3. Outcomes of an Emergency Department opioid alternatives Program implemented within a safety-net hospital system.

Abstract

Background: The ongoing opioid epidemic in the United States has reinforced the need to provide multimodal and non-opioid pain management interventions. The PAMI-ED ALT program employed a multifaceted approach in the Emergency Department (ED) developing electronic health record (EHR) pain management order panels and discharge panels, as well as educating patients, clinicians, and ED staff on opioid alternatives, including non-pharmacologic interventions. The primary objective of this analysis was to compare changes in opioid and non-opioid analgesic administrations and prescribing in ED patients with select pain conditions (renal colic, headache, low back, and non-low back musculoskeletal pain) before and after implementation of PAMI ED-ALT. Secondary outcomes included characterizing changes in 30-day ED all-cause recidivism and hospital all-cause admissions within these pain populations. Methods: Demographics, opioid and opioid alternative utilization, hospital admission, 30-day ED returns and change in pain intensity score were collected from January 2019-March 2020 (pre-program implementation) and January 2021-March 2023 (post-program implementation) for both the ED aggregate and program target pain populations. Results: Pain management order panel utilization increased throughout the post-implementation period. When comparing pre to post program data, there was a reduction in opioid administrations and prescriptions for most of the target pain conditions, as well as within the ED aggregate population. Opioid alternative administrations and prescriptions increased for all pain conditions except renal colic. Hospital admissions decreased significantly amongst those with low back pain and headache/migraine and 30-day ED returns significantly declined in those with musculoskeletal pain. Conclusion: Our findings demonstrate an opioid-alternatives program implemented within a safety-net hospital system serving a predominantly socially disadvantaged patient population can lead to changes in ED pain management and potentially reduce 30-day ED recidivism and hospitalizations. 9. The role of NF-κB signalling in T cell homeostasis and function

Abstract

Inhibitor of NF-κB kinase 1 (IKK1) and 2 (IKK2) form part of the functional IKK complex, necessary for NF-κB signalling. NF-κB is vital for survival, development, and function of multiple cell types. The aim of this study was to determine the function of NF-κB in thymic development and maintenance of the peripheral T cell compartment. For this, we used the Cre-loxP system in order to delete one or both of the Ikk1 and Ikk2 genes in T cells. Developmental deletion of Ikk1 and Ikk2 in CD4 CD8 double negative thymocytes revealed that NF-κB signalling was, surprisingly, redundant for early stages of thymic development and selection. Instead, late stage maturation of single positive thymocytes was abruptly halted in the absence of Ikk1 and Ikk2. Double deficient mice had a severe reduction in mature HSA low single positive thymocyte numbers and scarcely any peripheral T cells. Members of the TNF receptor superfamily (TNFRSF) are potent activators of NF-κB. We therefore tested the hypothesis that TNFRSF signalling could be regulating developing thymocytes. In vivo blockade of TNF in the IKK1/2 double deficient animals resulted in an almost complete rescue of thymocyte development. Cell culture experiments revealed that IKK1/2 double deficient thymocytes were specifically susceptible to TNF induced death. TNF receptor 1 (TNFR1) was identified as the key surface receptor, and TNFR1 IKK1/2 triple deficient mice had normal thymic development. Although TNFR1 is expressed throughout thymic development, transcriptomic analysis revealed cIAP2 and IL-7Rα as key targets of NF-κB dependent TNF signalling, but that co-operative developmental regulation of RIPK1 and caspase-8 underpinned the developmental susceptibility of mature SP thymocytes to TNF induced apoptosis. Our data reveal that developmental tuning of TNFR signalling and response genes is critical for normal T cell development and homeostasis.

Abstract

N,N-dimethyltryptamine (DMT) is a psychedelic substance produced within various Acacia species; however, the range of species that contain it and the quantity of the compound found within each species is relatively unknown. This work focused on the identification of DMT in 104 plant samples, representing 79 Acacia species, utilizing a reverse phase liquid chromatographic separation prior to mass spectral characterization with a robust Q-TOF-MS method. The findings of this work determined 51 DMT-containing species, 41 of which were previously unknown to contain the psychedelic. [ABSTRACT FROM AUTHOR]

Abstract

Carpal tunnel syndrome is the most common entrapment neuropathy, affecting quality of life for many people. Although it is a well recognised condition, new insights into epidemiology, diagnosis, and treatment have emerged in the past 6 years. The availability of disease-modifying treatments for rare systemic disorders associated with carpal tunnel syndrome (eg, amyloidosis) should alert clinicians to these diagnostic possibilities. Besides clinical evaluation and electrophysiology, the role of ultrasonography as a diagnostic tool has been confirmed and new ultrasound techniques have been applied, the clinical use and feasibility of which require further investigation. Surgical and non-surgical interventions are beneficial for the treatment of carpal tunnel syndrome and several treatment options are now available, giving clinicians the possibility to choose the best approach for every patient. New diagnostic and therapeutic techniques require further validation.

MBCT is based on the notion that meditation helps individuals effectively deploy and regulate attention to effectively manage and treat a range of psychological symptoms, including emotional responses to stress, anxiety, and depression. Several studies demonstrate that mindfulness approaches can effectively reduce negative emotional reactions that result from and/or exacerbate psychiatric difficulties and exposure to stressors among children, adolescents, and their parents. Mindfulness may be particularly relevant for youth with maladaptive cognitive processes such as rumination. Clinical experience regarding the utility of mindfulness-based approaches, including MBCT, is being increasingly supported by empirical studies to optimize the effective treatment of youth with a range of challenging symptoms. This paper provides a description of MBCT, including mindfulness practices, theoretical mechanisms of action, and targeted review of studies in adolescents. 29. Tracing the Impact: A Case of a Professional Development Programme in Mathematical Literacy

Abstract

The introduction of the subject Mathematical Literacy (ML) as part of the education reform process, led to the development of many large scale in-service programmes designed to train practising teachers to teach the new subject. The purpose of the study is to examine the challenges associated with providing in-service training for teachers of ML, by focusing on one district. We present a case study of one programme that was offered by two different universities in three large-scale funded cohorts in the province of KwaZulu-Natal (KZN). Data for the study was generated by student records, questionnaires and follow up surveys with each school in the district. By considering the success rate as the number of actual qualified teachers in relation to the number that was needed, we quantified the rate at 13%. Some of the factors which were responsible for this low rate are related to the processes of recruitment and selection of the appropriate teachers for the programme; the low pass rate as well as the deployment and management of the graduates at the end of the programme. It is recommended that future large scale teacher training initiatives be jointly managed by the universities and the education authorities, to ensure higher rates of success.

Abstract

In 2017, damage growth was observed within critical bonded step lap joints of the F/A-18 A-D that connected the composite wing skins to a specially designed titanium fitting attached to the fuselage. Ultimately, this led to the premature retirement of costly Australian military assets. The Defence Science and Technology Group (DSTG) took custodianship of one of the damaged wings for an in-depth examination and investigation into the root cause of the damage. In this follow-on phase of the retired asset examination program it was sought to assess the feasibility of performing a multiaxially loaded damage tolerance test of an extracted inner wing step lap joint (IWSLJ) specimen. All previous IWSLJ testing has been with uniaxial specimens which don't consider potentially damaging shear loading. The modelling results confirmed both that the shear loading is significant and that the DSTG Multiaxial Test rig is potentially capable of achieving representative multiaxial loading of a specimen or structural element extracted from a retired asset. The work has established stronger working relationships between Directorate of Aviation Engineering - Defence Aviation Safety Authority (DAVENG), DSTG and industry subject matter experts that is enabling current fleet managers to have enhanced preparedness. The work is also helping to develop further sovereign capabilities in bonded composite airframe sustainment for current and future fleets and increase opportunities for leveraging into other major international cooperative programs. 33. Relationships between Parental Attachment, Work and Family Roles, and Life Satisfaction

Abstract

The purpose of this study was to examine the relationship between parental attachment and satisfaction with work and family roles, as well as the relationship of these variables to life satisfaction. Results from a multiple regression analysis indicated that satisfaction with work and marriage, but not parenting satisfaction or parental attachment, was significantly related to life satisfaction. Responses to open-ended questions indicated that parental attachment relationships affected participants' career, marriage, parenting, and life satisfaction in varied and complex ways. Implications for counselors and directions for future research are discussed. (Contains 3 tables.)

34. The Effects of Role Congruence and Role Conflict on Work, Marital, and Life Satisfaction

Abstract

The impact of role congruence and role conflict on work, marital, and life satisfaction was studied using Super's life-span, life-space theory. A conceptual model of relationships between these variables was proposed, and gender differences were examined. Participants were 35 male and 60 female college graduates who completed surveys by mail. Results indicated adequate fit for the proposed model. Results of gender analyses indicated that women in this study participated more in parenting and housework, whereas men participated more in career and leisure activities. Despite the differences in actual role participation, no gender differences were found regarding ideal role participation. Both men and women reported that the parent role was most rewarding. Implications for counselors and directions for future research were discussed.

Abstract

BACKGROUND: Rapid diagnostic tests (RDTs) that rely on the detection of Plasmodium falciparum histidine-rich protein 2 (PfHRP2) have become key tools for diagnosing P. falciparum infection. The utility of RDTs can be limited by PfHRP2 persistence, however it can be a potential benefit in low transmission settings where detection of persistent PfHRP2 using newer ultra-sensitive PfHRP2 based RDTs can serve as a surveillance tool to identify recent exposure. Better understanding of the dynamics of PfHRP2 over the course of a malaria infection can inform optimal use of RDTs. METHODS: A previously published mathematical model was refined to mimic the production and decay of PfHRP2 during a malaria infection. Data from 15 individuals from volunteer infection studies were used to update the original model and estimate key model parameters. The refined model was applied to a cohort of patients from Namibia who received treatment for clinical malaria infection for whom longitudinal PfHRP2 concentrations were measured. RESULTS: The refinement of the PfHRP2 dynamic model indicated that in malaria naïve hosts, P. falciparum parasites of the 3D7 strain produce 33.6 × 10-15 g (95% CI 25.0-42.1 × 10-15 g) of PfHRP2 in vivo per parasite replication cycle, with an elimination half-life of 1.67 days (95% CI 1.11-3.40 days). The refined model included these updated parameters and incorporated individualized body fluid volume calculations, which improved predictive accuracy when compared to the original model. The performance of the model in predicting clearance of PfHRP2 post treatment in clinical samples from six adults with P. falciparum infection in Namibia improved when using a longer elimination half-life of 4.5 days, with 14% to 67% of observations for each individual within the predicted range. 44. The Role of Special Interest Groups in the Shaping of State Educational Policy Relative to School Finance: A Case Study.

Abstract

The Arizona state legislature met from November 1979 to April 1980 for the sole purpose of restructuring and codifying rules that define school financial policy and reforming tax structures that support educational institutions. This case study investigated the role of special interest groups in shaping state educational policy at that session. Data collection activities were structured around identifying (1) the special interest groups or individuals that attempted to influence the legislature; (2) the major issues or concerns of these groups; (3) the methods of contact employed; and (4) the extent to which those individuals or groups were successful. Data were collected by means of a mail survey of all 90 state legislators and all special interest groups and by open-ended interviews with 51 legislators and 52 representatives of special interest groups. Content analysis was used to analyze the data. The demographic and economic factors that precipated the special session are explained and the policy results of the special session reviewed. The special interest groups are identified, their concerns summarized, and methods of contact and influence discussed. The conclusion states that educational policy relative to school finance in Arizona is not being shaped by any special interest groups. (Author/MLF)

The persistent barriers to women's educational equity in educational administration may be eliminated through this project, which describes the development of an instrument for department self-assessment of sex equity and a model for institutional change. The procedures for developing, testing, and disseminating the instrument for self-assessment of sex equity are provided in detail. A set of standards, grouped by major area of activity, is presented that will ensure that women receive equitable experiences during their graduate studies. Strategies for change are described in the final chapter. It claims that authenticity exists on a continuum and content should not be seen as either completely authentic or as divorced from reality. It outlines the importance of authentic learning experiences, such as verbal experiences, visual experiences, vicarious experiences, simulated experiences, and direct experiences. The text provides tips for connecting the curriculum to real-life skills through project-based learning, which leads to the following results: engaged students learn more, studies take on a futuristic focus, and projects are compatible with the learning styles of students who have had difficulty succeeding in traditional classrooms. The volume also offers some cautions, such as recognizing that projects connected to low-level courses will not result in improved student performance.
