Introduction: This study explored treatment of executive functioning impairment in adults after acquired brain injury (ABI), clinician's confidence and support received, in a sample of occupational therapists in the United Kingdom. Methods: A 24-item online questionnaire was sent to 750 members of the Royal College of Occupational Therapists Specialist-Section in Neurological Practice. Data was collected at a nominal and ordinal level and included yes/no Likert-type scale and free field comments. Descriptive statistical analysis was completed. Results: Seventy-six occupational therapists working in a range of neurological settings completed the survey. Frequently used interventions included education (n = 57, 75%), task modification (n = 56, 73%), instrumental activities of daily living (ADL) (n = 54, 71%), personal ADL (n = 53, 70%) and goal setting (n = 53, 70%). Seventy-one percent used metacognitive strategies. Occupation-based metacognitive approaches were rarely used. Thirty-one (41%) participants reported being fairly confident and 28 (37%) were very confident. Support for clinical practice was accessed most frequently through joint clinical sessions (n = 30, 39%). Findings: Participants used various clinical interventions to treat service users with executive dysfunction after ABI. Meta-cognitive strategies were employed; however, occupation-based metacognitive approaches were infrequently utilised, suggesting uncertainty in adopting these in practice. 14. Defining falls risk factors in older adults with cognitive impairment living in residential care

Introduction Falls are common among older people and those with dementia are at higher risk of falls and injury. In cognitively intact older people, interventions targeted at specific risk factors have proved effective at reducing falls. There is limited evidence of effective interventions for those with dementia. In order to design effective interventions for those with dementia, better understanding of risk factors is required. The aim of this study was to define falls risk factors in older people with cognitive impairment. Methods Participants from residential care homes underwent baseline collection of demographics and medical factors, sensorimotor, balance and gait function, behavioural and psychiatric symptoms and neuropsychological performance and were followed up for 6 months to determine faller status. Data were analysed for differences between fallers and non-fallers. Results Study 1 collected readily available data from 240 residential care dwellers and found 7 independent predictors of falls which were used to develop a falls risk assessment tool. These were: use of a walking frame, poor standing balance, mini mental state examination <17, use of antidepressants or hypnotic/anxiolytics, previous falls and impulsivity. Study 2 collected detailed baseline data from 109 residential care dwellers and found 4 independent predictors of falls which were anxiety, impaired postural sway with eyes closed, poor attention and concentration and use of antidepressants. New scales to measure physical activity and impulsivity in this population were validated and used as baseline variables. A further test to measure judgement of balance abilities was developed but did not predict faller status. Conclusion Two new valid and reliable scales to measure impulsivity and physical activity were developed as well as a falls risk screening tool that accurately identified those at high risk of falls. Detailed data collection identified risk factors amenable to intervention. Targeted interventions are described in the discussion.

Background: Ageing populations globally have contributed to increasing numbers of people living with frailty, which has significant implications for use of health and care services and costs. The British Geriatrics Society defines frailty as "a distinctive health state related to the ageing process in which multiple body systems gradually lose their inbuilt reserves". This leads to an increased susceptibility to adverse outcomes, such as reduced physical function, poorer quality of life, hospital admissions, and mortality. Case management interventions delivered in community settings are led by a health or social care professional, supported by a multidisciplinary team, and focus on the planning, provision, and co-ordination of care to meet the needs of the individual. Case management is one model of integrated care that has gained traction with policymakers to improve outcomes for populations at high risk of decline in health and well-being. These populations include older people living with frailty, who commonly have complex healthcare and social care needs but can experience poorly co-ordinated care due to fragmented care systems.Objectives: To assess the effects of case management for integrated care of older people living with frailty compared with usual care.Search methods: We searched CENTRAL, MEDLINE, Embase, CINAHL, Health Systems Evidence, and PDQ Evidence and databases from inception to 23 September 2022. We also searched clinical registries and relevant grey literature databases, checked references of included trials and relevant systematic reviews, conducted citation searching of included trials, and contacted topic experts.Selection criteria: We included randomised controlled trials (RCTs) that compared case management with standard care in community-dwelling people aged 65 years and older living with frailty.Data collection and analysis: We followed standard methodological procedures recommended by Cochrane and the Effective Practice and Organisation of Care Group. We used the GRADE approach to assess the certainty of the evidence.Main results: We included 20 trials (11,860 participants), all of which took place in high-income countries. Case management interventions in the included trials varied in terms of organisation, delivery, setting, and care providers involved. Most trials included a variety of healthcare and social care professionals, including nurse practitioners, allied healthcare professionals, social workers, geriatricians, physicians, psychologists, and clinical pharmacists. In nine trials, the case management intervention was delivered by nurses only. Follow-up ranged from three to 36 months. We judged most trials at unclear risk of selection and performance bias; this consideration, together with indirectness, justified downgrading the certainty of the evidence to low or moderate. Case management compared to standard care may result in little or no difference in the following outcomes. • Mortality at 12 months' follow-up (7.0% in the intervention group versus 7.5% in the control group; risk ratio (RR) 0.98, 95% confidence interval (CI) 0.84 to 1.15; I2 = 11%; 14 trials, 9924 participants; low-certainty evidence) • Change in place of residence to a nursing home at 12 months' follow-up (9.9% in the intervention group versus 13.4% in the control group; RR 0.73, 95% CI 0.53 to 1.01; I2 = 0%; 4 trials, 1108 participants; low-certainty evidence) • Quality of life at three to 24 months' follow-up (results not pooled; mean differences (MDs) ranged from -6.32 points (95% CI -11.04 to -1.59) to 6.1 points (95% CI -3.92 to 16.12) when reported; 11 trials, 9284 participants; low-certainty evidence) • Serious adverse effects at 12 to 24 months' follow-up (results not pooled; 2 trials, 592 participants; low-certainty evidence) • Change in physical function at three to 24 months' follow-up (results not pooled; MDs ranged from -0.12 points (95% CI -0.93 to 0.68) to 3.4 points (95% CI -2.35 to 9.15) when reported; 16 trials, 10,652 participants; low-certainty evidence) Case management compared to standard care probably results in little or no difference in the following outcomes. • Healthcare utilisation in terms of hospital admission at 12 months' follow-up (32.7% in the intervention group versus 36.0% in the control group; RR 0.91, 95% CI 0.79 to 1.05; I2 = 43%; 6 trials, 2424 participants; moderate-certainty evidence) • Change in costs at six to 36 months' follow-up (results not pooled; 14 trials, 8486 participants; moderate-certainty evidence), which usually included healthcare service costs, intervention costs, and other costs such as informal care.Authors' conclusions: We found uncertain evidence regarding whether case management for integrated care of older people with frailty in community settings, compared to standard care, improved patient and service outcomes or reduced costs. There is a need for further research to develop a clear taxonomy of intervention components, to determine the active ingredients that work in case management interventions, and identify how such interventions benefit some people and not others.

Researchers face a challenge when evaluating the effectiveness of rehabilitation after a surgical procedure for hip fracture. Reported outcomes of rehabilitation will vary depending on the end point of the episode of care. Evaluation at an inappropriate end point might suggest a lack of effectiveness leading to the underuse of rehabilitation that could improve outcomes. The purpose of this article is to describe a conceptual framework for a continuum-care episode of rehabilitation after a surgical procedure for hip fracture. Definitions are proposed for the index event, end point, and service scope of the episode. Challenges in defining the episode of care and operationalizing the episode, and next steps for researchers are discussed. The episode described is intended to apply to all patients eligible for entry to rehabilitation after hip fracture and includes most functional recovery end points. This framework will provide a guide for rehabilitation researchers when designing and interpreting evaluations of the effectiveness of rehabilitation after hip fracture. - Abstract
Background: The scale of COVID-19 and its well documented long-term sequelae support a need to understand long-term outcomes including frailty. Methods: This prospective cohort study recruited adults who had survived hospitalisation with clinically diagnosed COVID-19 across 35 sites in the UK (PHOSP-COVID). The burden of frailty was objectively measured using Fried’s Frailty Phenotype (FFP). The primary outcome was the prevalence of each FFP group; robust (no FFP criteria), pre-frail (one or two FFP criteria) and frail (three or more FFP criteria) at 5 months and 1 year after discharge from hospital. For inclusion in the primary analysis, participants required complete outcome data for three of the five FFP criteria. Longitudinal changes across frailty domains are reported at 5 months and 1 year post-hospitalisation, along with risk factors for frailty status. Patient-perceived recovery and health-related quality of life (HRQoL) were retrospectively rated for pre-COVID-19 and prospectively rated at the 5 month and 1 year visits. This study is registered with ISRCTN, number ISRCTN10980107.Findings: Between March 5,2020 and March 31, 2021, 2,419 participants were enrolled with FFP data. Mean age was 57·9 (SD 12·6) years, 933 (38·6%) were female, and 429 (17·7%) had received invasive mechanical ventilation (IMV). 1,785 had measures at both timepoints, of which 240 (13·4%), 1138 (63·8%) and 407 (22·8%) were frail, pre-frail and robust respectively at 5 months compared with 123 (6·9%), 1,046 (58·6%) and 616 (34·5%) at 1 year. Factors associated with pre-frailty or frailty were IMV, older age, female sex, and greater social deprivation. Frail participants had a larger reduction in HRQoL compared with before their COVID-19 illness and were less likely to describe themselves as recovered.Interpretation: Physical frailty and pre-frailty are common following hospitalisation with COVID-19. Improvement in frailty was seen between five and 12 months although two-thirds of the population remained pre-frail or frail. This suggests comprehensive assessment and interventions targeting pre-frailty and frailty beyond the initial illness are required.

Introduction Increased mortality has been demonstrated in older adults with coronavirus disease 2019 (COVID-19), but the effect of frailty has been unclear. Methods This multi-centre cohort study involved patients aged 18 years and older hospitalised with COVID-19, using routinely collected data. We used Cox regression analysis to assess the impact of age, frailty and delirium on the risk of inpatient mortality, adjusting for sex, illness severity, inflammation and co-morbidities. We used ordinal logistic regression analysis to assess the impact of age, Clinical Frailty Scale (CFS) and delirium on risk of increased care requirements on discharge, adjusting for the same variables. Results Data from 5,711 patients from 55 hospitals in 12 countries were included (median age 74, interquartile range [IQR] 54–83; 55.2% male). The risk of death increased independently with increasing age (>80 versus 18–49: hazard ratio [HR] 3.57, confidence interval [CI] 2.54–5.02), frailty (CFS 8 versus 1–3: HR 3.03, CI 2.29–4.00) inflammation, renal disease, cardiovascular disease and cancer, but not delirium. Age, frailty (CFS 7 versus 1–3: odds ratio 7.00, CI 5.27–9.32), delirium, dementia and mental health diagnoses were all associated with increased risk of higher care needs on discharge. The likelihood of adverse outcomes increased across all grades of CFS from 4 to 9. Conclusion Age and frailty are independently associated with adverse outcomes in COVID-19. Risk of increased care needs was also increased in survivors of COVID-19 with frailty or older age.

- Abstract
Denne masteravhandlingen er en empirisk studie av norske unoterte selskaper der det undersøkes hvorvidt bytte av revisjonsselskap har betydning for regnskapsmanipulasjon. I studien defineres EY, PwC, KPMG, Deloitte og BDO som Big5, og øvrige revisjonsselskaper som ikke-Big5. Studiens fokus er bytte mellom ulike typer revisjonsselskap, samt bytter som skjer mellom samme type revisjonsselskap. Nærmere bestemt skal følgende problemstilling besvares: «Hvilken sammenheng er det mellom regnskapsmanipulasjon og bytte mellom ulike revisjonsselskaper? Og hvilken betydning har soliditet, endring i størrelse på revisjonshonorar og selskapsstørrelse for tendensen til å bedrive regnskapsmanipulasjon?» Studien tar for seg et utvalg på 440 revisorbytter i perioden 2014-2018. Ved måling av indikasjon på regnskapskvalitet benyttes M-Score modellen til Beneish (1999). Dette på grunnlag av modellens sammenheng, som hensyntar resultatregnskap, balanse og kontantstrøm, samtidig som forholdet mellom ulike nøkkeltall i regnskapet kommer til syne. Funnene i studien indikerer en sammenheng mellom regnskapsmanipulasjon og bytte mellom Big5 revisjonsselskap. Derimot fremkommer det ingen regnskapsmessige kvalitetsforskjeller ved bytter mellom øvrige revisjonsselskap. Dette kan bety at det ikke er noen vesentlig forskjell mellom de ulike typer revisjonsselskapene og hvorvidt de er tilbøyelig til å godta regnskap av lavere kvalitet. Selskapets soliditet har ingen effekt på indikasjon av regnskapsmanipulasjon, noe som er et interessant funn, da det står i motsetning til tidligere internasjonal forskning (Gul et al., 2018; Kluger & Shields, 1989). Studien fant at endring i revisjonshonorarstørrelse har betydning for tendensen til å bedrive regnskapsmanipulasjon. Samtidig viser studien at lavere tendenser til regnskapsmanipulasjon ved bytter mellom revisjonsselskaper fører til høyere revisjonshonorar. Avslutningsvis viser funnene en lavere tendens til å bedrive regnskapsmanipulasjon hos de større selskapene. This master's thesis is an empirical study of Norwegian unlisted companies where we investigate whether a change of audit firm has an impact on earnings management. In the thesis audit firms such as Deloitte, KPMG, EY, PwC and BDO are defined as Big5, and other audit firms as non-Big5. The study focuses on audit changes between different types of audit firms, as well as changes between the same type of audit firms. More specifically, the research question for this study is: “What is the relation between earnings managment and switching between different audit firms? And what significance does solvency, change in the size of audit fees and company size have for the tendency to engage in earnings management?” The study includes a sample of 440 auditor changes in the period 2014-2018. When measuring the indication of earnings management, the M-Score model by Beneish (1999) is used, as the model takes into account the relation between the income statement, balance sheet and cash flow, but also different ratios in the accounts. The findings indicate that there is a relationship between earnings management and switching between Big5 audit firms. On the other hand, there are no accounting quality differences when switching between other auditing firms. This may indicate that there is no significant difference between the different types of audit firms and whether they are inclined to accept lower quality financial statements. The company's solvency has no effect on the indication of earnings management, which is an interesting finding as it is in contrast to previous international research (Gul et al., 2018; Kluger & Shields, 1989). Furthermore, we found that a change in the size of audit fees is important for the tendency to engage in earnings management, and that switching between auditing firms that leads to higher auditing fees will result in a lower tendency for earnings management. In conclusion, the findings show that larger companies have a lower tendency to engage in earnings management.

Bacheloroppgave i Økonomistyring og investeringsanalyse fra Handelshøyskolen BI, 2018 Formålet med denne oppgaven er å finne Trønderfrakts kostnader knyttet til transporttjenestene for Engrospartner. Vi ønsker å avdekke om prisen de benytter i dag dekker de totale kostnadene ved å gjennomføre tjenestene. På bakgrunn av dette formulerte vi følgende problemstilling: Hva er kostnadene knyttet til transporttjenestene Trønderfrakt utfører for Engrospartner, og hvilken kilopris bør de benytte? Vi har gjennomført intervju med daglig leder og observert aktivitetene ved transporttjenestene for å samle inn primærdata som kan besvare problemstillingen. I tillegg har vi benyttet en rekke sekundærdata, som regnskap, offentlig statistikk og publikasjoner, for å finne relevant informasjon knyttet til temaet. Ved hjelp av TDABC har vi gjennomført en kostnadsanalyse av transporttjenestene. Gjennom analysen har vi kommet frem til at kostnadene for transporttjenestene Trønderfrakt utfører for Engrospartner er omtrent 78 000 kr per uke og 1,25 kr per kilo. Videre har vi avdekket at prisen de benytter i dag, på 1,28 kr per kilo, dekker kostnadene knyttet til tjenestene. Denne prisen gir en fortjeneste på cirka 2,5 %. Vi vil anbefale Trønderfrakt å øke prisen til 1,31 kr per kilo, som inkluderer en fortjeneste på 4,5%. Denne fortjenesten vil redusere risikoen prisformatet medfører. Ved hjelp av regresjon og hypotesetester har vi funnet ut at kostnadene for en rute er uavhengig av antall kilo varer som leveres. Avstanden fra terminalen forklarer derimot 96% av variasjonen i kostnadene. Dagens prisformat medfører at Trønderfrakt vil tjene på noen ruter, og tape på andre. På bakgrunn av dette har vi laget en prismodell som baserer seg på pris per kilo per distansesone, som tar hensyn til detaljistenes avstand fra terminalen. Prisformatet sikrer at inntekten fra tjenestene samsvarer bedre med de reelle kostnadene.
