Abstract

MoO2/C-composites have been fabricated for the first time by a tartaric acid/glucose-assisted sol-gel method with post-annealing at 500 {\deg}C for 1 h in N2 flow. The synthesized materials were fully characterized with respect to structure, morphology, and electrochemical properties. Compared with tartaric acid-assisted products, the adoption of glucose as carbon source effectively increases the carbon content in the composites. Irrespective of the organic component, the composites exhibit low crystallinity and small grain size. This results in good electrochemical performance of the anode materials as confirmed for the glucose-assisted materials which after additional post treatment delivers a competitive electrochemical capacity.

Abstract

Li3VO4/C composites have been synthesized by a sol-gel method and post-annealing at 650 {\deg}C for 1 h in N2 flow using either tartaric acid, malic acid, or glucose as both chelating agents and carbon source. The presence of these organic additives crucially affects morphology and crystallite size of the final product. It is found that the electrochemical properties of Li3VO4/C as anode material for Li-ion batteries (LIBs) are influenced by the morphology, texture and carbon content of the material. When using carboxylic acids as carbon source composites with mesoporous structure and a high surface area are obtained that display an enhanced electrochemical activity. Initially, reversible capacity of about 400 mAh g-1 is obtained. In contrast, Li3VO4/C synthesized with glucose outperforms in terms of cycling stability. It exhibits a discharge capacity of 299 mAh g-1 after 100 cycles corresponding to an excellent capacity retention of 96 %. The favorable effect of carbon composites on the electrochemical performance of Li3VO4 is shown.

Abstract

Background and Objectives COVID-19–related inflammation, endothelial dysfunction, and coagulopathy may increase the bleeding risk and lower the efficacy of revascularization treatments in patients with acute ischemic stroke (AIS). We aimed to evaluate the safety and outcomes of revascularization treatments in patients with AIS and COVID-19. Methods This was a retrospective multicenter cohort study of consecutive patients with AIS receiving intravenous thrombolysis (IVT) and/or endovascular treatment (EVT) between March 2020 and June 2021 tested for severe acute respiratory syndrome coronavirus 2 infection. With a doubly robust model combining propensity score weighting and multivariate regression, we studied the association of COVID-19 with intracranial bleeding complications and clinical outcomes. Subgroup analyses were performed according to treatment groups (IVT-only and EVT). Results Of a total of 15,128 included patients from 105 centers, 853 (5.6%) were diagnosed with COVID-19; of those, 5,848 (38.7%) patients received IVT-only and 9,280 (61.3%) EVT (with or without IVT). Patients with COVID-19 had a higher rate of symptomatic intracerebral hemorrhage (SICH) (adjusted OR 1.53; 95% CI 1.16–2.01), symptomatic subarachnoid hemorrhage (SSAH) (OR 1.80; 95% CI 1.20–2.69), SICH and/or SSAH combined (OR 1.56; 95% CI 1.23–1.99), 24-hour mortality (OR 2.47; 95% CI 1.58–3.86), and 3-month mortality (OR 1.88; 95% CI 1.52–2.33). 14. Acquisition of orthographic forms via spoken complex word training

Abstract

This study used a novel word-training paradigm to examine the integration of spoken word knowledge when learning to read morphologically complex novel words. Australian primary school children including Grades 3-5 were taught the oral form of a set of novel morphologically complex words (e.g., (/vɪbɪŋ/, /vɪbd/, /vɪbz/), with a second set serving as untrained items. Following oral training, participants saw the printed form of the novel word stems for the first time (e.g., vib), embedded in sentences, while their eye movements were monitored. Half of the stems were spelled predictably and half were spelled unpredictably. Reading times were shorter for orally trained stems with predictable than unpredictable spellings and this difference was greater for trained than untrained items. These findings suggest that children were able to form robust orthographic expectations of the embedded morphemic stems during spoken word learning, which may have occurred automatically without any explicit control of the applied mappings, despite still being in the early stages of reading development. Following the sentence reading task, children completed a reading-aloud task where they were exposed to the novel orthographic forms for a second time. The findings are discussed in the context of theories of reading acquisition.

Abstract

Aim: To compare safety and functional outcomes of intravenous thrombolysis (IVT) between females and males with acute ischaemic stroke (AIS) in relation to preadmission use of antiplatelets. Methods: Multicentre cohort study of patients admitted from 1 January 2014 to 31 January 2020 to hospitals participating in the Swiss Stroke Registry, presenting with AIS and receiving IVT. Primary safety outcome was in-hospital symptomatic intracerebral haemorrhage (sICH). Primary functional outcome was functional independence at 3 months after discharge. Multivariable logistic regression models were fitted to assess the association between sex and each outcome according to preadmission use of antiplatelets. Results: The study included 4996 patients (42.51 % females, older than males, median age 79 vs 71 years, p < 0.0001). Comparable proportions of females (39.92 %) and males (40.39 %) used antiplatelets before admission (p = 0.74). In total, 3.06 % females and 2.47 % males developed in-hospital sICH (p = 0.19), with similar odds (adjusted odds ratio, [AOR] 0.93, 95 % confidence interval, [CI] 0.63–1.39). No interaction was found between sex and preadmission use of either single or dual antiplatelets in relation to in-hospital sICH (p = 0.94 and p = 0.23). Males had higher odds of functional independence at 3 months (AOR 1.34, 95 % CI 1.09–1.65), regardless of preadmission use of antiplatelets (interaction between sex and preadmission use of either single or dual antiplatelets p = 0.41 and p = 0.58). Conclusion: No sex differences were observed in the safety of IVT regarding preadmission use of antiplatelets. Males showed more favourable 3-month functional independence than females; however, this sex difference was apparently not explained by a sex-specific mechanism related to preadmission use of antiplatelets.

16. Teaching Children to Read Irregular Words: A Comparison of Three Instructional Methods

Abstract

Purpose Children learning to read in English must learn to read words with varying degrees of grapheme-phoneme correspondence regularity, but there is very little research comparing methods of instruction for words with less predictable or irregular spellings. Therefore, we compared three methods of instruction for beginning readers. Method Eighty-five Kindergarten children were randomly assigned to either Look and Say (LSay), Look and Spell (LSpell), mispronunciation correction (MPC), or wait-list control conditions. Children were taught 12 irregular words over three sessions. Amount of instructional time and number of exposures to the written and spoken forms of the words was controlled across the three experimental conditions. After training, children were assessed on reading aloud and orthographic choice measures. Results Children showed evidence of superior learning of trained words in the LSpell and MPC conditions, compared to LSay and control conditions. Differences between the LSpell and MPC conditions were not significant. There was no evidence of generalization to untrained items. Conclusions Findings indicate that active processing of a word’s orthography is crucial for learning irregular words. These results have implications for initial reading instruction. Further research is required to determine whether differences between LSpell and MPC conditions emerge after longer periods of training.

Abstract

BACKGROUND Patients with stroke secondary to occlusions of the anterior cerebral artery (ACA) often have poor outcomes. The optimal acute therapeutic intervention for these patients remains unknown. METHODS Patients with isolated ACA-stroke were identified from 10 centers participating in the EndoVascular treatment And ThRombolysis in Ischemic Stroke Patients (EVATRISP) prospective registry. Patients treated with endovascular thrombectomy (EVT) were compared to those treated with intravenous thrombolysis (IVT). Odds ratios with 95% confidence intervals (OR; 95%CI) were calculated using multivariate regression analysis. RESULTS Included were 92 patients with ACA-stroke. Of the 92 ACA patients, 55 (60%) were treated with IVT only and 37 (40%) with EVT (��bridging IVT). ACA patients treated with EVT had more often wake-up stroke (24% vs. 6%, p��=��0.044) and proximal ACA occlusions (43% vs. 24%, p��=��0.047) and tended to have higher stroke severity on admission [NIHSS: 10.0 vs 7.0, p��=��0.054). However, odds for favorable outcome, mortality or symptomatic intracranial hemorrhage did not differ significantly between both groups. Exploration of the effect of clot location inside the ACA showed that in patients with A1 or A2/A3 ACA occlusions the chances of favorable outcome were not influenced by treatment allocation to IVT or EVT. DISCUSSION Treatment with either IVT or EVT could be safe with similar effect in patients with ACA-strokes and these effects may be independent of clot location within the occluded ACA.

Abstract

BACKGROUND AND OBJECTIVE The International Code of Practice (CoP) IAEA TRS-483 provides a formalism for dosimetry in small fields of MV photon beams. It includes specific guidance for suitable detectors, and summarizes their general characteristics, e.g., dose linearity, stability, dose rate linearity, and leakage. The guidance for dose linearity stipulates it to be “better than 0.1% over an absorbed dose range of at least three orders of magnitude”. The CoP did not provide any methodology for the measurement of dose linearity. The aim of our multicentre experimental study was to verify the guidance given in TRS-483 for dose linearity applying two alternative methods, using a large number of suitable detectors recommended in TRS-483 for small field dosimetry. METHODS All measurementsfor the determination of dose linearity were done following a strictstudy protocol, at each of the nine participating centres. Twenty different types of detectors (10 ionization chambers, 9 diodes, and 1 micro diamond detector) were used for testing the dose linearity, with 49 detectors in total. All measurements were performed using an isocentric set-up with SSD = 90 cm, a depth of 10 cm, gantry 0°, and field size of 4 x 4 cm2 in 6 and 10 MV photon beams with (WFF) and without (FFF) flattening filter on either Elekta Versa HDTM or Varian TrueBeamTM linear accelerators. To minimize fluctuations in ion chamber readings from linac output variation, an ionization chamber having sufficiently large cavity volume (PTW Semiflex or IBA CC13) was utilized as a reference detector. Detectors were irradiated with 5, 10, 20, 30, 50, 100, 200, 300, 500, and 1000 MUs, covering an approximate absorbed dose range of three orders of magnitude. We analyzed the results by two alternative methods, considering a pass criterion of 0.1% as stated in TRS-483. A. Dose Linearity A: Adapted formalism from “IEC 60731 Medical electrical equipment – Dosimeters with ionization chambers as used in radiotherapy” Dose linearity was calculated as shown in Eq. (1), where Mi = mi/mi, ref ; mi denotes a single, i-th measurement (data point) performed at a particular centre, for a selected detector, energy, and number of MUs, while mi, ref stands for the corresponding measurement with reference ionization chamber done at the same time. Mref was defined as shown in Eq. (2), where indices 50, 100, and 200 denote number of Mus. R2 indicates the proportion of the variation of the data explained by the best-fit linear function, where mi, ref and mi were considered as the independent and dependent variables, respectively. We also considered that the pass criteria of 0.1% is satisfied if R2 > 0.999. RESULTS Dose Linearity A: We found that the TRS-483 guidance on dose linearity (0.1%) was not met for the majority of the 1960 analyzed data points (Table 1). In particular, the dose linearity criterion was not satisfied for low number of MUs. Dose Linearity B: The coefficient of determination R2 was higher than 0.999 for all analyzed data sets (196) for the entire range of MUs investigated in this study. DISCUSSION AND CONCLUSIONS We tested dose linearity for 49 detectors (20 different types) using two methods. The present results show that the 0.1% tolerance for dose linearity cannot be met for the selected range of doses (MUs) if the first method is used (adapted methodology from IEC 60731) for the determination of dose linearity. A less stringent acceptability criterion is needed, especially for very small numbers of MUs. For instance, if the tolerance in linearity is set at 1.0%, then more than 90% of the data points with 20 or more MUs comply (Table 1). Alternatively, if we assume that R2 = 0.999 corresponds to 0.1% linearity criterion from TRS-483, the dose linearity acceptability criterion can be met with 100% of the data pointsfor the whole range of MUs investigated in this study. In our opinion, method B is an acceptable method for the determination of dose linearity of detectors for small field dosimetry. Therefore, we recommend reporting dose linearity in terms of the coefficient of determination R2. To avoid any potential ambiguity, the methodology for the determination of dose linearity should be specified along with the corresponding acceptability criterion in an eventual update of the TRS-483.

Abstract

Purpose The Thrombolysis in Ischemic Stroke Patients (TRISP) collaboration was a concerted effort initiated in 2010 with the purpose to address relevant research questions about the effectiveness and safety of intravenous thrombolysis (IVT). The collaboration also aims to prospectively collect data on patients undergoing endovascular treatment (EVT) and hence the name of the collaboration was changed from TRISP to EVA-TRISP. The methodology of the former TRISP registry for patients treated with IVT has already been published. This paper focuses on describing the EVT part of the registry. Participants All centres committed to collecting predefined variables on consecutive patients prospectively. We aim for accuracy and completeness of the data and to adapt local databases to investigate novel research questions. Herein, we introduce the methodology of a recently constructed academic investigator-initiated open collaboration EVT registry built as an extension of an existing IVT registry in patients with acute ischaemic stroke (AIS). Findings to date Currently, the EVA-TRISP network includes 20 stroke centres with considerable expertise in EVT and maintenance of high-quality hospital-based registries. Following several successful randomised controlled trials (RCTs), many important clinical questions remain unanswered in the (EVT) field and some of them will unlikely be investigated in future RCTs. Prospective registries with high-quality data on EVT-treated patients may help answering some of these unanswered issues, especially on safety and efficacy of EVT in specific patient subgroups. Future plans This collaborative effort aims at addressing clinically important questions on safety and efficacy of EVT in conditions not covered by RCTs. The TRISP registry generated substantial novel data supporting stroke physicians in their daily decision making considering IVT candidate patients. While providing observational data on EVT in daily clinical practice, our future findings may likewise be hypothesis generating for future research as well as for quality improvement (on EVT). The collaboration welcomes participation of further centres willing to fulfill the commitment and the outlined requirements. © 2021 BMJ Publishing Group. All rights reserved.

Abstract

Introduction: Wallerian degeneration and diaschisis are considered separate remote entities following ischemic stroke. They may, however, share common neurophysiological denominators, since they are both related to disruption of fiber tracts and brain atrophy over time. Therefore, with advanced multimodal neuroimaging, we investigate Wallerian degeneration and its association with diaschisis. Methods: In order to determine different characteristics of Wallerian degeneration, we conducted examinations on seventeen patients with chronic unilateral ischemic stroke and persisting large vessel occlusion, conducting high-resolution anatomical magnetic resonance imaging (MRI) and blood oxygenation-level dependent cerebrovascular reactivity (BOLD-CVR) tests, as well as Diamox $^{15}$(O)-H$_{2}$O-PET hemodynamic examinations. Wallerian degeneration was determined using a cerebral peduncle asymmetry index (% difference of volume of ipsilateral and contralateral cerebral peduncle) of more than two standard deviations away from the average of age-matched, healthy subjects (Here a cerebral peduncle asymmetry index > 11%). Diaschisis was derived from BOLD-CVR to assess the presence of ipsilateral thalamus diaschisis and/or crossed cerebellar diaschisis. Results: Wallerian degeneration, found in 8 (47%) subjects, had a strong association with ipsilateral thalamic volume reduction (r $^{2}$ = 0.60) and corticospinal-tract involvement of stroke (p < 0.001). It was also associated with ipsilateral thalamic diaschisis (p = 0.021), No cerebral peduncular hemodynamic differences were found in patients with Wallerian degeneration. In particular, no CBF decrease or BOLD-CVR impairment was found. Conclusion: We show a strong association between Wallerian degeneration and ipsilateral thalamic diaschisis, indicating a structural pathophysiological relationship.

46. Predictors for affected stroke territory and outcome of acute stroke treatments are different for posterior versus anterior circulation stroke

Abstract

Distinct patient characteristics have been proposed for ischaemic stroke in the anterior versus posterior circulation. However, data on functional outcome according to stroke territory in patients with acute stroke treatment are conflicting and information on outcome predictors is scarce. In this retrospective study, we analysed functional outcome in 517 patients with stroke and thrombolysis and/or thrombectomy treated at the University Hospital Zurich. We compared clinical factors and performed multivariate logistic regression analyses investigating the effect of outcome predictors according to stroke territory. Of the 517 patients included, 80 (15.5%) suffered a posterior circulation stroke (PCS). PCS patients were less often female (32.5% vs. 45.5%, p = 0.031), received thrombectomy less often (28.7% vs. 48.3%, p = 0.001), and had lower median admission NIHSS scores (5 vs. 10, p < 0.001) as well as a better median three months functional outcome (mRS 1 vs. 2, p = 0.010). Predictors for functional outcome were admission NIHSS (OR 0.864, 95% CI 0.790-0.944, p = 0.001) in PCS and age (OR 0.952, 95% CI 0.935-0.970, p < 0.001), known symptom onset (OR 1.869, 95% CI 1.111-3.144, p = 0.018) and admission NIHSS (OR 0.840, 95% CI 0.806-0.876, p < 0.001) in ACS. Acutely treated PCS and ACS patients differed in their baseline and treatment characteristics. We identified specific functional outcome predictors of thrombolysis and/or thrombectomy success for each stroke territory.
