in women with PCOS, such as smoking, alcohol, and family history, to substantiate the significance of these associations further. The interventions used to treat PCOS might also affect the risk of cancer and require further probing. Zika virus infection (ZIKV) was one of the most catastrophic epidemics. ZIKV in nonpregnant women is mild and sometimes asymptomatic. However, infection during pregnancy leads to congenital malformations in the fetus, while maternal signs of infection are preceded by a rash. The maternal-fetal infection begins with a rash that occurs early during pregnancy. The most severe pathologies were related to the first trimester of gestation, including microcephaly, musculoskeletal, genitourinary, craniofacial, ocular, and pulmonary manifestations. The prognosis may not be encouraging. Herd immunity increases CD8 + (cytotoxic T-lymphocytes) earlier and decreases in the resolution phase. However, CD4 + (T-helper cells) remains higher after infection. Recent ongoing vaccine development shows good immunity, control of the vector (Aedes mosquitoes), and treatment. ZIKV, anomalies, mortality, herd immunity, and vaccine were our main keywords. Ostracism-being ignored and excluded-has received considerable experimental research attention over the last 25 years. Ostracism signals social separation, isolation, and loss, and responses vary across time. Ostracism episodes as short as 2 min result in physiological pain responses, need threat, and emotional distress, followed by cognitive, emotional, motivational, and behavioral responses that either increase the likelihood of subsequent inclusion (at the cost of being socially pliable), or ensure further ostracism through aggression or solitude. Longer-term ostracism leads to resignation, accompanied by alienation, depression, helplessness, and feelings of unworthiness of attention by others. Parkinson's disease (PD) is a chronic neurodegenerative disease that is challenging to treat due to its progressive nature and its weaning response to therapy. Safinamide, a monoamine oxidase type-B inhibitor (MAOB-I), has shown promise in managing dyskinesias caused by levodopa (L-dopa), carbidopa, and PD features such as pain and depression. This systematic review aimed to evaluate safinamide's efficacy as a monotherapy and an add-on in tackling these issues. We composed this systematic review according to the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. Our group searched the following databases: Manchester University Library, ScienceDirect, Google Scholar, PubMed, PubMed Central, and MedLine for articles produced in the last ten years using various search terms and criteria, which we outlined in the search strategy and eligibility criteria sections. We excluded 722 out of the initially screened 730 records for multiple reasons, such as titles and abstracts being irrelevant to the topic, articles without free full access, articles originally not in the English language, and articles that did not score 70% or above on their respective quality assessment tools. Background: The emergence of COVID-19 exacerbated the existing epidemic of opioid use disorder (OUD) across the United States due to the disruption of in-person treatment and support services. Increased use of technology including telehealth and the development of new partnerships may facilitate coordinated treatment interventions that comprehensively address the health and well-being of individuals with OUD., Objective: The analysis of this pilot program aimed to determine the feasibility of delivering a COVID-19 telehealth care management program using SMS text messages for patients receiving OUD treatment., Methods: Eligible individuals were identified from a statewide opioid treatment program (OTP) network. Those who screened positive for COVID-19 symptoms were invited to connect to care management through a secure SMS text message that was compliant with Health Insurance Portability and Accountability Act standards. Care management monitoring for COVID-19 was provided for a period of up to 14 days. Monitoring services consisted of daily SMS text messages from the care manager inquiring about the participant's physical health in relation to COVID-19 symptoms by confirming their temperature, if the participant was feeling worse since the prior day, and if the participant was experiencing symptoms such as coughing or shortness of breath. If COVID-19 symptoms worsened during this observation period, the care manager was instructed to refer participants to the hospital for acute care services. The feasibility of the telehealth care management intervention was assessed by the rates of adoption in terms of program enrollment, engagement as measured by the number of SMS text message responses per participant, and retention in terms of the number of days participants remained in the program., Results: Between January and April 2021, OTP staff members referred 21 patients with COVID-19 symptoms, and 18 (82%) agreed to be contacted by a care manager. Participants ranged in age from 27 to 65 years and primarily identified as female (n=12, 67%) and White (n=15, 83%). The majority of participants were Medicaid recipients (n=14, 78%). There were no statistically significant differences in the demographic characteristics between those enrolled and not enrolled in the program. A total of 12 (67%) patients were enrolled in the program, with 2 (11%) opting out of SMS text message communication and choosing instead to speak with a care manager verbally by telephone. The remaining 10 participants answered a median of 7 (IQR 4-10) SMS text messages and were enrolled in the program for a median of 9 (IQR 7.5-12) days. No participants were referred for acute care services or hospitalized during program enrollment., Conclusions: These results demonstrate the feasibility of a novel telehealth intervention to monitor COVID-19 symptoms among OTP patients in treatment for OUD. Colorectal cancer (CRC) is a malignant condition of the colon and rectum. Generally, malignancies constitute a significant health threat to humans, and the result can be devastating. CRC is no exception. The gastrointestinal (GI) microbiome has long been suspected of impacting CRC. This review seeks to explore whether there is a connection between the two or not. For screening purposes, relevant articles were culled from various databases using key terms and phrases. Following a thorough search, the inclusion and exclusion criteria were applied, and a quality assessment was conducted. Most patients with sickle cell disease (SCD) seek hospital care because of pain symptoms. While some patients opt to treat themselves at home, some prefer to seek treatment in a hospital setting. There are, however, some patients with more complicated effects of the disease who seek treatment so often that they have been termed "super-users." This paper seeks to determine, across the board, the treatments available for vaso-occlusive crisis (VOC), the most common complication of SCD. Due to the frequency and unpredictable nature of VOC, it is no surprise that the lives of so many patients dealing with SCD are constantly disrupted by this complication.
