Central nervous system (CNS) injuries, including stroke, traumatic brain injury, and spinal cord injury, are leading causes of long-term disability. It is estimated that more than half of the survivors of severe unilateral injury are unable to use the denervated limb. Previous studies have focused on neuroprotective interventions in the affected hemisphere to limit brain lesions and neurorepair measures to promote recovery. However, the ability to increase plasticity in the injured brain is restricted and difficult to improve. Therefore, over several decades, researchers have been prompted to enhance the compensation by the unaffected hemisphere. Animal experiments have revealed that regrowth of ipsilateral descending fibers from the unaffected hemisphere to denervated motor neurons plays a significant role in the restoration of motor function. In addition, several clinical treatments have been designed to restore ipsilateral motor control, including brain stimulation, nerve transfer surgery, and brain–computer interface systems. Here, we comprehensively review the neural mechanisms as well as translational applications of ipsilateral motor control upon rehabilitation after CNS injuries.

BackgroundWhile the contralateral seventh cervical nerve (CC7) cross transfer was designed to reconstruct paralyzed arm function after stroke, improvement in language function was found in patients combined with aphasia.ObjectiveTo evaluate the effect of improvement in language function after CC7 cross transfer in stroke patients with chronic aphasia and explore its potential mechanism.MethodsIn a prospective observative cohort, patients diagnosed with hemiplegia combined with aphasia were included. The language function was evaluated through the changes of Aphasia Quotient evaluated by Western Aphasia Battery (WAB-AQ) as well as its four subtests from baseline to 1 week and 6 months after the surgery. Patients also received oral agility test by Boston Diagnostic Aphasia Examination (BDAE-OA). Resting-state functional MRI (rs-fMRI) was scanned before and over 6 months after the surgery to explore the potential central mechanism in language improvements.ResultsThe average increase of WAB-AQ was 8.08 points from baseline to 1 week post-operatively (PConclusionsImmediate and stable improvement in language function was detected after CC7 cross transfer in hemiplegic patients combined with aphasia, which may be realized through enhanced function of language network in the bilateral hemisphere.

To improve the application of molten salt method in carbon fibers (CFs) surface modification, the work explores the effect of the CFs-itself crystallinity on the formation and growth mechanism of TiC coating in the molten salt reaction. Three different types of CFs were used to prepare TiC coatings at 850°C, and the formation and growth mechanisms were studied. The results showed that CFs surface defects could preferentially initiate nucleation and cause inhomogeneity of TiC particles in the initial stage of TiC coating formation. The CFs surface with high crystallinity could obtain a finer and more uniform TiC coating because the growth rate of TiC particles is slower and the particles do not accumulate with each other during the coating growth. At the same time, the growth rate of TiC coating decreased with increasing CFs crystallinity during the TiC coating growth, which reason is that CFs with higher crystallinity require more activation energies of carbon atom counter diffusion. Therefore, the surface of CFs with higher crystallinity and fewer surface defects is easier to obtain a uniform and particle size TiC coating during the molten salt reaction.

BackgroundPost-stroke aphasia is a common but intractable sequela which still needs new and more effective treatments. Evidence from follow-ups after contralateral seventh cervical nerve transfer surgery indicated that nerve transection leads to immediate language improvements in patients with right post-stroke aphasia.ObjectiveThrough a prospective cohort design, this study aims to prove that C7 neurotomy at the intervertebral foramen (NC7) combined with a 3-week intensive speech and language therapy (iSLT) can improve the language function in post-stroke aphasia patients.MethodsIn this study, patients aged over 18 years old and had been diagnosed with post-stroke aphasia for 1 year or longer were included. Primary outcomes were the change in the ability to retrieve personally relevant words in Boston Naming Test (BNT) with follow-up assessment after three-weeks’ iSLT post-operatively. As well as several secondary outcome measures including the Western Aphasia Battery (WAB), daily communication abilities (measured by the Communication Activities of Daily Living Third Edition [CADL-3]) and Fugl-Meyer of upper limb part (UEFM).ResultsThe average increase of BNT score was 11.2 points from baseline to 3 weeks post-operatively (P=0.001, 95%CI: 8.1-14.1). The WAB and CADL-3 assessment showed 9.4, 10.4 points increasing in average (PConclusionsNC7 plus iSLT significantly improved the language function in patients with post-stroke aphasia, and did not significantly affect the motor function of the right limb. The mechanism of this surgery needs to be further explored.

Background Contralateral C7 (CC7) nerve transfer has successfully restored hand function in patients with spastic hemiplegia from chronic central nervous system injuries. However, little is known about the morphology and anatomy of the donor C7 nerve root in patients undergoing this procedure. This study quantified intraoperative measurements of donor C7 nerve roots during CC7 transfer surgery for spastic hemiplegia in patients treated at a high-volume center to describe observed anatomical variations for successful direct anastomosis. Methods A database of images from 21 patients (2 females, 19 males) undergoing CC7 surgery was searched for photographic data that contained a standard ruler measuring donor C7 nerve root length after surgical sectioning and before transfer. Two independent observers analyzed these images and recorded C7 nerve root diameter, length, and branch lengths. Results Mean (SD) values of donor C7 nerve measurements were length, 53.5 (8.0) mm; diameter, 5.1 (0.9) mm; branch length following surgical sectioning, 18.3 (6.3) mm. Right-sided donor C7 nerve roots yielded significantly longer branches compared with left-sided donor C7 nerve roots (P = 0.01). Other patient factors such as age, sex, or laterality of brain injury did not influence intraoperative anatomy. Conclusions We report detailed intraoperative measurements of the donor C7 root during CC7 nerve transfer for spastic hemiplegia. These findings describe existing variation in surgical C7 nerve root anatomy in patients undergoing this procedure and may serve as a general reference for the expected donor C7 length in successful direct anastomosis.

Restoring limb movements after central nervous system injury remains a substantial challenge. Recent studies proved that crossing nerve transfer surgery could rebuild physiological connectivity between the contralesional cortex and the paralyzed arm to compensate for the lost function after brain injury. However, the neural mechanism by which this surgery mediates motor recovery remains still unclear. Here, using a clinical mouse model, we showed that this surgery can restore skilled forelimb function in adult mice with unilateral cortical lesion by inducing cortical remapping and promoting corticospinal tract sprouting. After reestablishing the ipsilateral descending pathway, resecting of the artificially rebuilt peripheral nerve did not affect motor improvements. Furthermore, retaining the sensory afferent, but not the motor efferent, of the transferred nerve was sufficient for inducing brain remapping and facilitating motor restoration. Thus, our results demonstrate that surgically rebuilt sensory input triggers neural plasticity for accelerating motor recovery, which provides an approach for treating central nervous system injuries.

IntroductionAphasia affects many stroke survivors; therefore, effective treatments are urgently needed. Preliminary clinical findings have suggested an association between contralateral C7–C7 cross nerve transfer and recovery from chronic aphasia. Randomised controlled trials supporting the efficacy of C7 neurotomy (NC7) are lacking. This study will explore the efficacy of NC7 at the intervertebral foramen for improving chronic poststroke aphasia.Methods and analysisThis study protocol reports a multicentre, randomised, assessor-blinded active-controlled trial. A total of 50 patients with chronic poststroke aphasia for over 1 year and with a aphasia quotient calculated by Western Aphasia Battery Aphasia Quotient (WAB-AQ) score below 93.8 will be recruited. Participants will be randomly assigned to 1 of 2 groups (25 individuals each) to receive NC7 plus intensive speech and language therapy (iSLT), or iSLT alone programme. The primary outcome is the change in Boston Naming Test score from baseline to the first follow-up after NC7 plus 3 weeks of iSLT or iSLT alone. The secondary outcomes include the changes in the WAB-AQ, Communication Activities of Daily Living-3, International Classification of Functioning, Disability and Health (ICF) speech language function, Barthel Index, Stroke Aphasic Depression Questionnaire-hospital version and sensorimotor assessments. The study will also collect functional imaging outcomes of naming and semantic violation tasks through functional MRI and electroencephalogram to evaluate the intervention-induced neuroplasticity.Ethics and disseminationThis study was approved by the institutional review boards of Huashan Hospital, Fudan University, and all participating institutions. The study findings will be disseminated through peer-reviewed publications and conference presentations.Trial registration numberChiCTR2200057180.

Based on a survey of 2343 rural residents in China, this paper adopts a binary logistic regression model as the analytical tool to study the impact of rural residents’ social network and sense of responsibility regarding their participation in environmental governance. The results show that the cost, frequency and scope of social network activities have positive and significant influences on resident participation in rural environmental governance. The cost of a social network is conducive to building a rural social network, enhancing the connection of interests and promoting the formation of a rural community. Extending social network objectives from family members to villagers can improve the cultural identity and emotional identity of rural residents. The increase in the frequency of social network activities can not only enhance trust among residents, but also reduce the cost of environmental governance mobilization. The scope of a social network acts as an inhibitor whereby social interaction beyond the scope of rural areas will reduce identification with rural emotions. The four dimensions, including responsibility cognition, responsibility will, responsibility emotion and responsibility behavior have significant influences on resident participation in rural environmental governance. Residents’ sense of responsibility plays the role of an introverted driving force for them to take part in rural environmental governance, which itself helps to overcome “non-participation” behaviors of “rational smallholders” to a certain extent. Furthermore, it endows rural environmental governance with resilience. So, it is of significance to enhance rural residents’ social networks and to improve rural residents’ cognition of collective responsibility.

Photoreceptor degeneration diseases, such as retinitis pigmentosa and age-related macular degeneration, are major causes of blindness. Photovoltaic devices or materials activates retinal cells in response to light, hence are promising retinal prosthesis in treating blind patients. The restoration of image-forming vision with high sensitivity is the key. Here we demonstrated that Au nanoparticle-coated titania (TiO2-x) nanowire (NW) arrays can restore vision in blind mice and non-human primates with photoreceptor degeneration. We first showed that blind mice with subretinal implant of NW arrays were capable of detecting static, moving and flashing objects with low light intensity threshold for 22 months, having a visual acuity of 0.3 cpd (0.4 cpd in normal mice). Results in chronic in vivo calcium imaging in primary visual cortex (V1) collectively suggested the plastic change in V1 neurons as well as the improvement in visual information encoding for natural images after NW implant. What’s more, macaque monkeys with subretinal implant of NW arrays were capable of detecting an LED of 0.5º in diameter at 10 μW·mm-2 in visually-guided saccade experiments. Our findings opened up the possibility to utilize nanomaterials as artificial photoreceptors to ameliorate visual deficits of patients with photoreceptor degeneration.

Sandalwood oil has been widely used in perfumery industries and aromatherapy. Santalols are its major components. Herein, we attempted to construct santalol-producing yeasts. To alter flux from predominant triterpenoid/steroid biosynthesis to sesquiterpenoid production, expression of ERG9 (encoding yeast squalene synthase) was depressed by replacing its innate promotor with PHXT1 and fermenting the resulting strains in galactose-rich media. And the genes related to santalol biosynthesis were overexpressed under control of GAL promotors, which linked santalol biosynthesis to GAL regulatory system. GAL4 (a transcriptional activator of GAL promotors) and PGM2 (a yeast phosphoglucomutase) were overexpressed to overall promote this artificial santalol biosynthetic pathway and enhance galactose uptake. 1.3 g/L santalols and 1.2 g/L Z-α-santalol were achieved in the strain WL17 expressing SaSS (α-santalene synthase from Santalum album) and WL19 expressing SanSyn (α-santalene synthase from Clausena lansium) by fed-batch fermentation, respectively. This study constructed the microbial santalol-producing platform for the first time.

Corticospinal neurons (CSNs) undertake direct cortical outputs to the spinal cord and innervate the upper limb through the brachial plexus. Our previous study has shown that the contralateral middle trunk transfer to the paralyzed upper extremity due to cerebral injury can reconstruct the functional cerebral cortex and improve the function of the paralyzed upper extremity. To interpret the cortical reconstruction and the motor improvement after the middle trunk transfer, we explored the distribution of CSNs connecting to the middle, upper, and lower trunk of the brachial plexus by retrograde trans-neuronal tracing using pseudorabies virus (PRV-EGFP or PRV-mRFP). We show that, rather than an individual specific area, these CSNs labelled by each trunk of the brachial plexus were widespread and mainly assembled within the primary motor cortex (M1), secondary motor cortex (M2), primary somatosensory cortex (S1), and slightly within the secondary somatosensory cortex (S2). The three trunk-labelled CSNs were intermingled in these cortices, and mostly connected to more than two trunks, especially the middle trunk-labelled CSNs with higher proportion of co-labelled neurons. Our findings revealed the distribution features of CSNs connecting to the adjacent spinal nerves that innervate the upper limb, which can improve our understanding of the corticospinal circuits associated with motor improvement and the functional cortical reconstruction after the middle trunk transfer.

Rural health governance is an important part of low-carbon green life, which is also related to the sustainable development and population health project in rural areas. Based on the survey data of 2343 rural residents in China, this study adopted a binary logistic regression model to explore the effects of rural residents’ social network and cognition on their participation in rural health governance. The research results show that only less than 30% of the respondents participated in rural health governance, and the proportion of rural resident participating is not high. Both their social network and cognition have a significant impact on their participation in rural health governance. Introverted communication helps strengthen the connection between rural residents, form the network and structure of rural social communication, build emotional links and common interests, and form a common cultural understanding paradigm and action framework. The extraverted communication means that rural residents gradually break away from the social network of acquaintances, which is not conducive to building a rural community. Rural residents’ understanding of behavior begins to deviate from rural culture, customs and emotional values, and the binding force of traditional culture is reduced, making it difficult to motivate them to participate in rural public life. Policy cognition can improve rural residents’ recognition of the value and significance of health governance. Responsibility awareness is the internal driving force for rural residents to participate in health governance, which can also reduce the governance cost of rural managers. Based on this, increasing rural residents’ introverted communication and cultivating their sense of responsibility are key to promoting their participation in rural health governance.

In the post-pandemic era, the need for resilient and flexible COVID-19 prevention strategies in rural areas has become increasingly prominent. Based on a sample of 2229 rural residents nationwide, the Structural Equation Model was adopted to analyze the influence of social capital and technological empowerment on pandemic resilience in rural areas. The proportion of diversity, adequacy, and effectiveness of pandemic prevention measures taken by communities was about 57%. Social capital (0.667) and technological empowerment (0.325) had a significant positive impact on rural resilience and pandemic prevention. Social capital plays a mediating role between technological empowerment and pandemic resilience in rural areas. The risk of disease in society stimulates the inherent social capital factors in villages, with the individual social network generating strong social support. Technological empowerment can not only provide new methods for the connection of social capital, but also bring new means for rural authorities to improve their governance capabilities. Social trust in social capital plays an important role in rural resilience and pandemic prevention. The indirect effect of technological empowerment through social capital on pandemic resilience is greater than its direct effect. Social capital construction is the key to rural resilience and pandemic prevention.

Photoreceptor degeneration diseases, such as retinitis pigmentosa and age-related macular degeneration, are major causes of blindness. Here we evaluate the performance of Au nanoparticle-coated titania (TiO2-x) nanowire (NW) arrays as artificial photoreceptors to restore image-forming vision in blind mice and non-human primates with photoreceptor degeneration. We first demonstrated that blind mice with the subretinal implant of NW arrays were capable of detecting static, moving and flashing objects, with a visual acuity of 0.3 cpd, measured up to 9 months after the implant surgery. Using chronic in vivo calcium imaging, we continuously monitored light-induced responses of neurons in the primary visual cortex (V1) in NW-implanted blind mice for over 14 weeks. Light responses of V1 neurons first appeared at 2 days after the surgery and the number of light-responsive V1 neurons peaked 1 week after the implant surgery, and the latency of these responses decreased over time. These results collectively suggested the plasticity and improvement in the encoding of visual information in V1 after NW implant surgery. We then implanted NW arrays subretinally in macaque monkeys, showing that the NW arrays did not cause adverse responses 8 weeks post surgery. Finally, visually-guided saccades were restored in macaque monkeys with the subretinal implant of NW arrays. Our findings demonstrated the possibility to utilize nanomaterials as artificial photoreceptors to ameliorate visual deficits of patients with photoreceptor degeneration.

Our previous investigation suggested that faster seventh cervical nerve (C7) regeneration occurs in patients with cerebral injury undergoing contralateral C7 transfer. This finding needed further verification, and the mechanism remained largely unknown. Here, Tinel's test revealed faster C7 regeneration in patients with cerebral injury, which was further confirmed in mice by electrophysiological recordings and histological analysis. Furthermore, we identified an altered systemic inflammatory response that led to the transformation of macrophage polarization as a mechanism underlying the increased nerve regeneration in patients with cerebral injury. In mice, we showed that, as a contributing factor, serum amyloid protein A1 (SAA1) promoted C7 regeneration and interfered with macrophage polarization in vivo. Our results indicate that altered inflammation promotes the regenerative capacity of the C7 nerve by altering macrophage behavior. SAA1 may be a therapeutic target to improve the recovery of injured peripheral nerves.

Extracellular glutamate contributes to brain damage in ischemia. Under physiological conditions, glutamate transporters are responsible for regulating its intracellular/extracellular concentrations in the brain. However, how the extracellular glutamate is regulated in ischemia remains unclear. Here, we showed that the sonic hedgehog (SHH)–Smoothened (SMO)–GLT-1 pathway controlled extracellular glutamate and blocking SMO reduced ischemic brain damage in rodents. SHH was quickly released in a rodent model of ischemia, and activation of its pathway was associated with neuronal damage. Inhibiting SMO, the mediator of SHH signaling, maintained GLT-1 membrane expression, lowered extracellular glutamate, reduced infarct volume, and improved neurological functions in mice. Mechanistically, SHH suppressed GLT-1 membrane expression via PKCα phosphorylation of serine-562 on GLT-1. Last, administration of NVP-LDE225, an FDA-approved SMO antagonist used for cancer treatment in clinic, had protective effects in mice and cynomolgus monkeys subjected to ischemia. Together, these results suggest that SMO could be targeted for treating glutamate toxicity in ischemia.

Plasticity of neurons in the ventral tegmental area (VTA) is critical for establishment of drug dependence. However, the remodeling of the circuits mediating the transition between positive and negative effect remains unclear. Here, we used neuronal activity-dependent labeling technique to characterize and temporarily control the VTA neuronal ensembles recruited by the initial morphine exposure (morphine-positive ensembles, Mor-Ens). Mor-Ens preferentially projected to NAc, and induced dopamine-dependent positive reinforcement. Electrophysiology and rabies viral tracing revealed the preferential connections between the VTA-projective corticotrophin-releasing hormone (CRH) neurons of central amygdala (CRHCeA→VTA) and Mor-Ens, which was enhanced after escalating morphine exposure and mediated the negative effect during opiate withdrawal. Pharmacologic intervention or CRISPR-mediated repression of CRHR1 in Mor-Ens weakened the inhibitory CRHCeA→VTA inputs, and alleviated the negative effect during opiate withdrawal. These data suggest that neurons encoding opioid reward experience are inhibited by enhanced CRHCeA→VTA inputs induced by chronic morphine exposure, leading to negative effect during opiate withdrawal, and provide new insight into the pathological changes in VTA plasticity after drug abuse and mechanism of opiate dependence.

With the rapid development of the Internet of Things, artificial intelligence and 5G, a series of IoT technology standard protocols, such as OneM2M, have been developed by standard-setting bodies. Although smart homes are growing rapidly, they have not yet formed uniform standards and full-scale home systems. Based on this, it is proposed that the public service entity (CSE) in the oneM2M standard be layered management, MN-CSE as the core node to achieve threshold detection, and the values are forwarded to IN-CSE for in-AE data operation of the smart home deployment scheme. Experiments have verified that the solution can significantly reduce the device requirements of the home core node MN-CSE and effectively reduce the cost of intelligent home deployment maintenance.

Tears of the proximal component of the triangular fibrocartilage complex (pc-TFCC) lead to instability in the distal radioulnar joint. The aim of this study was to measure the load-bearing radioulnar distances (RaUls) and to evaluate its diagnostic suitability in patients with pc-TFCC tears.We retrospectively assessed and compared the lateral wrist radiographs of 61 adult patients with arthroscopically confirmed tears of pc-TFCC with those of a control group of 64 healthy participants. The RaUl was measured on lateral radiographs, and the difference in RaUl (D-value) between load-bearing and nonloading conditions was calculated in the 2 groups. Receiver operator characteristic curves were plotted to determine the diagnostic accuracy and optimal cutoff-score of load-bearing RaUl and RaUl D-value. The diagnostic performance was verified in a validation sample of patients (30 wrists) with pc-TFCC tears and a control group of healthy individuals (30 wrists).In the training sample, load-bearing RaUls of the affected wrists were higher than the same side of the controls (12.0 mm vs 7.1 mm). The TFCC-injury group showed a significantly higher RaUl D-value than the control group (8.5 mm vs 3.4 mm). Using a receiver operator characteristic curve, the cutoff value of load-bearing RaUl was 10 mm (sensitivity = 97.6%, specificity = 85.7%) and that of RaUl D-value was 6.5 mm (sensitivity = 90.2%, specificity = 78.5%). The areas under the curve of load-bearing RaUl and RaUl D-value were 0.96 and 0.88, respectively. In the test sample, the sensitivity, specificity, and accuracy of RaUl were 0.93, 0.70, and 0.82 and those of RaUl D-value were 0.77, 0.83, and 0.80 respectively.Load-bearing RaUl measurement is a simple method to diagnose an unstable distal radioulnar joint in patients with TFCC injury. The load bearing RaUl of10 mm or RaUl D-value of6.5 mm can be used to differentiate TFCC injuries and showed acceptable accuracy.Diagnostic II.

Background: Since January 2020, COVID-19 has spread globally to more than 210 countries, and the epidemic lasted for more than 11 months. Scientists have studied many effective prevention and control strategies. Based on real-time big data on COVID-19 pneumonia captured by the Baidu App, our study aimed to analyse the characteristics of the COVID-19 pandemic in 10 epicentre countries. It also aimed to discuss the impact of politics, economy, and humanities on COVID-19 as well as effective strategies of prevention and control the epidemic. Methods: Based on the real-time big data report of the new coronavirus pneumonia epidemic in Baidu APP, collected from January 27 to December 22, 2020. Incidence and death data in the world and China, India, Europe UK, Italy, France, Spain, Russia, America Brazil, the United States, Mexico were collected.1 Excel 2016 was used to build a database and performed linear fitting analysis, R3.6.3 and ggplot2 package was used for descriptive statistical analysis. Current status and trends of morbidity and mortality in the world and epicenter countries were analyze. Findings: To December 22, 2020, in the recent 5 days, the top two incidence cases were America with 1,079,385 cases, and the United Kingdom with 197,037 cases; the bottom two were China with 507 cases, and Mexico 48,416 cases. The top two mortality cases were America with 12,180 cases, and Brazil with 3,500 cases; the bottom two were China 7 Cases, and Spain 664 cases. The top two incidence rates were in America 0.3303%, and the United Kingdom 0.2959%;the bottom two were in China 0.0000%,and India 0.0097%.The top two mortality rates were in Italy 0.0044%, and the United Kingdom 0.0042%;the bottom two were in China 0.0000%,and India 0.0001%.From January 27 to December 22, 2020,The top two cumulative confirmed cases were in America with 18,494,265 cases, and India with 10,094,801 cases; the bottom two were China 95,998 cases, and Mexico 1,325,915 cases. The mutation of Coronavirus has made the already serious epidemic worse.
