Fragestellung: Vorliegende Studie evaluiert die Langzeiteffekte eines vorschulischen Trainings in phonologischer Bewusstheit und in Buchstaben-Laut Korrespondenz. Methodik: Die Kinder der Experimental- und Kontrollgruppe der Ursprungsstudie ( Blaser, Preuss, Groner, Groner & Felder, 2007 ) wurden am Ende des 3. und 4. Schuljahres erneut mit Lese- und Rechtschreibtests nachuntersucht. Ergebnisse: Die Ergebnisse zeigen keine signifikanten Unterschiede zwischen der Experimental- und Kontrollgruppe in den durchschnittlichen Lese- und Rechtschreibleistungen. Trainingseffekte zeigen sich in einer Reduktion der Anzahl von Kindern mit Rechtschreibschwierigkeiten in der Trainingsgruppe. Schlussfolgerungen: Zusammenfassend werden die Ergebnisse als Bestätigung der Wirksamkeit des Trainings in der primären Prävention von Rechtschreibschwierigkeiten interpretiert. Trotz dieser positiven Ergebnisse bleibt ein kleiner Anteil an Kindern, die trotz vorschulischer Förderung später Lese- und Rechtschreibschwierigkeiten entwickeln. Auf diese Kinder sollte sich die Aufmerksamkeit der Forschung in Zukunft richten.

Objective Cognitive–perceptive ‘basic symptoms’ are used complementary to ultra-high-risk criteria in order to predict onset of psychosis in the pre-psychotic phase. The aim was to investigate the prevalence of a broad selection of ‘basic symptoms’ in a representative general adolescent population sample (GPS; N = 96) and to compare it with adolescents first admitted for early onset psychosis (EOP; N = 87) or non-psychotic psychiatric disorders (NP; N = 137). Methods Subjects were assessed with the Bonn Scale for the Assessment of Basic Symptoms (BSABS). Prevalence of at least one ‘basic symptom’ and mean numbers were compared across the three groups. Logistic regression was used to predict group membership by BSABS subscales; risk ratios were calculated to identify ‘basic symptoms’ which best discriminated between groups. Results The prevalence of at least any one ‘basic symptom’ was 30.2% in GPS compared to 81% in NP and 96.5% in EOP. Correct classification of EOP when compared to GPS was high (94.0%) and lower when compared to NP (78.6%). Cognitive symptoms discriminated best between EOP and NP. Conclusion Alike other prodromal- and psychotic-like experiences, ‘basic symptoms’ are prevalent in the general adolescent population, yet at a lower rate compared to EOP and NP. The usage of ‘at least one basic symptom’ as a screening criterion for youth at risk of developing a psychotic disorder is not recommended in the general population or in unselected psychiatrically ill adolescents. However, particularly cognitive ‘basic symptoms’ may be a valuable criteria to be included in future ‘at risk’ studies in adolescents.

Zusammenfassung: Fragestellung: Die vorliegende Studie hatte zum Ziel, die kurz, mittel- und längerfristige Wirksamkeit eines vorschulischen Trainings in phonologischer Bewusstheit und in Buchstaben-Laut-Korrespondenz, durchgeführt in Schweizer Mundart an einer Schweizer Stichprobe, an Hand der Outcomevariablen phonologische Bewusstheit und Lese- und Rechtschreibleistungen zu überprüfen. Methodik: Aus insgesamt 109 Kindern mit vollständigen Datensätzen über den Erhebungszeitraum wurde für die statistischen Analysen eine gepaarte Stichprobe aus je 26 Kindern aus Experimental- und Kontrollgruppe extrahiert. Es wurden einerseits Effekte des Trainings auf die kurz, mittel- und längerfristige Entwicklung der phonologischen Bewusstheit untersucht, andererseits die mittleren Lese- und Rechtschreibleistungen von Trainings- und Kontrollgruppe sowie der Anteil an lese- und rechtschreibschwachen Kindern in den jeweiligen Gruppen am Ende des 1. und 2. Schuljahres verglichen. Ergebnisse: Effekte des Trainings auf die phonologische Bewusstheit ließen sich bis zum Beginn des 1. Schuljahres nachweisen. Auf die mittleren Lese- und Rechtschreibleistungen am Ende beider Schuljahre zeigte das Training keinen Effekt, hingegen konnte der Anteil an rechtschreibschwachen Kindern durch das Training deutlich reduziert werden. Schlussfolgerungen: Insgesamt werden die Befunde als Bestätigung der Wirksamkeit des Trainings in der primären Prävention von Lese- und Rechtschreibschwierigkeiten gewertet.

The aim of this study was to assess patterns and correlates of family variables in 31 adolescents treated for their first episode of a schizophrenia spectrum disorder (early-onset schizophrenia [EOS]). Expressed emotion, perceived criticism, and rearing style were assessed. Potential correlates were patient psychopathology, premorbid adjustment, illness duration, quality of life (QoL), sociodemographic variables, patient and caregiver "illness concept," and caregiver personality traits and support. Families were rated as critical more frequently by patients than raters (55% vs. 13%). Perceived criticism was associated with worse QoL in relationship with parents and peers. An adverse rearing style was associated with a negative illness concept in patients, particularly with less trust in their physician. Future research should examine perceived criticism as a predictor of relapse and indicator of adolescents with EOS who need extended support and treatment. Rearing style should be carefully observed because of its link with patients' illness concept and, potentially, to service engagement and medication adherence.

A questionnaire on different aspects of divorce was given to 1380 adolescents from a commercial school in Berne. Subjects were between 15 and 22 years of age. The results reported in this study are based on the perceptions of the 245 adolescents who had separated or divorced parents, on visitation arrangements and factors that influenced parent-child contact. The rate of visits depended on the child's age at the time of separation and on the remarriage of one or both parents. The effects a new partnership had on the rate of visits were not directly comparable to the effects of a remarriage. Factors that may burden parent-child contact (such as interparental conflict, cancellations of visits, or lack of support from the custodial parent) correlated with the emotions the adolescent had before the visits. The present study intended to evaluate the short-, middle and long-term effects of preschool training in phonological awareness and letter-sound correspondence, conducted in Swiss dialect in Swiss kindergartens, on the basis of the outcome variables phonological awareness and reading and spelling competence.From a total sample of 109 children with complete datasets over all measurement points, a paired sample of 26 children each from the experimental and from the control group was selected for statistical analyses. Evaluated on the one hand were the short- middle and longer-term effects of the training on the development of the phonological awareness, while on the other, the average reading and spelling competencies and the percentage of reading-and-spelling disabled children in the training and in the control group were compared for the 1st and 2nd grades.Effects of the training on phonological awareness could be documented up to the beginning of the 1st grade. The average reading and spelling competencies did not differ be-tween the groups, whereas the percentage of spelling-disabled children was markedly reduced in the training group.As a whole, the results are seen as a confirmation of the efficacy of the training in the primary prevention of reading and writing disorders.

14. Course of Schizoaffective Psychoses: Results of a Followup Study

The course of illness was investigated retrospectively and prospectively in a sample of 150 hospitalized schizoaffective patients and 95 hospitalized bipolar manic-depressive patients. The two disorders showed more similarities than differences in their course; this was true for the age of onset (31.8 versus 34.7 years) and the length of illness until the end of the observation period (22.5 versus 23.8 years). Schizoaffective disorders took a more benign course than bipolar disorders, as measured by the frequency of the episodes (median 7 versus 9 episodes). Accordingly, periods of remission between episodes were longer in schizoaffective patients. The length of the episodes themselves was about the same in both disorders (median 3 months). Although subjects had reached an average age of 60 years, 63 percent of schizoaffecti ve and 82 percent of bipolar patients had experienced new episodes during the last 5 years of the observation period. Patients with schizoaffective psychoses were less likely to achieve a full remission than patients with bipolar disorders. A residual state was observed in 57 percent of schizoaffective and 24 percent of bipolar patients during the intervals between episodes. Because both schizoaffective and bipolar disorders showed a highly recurrent course, many patients received long-term treatment with neuroleptic drugs, antidepressants, or lithium.
