Abstract

Postnatally acquired cytomegalovirus (CMV) is commonly acquired via breast milk, with premature infants more frequently developing symptoms of CMV infection in comparison to term infants. Meningitis is a rare clinical manifestation of CMV infection. The diagnosis of meningitis is difficult to make in infants, particularly those who are preterm. Consequentially, broad-spectrum empiric antimicrobial coverage is often administered for several days while waiting for current gold standard CSF testing to result. The BioFire FilmArray (BFA) simultaneously tests for 14 different pathogens, including CMV, allowing for quicker diagnosis and shorter time to definitive treatment. Here, we report a very low birth weight infant with postnatally acquired CMV meningitis, the first to our knowledge to be diagnosed using the BioFire FilmArray. [ABSTRACT FROM AUTHOR]

Contains fulltext : 200176.pdf (Publisher’s version ) (Open Access), BACKGROUND: Placebo and nocebo effects occur in clinical or laboratory medical contexts after administration of an inert treatment or as part of active treatments and are due to psychobiological mechanisms such as expectancies of the patient. Placebo and nocebo studies have evolved from predominantly methodological research into a far-reaching interdisciplinary field that is unravelling the neurobiological, behavioural and clinical underpinnings of these phenomena in a broad variety of medical conditions. As a consequence, there is an increasing demand from health professionals to develop expert recommendations about evidence-based and ethical use of placebo and nocebo effects for clinical practice. METHODS: A survey and interdisciplinary expert meeting by invitation was organized as part of the 1st Society for Interdisciplinary Placebo Studies (SIPS) conference in 2017. Twenty-nine internationally recognized placebo researchers participated. RESULTS: There was consensus that maximizing placebo effects and minimizing nocebo effects should lead to better treatment outcomes with fewer side effects. Experts particularly agreed on the importance of informing patients about placebo and nocebo effects and training health professionals in patient-clinician communication to maximize placebo and minimize nocebo effects. CONCLUSIONS: The current paper forms a first step towards developing evidence-based and ethical recommendations about the implications of placebo and nocebo research for medical practice, based on the current state of evidence and the consensus of experts. Future research might focus on how to implement these recommendations, including how to optimize conditions for educating patients about placebo and nocebo effects and providing training for the implementation in clinical practice.

16. Psychophysiological characteristics of abdominal pain in children and adolescents: A systematic review of literature

Abstract

Background. Frequent abdominal pain (AP) in children and adolescents is often designated as functional gastrointestinal disorder. In contrast to research on psychological and social influences on the experience of AP in this population, psychophysiological features such as function of the autonomic nervous system, the central nervous system, or the endocrine system have rarely been studied. Methods. We conducted a systematic literature search for peer-reviewed journal articles referring to children with AP between 4 and 18 years. Studies on experimental baseline characteristics or reactivity of psychophysiological outcome parameters (autonomous nervous system, central nervous system, and endocrine parameters) were included. Key Results. Twelve of 18 included studies found psychophysiological differences between children with AP and healthy ones. These studies indicate a possible autonomic dysregulation and hypersensitivity of the central nervous system in children with AP following stimulation with stress or other intense stimuli. Mainly conflicting results were found regarding baseline comparisons of autonomic and endocrine parameters. Conclusions and Inferences. Frequent AP in children may be associated with an altered psychophysiological reaction on intense stimuli. It has to be considered that the current literature on psychophysiological characteristics of childhood AP is small and heterogeneous. In particular, multiparameter studies using validated experimental paradigms are lacking.

36. Effects of Various Ratios of Corn and Corn Silage and Slaughter Weight on the Performance of Steers Individually Fed

Abstract

Fifty-six Hereford steers were used in this experiment designed to determine the effects of different combinations of corn and corn silage and slaughter weight on cattle performance. Eight of the calves were slaughtered initially (227 kg) to determine beginning composition and the other 48 were assigned in equal numbers to the 12 treatments. The rations fed were isonitrogenous and included the following ratios of corn to corn silage (dry basis): (A) 30:70; (B) 50:50; (C) 70:30; and (D) 80:20. Ending weights were: (1) 341 kg; (2) 454 kg; and (3) 545 kilograms. The steers were individually fed throughout the test. Bomb calorimeter determinations were made on the feed and body tissues to enable the calculation of energetic efficiency.Steers fed the high silage A ration were slower gaining (P<.05) averaging .90 kg per day. Although cattle performance on the other rations was quite similar (1.06, 1.13 and 1.11 kg/day for B, C and D, respectively) feeding a higher level of corn grain than that of ration C does not appear advantageous. Most carcass traits were quite similar; however the fastest gaining, ration C cattle did produce the fatter, higher grading carcasses.Gross energetic efficiency (Meal energy gained/Mcal G.E. consumed) increased as slaughter weight increased with peak conversions of 16.5 and 11.9% for empty body and carcass gains, respectively. Hence, the fattening process appears to be a more efficient process (calorically) than the growth process (protein deposition).

37. Effects of Ration Energy and Slaughter Weight on Composition of Empty Body and Carcass Gain of Beef Cattle

Abstract

Empty body and carcass protein, fat, water and ash were determined on 56 Hereford steers representing four slaughter groups. Eight calves were slaughtered at 227 kilograms. The other 48 were fed one of four rations and slaughtered at either 341, 454 or 545 kilograms. The rations consisted of the following ratios of corn to corn silage (dry basis): (A) 30:70; (B) 50:50; (C) 70:30; and (D) 80:20. All 12 treatments were represented by an equal number of experimental animals. Composition of empty body and carcass gain were determined via the comparative slaughter technique. Caloric gains were computed using published gross energy values for protein and fat.The composition of empty body and carcass gain for a given weight were not affected (P<.05) by ration. Fat gain, as a percentage of empty body and carcass gain ranged from 25.11 to 44.02% and 27.48 to 48.57%, respectively, as slaughter weight increased from 341 to 545 kilograms. The greatest increase in fat content occurred after the cattle reached 341 kilograms. Fat, as a percentage of empty body and carcass gain from 454 to 545 kg was 63.37 and 68.26%, respectively. Energy gain per day was less (P<.05) for the cattle fed ration A to 545 kilograms.The following equation was developed to predict empty body weight: −13.577 + .972X, where × refers to kilograms if shrunk live weight (R2=.998).
