1. WeatherProof: Leveraging Language Guidance for Semantic Segmentation in Adverse Weather

Abstract

We propose a method to infer semantic segmentation maps from images captured under adverse weather conditions. We begin by examining existing models on images degraded by weather conditions such as rain, fog, or snow, and found that they exhibit a large performance drop as compared to those captured under clear weather. To control for changes in scene structures, we propose WeatherProof, the first semantic segmentation dataset with accurate clear and adverse weather image pairs that share an underlying scene. Through this dataset, we analyze the error modes in existing models and found that they were sensitive to the highly complex combination of different weather effects induced on the image during capture. To improve robustness, we propose a way to use language as guidance by identifying contributions of adverse weather conditions and injecting that as "side information". Models trained using our language guidance exhibit performance gains by up to 10.2% in mIoU on WeatherProof, up to 8.44% in mIoU on the widely used ACDC dataset compared to standard training techniques, and up to 6.21% in mIoU on the ACDC dataset as compared to previous SOTA methods., Comment: arXiv admin note: substantial text overlap with arXiv:2312.09534

Abstract

IntroductionThe purpose of this study was to quantitatively evaluate the contribution of synovial lymphoid aggregates to autoantibody (rheumatoid factor [RF] and anti-cyclic citrullinated peptide [anti-CCP]) and total immunoglobulin (IgG and IgM) production in rheumatoid arthritis (RA) patients and the effect thereon of the B-cell-depleting antibody, rituximab, in the ARISE (Assessment of Rituximab's Immunomodulatory Synovial Effects) trial.MethodsAutoantibodies as well as total IgM and IgG were quantified by enzyme-linked immunosorbent assay in extracts of synovial tissues and matched serum from patients with RA or osteoarthritis (OA). Synovial biopsies and serum were obtained at baseline and 8 weeks following rituximab therapy in 14 RA patients. A synovial/serum index (SSI) was calculated as the ratio of synovial to serum antibody/albumin, with values above 1 representing synovial enrichment. Lymphoid aggregates were evaluated histologically.ResultsAnti-CCP IgG, but not RF-IgM, was significantly enriched in RA synovia compared with serum. Total IgM and IgG were also enriched in RA, but not in OA. SSI correlated significantly with mRNA content for both IgM and IgG, demonstrating that it reflected synovial immunoglobulin production. RA synovia with lymphocyte aggregates contained significantly elevated RF-IgM and anti-CCP IgG compared with tissues with diffuse lymphoid infiltration. Rituximab treatment did not affect synovial autoantibody or total immunoglobulin SSI overall. However, in aggregate-containing tissues, rituximab significantly reduced total IgM and IgG SSI as well as IgM and IgG1 mRNA. Surprisingly, RF-IgM and anti-CCP IgG SSIs were unchanged by rituximab in aggregate-containing synovia.ConclusionsCombined with earlier observations that synovial lymphoid aggregates are unaltered by rituximab treatment, these data suggest that lymphoid aggregates may provide a protective niche for autoantibody-producing cells.

Abstract

Acute myeloid leukaemia (AML) continues to have a poor prognosis, warranting new therapeutic strategies. The bone marrow (BM) microenvironment consists of niches that interact with not only normal haematopoietic stem cells (HSC) but also leukaemia cells like AML. There are many adhesion molecules in the BM microenvironment; therein, integrins have been of central interest. AML cells express integrins that bind to ligands in the microenvironment, enabling adhesion of leukaemia cells in the microenvironment, thereby initiating intracellular signalling pathways that are associated with cell migration, cell proliferation, survival, and drug resistance that has been described to mediate cell adhesion‐mediated drug resistance (CAM‐DR). Identifying and targeting integrins in AML to interrupt interactions with the microenvironment have been pursued as a strategy to overcome CAM‐DR. Here, we focus on the BM microenvironment and review the role of integrins in CAM‐DR of AML and discuss integrin‐targeting strategies. 29. Repository corticotropin injection as adjunctive therapy in patients with rheumatoid arthritis who have failed previous therapies with at least three different modes of action

Theresa Gillis, Megan Crane, Carly Hinkle, Nathan Wei Arthritis Treatment Center, Frederick, MD, USA Objective: Many types of treatment are available for patients with rheumatoid arthritis (RA), however, some patients fail to achieve remission. This report aims to determine the safety and efficacy of using repository corticotropin injection (RCI) as an adjunctive therapy in patients with RA refractory to at least three therapeutics with different mechanisms of action. Method: In this open-label, interventional, single-group study, patients received 80 U RCI twice weekly via subcutaneous injection over 12 weeks. Changes in the Ritchie–Camp Articular Index and health assessment questionnaire scores were monitored for changes from baseline measures. Results: Eight patients were enrolled and consisted of seven females and one male with an average age of 64.6 years and disease duration of 20.9 years. Use of RCI resulted in significant improvement in swollen and tender joint counts. The disease activity score 28 and the physician and patient visual analog scale scores were significantly reduced at treatment week 12. The reduction in health assessment questionnaire scores did not reach statistical significance after RCI treatment. Once RCI therapy was discontinued, all improvements in disease activity score 28, physician and patient visual analog scale, and tender and swollen joint counts achieved during treatment were lost by the week 16 follow-up visit. Conclusion: While larger clinical trials are necessary to further confirm the efficacy of RCI in patients with refractory RA, the response of patients with refractory RA in this study suggests that RCI can be an effective add-on therapy for patients who have exhausted several classes of treatments. Furthermore, this study suggests that RCI has an alternative mode of action, compared to other available antirheumatic drugs. Keywords: refractory rheumatoid arthritis, rheumatoid arthritis

Anecdotal and uncontrolled experience suggests that platelet-rich plasma (PRP) can be used for soft tissue injuries and autologous stem cells used in combination with PRP may be effective for tissue repair and cartilage regeneration. Large multicenter controlled studies of PRP are lacking, but there is enough anecdotal evidence to encourage its use as a 'matrix tissue graft.'Injuries that respond to PRP include enthesopathies, such as epicondylitis and plantar fasciitis. A second phase of regenerative technique involves using PRP in tandem with stem cells. Study results suggest that the therapeutic use of mesenchymal stem cells for regeneration of cartilage in patients with osteoarthritis is feasible. Abstract

Purpose: Studies of disease-modifying osteoarthritis drug (DMOAD) safety and effectiveness have relied on surrogate markers of disease activity (medical history, physical examination, radiographs, and magnetic resonance imaging). These often have low interobserver reliability and provide no information about cartilage quality. Assessment of cartilage histomorphology is a potentially valuable method for evaluating the extent of osteoarthritis (OA) and the effect of DMOADS on OA. Data on 126 arthroscopically guided small diameter (2 mm) Jamshidi needle cartilage biopsies from 32 patients (63 procedures with 2 biopsies obtained at each procedure) are reported. Type of Study: Retrospective analysis and description of technique. Methods: One hundred and twenty-six arthroscopically guided Jamshidi needle biopsies of articular cartilage were performed in 32 volunteers (10 men, 22 women; age range, 43-81 years) with Kellgren-Lawrence radiographic stage 1-3 OA of the knee in a randomized double-blinded placebo controlled study of insulin-like growth factor (IGF-1). Biopsies were obtained before treatment and also after 13 weeks of intra- articular injections. Patients were randomized to one of the 3 treatment groups: IGF given weekly or biweekly or placebo. Biopsies were obtained from the weight-bearing portion of the medial femoral condyle using a 2-mm Jamshidi needle and consisted of 2-mm diameter plugs of cartilage and underlying subchondral bone. Results: Adequate material for interpretation was obtained from the biopsies. Abstract

OBJECTIVE: To assess safety and clinical outcomes in patients with inflammatory arthritis after intraarticular (IA) injection of rAAV2-TNFR:Fc, a recombinant adeno-associated viral vector containing the human tumor necrosis factor (TNF) receptor-immunoglobulin (IgG1) Fc fusion (TNFR:Fc) gene. METHODS: In this phase 1/2 randomized study, adults with persistent moderate or severe inflammation in a target joint, being treated with or without systemic anti-TNF therapy, received a single IA injection of either rAAV2-TNFR:Fc (1 x 1011, 1 x 1012, or 1 x 1013DNase-resistant particles/ml joint volume) or placebo, followed by open-label rAAV2-TNFR:Fc 12–30 weeks later, depending on when the target joint met predetermined criteria for reinjection. RESULTS: 127 subjects received the first injection of blinded study drug; 95 subjects received open-label rAAV2-TNFR:Fc. Administration site reactions, consisting of transient mild to moderate increases in tenderness and swelling of the injected joint, occurred after 23/191 (12%) rAAV2-TNFR:Fc injections and were dose-dependent. Rates of other adverse events were not dose-dependent. Notable serious adverse events (SAE) included culture-negative septic arthritis in a subject receiving leflunomide and fatal disseminated histoplasmosis considered unrelated to rAAV2-TNFR:Fc in a subject receiving adalimumab. In the phase 2 portion of the study, a 30% decrease in target joint global visual analog scale was observed in 21/50 (42%) rAAV2-TNFR:Fc subjects and 3/16 (19%) placebo subjects 12 weeks after first injection (p = 0.14). CONCLUSION: IA rAAV2-TNFR:Fc resulted in administration site reactions after 12% of injections. A fatal SAE, disseminated histoplasmosis, was considered not related to study agent. Patient-reported outcome measures of clinical response showed greater improvement in treated patients than placebo patients.
