4. Pancreatic cancer with multiple liver metastasis complicating multi organ infarcts from Marantic endocarditis and Trousseau's syndrome.

Abstract

Background: Marantic endocarditis and Trousseau's syndrome are historically linked with pancreatic cancer. The patient had catastrophic embolic events which caused multi organ infarct due to underlying advanced pancreatic cancer. Doctors should be aware of early signs and symptoms of pancreatic cancer and conduct necessary clinical assessments and investigations that can prevent any severe complications. Case Presentation: A middle-aged healthy gentleman who presented with left-sided weakness, slurred speech, left-sided sensory neglect, mild headache, and fever. CT head showed acute right-sided temporal ischemic changes with subsequent magnetic resonance imaging (MRI) head showed multiple infarcts in the brain. A transesophageal echocardiogram reported possible MV vegetation. MRI of the liver was done due to deranged LFTs which showed multiple liver metastasis with a primary mass in the tail of the pancreas. Left leg ultrasound Doppler showed a large left leg deep vein thrombosis involving the femoral vein as he complained of left leg pain for 2 months and went to GP with right leg pain 3 months ago which was treated as a right ankle sprain. The clinical events explained that most likely he developed Trousseau's syndrome 3 months ago and the embolic phenomena were due to rare Marantic endocarditis (Non-bacterial thrombotic endocarditis) secondary to pancreatic cancer. Ultrasound-guided biopsy of liver metastasis was planned, but sadly the patient passed away the next day. Conclusion: The diagnosis of early pancreatic cancer with imaging poses a challenge sometimes, therefore the pancreatic protocol of CT or MRI is the first line of investigation. 5. Redox active molecules with molecular electronics and synthetic applications

Abstract

Redox active molecules are ubiquitous to nature and have properties that make them coveted targets for applications in areas of synthesis as well as for the development of materials. This thesis describes the synthesis and characterisation of several flavin donor-acceptor dyads designed around an oligothiophene donor backbone and a flavin acceptor moiety. These show potential applications as optoelectronic materials. It also describes the synthesis of a ferrocene-flavin tetracyanobutadiene super-acceptor compound which showed preliminary evidence of non-linear-optic effects. Finally, a novel method was developed to investigate the redox umpolung activated reactions of vinylferrocene. The vinyl group of vinylferrocene was activated by polarity inversion of ferrocene to ferrocenium and was able to undergo Diels-Alder cycloadditions with cyclobutadiene and furan, as well as, Markovnikov addition of thiols. These reactions were then used to explore the use of vinylferrocene as a redox auxiliary and as a redox active tag in polymers and have the potential to be used in nanoparticles as well as biological systems.

Abstract

Objective: Thiopurines are widely used as maintenance therapy in inflammatory bowel disease (IBD) but the evidence base for their use is sparse and their role increasingly questioned. Using the largest series reported to date, we assessed the long-term effectiveness of thiopurines in ulcerative colitis (UC) and Crohn’s disease (CD), including their impact on need for surgery. Design: Outcomes were assessed in 11 928 patients (4968 UC, 6960 CD) in the UK IBD BioResource initiated on thiopurine monotherapy with the intention of maintaining medically induced remission. Effectiveness was assessed retrospectively using patient-level data and a definition that required avoidance of escalation to biological therapy or surgery while on thiopurines. Analyses included overall effectiveness, time-to-event analysis for treatment escalation and comparison of surgery rates in patients tolerant or intolerant of thiopurines. Abstract

Organic hole-transporting materials (HTM) have shown excellent ability in achieving high efficiency perovskite solar cells. However, their high cost significantly limits their applications in scaling-up perovskite solar cells. Here, we report an efficient, low-cost HTM, called TAT-tBuSty, comprising a central triazatruxene moiety with 3 terminal tert-butoxy styrene groups and 3 hexyl side chains on the indole nitrogen. For the first time, we use an inexpensive liquid precur-sor to attach the donor ligand to the molecule. This enables purification of the HTM using a silica plug and recrystallization without the need for complicated column chromatography, which is typically required to purify solid-precursor-prepared HTMs. We estimate that the cost for the purified TAT-tBuSty is $69.90 g-1, which is 35% cheaper than the widely-used 2,2′7,7′-tetrakis-(N,N-di-p-methoxyphenyl-amine)-9,9′-spirobifluorene (spiro-OMeTAD; ca. $112.05 g-1). Using this low-cost TAT-tBuSty as the HTM in a caesium-formamidinium mixed-cation perovskite solar cell, we have successfully achieved a champion power conversion efficiency (PCE) of 20.3% (19.4% stabilized efficiency), which is comparable to the efficiency of devices using the reference spiro-OMeTAD HTM. Our work is crucial for up-scaling of next-generation high-performance metal-halide perovskite solar cells.

Abstract

We simply synthesize coplanar metal-free organic sensitizers featuring a thienylethynyl spacer, CSD-01 and CSD-02, from commercially available thiophene derivatives. The dye-sensitized solar cells (DSSCs) based on the CSD-01 and CSD-02 without any coadsorbate show the power conversion efficiencies (η) of 4.60% and 5.32%, respectively. The study in the dye-adsorption time suggests the increase in the organic DSSC performance with decreasing soaking time.

Abstract

Organic compounds displaying second-order non-linear optical properties are key materials for optoelectronics. Here, three push-pull flavin derivates (FLA-A-C) have been synthesised and their optical, redox and second harmonic generation (SHG) properties have been investigated. In particular, we describe the role differing electron donor units (N, N-dimethylaniline-, N, N-diphenylaniline- and ferrocenyl-) attached through the C(8) position of the flavin unit have on the optical and redox properties of the latter. Hyper-Rayleigh scattering in dioxane solutions using a fundamental wavelength of 1064 nm was employed to evaluate their second-order nonlinear optical properties. Our experiments have indicated that their first hyperpolarisabilities (beta) are influenced by the electronic nature and strength of the donor groups. N, N-dimethylaniline-functionalised derivative FLA-A, exhibited the largest first hyperpolarisability (beta = 9550 x 10(-30) esu, using the T convention) and high decomposition temperature (T-d = 312 degrees C), thus indicating its potential application as a useful material for incorporation into non-linear optical devices., GC thanks EPSRC (EP/E036244/1). Abstract

Redox active molecules are ubiquitous to nature and have properties that make them coveted targets for applications in areas of synthesis as well as for the development of materials. This thesis describes the synthesis and characterisation of several flavin donor-acceptor dyads designed around an oligothiophene donor backbone and a flavin acceptor moiety. These show potential applications as optoelectronic materials. It also describes the synthesis of a ferrocene-flavin tetracyanobutadiene super-acceptor compound which showed preliminary evidence of non-linear-optic effects. Finally, a novel method was developed to investigate the redox umpolung activated reactions of vinylferrocene. The vinyl group of vinylferrocene was activated by polarity inversion of ferrocene to ferrocenium and was able to undergo Diels-Alder cycloadditions with cyclobutadiene and furan, as well as, Markovnikov addition of thiols. These reactions were then used to explore the use of vinylferrocene as a redox auxiliary and as a redox active tag in polymers and have the potential to be used in nanoparticles as well as biological systems.
