Abstract

BackgroundStudies have shown that individuals with schizophrenia suffer from memory impairments. In this study, we combined proton magnetic resonance spectroscopy (¹H-MRS) and functional magnetic resonance imaging (fMRI) to clarify the neurobiology of memory deficits in schizophrenia.MethodsWe used single-voxel MRS acquired in the left hippocampus and fMRI during performance of a memory task to obtain measures of neurochemistry and functional response in 28 stable, medicated participants with schizophrenia (SZ) and 28 matched healthy controls (HC).ResultsThe SZ group had significantly decreased blood oxygen level-dependent (BOLD) signal in left inferior frontal gyrus (IFG) during encoding and in the anterior cingulate cortex (ACC) and superior temporal gyrus (STG) during retrieval. We did not find significant differences in N-acetylaspartate/creatine (NAA/Cr) or glutamate+glutamine (Glx/Cr) levels between the groups, but did find a significant positive correlation between NAA/Cr and Glx/Cr in the HC group that was absent in the SZ group. There were no significant correlations between BOLD and MRS measured in the hippocampus. Further analyses revealed a negative correlation between left IFG BOLD and task performance in the SZ group. Finally, in the HC group, the left IFG BOLD was positively correlated with Glx/Cr.ConclusionsWe replicated findings of reduced BOLD signal in left IFG and of an altered relationship between IFG BOLD response and task performance in the SZ. The absence of correlation between NAA/Cr and Glx/Cr levels in patients might suggest underlying pathologies of the glutamate-glutamine cycle and/or mitochondria.

19. Getting Vaccinated against HPV: Attitudes, Intentions and Perceived Barriers of Female Undergraduates

Abstract

This study examines college women's intention to receive the Human Papilloma Virus (HPV) vaccine and their perceived barriers to being vaccinated. The study reports findings from an online questionnaire completed by 856 undergraduate women enrolled in a required personal health course at a large (27,000 plus) southeastern university. The majority of the respondents (78.6%) indicated that they would get the vaccine; however, only 60.7% of the respondents who self-identified as not yet sexually active reported that they plan to be vaccinated. Side effects, costs and lack of knowledge about the vaccine were most often indicated as barriers to receiving the vaccine. Education about HPV and the vaccine is needed to address the perceived barriers to vaccination. (Contains 3 tables.)

Abstract

This is the final report and critique which investigated the law school admissions process, and especially the role of the Law School Admission Test (LSAT) within that process, for possible bias against minority applicants. The study involved the reanalysis of existing data. Results show that current admission policies unfairly limit the enrollment of minority applicants. The report begins by reviewing the Bakke decision. It then examines each of the components of the Admissions Index which is a weighted combination of the undergraduate grade point average (UGPA) and the LSAT score. Overall, the UGPA is less biased against minorities than is the LSAT. The report presents evidence that shows the differential effect of adding LSAT scores to UGPA's for minorities vs. whites. The author examines items from the "Law School Admission Bulletin and LSAT Preparation Material" which is commonly used for practice by potential test takers. Factors inherent in the LSAT which might be contributing to low test performance for minorities are explained. Finally the Thorndike and Cole models for admissions decisions are evaluated. The report's recommendations include: adjusting the LSAT scores of minority applicants in recognition of possible cultural bias in the test; evaluating LSAT scores on an individual basis through extensive review of applicant files; separating evaluations for minority applicants; and disregarding LSAT scores. Without exception, reviewers, who critiqued the study agreed with the author's concern for the rethinking of current admissions policies. However, many reviewers pointed out problems with the methodology used, with the validity of the report's conclusions, and with the final set of recommendations. (Author/RM)

Abstract

The FACTRETRIEVAL2 test battery, which assesses both retrieval of general information from memory and metacognition about the retrieval, was administered to people before and after a recent expedition to Mount Everest and at extreme altitudes above 6,400 m (higher than any mountain in North America or Europe). The major findings were as follows. First, the same extreme altitudes already known to impair learning did not affect either accuracy or latency of retrieval, and this robustness of retrieval occurred for both recall and forced-choice recognition. Second, extreme altitude did affect metacognition: The climbers showed a decline in their feeling of knowing both while at extreme altitude and after returning to Kathmandu (i.e., both an effect and an aftereffect of extreme altitude). Third, extreme altitude had different effects than alcohol intoxication (previously assessed by Nelson, McSpadden, Fromme, & Marlatt, 1986). Alcohol intoxication affected retrieval without affecting metacognition, whereas extreme altitude affected metacognition without affecting retrieval; this different pattern for extreme altitude versus alcohol intoxication implies that (a) hypoxia does not always yield the same outcome as alcohol intoxication and (b) neither retrieval nor metacognition is strictly more sensitive than the other for defecting changes in independent variables.

Abstract

Background: Bile duct calculi (BDC) can be cleared or treated with modern endoscopic techniques in most patients. However, large stones, bile duct strictures, or unusual anatomy may make endoscopic clearance difficult. The purpose of the present study was to determine the efficacy of extracorporeal shock-wave lithotripsy (ESWL) in treating patients with complicated BDC. Methods: Between 1989 and January 1995, 16 patients with BDC were treated at our institution with ESWL using a Dornier HM-3 lithotropter. The average age of patients was 62 years (range 32 to 88). Endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy and attempted stone extraction (100%), nasobiliary drainage (83%), and biliary stents (6%) were used prior to ESWL. Eleven patients (61%) had solitary stones, ranging in diameter from 0.5 to 2.6 cm, whereas 7 patients had multiple stones, ranging in diameter from 0.5 to 5.0 cm. The indications for ESWL were stone impaction (56%), stone size (38%), and bile duct stricture (6%). Results: The 16 patients received 27 ESWL treatments (mean = 2101 shock at 21 kV); with 4 patients (22%) requiring multiple treatments. Stone fragmentation was achieved in 94% of patients. All patients had ERCP performed post-ESWL, and only 2 (13%) patients required immediate operations. At discharge, 94% of patients were stone-free. Minor complications (eg, pain, hematuria) were common. With an average follow-up of 3 years, only 1 patient (6%) has required retreatment for BDC. Hepatic transplantation was required in an additional patient. Conclusions: In this cohort of patients with both major medical comorbidities and/or technical contraindications to standard methods of endoscopic and surgical clearance of BDC, we found that ESWL facilitated stone clearance in 94% of patients with minimal morbidity and no mortality. Abstract

The National Animal Disease Center (NADC) conducts basic and applied research on endemic animal diseases of high priority that adversely affect US livestock production or trade. Experiments conducted at this center vary in range and scope with a subset involving synthetic or recombinant nucleic acids (DNA), microorganisms, and/or animals. Historically, the NADC used hard copy paper and filing systems to catalog and monitor these types of experiments, but to improve communication, tracking, searching, reporting, and documentation of Institutional Biosafety Committee (IBC) actions, this institution has transitioned to using a commercially available software tool to digitally manage protocols in our ongoing efforts to maintain excellence in regulatory compliance. In addition, similar to many other research institutions and universities, the scope of the IBC has expanded to include risk assessments on all work conducted at the center. This process has been streamlined using the Biosafety RAM open source software, developed by Sandia National Laboratories, and has stimulated productive discussions on best practices to safely conduct animal and microbiological experiments at the center. Although some initial challenges arose, successful implementation of these two software tools at the NADC has simplified the management of IBC compliance requirements and facilitated review processes at a high-containment government research facility.

43. Neurologic syndrome in 25 workers from an aluminum smelting plant

Abstract

* Background. - This article expands on an earlier series of three patients with a neurologic syndrome, who had all worked in an aluminum smelting plant. Methods. - Twenty-five symptomatic workers from the same plant were referred for a standardized evaluation, including completion of a health questionnaire, neurologic examination, and neuropsychologic evaluation. An exposure index was calculated for each worker based on level and duration of exposure in the potroom, where exposures were the greatest. This index was correlated with symptoms, signs, and neuropsychologic test scores. Results. - Twenty-two (88%) of the patients reported frequent loss of balance, and 21 (84%) reported memory loss. Neurologic examination revealed signs of incoordination in 21 (84%) of the patients. Neuropsychologic test results showed preservation in certain spheres of functioning, such as verbal IQ, with substantial impairment in others, particularly memory functioning. On memory test, 70% to 75% showed mild or greater impairment. The majority (17 of 19 tested, or 89%) showed depression on the Minnesota Multiphasic Personality Inventory. The exposure index was significantly correlated with signs and symptoms of incoordination. Conclusions. - This study and others in humans and animals support the existence of a syndrome characterized by incoordination, poor memory, impairment in abstract reasoning, and depression. Aluminum exposure in the potroom seems the most likely cause. (Arch Intern Med. 1992; 152:1443-1448)

44. Affect and Causal Attribution in Marital Conflicts: An Exploratory Study.

Abstract

Recent attempts to predict marital success or failure have explored the explanations couples offer for interpersonal events. To investigate whether positive and negative affect would lead to different causal attributions in a conflict-resolution conversation, 20 married couples were asked to observe a conflict-resolution and a control conversation in which they had engaged. They were asked to segment a videotape for both conversations whenever anything occurred that affected them positively or negatively. Among those segments the most positive and negative events were replayed, and causal attributions for self and for partner were collected. Results indicated that behaviors were seen as positively-motivated in those segments when spouses reported negative affect. Further, behaviors were seen as positively-motivated during conflict-resolution conversations as opposed to control conversations. Wives viewed their behaviors in negative segments as intended, indicating that they accepted responsibility for contributing to negative exchanges. Husbands in better marriages perceived their behavior in negative segments of conflict conversations as intended and negatively-motivated. Thus, self-blame may be related to satisfaction in husbands but to dissatisfaction in wives. (Author/JAC)

Topics in this book include: (1) a description of the test development process that is involved in compiling an actual LSAT; (2) a detailed analysis of the structure of each section of the LSAT; (3) an analysis of answer options; (4) a description of tricks for answering specific types of items; and (5) examples of the tricks from published materials and commentary on the tricks. Topics in this book include: (1) a description of the test development process that is involved in compiling an actual GMAT; (2) a detailed analysis of the structure of each section of the GMAT; (3) an analysis of answer options; (4) a description of tricks for answering specific types of items; and (5) examples of the tricks from published material and commentaries. Each suggested trick is accompanied by a frequency breakdown, which provides specific data about the trick's effectiveness on 20 previously disclosed editions of the GMAT. There is a special chapter on test bias which focuses on bias against minorities, women, and low income applicants. Results of a study designed to assess knowledge about chlamydial infection among university students (N=258) revealed serious gaps in knowledge. Data suggests that students held several misconceptions, or had no knowledge, regarding symptoms, transmission, treatment, and testing for chlamydial infection. (IAH)
