- Abstract
Introduction: The Covid-19 pandemic amplified access to care challenges faced by many priority populations, including First Nations and Inuit Peoples. During wave 1 of the pandemic in Canada, dental clinicians were mandated to postpone non-emergency dental services, and in many parts of the country, all dental operations ceased. Concerning this, the Non-Insured Health Benefits (NIHB) program approved “virtual dental consultation services” and decided to temporarily cover fees for consultation services by phone or other virtual methods to help assess clients’ needs and facilitate access to emergency dental care. The purpose of this study was to investigate the utilization of the virtual code by Canadian dentists during the beginning of the Covid-19 pandemic for children 18 years of age or younger. Methods: NIHB provided a secure data transfer portal for dental claims paid for the procedure “consultation with client for the emergency management and triage of client’s acute oral health care needs by phone or other virtual methods” and follow-up treatment with a date of service from April 1, 2020 to August 31, 2021. Data were analyzed using NCSS 2022 Statistical Software. Statistical analyses included frequencies, descriptive statistics, analysis of variance (ANOVA), and Chi-squared test. Results: Overall, 1040 patients 18 years of age or younger had a virtual visit with follow-up treatment between April 2020 and August 2021. The mean age was 10.5 ± 4.9 years and 54.3% were female. Most of the virtual consultations were completed by general practitioners 951 (91.4%) and pediatric dentists 86 (8.3%) and most initial virtual visits were completed in Quebec 358 (34.4%), Saskatchewan 266 (25.6%), Alberta 158 (15.2%), and Manitoba 120 (11.5%). More extractions were completed than any other type of dental treatment during the first visit following the initial consultation with a total of 417 extractions. The present research examines the impact of type of aggression (physical/ psychological) and type of dyad (male aggressor/ female victim and female aggressor/ male victim) on perceptions of a conflict scenario and its combatants. Participants read scenarios depicting a conflict between a married heterosexual couple and reported their impressions of the aggressiveness of the encounter and of the aggressor and victim. Physical aggression was evaluated more negatively (both in terms of the encounter and its combatants) than psychological aggression. Male to female violence was judged more harshly (both in terms of the aggressiveness of the encounter and impressions of the combatants) than female to male violence. Study 2 extended Study 1, assessing the relationship of experience with physical and psychological aggression on perceptions. The results from Study 1 were replicated. Contrary to predictions, experience with physical and psychological aggression did not consistently relate to perceptions of these types of aggression. Aims:\ud The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials.\ud \ud Methods and Results:\ud Adults with established HFrEF, New York Heart Association functional class (NYHA) ≥ II, EF ≤35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. GALACTIC‐HF randomized patients representative of recent HF registries and trials with substantial numbers of patients also having characteristics understudied in previous trials including more from North America (n = 1386), enrolled as inpatients (n = 2084), systolic blood pressure

Abstract Introduction Inotropes are associated with adverse outcomes in heart failure (HF), raising concern they may accelerate myocardial injury. Whether biomarkers of myocardial necrosis, inflammation and apoptosis change in response to acute milrinone administration is not well established. Methods Ten patients with severe HF and reduced cardiac output who were to receive milrinone were studied. Blood samples were taken just before initiation of milrinone and after 24 hours of infusion. Dosing was at the discretion of the patient's attending physician (range 0.25–0.5 mcg/kg/min). Plasma measurements of troponin, myoglobin, N-terminal-pro-BNP, interleukin-6, tumor necrosis factor-α, soluble Fas, and soluble Fas-ligand were performed at both time points. Results Troponin was elevated at baseline in all patients (mean 0.1259 ± 0.17 ng/ml), but there was no significant change after 24 hours of milrinone (mean 0.1345 ± 0.16 ng/ml, p = 0.44). There were significant improvements in interleukin-6, tumor necrosis factor-α, soluble Fas, and soluble Fas-ligand (all p < 0.05) indicative of reduced inflammatory and apoptotic signaling compared to baseline. Conclusion In conclusion, among patients with severe HF and low cardiac output, ongoing myocardial injury is common, and initiation of milrinone did not result in exacerbation of myocardial injury but instead was associated with salutary effects on other biomarkers.

- Abstract
Aims The objective of the Aliskiren Trial on Acute Heart Failure Outcomes (ASTRONAUT) was to determine whether aliskiren, a direct renin inhibitor, would improve post-discharge outcomes in patients with hospitalization for heart failure (HHF) with reduced ejection fraction. Pre-specified subgroup analyses suggested potential heterogeneity in post-discharge outcomes with aliskiren in patients with and without baseline diabetes mellitus (DM). Methods and results ASTRONAUT included 953 patients without DM (aliskiren 489; placebo 464) and 662 patients with DM (aliskiren 319; placebo 343) (as reported by study investigators). Study endpoints included the first occurrence of cardiovascular death or HHF within 6 and 12 months, all-cause death within 6 and 12 months, and change from baseline in N-terminal pro-B-type natriuretic peptide (NT-proBNP) at 1, 6, and 12 months. Data regarding risk of hyperkalaemia, renal impairment, and hypotension, and changes in additional serum biomarkers were collected. The effect of aliskiren on cardiovascular death or HHF within 6 months (primary endpoint) did not significantly differ by baseline DM status (P = 0.08 for interaction), but reached statistical significance at 12 months (non-DM: HR: 0.80, 95% CI: 0.64-0.99; DM: HR: 1.16, 95% CI: 0.91-1.47; P = 0.03 for interaction). Risk of 12-month all-cause death with aliskiren significantly differed by the presence of baseline DM (non-DM: HR: 0.69, 95% CI: 0.50-0.94; DM: HR: 1.64, 95% CI: 1.15-2.33; P < 0.01 for interaction). Among non-diabetics, aliskiren significantly reduced NT-proBNP through 6 months and plasma troponin I and aldosterone through 12 months, as compared to placebo. Among diabetic patients, aliskiren reduced plasma troponin I and aldosterone relative to placebo through 1 month only. There was a trend towards differing risk of post-baseline potassium ≥6 mmol/L with aliskiren by underlying DM status (non-DM: HR: 1.17, 95% CI: 0.71-1.93; DM: HR: 2.39, 95% CI: 1.30-4.42; P = 0.0

47. The effect of text message prompts on breast, cervical, and colorectal cancer screening rates among US women: a systematic review

- Abstract
Background: Screening rates for breast, cervical, and colorectal cancer for US women are below targeted goals established by Healthy People 2020. Evidenced-based screening interventions focus on effective communication to patients. In our modern society, text messaging has become a preferred mechanism of communication. Text messaging in healthcare has preliminarily been shown to have positive effects on patient behavior as related to appointment and medication adherence, as well as clinical outcomes of smoking cessation and weight loss. The objective of this study was to conduct a systematic review of the effect text message reminders on breast, cervical, and colorectal screening rates for US women. Additionally, this systematic review will assess intention and acceptability to screen for breast, cervical, and colorectal cancers among US women. Methods: We searched Medline, Web of Science, and CINAHL databases for studies assessing text message reminders on screening for breast, cervical and colorectal cancer. Two reviewers independently selected studies for inclusion based on pre-specified criteria. Trials and observational studies examining any of the following outcomes were eligible: effect of text message reminders on breast, cervical, and colorectal screening rates, effect of text message reminders on intentionality to screen for breast, cervical, or colorectal cancer, and acceptability of text message reminders to screen for breast, cervical, or colorectal cancer. Results: Four studies are included for analysis: one randomized control trial, one pre-post intervention study, and two cross-sectional surveys. The RCT and pre-post intervention study address the effect on text messaging on screening rates. The RCT reviewed is fair quality and the effect of the text message reminder cannot be discerned; the text message reminder is embedded in a multifaceted intervention. Additionally, the pre-post intervention has poor internal validity and outcomes for effect on screening rates are based on self-reported data. The two cross-sectional studies and the pre-post intervention study assessed acceptability; the range (37 percent to 83 percent) and inconsistent definitions of acceptability do not provide evidence for a conclusive assessment. The pre-post intervention was the only study that addressed intentionality. Again, a poor quality rating for this study and a low rate (24 percent) of intention do not provide enough evidence for an intentionality conclusion. Conclusion: Based on this systematic review it is not clear whether text message reminders effect screening rates for breast, cervical, or colorectal cancer screening for US women. It also is not clear if text message reminders effect intentionality or acceptability to screen for breast, cervical, or colorectal cancer among US women. The studies reviewed were limited and the majority of studies were of poor quality. Future research should consist of more RCTs with diverse sample populations with with respect to age, race/ethnicity, insurance status, and geographic setting. Furthermore, text message interventions need to be isolated and clearly defined as reminders versus educational.
