Objetivo Identificar la medicación potencialmente inapropiada en adultos mayores en un hospital del caribe colombiano. Métodos Estudio observacional descriptivo de corte transversal prospectivo, que incluyó a 181 adultos mayores (≥60 años de edad), internados en un hospital universitario del caribe colombiano. La medicación fue evaluada a través de la aplicación de los criterios STOPP contenidos en la herramienta STOPP/START. Resultados El promedio de edad fue de 75,14 años con DE +/- 9,09 años. La mayoría de los pacientes fueron de sexo masculino (50,30%). El 30,94 % de los pacientes estudiados presentó al menos un criterio de medicación potencialmente inapropiada. El criterio más prevalente fue el uso de benzodiazepinas dentro de los fármacos que aumentan en forma predecible el riesgo de caídas en personas mayores con un 22,00 %. La polimedicación y presencia de enfermedades crónicas fueron variables relacionadas con la presencia de medicación potencialmente inapropiada, con un OR= 5,14 (IC95% 1,15 – 22,82) y OR= 4,41 (IC95% 1,27 – 15,30), respectivamente. Conclusión La MPI es un problema frecuente en los adultos mayores, especialmente en aquellos que presentan enfermedades crónicas y polimedicación.

BackgroundDiabetic retinopathy (DR) and diabetic macular edema (DME) are potentially blinding, microvascular retinal diseases in people with diabetes mellitus. Preclinical studies support a protective role of the hormone prolactin (PRL) due to its ocular incorporation and conversion to vasoinhibins, a family of PRL fragments that inhibit ischemia-induced retinal angiogenesis and diabetes-derived retinal vasopermeability. Here, we describe the protocol of an ongoing clinical trial investigating a new therapy for DR and DME based on elevating the circulating levels of PRL with the prokinetic, dopamine D2 receptor blocker, levosulpiride.MethodsIt is a prospective, randomized, double-blind, placebo-controlled trial enrolling male and female patients with type 2 diabetes having DME, non-proliferative DR (NPDR), proliferative DR (PDR) requiring vitrectomy, and DME plus standard intravitreal therapy with the antiangiogenic agent, ranibizumab. Patients are randomized to receive placebo (lactose pill, orally TID) or levosulpiride (75 mg/day orally TID) for 8 weeks (DME and NPDR), 1 week (the period before vitrectomy in PDR), or 12 weeks (DME plus ranibizumab). In all cases the study medication is taken on top of standard therapy for diabetes, blood pressure control, or other medical conditions. Primary endpoints in groups 1 and 2 (DME: placebo and levosulpiride), groups 3 and 4 (NPDR: placebo and levosulpiride), and groups 7 and 8 (DME plus ranibizumab: placebo and levosulpiride) are changes from baseline in visual acuity, retinal thickness assessed by optical coherence tomography, and retinal microvascular abnormalities evaluated by fundus biomicroscopy and fluorescein angiography. Changes in serum PRL levels and of PRL and vasoinhibins levels in the vitreous between groups 5 and 6 (PDR undergoing vitrectomy: placebo and levosulpiride) serve as proof of principle that PRL enters the eye to counteract disease progression. Secondary endpoints are changes during the follow-up of health and metabolic parameters (blood pressure, glycated hemoglobin, and serum levels of glucose and creatinine). A total of 120 patients are being recruited.DiscussionThis trial will provide important knowledge on the potential benefits and safety of elevating circulating and intraocular PRL levels with levosulpiride in patients with DR and DME.Ethics and disseminationEthics approval has been obtained from the Ethics Committees of the National University of Mexico (UNAM) and the Instituto Mexicano de Oftalmología, I.A.P. In order to assess if an association exists between the risk of major depression (RMD) and physical activity (PA), controlling for demographic and academic variables in workers enrolled in undergraduate studies at a private university in Lima, Peru, we carried out a cross-sectional study of 1,111 people. We used the Major Depression Inventory (MDI) and the International Physical Activity Questionnaire (IPAQ) to measure RMD and PA, respectively. RMD prevalence was 4.2%. In the multiple regression model adjusted for age, gender, unemployment and hours of sleep, low levels of PA were associated with increased odds of RDM (OR: 2.15, 95% CI: 1.16 to 4.00). We conclude that there is an association between RMD and PA in the study population, independent of demographic and academics factors. Strategies to improve screening and development of longitudinal studies to assess causality are suggested.

Con el objetivo de evaluar si existe asociación entre el riesgo de depresión mayor (RDM) y la actividad física (AF) controlado por variables sociodemográficas y académicas en trabajadores que cursan estudios de pregrado en una universidad privada de Lima, se desarrolló un estudio de corte transversal analítico en 1111 personas. Se utilizó el inventario de depresión mayor y el cuestionario internacional de actividad física, para medir RDM y AF, respectivamente. La prevalencia de RDM fue 4,2 %. En el modelo de regresión de múltiples variables - ajustado por edad, sexo, desempleo y horas de sueño- la AF baja se asocia con un incremento de la odds de RDM (OR 2,15; IC 95%:1,16-4,00). Se concluye que existe asociación entre la RDM y la AF en la población estudiada, la cual es independiente de factores sociodemográficos y académicos. Se sugieren mejorar estrategias de tamizaje y el desarrollo de estudios longitudinales para evaluar causalidad.

Introduction: An amebic liver abscess is the most common presentation of extraintestinal amebiasis. This condition is the result of a parasite infection caused by Entamoeba histolytica. Materials and Methods: We report a case of a 53-year-old male who presented with abdominal pain in the right upper quadrant, jaundice, and a 10-kg weight loss within a 1-month span. Results and Conclusion: A wide range of symptoms and findings in the imaging tests suggestive of neoplasia, elevated levels of CA 19-9 and CA 125, and the presentation of biliary peritonitis as a complication makes this case a challenge for its approach and management.
