OBJECTIVES: Accountable care organizations (ACOs) are groups of healthcare providers responsible for quality of care and spending for a defined patient population. The elimination of low-value medical services will improve quality and reduce costs and, therefore, ACOs should actively work to reduce the use of low-value services. We set out to identify ACO characteristics associated with implementation of strategies to reduce overuse. STUDY DESIGN: Survey analysis. METHODS: We used the National Survey of ACOs to determine the percentage of responding ACOs aware of the Choosing Wisely campaign and to what degree ACOs have taken steps to reduce the use of low-value services. We identified characteristics of ACOs associated with implementing low-value care-reducing strategies using 3 statistical models (stepwise and LASSO logistic regression and random forest). RESULTS: Responding executives of 155 of 267 ACOs (58%) were aware of Choosing Wisely. Eighty-four of those 155 ACO leaders said that their ACOs also actively implemented strategies to reduce the use of low-value services, largely through educating physicians and stimulating shared decision making. All 3 models identified the presence of at least 1 commercial payer contract and prior joint experience pursuing risk-based payment contracts as the most important predictors of an ACO actively implementing strategies to reduce low-value care. CONCLUSIONS: In the first year of implementation, just one-third of ACOs had taken steps to reduce the use of low-value medical services. Safety-net ACOs and those with little experience as a risk-bearing organization need more time and support from healthcare payers and the Choosing Wisely campaign to prioritize the reduction of overuse.

In de media en in de wetenschap wordt in toenemende mate aandacht besteed aan de kwaliteit van medische zorg. Kwaliteit in de gezondheidszorg is belangrijk voor zowel patienten die een goede behandeling wensen, als voor artsen die goede kwaliteit willen leveren. Maar hoe kun je de kwaliteit van de gezondheidszorg inzichtelijk maken? Indicatoren kunnen mogelijk uitkomst bieden. In dit onderzoek is een set indicatoren ontwikkeld om de kwaliteit van de heelkunde in het ziekenhuis te meten. De Inspectie voor de Gezondheidszorg (IGZ) heeft opdracht gegeven aan het Rijksinstituut voor Volksgezondheid en Milieu (RIVM) om deze set indicatoren te ontwikkelen om daarmee haar taak als toezichthouder te ondersteunen. Hierbij is intensief samengewerkt met de Nederlandse Vereniging voor Heelkunde (NVvH). De ontwikkeling van de indicatoren is in een aantal stappen uitgevoerd. De eerste stap betrof het identificeren van terreinen binnen de heelkunde waar de zorg potentieel verbeterd kon worden. In de tweede stap heeft literatuuronderzoek en de inbreng van experts een aantal potentiele indicatoren opgeleverd. In de derde stap is na een consensusprocedure een set van negen indicatoren gedefinieerd: het percentage reoperaties binnen dertig dagen, het percentage ziekenhuismortaliteit, het percentage postoperatieve wondinfecties, het percentage choledochusletsels, de gemiddelde opnameduur na appendectomie, de postoperatieve opnameduur na colonresecties, de wachttijd bij heupfracturen, het percentage proefthoracotomie en het percentage pneumonectomie versus lobectomie. In een haalbaarheidsonderzoek is onderzocht in hoeverre de gegevens beschikbaar waren en op welke manier de gegevens ontsloten kunnen worden. Door de gebrekkige aansluiting bij bestaande systemen kostte de registratie veel tijd. De ontwikkeling van een goede structuur voor dataverzameling en verwerking is essentieel voor een goede implementatie van de indicatoren. Indicators may be helpful in measuring quality of care. In this study we developed and tested a set of quality-indicators for surgical care. The Dutch Health Care Inspectorate requested the National Institute for Public Health and the Environment (RIVM)to develop this set of indicators to support their inspection activities. This study was carried out in close collaboration with the Dutch Society for Surgeons (NVvH). To define a limited set of indicators for surgical care, three steps were taken. In the first step a working group, with members of the NVvH and researchers of RIVM, identified risks of suboptimal care. In the second step, indicators were identified to measure suboptimal care, based on the literature and expert opinion. In the third step, after a consensus procedure, a set of 9 indicators was selected: percentage reoperations within 30 days (planned or unplanned), percentage in-hospital mortality, percentage postoperative wound infections, percentage biliary duct injury, length of stay appendectomy, postoperative length of stay after colon resections, time between admission and hip fracture surgery, percentage exploratory thoracotomy and percentage pneumectomy versus pulmonary lobectomy. To evaluate the feasibility of the registration of these indicators, we tested the registration in several hospitals. Further development of a system for data registration and processing is necessary for successful implementation of the indicators. RIVM rapport:In deze studie is een set van 35 kwaliteitsindicatoren ontwikkeld voor de verloskundige zorg in het ziekenhuis. Deze indicatoren zijn gedurende zes maanden getest op bruikbaarheid in dertien ziekenhuizen. Uit de evaluatie blijkt dat de registratie haalbaar is en dat verdere implementatie van de indicatoren ondersteund wordt door de betrokken zorgverleners in het ziekenhuis. In de komende jaren wordt door de Nederlandse Vereniging voor Obstetrie en Gynaecologie (NVOG) gewerkt aan de implementatie van de set indicatoren om de kwaliteit van de obstetrische zorg in het ziekenhuis te stimuleren. Het doel van deze indicatoren is om gynaecologen een hulpmiddel te geven om de kwaliteit van de door hen verleende obstetrische zorg te monitoren en te bewaken. Bij het ontwikkelen van de kwaliteitsindicatoren zijn de richtlijnen van de NVOG als uitgangspunt genomen. Er is rekening gehouden met de mate waarin de indicatoren eenvoudig meetbaar en snel beschikbaar zijn en in hoeverre zij een bijdrage kunnen leveren aan verbeteringen in de zorg. De Landelijke Verloskunde Registratie (LVR) is de belangrijkste informatiebron voor de indicatoren. Na drie beoordelingsrondes door experts is een set van 35 kwaliteitsindicatoren tot stand gekomen, waarvan negentien structuur-, dertien proces- en drie uitkomstindicatoren. Structuurindicatoren hebben betrekking op de organisatie van de zorg, zoals een structurele complicatiebespreking. Voorbeelden van geselecteerde procesindicatoren zijn het percentage inleidingen en het percentage kunstverlossingen. Further development of a system for data registration and processing is necessary for successful implementation of the indicators. Kwaliteit in de gezondheidszorg is belangrijk voor zowel patienten die een goede behandeling wensen, als voor artsen die goede kwaliteit willen leveren. Maar hoe kun je de kwaliteit van de gezondheidszorg inzichtelijk maken? Indicatoren kunnen mogelijk uitkomst bieden. In dit onderzoek is een set indicatoren ontwikkeld om de kwaliteit van de heelkunde in het ziekenhuis te meten. De Inspectie voor de Gezondheidszorg (IGZ) heeft opdracht gegeven aan het Rijksinstituut voor Volksgezondheid en Milieu (RIVM) om deze set indicatoren te ontwikkelen om daarmee haar taak als toezichthouder te ondersteunen. Hierbij is intensief samengewerkt met de Nederlandse Vereniging voor Heelkunde (NVvH). De ontwikkeling van de indicatoren is in een aantal stappen uitgevoerd. De eerste stap betrof het identificeren van terreinen binnen de heelkunde waar de zorg potentieel verbeterd kon worden. In de tweede stap heeft literatuuronderzoek en de inbreng van experts een aantal potentiele indicatoren opgeleverd. In de derde stap is na een consensusprocedure een set van negen indicatoren gedefinieerd: het percentage reoperaties binnen dertig dagen, het percentage ziekenhuismortaliteit, het percentage postoperatieve wondinfecties, het percentage choledochusletsels, de gemiddelde opnameduur na appendectomie, de postoperatieve opnameduur na colonresecties, de wachttijd bij heupfracturen, het percentage proefthoracotomie en het percentage pneumonectomie versus lobectomie. In een haalbaarheidsonderzoek is onderzocht in hoeverre de gegevens beschikbaar waren en op welke manier de gegevens ontsloten kunnen worden. Door de gebrekkige aansluiting bij bestaande systemen kostte de registratie veel tijd. BackgroundSince, at the health system level, there is little research into the possible interrelationships among the various indicators of health, healthcare performance, non-medical determinants of health, and community and health system characteristics, we conducted this study to explore such interrelationships using the Canadian Health Indicators Framework.MethodsWe conducted univariate correlational analyses with health and healthcare performance as outcomes using recent Canadian data and the ten Canadian provinces and three territories as units of the analyses. For health, 6 indicators were included. Sixteen healthcare performance indicators, 12 non-medical determinants of health and 16 indicators of community and health system characteristics were also included as independent variables for the analysis. A set of decision rules was applied to guide the choice of what was considered actual and preferred performance associations.ResultsHealth (28%) correlates more frequently with non-medical determinants than healthcare does (12%), in the preferred direction. Better health is only correlated with better healthcare performance in 13% of the cases in the preferred direction. Better health (24%) is also more frequently correlated with community and health system characteristics than healthcare is (13%), in the preferred direction.ConclusionCanadian health performance is a function of multiple factors, the most frequent of which may be the non-medical determinants of health and the community characteristics as against healthcare performance. The contribution of healthcare to health may be limited only to relatively small groups which stand to benefit from effective healthcare, but its overall effect may be diluted in summary measures of population health. Interpreting multidimensional, multi-indicator performance data in their proper context may be more complex than hitherto believed.

RIVM rapport:Doel van dit project is om door middel van het koppelen van bestaande registraties inzicht te krijgen in het transmuraal zorggebruik van chronisch zieken in verschillende zorgsectoren (huisartsen- en ziekenhuiszorg). Op deze manier wordt getracht meer inzicht te verschaffen in de afstemming en samenhang van de verschillende schakels in de (keten)zorg van chronisch zieken. Dit rapport beschrijft een geanonimiseerde koppeling van huisartsgegevens aan landelijke ziekenhuisregistraties. De koppeling is uitgevoerd voor een aantal chronische ziekten: diabetes mellitus, artrose, lage rugklachten, nek- en schouderklachten, heup- en knieklachten, coronaire hartziekten, beroerte (inclusief TIA), hypertensie, astma en long emfyseem. De koppeling is uitgevoerd op basis van de volgende koppelsleutel: geboortedatum, 4-cijferige postcode en geslacht van de patient. In de koppeling kan 87% van de patienten, die door de huisarts is doorverwezen naar de tweede lijn, gekoppeld worden aan de ziekenhuisregistraties. Dit rapport beschrijft het gebruik van huisartsen- en ziekenhuiszorg van chronisch zieken. Men zou verwachten dat de diagnose waarmee de patient door de huisarts is doorverwezen naar het ziekenhuis gelijk is aan de diagnose die wordt geregistreerd in het ziekenhuis bij ontslag. Bij het grootste deel van de patienten is dit het geval, maar de overeenstemming is niet 100%. Bijvoorbeeld voor artrose en voor heup- en knieklachten is het overeenstemmingspercentage ruim 60%. Na ontslag uit het ziekenhuis heeft ongeveer de helft (47%) binnen een week contact met de huisarts., This report focuses on the health care utilisation of chronically ill people by linking data on referrals by general practitioners (GPs) with data on health care utilisation in the Netherlands. The GP data were obtained from the Second Dutch National survey of General Practice, carried out by the National Institute of Health Services Research (Nivel) and the National Institute for Public Health and the Environment (RIVM). Data on hospital utilisation were derived from the National Medical Register (LMR) and the National Register of ambulatory care (LAZR). Prismant maintains both databases. The linkage was carried out for a selection of patients with a referral by their GP and with one of the following chronic diseases: diabetes mellitus, coronary heart disease, stroke, hypertension, arthrosis, chronic low back pain, chronic hip and knee complains, chronic neck and shoulder complains, chronic obstructive pulmonary disease (COPD) and asthma. In the absence of a unique identifier in the Netherlands, the record linking was carried out using the combination of date of birth, gender and four-digit postal code of the patients. Of all patients referred by their GP to a medical specialist 87% could be linked to a hospital admission, an outpatient treatment or an outpatient visit. This report presents the multidisciplinary health care utilisation of chronically ill patients. We investigated also to what extent the diagnosis of the GP matches the diagnosis at discharge from the hospital. For the greater part, the diagnosis matches to the hospital diagnosis. For patients with arthrosis and hip and knee complains the diagnosis of the GP matches in 60% of the cases with the diagnosis at discharge from the hospital. In addition to that, we also studied the follow-up by GPs for patients with a hospital admission. GPs have contact with 47% of the chronically ill patients within a week after hospital discharge.

RIVM rapport:Het Ministerie van VWS wil de kwaliteit van de diabeteszorg verhogen door middel van het verbeteren van de samenwerking tussen zorgverleners. Deze samenwerking tussen zorgverleners (zoals huisartsen, diabetesverpleegkundigen, internisten, dietisten, etc.) vindt plaats in transmurale diabeteszorgprojecten. In de afgelopen jaren is er al een aantal van zulke initiatieven op dit gebied ondernomen door zorgverleners zelf. Het Ministerie heeft het RIVM gevraagd om te onderzoeken in hoeverre de reeds bestaande samenwerkingsverbanden leiden tot een betere zorg voor mensen met diabetes. Transmurale diabeteszorg leidt er toe dat mensen met diabetes vaker worden gecontroleerd. Daarnaast is onderzocht wat het effect is van transmurale diabeteszorg voor de patienten . Er wordt daarbij gekeken naar de hoogte van de bloedsuikerspiegel (HBA1c), bloeddruk, cholesterol en lichaamsgewicht. Door de transmurale zorg is de bloedsuikerspiegel verlaagd en lijkt ook het cholesterol gehalte te dalen. Het effect van transmurale zorg op het ontwikkelen van complicaties van diabetes mellitus zoals diabetische voet, oogaandoeningen of hart- en vaatziekten is nog onduidelijk vanwege de korte follow-up duur in de beschikbare gegevens. Het is aannemelijk dat de verlaagde bloedsuikerspiegel en cholesterol op termijn leiden tot een afname in diabetescomplicaties. De onderzochte factoren zijn immers risicofactoren voor complicaties van diabetes. Ondanks de al gevonden verbeteringen in de onderzochte transmurale diabeteszorgprojecten is er nog ruimte voor verdere verbetering, met name op het gebied van de preventie van risicofactoren voor hart- en vaatziekten (bloeddruk en lichaamsgewicht)., The Ministry of Health wants to improve the quality of diabetes care in the Netherlands by improving the coordination between health care providers. This coordination between health care providers (like general practitioners, specialised diabetes nurses, specialist and dieticians, etc.) takes place in transmural diabetes care projects. During the last years, many transmural diabetes care projects were already set up by the health care providers themselves. The Ministry of Health has asked the RIVM to investigate to what extent the diabetes transmural care projects have improved the care of patients with diabetes mellitus. Transmural diabetes care result in a better control of patients with diabetes mellitus. In addition, we investigate what the effects of transmural care were on patient outcomes like blood glucose level (Hba1c), blood pressure, cholesterol and bodyweight (BMI). Transmural care lowers the Hba1c levels and probably also have positive effects on the cholesterol level of patients with diabetes mellitus. We could be, however, less conclusive about on cardiovascular risk factors and diabetes related complications like a diabetic foot, eye complications or cardiovascular diseases. It is likely that good Hba1c levels and cholesterol levels will lower the number of diabetes related complications, since high Hba1c and cholesterol levels are risk factors for diabetes related complications. Although transmural care project has resulted in improvements in Hba1c and cholesterol levels, still improvements can be made, mainly on the prevention of cardiovascular risk factors (blood pressure and body weight).
