Abstract

Left ventricular outflow tract obstruction (LVOTO) is common in hypertrophic cardiomyopathy (HCM), but relationships between anatomical metrics and obstruction are poorly understood. We aimed to develop machine learning methods to evaluate LVOTO in HCM patients and quantify relationships between anatomical metrics and obstruction. This retrospective analysis of 1905 participants of the HCM Registry quantified 11 anatomical metrics derived from 14 landmarks automatically detected on the three-chamber long axis cine CMR images. Linear and logistic regression was used to quantify strengths of relationships with the presence of LVOTO (defined by resting Doppler pressure drop of > 30 mmHg), using the area under the receiver operating characteristic (AUC). Intraclass correlation coefficients between the network predictions and three independent observers showed similar agreement to that between observers. Abstract

Hypertrophic cardiomyopathy (HCM) is a common, serious, genetic heart disorder. Rare pathogenic variants in sarcomere genes cause HCM, but with unexplained phenotypic heterogeneity. Moreover, most patients do not carry such variants. We report a genome-wide association study of 2,780 cases and 47,486 controls that identified 12 genome-wide-significant susceptibility loci for HCM. Single-nucleotide polymorphism heritability indicated a strong polygenic influence, especially for sarcomere-negative HCM (64% of cases; h2g = 0.34 ± 0.02). A genetic risk score showed substantial influence on the odds of HCM in a validation study, halving the odds in the lowest quintile and doubling them in the highest quintile, and also influenced phenotypic severity in sarcomere variant carriers. Mendelian randomization identified diastolic blood pressure (DBP) as a key modifiable risk factor for sarcomere-negative HCM, with a one standard deviation increase in DBP increasing the HCM risk fourfold. Common variants and modifiable risk factors have important roles in HCM that we suggest will be clinically actionable.

Abstract

Importance It is unknown whether coronary revascularization, when added to optimal medical therapy, improves prognosis in patients with stable ischemic heart disease (SIHD) at increased risk of cardiovascular events owing to moderate or severe ischemia. Objective To describe baseline characteristics of participants enrolled and randomized in the International Study of Comparative Health Effectiveness With Medical and Invasive Approaches (ISCHEMIA) trial and to evaluate whether qualification by stress imaging or nonimaging exercise tolerance test (ETT) influenced risk profiles. Design, Setting, and Participants The ISCHEMIA trial recruited patients with SIHD with moderate or severe ischemia on stress testing. Abstract

Although current guidelines on the management of stable coronary artery disease acknowledge that multiple mechanisms may precipitate myocardial ischemia, recommended diagnostic, prognostic and therapeutic algorithms are still focused on obstructive epicardial atherosclerotic lesions, and little progress has been made in identifying management strategies for non-atherosclerotic causes of myocardial ischemia. The purpose of this consensus paper is three-fold: 1) to marshal scientific evidence that obstructive atherosclerosis can co-exist with other mechanisms of ischemic heart disease (IHD); 2) to explore how the awareness of multiple precipitating mechanisms could impact on pre-test probability, provocative test results and treatment strategies; and 3) to stimulate a more comprehensive approach to chronic myocardial ischemic syndromes, consistent with the new understanding of this condition.

Abstract

Purpose of review: Implementation of effective interventions often requires evidence regarding value, that is, whether they are worth what we pay for them. This review explores recent evidence concerning cost-effectiveness in familial hypercholesterolemia, and discusses the cause of, and likelihood of solutions to, the paucity of such evidence. Recent findings: Cost-effectiveness analysis in familial hypercholesterolemia has been limited almost exclusively to adult populations. However, there is growing evidence that childhood intervention offers substantial benefit in terms of downstream health gains. Statin therapy in adults has been demonstrated to be cost-effective, but the range of novel agents that might be used will require de novo economic evaluation alongside exploration of their effect and safety profile. Summary: The familial hypercholesterolemia field has limited evidence regarding cost-effectiveness, which limits optimum allocation of resources. Economic evaluations are necessary to appraise new agents and optimal timing of management approaches. Evaluations often have substantial data demands; consequentially, their applicability to medical decision-making or policy will be partly determined by the availability of data, particularly those providing information about the long-term trajectory of health benefit from familial hypercholesterolemia treatment. Abstract

Согласно современной патофизиологической модели хронической ишемической болезни сердца (ИБС), ишемия миокарда и стенокардия напряжения возникают в результате наличия атеросклеротической бляшки в коронарной артерии, в связи с чем клиническое лечение ИБС сосредоточено на выявлении и устранении стеноза. Хотя данный подход применяется на практике уже много лет, некоторые факты, включая недостаточное влияние этого подхода на прогноз, свидетельствуют о том, что такая прямая связь может представлять собой слишком упрощенный взгляд на ИБС. В частности, в многочисленных исследованиях было показано, что ИБС может быть как при наличии, так и в отсутствии коронарной болезни сердца, и что атеросклероз всего лишь один из элементов в комплексном многофакторном патофизиологическом процессе, включающем также воспаление, нарушение микроциркуляции, эндотелиальную дисфункцию, тромбоз и ангиогенез. Более того, высокая частота рецидивов выделяет тот факт, что у пациентов со стабильной ИБС устранение стеноза не оказывает воздействия на лежащие в основе патологические механизмы, что ведет к прогрессированию других поражений. Предлагаемая в этом обзоре модель смещает фокус с обструктивного атеросклероза коронарных артерий на клетки миокарда, где, собственно, и возникает ишемия. Если клетку миокарда расположить в центре этой модели, появляется возможность учесть все возможные потенциальные патологические факторы и, как следствие, разработать стратегии для защиты кардиомиоцитов от ишемического повреждения, независимо от причинного механизма.

Abstract

The highly selective reactions 12C(7Li,d)17O and 12C(6Li,p)17O have been used to populate high-lying excited states in 17O up to 16 MeV in excitation. Several of the states are newly observed, and the existence of others in a previous study of 12C(6Li,p)17O is confirmed. The observed spectra show a clear gap of about 3 MeV, indicating an energy gap between 3p-2h and 5p-4h states in 17O. Differential cross section angular distributions have been extracted from the data for both reactions and they have been compared with finite-range DWBA calculations by assuming a “5He” cluster transfer. Possible spins and parities are reported for states at 11.82 MeV (7/2+), 12.00 MeV (9/2+),12.22 MeV (7/2-), and 12.42 MeV (9/2+).

Abstract

The yrast sequence in 183Tl has been studied for the first time in recoil-mass selected γ-ray spectroscopic measurements. A rotational-like cascade of seven transitions is established down to the band head with probable spin and parity (13/2+). Unlike in the adjacent odd-mass Tl nuclei, prompt γ decay from the yrast band to a lower lying weakly deformed (oblate) structure is not observed. These features are consistent with the predicted drop of the prolate band head in 183Tl compared to 185Tl. The implications for the prolate energy minimum in odd-mass Tl nuclei at the neutron i 13/2 midshell (N = 103) are discussed.

Abstract

Spectroscopic studies of weakly populated proton i13/2 bands in superdeformed Tl nuclei (around mass 190) and in the normally deformed, very light 183Tl nucleus are discussed. Among the results presented, the first measurement of a superdeformed quadrupole moment in an odd-Z nucleus, 191Tl, is reported. The experiments were conducted with the Gammasphere array as "stand-alone" device and coupled with the Argonne Fragment Mass Analyzer.

Abstract

Two sets of strongly coupled rotational bands have been identified in Zn-62. These bands have been observed up to the terminating states of their respective configurations. Lifetime measurements indicate that the transition quadrupole moments in these bands decrease as termination is approached. These results establish the first terminating states of rotational bands in the A similar to 60 mass region and confirm the predicted loss of collectivity associated with smooth band termination.

Abstract

A high-spin rotational cascade of six gamma rays has been observed in Zn-62. The quadrupole moment of 2.7(-0.5)(+0.7) e b measured for this band corresponds to a deformation beta(2) = 0.45(-0.07)(+0.10). The properties of this band are in excellent agreement with calculations that predict high-spin superdeformed bands in Zn-62 with deformations beta(2) = 0.41-0.49. These results establish a new region of superdeformation for nuclei with neutron and proton numbers N, Z approximate to 30-32.
