It is responsible for surface and deep tissue destruction leading to perforations and hemorrhage. Its pathogenesis represented by an angio-invasion is at the origin of a local infarction and a vascular thrombosis. We report a case of gastrointestinal (GI) mucormycosis-induced multiple gastric ulcers, GI bleeding and rectal perforation.Case Presentation: A 75-year-old man, with type II diabetes mellitus, was admitted to the intensive care unit for an acute abdominal pain associated with massive hematochezia. Clinical examination was that of an acute peritonitis and a hemorrhagic shock state. Abdominal and pelvic CT scan with intravenous contrast concluded to a perforation of the anterior wall of the rectum. He underwent immediate laparotomy with temporary colostomy. Several upper GI endoscopies had shown multiple gastric ulcer lesions. Lower GI endoscopy showed a fistulous orifice of the rectum on its anterior surface. Histopathology of the gastric biopsy showed acute and subacute inflammatory changes with filamentous elements suggesting mucormycosis. Histopathology of the rectal biopsy showed a subacute non-specific inflammation. Culture of the secretions from the rectal fistula orifice showed the strain Rhizopus sp. Antifungal susceptibility testing reported sensitivity to liposomal amphotericin B. The diagnosis of GI mucormycosis-induced multiple gastric ulcers, rectal perforation and pulmonary embolism in the patient with type II diabetes mellitus was retained. The outcomes were favorable after 6 weeks of treatment with liposomal amphotericin B associated with temporary colostomy and appropriate diabetes management.Conclusion: GI mucormycosis remains a multidisciplinary diagnostic challenge, less frequent in clinical practice, with a long diagnostic pathway. This opportunistic systemic mycosis can lead to numerous GI complications including perforation, massive GI bleeding and even multiple extra-GI complications. 5. Seed particle formation for silicate dust condensation by SiO nucleation

Abstract

Clustering of the abundant SiO molecules has been discussed as a possible mechanism of seed particle formation for silicate dust in stellar outflows with an oxygen rich element mixture. Previous results indicated that condensation temperatures based on this mechanism are significant lower than what is really observed. This negative result strongly rests on experimental data on vapour pressure of SiO. New determinations show the older data to be seriously in error. Here we aim to check with improved data the possibility that SiO nucleation triggers the cosmic silicate dust formation. First we present results of our measurements of vapour pressure of solid SiO. Second, we use the improved vapour pressure data to re-calibrate existing experimental data on SiO nucleation from the literature. Third, we use the re-calibrated data on SiO nucleation in a simple model for dust-driven winds to determine the condensation temperature of silicate in stellar outflows from AGB stars. We show that onset of nucleation under circumstellar conditions commences at higher temperature than was previously found. Calculated condensation temperatures are still by about 100 K lower than observed ones, but this may be due to the greenhouse effect of silicate dust temperatures. The assumption that the onset of silicate dust formation in late-type M stars is triggered by cluster formation of SiO is compatible with dust condensation temperatures derived from IR observations., Comment: 11 pages, 11 figures

Abstract

Background and Objectives COVID-19–related inflammation, endothelial dysfunction, and coagulopathy may increase the bleeding risk and lower the efficacy of revascularization treatments in patients with acute ischemic stroke (AIS). We aimed to evaluate the safety and outcomes of revascularization treatments in patients with AIS and COVID-19. Methods This was a retrospective multicenter cohort study of consecutive patients with AIS receiving intravenous thrombolysis (IVT) and/or endovascular treatment (EVT) between March 2020 and June 2021 tested for severe acute respiratory syndrome coronavirus 2 infection. With a doubly robust model combining propensity score weighting and multivariate regression, we studied the association of COVID-19 with intracranial bleeding complications and clinical outcomes. Subgroup analyses were performed according to treatment groups (IVT-only and EVT). Results Of a total of 15,128 included patients from 105 centers, 853 (5.6%) were diagnosed with COVID-19; of those, 5,848 (38.7%) patients received IVT-only and 9,280 (61.3%) EVT (with or without IVT). Patients with COVID-19 had a higher rate of symptomatic intracerebral hemorrhage (SICH) (adjusted OR 1.53; 95% CI 1.16–2.01), symptomatic subarachnoid hemorrhage (SSAH) (OR 1.80; 95% CI 1.20–2.69), SICH and/or SSAH combined (OR 1.56; 95% CI 1.23–1.99), 24-hour mortality (OR 2.47; 95% CI 1.58–3.86), and 3-month mortality (OR 1.88; 95% CI 1.52–2.33). Patients with COVID-19 also had an unfavorable shift in the distribution of the modified Rankin score at 3 months (OR 1.42; 95% CI 1.26–1.60). Discussion Patients with AIS and COVID-19 showed higher rates of intracranial bleeding complications and worse clinical outcomes after revascularization treatments than contemporaneous non–COVID-19 patients receiving treatment. The s wave potentials for electroproduction and meson-baryon scattering are derived from a chiral effective Lagrangian which includes the eta-prime as an explicit degree of freedom and incorporates important features of the underlying QCD Lagrangian such as the axial U(1) anomaly. The effective potentials are iterated in a Bethe-Salpeter equation and cross sections for eta, eta-prime photo- and electroproduction from protons are obtained. The results for the eta-prime photoproduction cross section reproduce the appearance of an S_(11) resonance around 1.9 GeV observed at ELSA. We include the eta-prime in chiral perturbation theory without employing 1/N_c counting rules. The method is illustrated by calculating the masses and decay constants of the Goldstone boson octet (pions, kaons, eta) and the singlet eta-prime up to one-loop order. Abstract

We extend our recent non-perturbative chiral SU(3) coupled channel approach to pion- and photon-induced $\eta$- and $K$-meson production off protons by including all strong and electromagnetic p-wave multipoles. We identify the p-wave amplitudes of the next-to-leading order SU(3) chiral meson-baryon Lagrangian with a coupled channel potential which is iterated to infinite orders in a separable Lippmann-Schwinger equation. Our approach to $\eta$- and $K$-photoproduction introduces no additional free parameters. By adjusting a few finite range parameters and the unknown parameters in the Lagrangian, we are able to simultaneously describe a very large amount of low-energy data. These include the total and differential cross sections of the $\pi$-induced reactions $\pi^- p \to \eta n, K^0 \Lambda, K^0 \Sigma^0, K^+ \Sigma^-$ and $\pi^+ p \to K^+ \Sigma^+$ as well as those of photoproduction $\gamma p \to \eta p, K^+\Lambda, K^+ \Sigma^0, K^0 \Sigma^+$. The polarization observables measured in $\eta$- and $K$-photoproduction are particularly sensitive to interference terms between the s- and p-wave multipoles. The total cross section data are remarkably well reproduced in all channels.
