The emergency nature of cSSTI hospitalizations requires starting immediate empiric intravenous (IV) antibiotic treatment, making the appropriate choice of initial antibiotic therapy crucial.Patients and Methods: The use of ceftaroline fosamil (CFT) as an alternative to other IV antibiotic therapies for the empiric treatment of hospitalized adults with cSSTI (vancomycin, linezolid, daptomycin, cloxacillin, tedizolid) was evaluated through cost consequences analysis. The model structure was a decision tree accounting for four different pathways: patients demonstrating early response (ER) either discharged early (with oral antibiotic) or remaining in hospital to continue the initial therapy; non-responders either remaining on the initial IV therapy or switching to a second-line antibiotic. The model perspective was the Spanish National Health System.Results: CFT resulted in average percentage of patients discharged early (PDE) of 24.6% (CI 19.49– 30.2%) with average total cost per patient of € 6763 (€ 6268–€ 7219). Vancomycin, linezolid, daptomycin and tedizolid resulted in average PDE of 22% (17.34– 27.09%), 26.4% (20.5– 32.32%), 28.6% (22.08– 35.79%) and 26.5% (20.39– 33.25%), respectively, for a total cost per patient of € 6,619 (€ 5,902–€ 6,929), € 6,394 (€ 5,881–€ 6,904), € 6,855 (€ 5,800–€ 7,410) and € 7,173 (€ 6,608–€ 7,763), respectively. Key model drivers were ER and antibiotic treatment duration, with hospital costs accounting for over 83% of the total expenditures.Conclusion: Given its clinical and safety profile, CFT is an acceptable choice for cSSTI empiric therapy providing comparable ER and costs to other relevant antibiotic options.Keywords: complicated skin and soft tissue infection, cost-consequences, Spain, ceftaroline fosamil

Abstract

Background: Sleep and circadian disruption (SCD) is common and severe in the ICU. On the basis of rigorous evidence in non-ICU populations and emerging evidence in ICU populations, SCD is likely to have a profound negative impact on patient outcomes. Thus, it is urgent that we establish research priorities to advance understanding of ICU SCD. Methods: We convened a multidisciplinary group with relevant expertise to participate in an American Thoracic Society Workshop. Workshop objectives included identifying ICU SCD subtopics of interest, key knowledge gaps, and research priorities. Members attended remote sessions from March to November 2021. Recorded presentations were prepared and viewed by members before Workshop sessions. Workshop discussion focused on key gaps and related research priorities. The priorities listed herein were selected on the basis of rank as established by a series of anonymous surveys. Results: We identified the following research priorities: establish an ICU SCD definition, further develop rigorous and feasible ICU SCD measures, test associations between ICU SCD domains and outcomes, promote the inclusion of mechanistic and patient-centered outcomes within large clinical studies, leverage implementation science strategies to maximize intervention fidelity and sustainability, and collaborate among investigators to harmonize methods and promote multisite investigation. Conclusions: ICU SCD is a complex and compelling potential target for improving ICU outcomes. Given the influence on all other research priorities, further development of rigorous, feasible ICU SCD measurement is a key next step in advancing the field.

5. Preschoolers with Developmental Speech and/or Language Impairment: Efficacy of the Teaching Early Literacy and Language (TELL) Curriculum

Abstract

Problem/Purpose: Young children with developmental speech and/or language impairment (DSLI) often fail to develop important oral language and early literacy skills that are foundational for subsequent schooling and reading success. The purpose of this investigation was to examine the efficacy of the TELL curriculum and associated evidence-based teaching practices in promoting the acquisition of oral language and early literacy skills for preschool children with DSLI. Participants: Participants included 202 male and 87 female preschoolers with DSLI in the absence of other developmental impairment. Children ranged in age from 43 to 63 months. They were enrolled in 91 inclusive preschool classes and their corresponding classroom teachers were all female. Method: In this cluster RCT, classroom teachers were randomly assigned to implement the TELL curriculum or to continue with their business-as-usual (BAU) curriculum. Proximal outcomes were assessed with investigator-developed curriculum-based measures (CBM) administered six times over the school year and an investigator-developed assessment of vocabulary targeted in TELL. Standardized tests of oral language ("Clinical Evaluation of Language Fundamentals Preschool - 2nd Edition"), and early literacy skills ("Test of Preschool Early Literacy"), and a benchmarked early literacy assessment ("Phonological Awareness and Literacy Screening PreK") were administered at the beginning and end of the school year to determine impact on more distal outcomes. Results: Results indicated a significant TELL effect for all CBMs at later measurement points with Cohen's "ds" in the medium (0.43) to very large (1.25) range. TELL effects were also noted for the vocabulary measures with small to medium between-group effect sizes (Cohen's f^2 range from 0.02 to 0.44). There were no significant TELL effects for the more distal measures. Conclusion: Based on progress measures, the TELL curriculum was effective for improving the oral language and early literacy skills of young children with DSLI. 9. Do Family and Peer Academic Social Supports Predict Academic Motivations and Achievement of First-Year College Students?

Abstract

Purpose: This study examines family and peer academic social supports as correlates of academic motivation and first-semester GPA. Design/methodology/approach: Using a correlational design, 468 first-semester college students completed an online survey. Demographic characteristics, high school GPA and first-semester GPA were gathered from institutional data and linked to student survey responses. Findings: Bivariate and multiple regression analyses revealed that family and peer academic social supports are predictive of academic motivation and first-semester GPA. The relationships identified were small- to medium-sized and of theoretical and substantive interest. Research limitations/implications: The main limitations of the study are the measures are self-reported and the study is correlational, the latter limiting the strength of causal inferences. However, the study provides further understanding of the importance of college students' family and peer social supports in relationship to academic motivation and achievement. Practical implications: The research has practical implications for higher educators developing programs to improve family and peer social support. If future research establishes causal relationships, interventions to enhance family and peer academic supports may prove beneficial in promoting academic motivation. Further, encouraging families to broadly discuss academic topics may be ineffective in terms of academic self-efficacy and achievement for historically underrepresented students. Originality/value: This study makes a unique contribution to the literature by establishing relationships between family and peer supports with academic motivation. Statistical interactions between family and peer supports and with demographic characteristics in predicting academic motivation were identified. If the interactions are replicable, the findings provide avenues for future correlational and intervention research.

Abstract

Purpose: The purpose of this study was to test the efficacy of the Teaching Early Literacy and Language (TELL) curriculum package for improving the early literacy and oral language skills of preschoolers from low-income families. Method: In a randomized controlled trial (RCT), TELL was implemented in 30 TELL and 28 business-as-usual classrooms. TELL is a Tier 1 whole-class curriculum package that includes a scope and sequence of instruction, lesson plans and materials, high-quality books, curriculum-based measures (CBMs), and professional development training and coaching. Results: Implementation fidelity was high; however, due to COVID-19 school closures, we did not collect end-of-year data for our third cohort of teachers. Results indicated significant TELL effects on all code-related CBMs and two code-related distal measures as well as receptive and expressive vocabulary CBMs. No significant differences were found on distal measures of oral language/vocabulary or listening comprehension. Conclusions: Results of the present study, paired with findings from an earlier TELL RCT with children with developmental speech and/or language impairments, suggest that children enrolled in TELL classrooms achieve higher outcomes on a variety of code-related and vocabulary measures by the end of their final preschool year. Abstract

The Positive and Negative Syndrome Scale (PANSS) is frequently described with five latent factors, yet published factor models consistently fail to replicate across samples and related disorders. We hypothesize that (1) a subset of the PANSS, instead of the entire PANSS scale, would produce the most replicable five-factor models across samples, and that (2) the PANSS factor structure may be different depending on the treatment phase, influenced by the responsiveness of the positive symptoms to treatment. Using exploratory factor analysis, confirmatory factor analysis and cross validation on baseline and post-treatment observations from 3647 schizophrenia patients, we show that five-factor models fit best across samples when substantial subsets of the PANSS items are removed. The optimal model at baseline (five factors) omits 12 items: Motor Retardation, Grandiosity, Somatic Concern, Lack of Judgment and Insight, Difficulty in Abstract Thinking, Mannerisms and Posturing, Disturbance of Volition, Preoccupation, Disorientation, Excitement, Guilt Feelings and Depression. The PANSS factor models fit differently before and after patients have been treated. Patients with larger treatment response in positive symptoms have larger variations in factor structure across treatment stage than the less responsive patients. Negative symptom scores better predict the positive symptoms scores after treatment than before treatment. We conclude that sparse factor models replicate better on new samples, and the underlying disease structure of Schizophrenia changes upon treatment.

Abstract

We consider the detector size, location, depth, background, and radio-purity required of a mid-Pacific deep-ocean instrument to accomplish the twin goals of making a definitive measurement of the electron anti-neutrino flux due to uranium and thorium decays from Earth's mantle and core, and of testing the hypothesis for a natural nuclear reactor at the core of Earth. We take the experience with the KamLAND detector in Japan as our baseline for sensitivity and background estimates. We conclude that an instrument adequate to accomplish these tasks should have an exposure of at least 10 kilotonne-years (kT-y), should be placed at least at 4 km depth, may be located close to the Hawaiian Islands (no significant background from them), and should aim for KamLAND radio-purity levels, except for radon where it should be improved by a factor of at least 100. With an exposure of 10 kT-y we should achieve a 24% measurement of the U/Th content of the mantle plus core. Designed for early childhood educators working with children who have a wide variety of disabilities, this manual discusses the ways in which assistive technology can facilitate the child's active participation in developmentally appropriate activities and offers ideas for incorporating devices and strategies into daily school and home routines. Chapters 1-5 provide an opportunity to explore and review the foundations of assistive technology including relevance to real life tasks, legislative mandates which have created the framework for accessible technology, services to people with disabilities, and the importance of a team-based technology selection and service delivery process. Specific assistive technologies for positioning, access, communication, play, and computer use that are available to enhance a child's independence and skill development are introduced. Chapters 6, 7, and 8 focus on implementation issues: curricular issues that foster and support a "big picture" approach to assistive technology use in educational settings; the relationship between the Individualized Education Program process, technology implementation, and functional outcomes; and funding options. 35. Multiple understandings of executive coaching : an exploratory study of Irish experiences

This research study explores executives’ perceptions of factors that impede and facilitate the effectiveness of a coaching intervention designed to enhance leadership skills. It also explores research participants’ perceptions of the effectiveness of the intervention. Because research studies have rarely given prominence to the voice of the executive (Kilburg, 2004, Lowman, 2005, Turner, 2006; Styhre, 2008), this aspect of coaching is still largely unexplored, thus the purpose of the research is to bring the voice of the executives to the fore via an instrumental case study whose focus is the experiences of the executives. A large indigenous Irish company facilitated the research, which took place in Dublin. The research design is a series of in-depth, semi-structured interviews with a cohort of four executives who had engaged in a pilot coaching programme, with the HR Director who initiated the coaching intervention, with the Coach, and with the Divisional CEO whose budget paid for the coaching. The executives’ stories are told via a ‘montage’ of rich descriptions of their views on the organisation, their leader, and their coaching experiences. The views of the other players, (HR Director, Coach and CEO) are presented independently. A review of current literature on executive coaching discusses recent research studies and notes the dominance of North American research and the dearth of studies that address executives’ perspectives. The literature reviews salient inputs to the coaching process: the role of the organisation, the skill sets of the coach, and the readiness of executives to be coached. The research found that coaching yielded a number of positive outcomes for all executives, although perceptions of gains varied across all research participants; the most significant evaluation discrepancy was between the HR Director and the CEO. While the Coach had many strengths, which were acknowledged by all executives, his description of his ‘structured’ approach was at variance with that experienced by the executives, some of whom were frustrated by lack of continuity and by his failure to measure progress. The CEO, who had also taken part in the coaching programme, was particularly frustrated by what he saw as the lack of engagement by the Coach. Neither the executives nor the CEO challenged the Coach to change his approach. A key finding was that neither the Coach nor the Organisation (as represented by the HR Director and the CEO) managed the process to produce a satisfactory results oriented experience for all the executives. The findings from this research study inform a conceptual framework that highlights the facilitators and inhibitors of executive coaching as articulated by the stakeholders to the coaching programme. The findings have practical implications for coaches, HR professionals and executives on how to behave in a coaching situation and the study adds to the body of knowledge on what facilitates and hinders the success of executive coaching and the factors that influence executives’ evaluation of the coaching process.

We discuss new photometric data collected on the gamma Dor variables HD 224945 and HD 224638. Multiperiodicity was detected in both stars, thanks to the clear spectral window of a multisite campaign that involved five observatories. HD 224945 shows the shortest period among the gamma Dor stars, i.e., 0.3330 d. The pulsation behaviour is very different: HD 224945 displays a set of frequencies spread over an interval much wider than that of HD 224638. We clearly found evidence for amplitude variations in the excited modes by comparing data from different years. HD 224945 and HD 224638 are among the best examples of $\gamma$ Dor stars that show multimode pulsations, which make them very interesting from an asteroseismological point of view., Comment: 8 pages (in A&A style), 3 eps figures. Abstract

We report the possible identification of weak methane spectral features in the near-infrared K band in two late L dwarfs, DENIS 0205-11 (L7) and 2MASS 1523+30 (L8). New, high signal-to-noise ratio flux-calibrated spectra, spanning the wavelength interval 1.10 - 2.35 microns with an average resolving power R = 1800 were obtained using NIRSPEC on the Keck II telescope. Results are reported and compared for three late L dwarfs (L5, L7, and L8) and two T dwarfs (T1 and T6). The spectra, which are continuous through the atmospheric absorption bands, show the development of deep steam bands and the weakening of iron hydride features through the L dwarfs and the emergence of strong methane bands in the T dwarfs. A detailed comparison of the K band regions with synthetic spectra suggests that the weak features seen in the L7 and L8 dwarfs at 2.20 microns are likely to be methane. We see no evidence for methane in the H band. At the R = 1800 resolution, significant differences are evident between the spectral signatures of the L8 and the T1, leaving room for additional transition objects (L9 or T0)., Comment: Accepted for publication in the Astrophysical Journal Letters

Abstract

Measurements of [OIII] emission in Lyman Break galaxies (LBGs) at z>3 are presented. Four galaxies were observed with narrow-band filters using the Near-IR Camera on the Keck I 10-m telescope. A fifth galaxy was observed spectroscopically during the commissioning of NIRSPEC, the new infrared spectrometer on Keck II. The emission-line spectrum is used to place limits on the metallicity. Comparing these new measurements with others available from the literature, we find that strong oxygen emission in LBGs may suggest sub-solar metallicity for these objects. The [OIII]5007 line is also used to estimate the star formation rate (SFR) of the LBGs. The inferred SFRs are higher than those estimated from the UV continuum, and may be evidence for dust extinction., Comment: 25 pages, including 6 figures. Accepted for publication in ApJ

Abstract

Moderate resolution, near-IR spectroscopy of MS1512-cB58 is presented, obtained during commissioning of the the Near IR Spectrometer (NIRSPEC) on the Keck II telescope. The strong lensing of this z=2.72 galaxy by the foreground cluster MS1512+36 makes it the best candidate for detailed study of the rest-frame optical properties of Lyman Break Galaxies. A redshift of z=2.7290+/-0.0007 is inferred from the emission lines, in contrast to the z=2.7233 calculated from UV observations of interstellar absorption lines. Using the Balmer line ratios, we find an extinction of E(B-V)=0.27. Using the line strengths, we infer an SFR=620+/-18 Msun/yr (H_0=75, q_0=0.1, Lambda =0), a factor of 2 higher than that measured from narrow-band imaging observations of the galaxy, but a factor of almost 4 lower than the SFR inferred from the UV continuum luminosity. The width of the Balmer lines yields a mass of M_vir=1.2x10^10 Msun. We find that the oxygen abundance is 1/3 solar, in good agreement with other estimates of the metallicity. However, we infer a high nitrogen abundance, which may argue for the presence of an older stellar population., Comment: 14 pages, including 3 figures. Accepted for publication in ApJ Letters

We imaged the star clusters in the vicinity of the southern nucleus (NGC 4039) in 0.39" seeing in K-band using NIRSPEC's slit-viewing camera. The brightest star cluster revealed in the near-IR (M_K(0) = -17.9) is insignificant optically, but coincident with the highest surface brightness peak in the mid-IR (12-18 um) ISO image presented by Mirabel et al (1998). We obtained high signal-to-noise 2.03-2.45 um spectra of the nucleus and the obscured star cluster at R = 1900. The cluster is very young (age ~ 4 Myr), massive (M ~ 16E6 M_sun), and compact (density ~ 115 M_sun pc^(-3) within a 32 pc half-light radius), assuming a Salpeter IMF (0.1-100 M_sun). Its hot stars have a radiation field characterized by T_eff ~ 39,000 K, and they ionize a compact HII region with n_e ~ 10^4 cm^(-3). Abstract

This paper presents infrared spectra taken with the newly commissioned NIRSPEC spectrograph on the Keck Telescope of the High Redshift Radio Galaxy MRC 2025-218 (z=2.630) and an extremely red galaxy (R-K > 6 mag) 9'' away. These observations represent the deepest infrared spectra of a radio galaxy to date and have allowed for the detection of Hbeta, OIII (4959/5007), OI (6300), Halpha, NII (6548/6583) and SII (6716/6713). The Halpha emission is very broad (FWHM~6000 km/s) and strongly supports AGN unification models linking radio galaxies and quasars. The line ratios are most consistent with a partially obscured nuclear region and very high excitation. The OIII (5007) line is extended several arcseconds and shows high velocity clouds in the extended emission. The nucleus also appears spectrally double and we argue that the radio galaxy is undergoing a violent merger process. The red galaxy, by comparison, is very featureless even though we have a good continuum detection in the H and K bands.
