5. Evaluation of a Novel Point-of-Care Ultrasound Curriculum for First-Year Pediatric Residents

Abstract

The aim of the study is to evaluate a novel point-of-care ultrasound (POCUS) educational curriculum for pediatric residents.The cohort study in graduate medical education was completed from January 2017 to March 2019. Postgraduate year 1 (PGY1) pediatric residents attended the educational curriculum that consisted of 3 half-day sessions over a 3-month period. Each session consisted of a lecture (introduction, extended focused assessment with sonography for trauma, soft tissue/musculoskeletal, cardiac, and resuscitative applications) followed by supervised hands-on scanning sessions. Group ratio was 3 learners to 1 machine/expert instructor. Main outcome measures included pre- and post-written test scores, as well as objective structured clinical examination (OSCE) scores.Forty-nine PGY1 residents (78% women) completed the curriculum. The mean (SD) pretest score was 68% (8.5), and the mean posttest score was 83% (8.3) with a difference of 15 (95% confidence interval, 12.5-17.6; P0.001). Mean (SD) focused assessment with sonography for trauma OSCE score after the curriculum was 88.7% (11.9). The number of PGY1 pediatric residents that were comfortable performing POCUS examinations increased from pretraining to posttraining for soft tissue/musculoskeletal (14%-61%, P0.001), extended focused assessment with sonography for trauma (24%-90%, P0.001), and cardiac (18%-86%, P0.001). All participants found the curriculum useful, and 42 of 49 (86%) stated the curriculum increased their ability to acquire and interpret images.Postgraduate year 1 pediatric residents learned the basics of POCUS through 3 brief educational sessions. The increase in posttest scores demonstrated improved POCUS knowledge, and the high OSCE score demonstrated their ability to acquire ultrasound images. Point-of-care ultrasound guidelines are needed for pediatric residency programs.

33. Using poultry litter as a protein source in meat goat diets. (Small Ruminant Production III)

Abstract

Twenty female and castrated male crossbred meat goats (113 [+ or -] 7 d of age) were fed one of two diets to determine if poultry litter can be used as a suitable protein source in diets for meat goats. Animals were fed either a control, commercial diet (75% commercial diet, 25% alfalfa pellets), or a poultry litter-based diet (40% poultry litter, 35% corn and 25 % alfalfa pellets) with 3 pens/treatment and 10 goats/treatment. Animals that had been exposed to creep feed while nursing were weaned and placed in an indoor facility with 8 X 8 pens and slatted concrete floors and allowed ad libitum access to feed, water and minerals. The goats were allowed a 14-d adjustment period in which they were gradually introduced to the treatment diet such that at the end of the adjustment period, goats were receiving 100% of the treatment diet and no digestive problems were observed. Goats were weighed weekly and feed consumption determined biweekly for the eight-week trial. Total gain for goats on the control diet (20.1 [+ or -] 3.26 kg) was greater (P < 0.05) than for those receiving the poultry litter-based diet (6.2 [+ or -] 1.52 kg). Feed efficiency (gain/feed) for the control group (0.12 [+ or -] 0.04) was not different from animals receiving the poultry litter-based diet (0.07 [+ or -] 04). In summary, goats fed a commercial goat diet gained faster but had a similar feed efficiency when compared to goats fed the poultry litter-based diet. However, it was observed that goats were sorting the poultry litter-based diet, so further studies are needed using a pelleted diet to minimize sorting. Also, it is important to note that near the end of the study, two animals receiving the poultry litter-based diet died of urinary calculi. Key Words: Meat goat, Poultry litter, Feed intake

An adamantinoma of the tibia is presented, for which the CT and MRI characteristics are described. Both imaging modalities were excellent in providing information as to the extent and invasiveness of the tumor, although MRI had the advantage of providing immediate high quality sagittal visualization. Abstract

The radiologic evaluation of growth plate injuries is vital to the surgical management of children with partial growth plate arrests. Plain film radiographs are of limited value, and in some cases, even high quality tomography does not provide adequate detail. Computed tomography (CT) can however provide a cross-sectional display and may also aid in defining the relative position of bone and cartilage in the affected area. This may prove invaluable to the surgeon in the planning of his operation. The vast majority of cases of partial growth arrest can be adequately examined by conventional means, but CT can provide additional information in cases where plain film examination is inadequate. 48. An update on estrus synchronization in goats: A minor species1

Abstract

Estrus synchronization allows for parturition at suitable times to take advantage of niche markets, feed supplies, labor, and rising price trends. In the past, synchronization of estrus in goats has focused primarily on dairy goats to allow for optimal timing of milk production. However, recent interest in meat goat production has resulted in attempts to use dairy goat, sheep, and cattle synchronization regimens in meat goat management systems. Methods of synchronization have included techniques as simple as alteration of light patterns or manipulation of social inputs (i.e., the buck effect) and as complex as varying timed hormonal treatments combined with light alteration and the buck effect. The synchronization of estrus using timed hormonal treatments seems to be more convenient in many meat goat production situations. Examples of hormones used include melatonin, progestogens (administered orally, as an injection, or by using intravaginal releasing devices), gonadotropins/GnRH (or agonists), and PG alone or in combination. As is seen with sheep and cattle, breed and/or breed type, stage of production, and environmental effects can influence synchronization success in goats. The introduction of breeds developed in other countries for rapid growth, such as the Boer goat, and increased consumer and producer interest have added to the impetus for developing cost-efficient and highly effective estrus synchronization regimens. New research is being conducted and various synchronization methods are being attempted in goats, a minor species, and the objective of this paper is to review these efforts.
