Abstract

BackgroundReformulating opioid analgesics to deter abuse is one approach toward improving their benefit-risk balance. To assess sentiment and attempts to defeat these products among difficult-to-reach populations of prescription drug abusers, evaluation of posts on Internet forums regarding reformulated products may be useful. A reformulated version of OxyContin (extended-release oxycodone) with physicochemical properties to deter abuse presented an opportunity to evaluate posts about the reformulation in online discussions. ObjectiveThe objective of this study was to use messages on Internet forums to evaluate reactions to the introduction of reformulated OxyContin and to identify methods aimed to defeat the abuse-deterrent properties of the product. MethodsPosts collected from 7 forums between January 1, 2008 and September 30, 2013 were evaluated before and after the introduction of reformulated OxyContin on August 9, 2010. A quantitative evaluation of discussion levels across the study period and a qualitative coding of post content for OxyContin and 2 comparators for the 26 month period before and after OxyContin reformulation were conducted. Product endorsement was estimated for each product before and after reformulation as the ratio of endorsing-to-discouraging posts (ERo). Post-to-preintroduction period changes in ERos (ie, ratio of ERos) for each product were also calculated. Additionally, post content related to recipes for defeating reformulated OxyContin were evaluated from August 9, 2010 through September 2013. ResultsOver the study period, 45,936 posts related to OxyContin, 18,685 to Vicodin (hydrocodone), and 23,863 to Dilaudid (hydromorphone) were identified. The proportion of OxyContin-related posts fluctuated between 6.35 and 8.25 posts per 1000 posts before the reformulation, increased to 10.76 in Q3 2010 when reformulated OxyContin was introduced, and decreased from 9.14 in Q4 2010 to 3.46 in Q3 2013 in the period following the reformulation. Abstract

Extracellular vesicles (EVs) are lipid bilayer particles released by virtually all cells, with prominent roles in both physiological and pathological processes. The size, number, and molecular composition of released EVs correlate to the cells of origin, modulated by the cell's environment and pathologic state. The proteins, DNA, RNA, and protein cargo carried by EVs are protected by degradation, with a prominent role in targeted intercellular signaling. These properties make EVs salient targets as both carriers of biomarkers and potential therapeutic delivery vehicles. The majority of EV research has focused on blood, urine, saliva, and cerebrospinal fluid due to easy accessibility. EVs have also been identified and studied in all ocular biofluids, including the vitreous humor, the aqueous humor, and the tear film, and the study of EVs in ocular disease is a new, promising, and underexplored direction with unique challenges and considerations. This review covers recent advances in the diagnostic and therapeutic use of ocular EVs, with a focus on human applications and key preceding in vitro and in vivo animal studies. Bardet–Biedl syndrome (BBS), an autosomal recessive ciliopathy with pleiotropic effects, manifests as a spectrum of anomalies involving multiple genes and affects fewer than 3,000 individuals in the USA. Due to its rarity and phenotypic variability, early diagnosis of BBS poses a significant challenge. 21. Identifying individual differences in the neural correlates of language processing using fMRI

Abstract

Mapping language functions in the brain is of profound theoretical and clinical interest. The aim of the current Ph.D. project was to develop an fMRI paradigm to assesses different language processes (i.e., phonological, semantic, sentence processing) and modalities (listening, reading, repetition) in a stimulus-driven manner, keeping non-linguistic task demands to a minimum. Cortical activations and functional connectivity patterns were largely in line with previous research, validating the suitability of the paradigm for localizing different language processes. The first empirical chapter of the thesis investigated sentence comprehension in listening and reading, which elicited largely overlapping activations for the two modalities and for semantic and syntactic integration in the left anterior temporal lobe (ATL). Functional connectivity of the left ATL with other parts of the cortical language network differed between the modalities and processes. The second empirical chapter explored individual differences in brain activity in relation to verbal ability. Results supported the notion of more extended as well as stronger activations during language processing in individuals with higher verbal ability, possibly reflecting enhanced processing. The third empirical chapter further investigated individual differences in brain activity, focusing on lateralization in activity as a fundamental principle of how language processing is functionally organized in the brain. Degrees of left-lateralization differed significantly between language processes and were positively related to behaviorally assessed language lateralization. Furthermore, the results provided new evidence supporting a positive relationship between left-lateralization and verbal ability. The thesis concludes with a discussion of the significance of the results with regard to general principles of brain functioning and outlines potential clinical implications.

26. A Matter of Perspective? Exploring Self-Distanced Reflections as an Emotion Regulation Strategy for Individuals in High-Stress Occupations

Abstract

This study investigates the effects of a linguistic self-distancing technique during reflection on affect, challenge versus threat appraisal, and anticipatory stress for individuals in high-stress occupations. Research in the laboratory suggests that reflections using non-first-person pronouns can reduce emotional reactivity to stressors, improve challenge appraisal, and reduce stress anticipation for future stressors. This study was conducted through an online questionnaire including a short reflection task. Participants were asked to reflect on a recent stressful work event either from a self-distanced or a self-immersed perspective. Self-assessments of global affect were collected pre- and post-reflection, while post-reflection reports of positive and negative affect in particular, as well as challenge versus threat appraisal and anticipatory stress were recorded. General measures of work stress and emotion regulation skills were included as covariates. A repeated-measures ANCOVA revealed no significant changes in global affect at the group level, but the interaction of gender and time of assessment showed a significant decrease in global affect scores only for female participants in the control group. Univariate ANCOVAs further showed no significant effects concerning positive and negative affect scores, or challenge versus threat appraisal. Interestingly, there was a significant group difference in anticipatory stress, revealing higher stress scores for the self-distanced over the self-immersed condition. The results contradict current research and emphasise the need to consider gender-specific differences in the effects of self-distanced reflections. Implications towards their suitability in unguided, naturalistic contexts and for regulating occupational stress in specific will be discussed with regards to methodological limitations.

Abstract

Background In South Africa, an estimated 11% of the population have high alcohol use, a major risk factor for TB. Alcohol and other substance use are also associated with poor treatment response, with a potential mechanism being altered TB drug pharmacokinetics. Objectives To investigate the impact of alcohol and illicit substance use on the pharmacokinetics of first-line TB drugs in participants with pulmonary TB. Methods We prospectively enrolled participants ≥15 years old, without HIV, and initiating drug-susceptible TB treatment in Worcester, South Africa. Alcohol use was measured via self-report and blood biomarkers. Other illicit substances were captured through a urine drug test. Plasma samples were drawn 1 month into treatment pre-dose, and 1.5, 3, 5 and 8 h post-dose. Non-linear mixed-effects modelling was used to describe the pharmacokinetics of rifampicin, isoniazid, pyrazinamide and ethambutol. Alcohol and drug use were tested as covariates. Results The study included 104 participants, of whom 70% were male, with a median age of 37 years (IQR 27–48). Alcohol use was high, with 42% and 28% of participants having moderate and high alcohol use, respectively. Rifampicin and isoniazid had slightly lower pharmacokinetics compared with previous reports, whereas pyrazinamide and ethambutol were consistent. No significant alcohol use effect was detected, other than 13% higher ethambutol clearance in participants with high alcohol use. Methaqualone use reduced rifampicin bioavailability by 19%. Conclusion No clinically relevant effect of alcohol use was observed on the pharmacokinetics of first-line TB drugs, suggesting that poor treatment outcome is unlikely due to pharmacokinetic alterations. 44. Connecting the Visible World with the Invisible

Abstract

Research suggests that connecting the visible (macroscopic) world of chemical phenomena to the invisible (particulate) world of atoms and molecules enhances student understanding in chemistry. This approach aligns with the science standards and is fundamental to the redesigned AP Chemistry curriculum. However, chemistry is usually taught at the abstract symbolic level, rarely incorporating particulate-level instruction. This article addresses that shortcoming by describing how to use particulate diagrams in a chemistry course. Using particulate diagrams in such topics as nature of matter, chemical/physical changes, ionic compounds, balancing equations, colligative properties, and acids/bases enhances students' critical-thinking skills as they experience the science and engineering practice of Developing and Using Models. The diagrams were incorporated into pre- and post-assessment, lab questions, and explanations given in class.
