Objective: The Bridging the Gap Between Physical and Mental Illness in Community Pharmacy ("PharMIbridge") randomised controlled trial (RCT) aims to test the effectiveness of a pharmacist-led support service in improving medication adherence, and the physical and mental health of people living with severe and persistent mental illness compared to a standard medication review service. Method: Using the six-step intervention mapping (IM) framework, this paper describes the development and content of the "PharMIbridge" pharmacist training programme, an integral part of the RCT implementation, and continuous adaptations made to the process to keep pace with the evolving severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic in Australia. A Training Working Group comprising health educators, practitioners, mental health consumers and researchers, refined the programme objectives and assisted with developing content and troubleshooting issues related to training delivery for pharmacists randomised to the RCT intervention arm. Results: A 2-day training programme was developed, which included Mental Health First Aid, simulated patient role-plays, and four pre-recorded modules using lectures, demonstration case vignettes, role-play activities and discussion. The programme, co-facilitated by project team members and mentors (pharmacist and consumer educators), aims to enhance pharmacists' mental health literacy, skills and confidence and empower them to engage with this vulnerable population using a strengths-based approach. Pre- and post-training questionnaires and interviews will be used to evaluate the impact of the "PharMIbridge" training programme. Conclusion: The systematic stepwise method provided by the IM framework highlights the solution-focused approach adopted by the project team and characteristics including adaptability and resilience which enabled training development and implementation across four Australian regions during the SARS-CoV-2 pandemic.

Background: Episodic spikes in air pollution due to landscape fires are increasing, and their potential for longer term health impacts is uncertain. Objective: Our objective is to evaluate associations between exposure in utero and in infancy to severe pollution from a mine fire, background ambient air pollution, and subsequent hospital care. Methods: We linked health records of births, emergency department (ED) visits, and hospitalizations of children born in the Latrobe Valley, Australia, 2012-2015, which included a severe pollution episode from a mine fire (9 February 2014 to 25 March 2014). We assigned modeled exposure estimates for fire-related and ambient particulate matter with an aerodynamic diameter of 2.5 [micro]m (P[M.sub.2.5]) to residential address. We used logistic regression to estimate associations with hospital visits for any cause and groupings of infectious, allergic, and respiratory conditions. Outcomes were assessed for the first year of life in the in utero cohort and the year following the fire in the infant cohort. We estimated exposure-response for both fire-related and ambient P[M.sub.2.5] and also employed inverse probability weighting using the propensity score to compare exposed and not/minimally exposed children. Results: Prenatal exposure to fire-related P[M.sub.2.5] was associated with ED presentations for allergies/skin rash [odds ratio (OR) = 1.34, 95% confidence interval (CI): 1.01, 1.76 per 240[micro]g/[m.sup.3] increase]. Exposure in utero to ambient P[M.sub.2.5] was associated with overall presentations (OR= 1.18, 95% CI: 1.05, 1.33 per 1.4 [micro]g/[m.sup.3]) and visits for infections (ED: OR = 1.13, 95% CI: 0.98, 1.29; hospitalizations: OR = 1.23, 95% CI: 1.00, 1.52). Exposure in infancy to fire-related P[M.sub.2.5] compared to no/minimal exposure, was associated with ED presentations for respiratory (OR = 1.37, 95% CI: 1.05, 1.80) and infectious conditions (any: OR = 1.21, 95% CI: 0.98, 1.49; respiratory-related: OR= 1.39, 95% CI: 1.05, 1.83). Early life exposure to ambient P[M.sub.2.5] was associated with overall ED visits (OR = 1.17, 95% CI: 1.05, 1.30 per 1.4 [micro]g/[m.sup.3] increase). Discussion: Higher episodic and lower ambient concentrations of P[M.sub.2.5] in early life were associated with visits for allergic, respiratory, and infectious conditions. IntroductionNumerous studies have found associations between characteristics of urban environments and risk factors for dementia and cognitive decline, such as physical inactivity and obesity. However, the contribution of urban environments to brain and cognitive health has been seldom examined directly. This cohort study investigates the extent to which and how a wide range of characteristics of urban environments influence brain and cognitive health via lifestyle behaviours in mid-aged and older adults in three cities across three continents.Methods and analysisParticipants aged 50-79 years and living in preselected areas stratified by walkability, air pollution and socioeconomic status are being recruited in Melbourne (Australia), Barcelona (Spain) and Hong Kong (China) (n=1800 total; 600 per site). Two assessments taken 24 months apart will capture changes in brain and cognitive health. Cognitive function is gauged with a battery of eight standardised tests. Brain health is assessed using MRI scans in a subset of participants. Information on participants' visited locations is collected via an interactive web-based mapping application and smartphone geolocation data. Environmental characteristics of visited locations, including the built and natural environments and their by-products (e.g., air pollution), are assessed using geographical information systems, online environmental audits and self-reports. Data on travel and lifestyle behaviours (e.g., physical and social activities) and participants' characteristics (e.g., sociodemographics) are collected using objective and/or self-report measures.Ethics and disseminationThe study has been approved by the Human Research Ethics Committee of the Australian Catholic University, the Institutional Review Board of the University of Hong Kong and the Parc de Salut Mar Clinical Research Ethics Committee of the Government of Catalonia. Purpose: The purpose of this paper is to explore the experiences of students and teachers who had participated in a postgraduate work-based praxis course within a Master of mental health practice qualification. Design/methodology/approach: This qualitative study used an interpretative phenomenological approach to understand the lived experience of students and course convenors participating in a work-based praxis course. Seven students and two convenors were recruited. Interview and reflective portfolio data were analysed thematically. Findings: The main themes identified were the importance of planning, the value of partnerships, the significance of learning in the workplace and how the facilitation of work-based learning differs from coursework. Originality/value: Work-based learning within postgraduate coursework qualifications can support higher-level learning, knowledge and skills has received limited attention in the literature. This study supported the value of providing postgraduate students with work-based learning opportunities, resulting in the application of new or advanced skills, within their existing work roles. This study is important, because it provides insights into the student experience of postgraduate work-based learning and the impact of this learning on professional practice.

Decades of effort has led to persistent repetition in what the research priorities of people with lived-experience of mental ill-health are.This was a narrative review and synthesis of published literature reporting mental health research priorities (2011-2023).A narrative framework was established with the questions: (1) who has been involved in priority setting? With whom have priorities been set? Which priorities have been established and for whom? What progress has been made? And, whose priorities are being progressed?Seven papers were identified. Two were Australian, one Welsh, one English, one was from Chile and another Brazilian and one reported on a European exercise across 28 countries (ROAMER). Hundreds of priorities were listed in all exercises. Prioritisation mostly occured from survey rankings and/or workshops (using dots, or post-it note voting). Most were dominated by clinicians, academics and government rather than people with lived-experience of mental ill-health and carer, family and kinship group members.One lived-experience research led survey was identified. Few studies reported lived-experience design and development involvement. Five of the seven papers reported responses, but no further progress on priorities being met was reported.This review followed PRISMA guidance for search strategy development and systematic review and reporting. Aim: Prescribed burning is the most common method employed to reduce fuel loads in flammable landscapes. This practice is designed to reduce the hazard associated with uncontrolled bushfires. Prescribed burns are frequently conducted close to residential areas, and the associated smoke impacts can adversely affect community health. Particulate matter is the predominant pollutant within the smoke and is strongly and consistently linked with adverse health effects. Outdoor smoke readily infiltrates buildings and reduces the quality of indoor air. Portable air cleaners containing highefficiency particulate air (HEPA) filters are a promising indoor air quality intervention for reducing outdoor smoke exposure. Methods: We provided 10 homes from semirural regions of Victoria, Australia, with HEPA cleaners and conducted continuous monitoring of indoor and outdoor fine particulate matter (PM2.5) for 2-4 weeks during prescribed burning periods. We calculated the potential improvements to indoor air quality when operating a HEPA cleaner during a smoke episode. Ventilation measures were conducted to identify points of smoke ingress and housing characteristics that could lead to higher infiltration rates. Results: Depending on the house, the use of HEPA cleaners resulted in a reduction in indoor PM2.5 concentrations of 30-74%. Conclusions: HEPA cleaners have the potential to substantially improve indoor air quality during episodic smoke episodes. [ABSTRACT FROM AUTHOR]

Background and Objective: Chronic, low‐intensity air pollution exposure has been consistently associated with reduced lung function throughout childhood. However, there is limited research regarding the implications of acute, high‐intensity air pollution exposure. We aimed to determine whether there were any associations between early life exposure to such an episode and lung growth trajectories. Methods: We conducted a prospective cohort study of children who lived in the vicinity of the Hazelwood coalmine fire. Lung function was measured using respiratory oscillometry. Z‐scores were calculated for resistance (R5) and reactance at 5 Hz (X5) and area under the reactance curve (AX). Two sets of analyses were conducted: (i) linear regression to assess the cross‐sectional relationship between post‐natal exposure to mine fire‐related particulate matter with an aerodynamic diameter of less than 2.5 micrometres (PM2.5) and lung function at the 7‐year follow‐up and (ii) linear mixed‐effects models to determine whether there was any association between exposure and changes in lung function between the 3‐ and 7‐year follow‐ups. Results: There were no associations between mine fire‐related PM2.5 and any of the lung function measures, 7‐years later. There were moderate improvements in X5 (β: −0.37 [−0.64, −0.10] p = 0.009) and AX (β: −0.40 [−0.72, −0.08] p = 0.014), between the 3‐ and 7‐year follow‐ups that were associated with mean PM2.5, in the unadjusted and covariance‐adjusted models. Similar trends were observed with maximum PM2.5. Conclusion: There was a moderate improvement in lung stiffness of children exposed to PM2.5 from a local coalmine fire in infancy, consistent with an early deficit in lung function at 3‐years after the fire that had resolved by 7‐years. The independent and joint effects of within- and between-city contrasts in air pollution on mortality have been investigated rarely. To examine the differential effects of between- versus within-city contrasts in pollution exposure, we used both ambient measurements and land use regression models to assess associations with mortality and exposure to nitrogen dioxide (NO2) among ~735,600 adults in 10 of the largest Canadian cities. We estimated exposure contrasts partitioned into within- and between-city contrasts, and the sum of these as overall exposures, for every year from 1984 to 2006. Residential histories allowed us to follow subjects annually during the study period. We calculated hazard ratios (HRs) adjusted for many personal and contextual variables. In fully-adjusted, random-effects models, we found positive associations between overall NO2 exposures and mortality from non-accidental causes (HR per 5 p.p.b.: 1.05; 95% confidence interval (CI): 1.03-1.07), cardiovascular disease (HR per 5 p.p.b.: 1.04; 95% CI: 1.01-1.06), ischaemic heart disease (HR per 5 p.p.b.: 1.05; 95% CI: 1.02-1.08) and respiratory disease (HR per 5 p.p.b.: 1.04; 95% CI: 0.99-1.08), but not from cerebrovascular disease (HR per 5 p.p.b.: 1.01; 95% CI: 0.96-1.06). We found that most of these associations were determined by within-city contrasts, as opposed to by between-city contrasts in NO2. Our results suggest that variation in NO2 concentrations within a city may represent a more toxic mixture of pollution than variation between cities.

Aim: To examine associations of neighborhood greenery, air pollution and walkability with cardiometabolic disease in adults aged ≥45 years in the Frankston–Mornington Peninsula region, Victoria, Australia. Methods: A cross‐sectional, ecological study design was used. We assessed mean annual neighborhood greenery using the Normalized Difference Vegetation Index; air pollution (fine particulate matter of diameter ≤2.5 μm [PM2.5] and NO2) using land‐use regression models; and walkability using Walk Score (possible values 0–100). Medically diagnosed diabetes (~95% type‐2), heart disease and stroke were self‐reported in the Australian Census (2021). Multivariable regression was used to model associations between environmental exposures and area‐level (neighborhood) cardiometabolic disease prevalence (age group ≥45 years), with socioeconomic status, age and sex as covariates. Air pollution was examined as a mediator of associations between greenery and disease prevalence. Results: Our sample comprised 699 neighborhoods with the following mean (SD) values: Normalized Difference Vegetation Index 0.47 (0.09), PM2.5, 8.5 (0.6) μg/m3 and NO2, 5.2 (1.6) ppb. Disease prevalences were: heart disease, mean 8.9% (4.5%); diabetes, mean 10.3% (4.7%); and stroke, median 1.2% (range 0–10.9%). Greenery was negatively associated with diabetes (β = −5.85, 95% CI −9.53, −2.17) and stroke prevalence (β = −1.26, 95% CI −2.11, −0.42). PM2.5 and NO2 were positively associated with diabetes (β = 1.59, 95% CI 1.00, 2.18; β = 0.42, 95% CI 0.22, 0.62) and stroke prevalence (β = 0.15, 95% CI 0.01, 0.29; β = 0.06, 95% CI 0.01, 0.10). The association between greenery and diabetes was partially mediated by PM2.5 (mediated effect −5.38, 95% CI −7.84, −3.03). Conclusions: Greenery and air pollutants were associated with lower and higher prevalence, respectively, of self‐reported diabetes and, to a lesser extent, stroke.
