Abstract

Abstract Background Human airway surface liquid (ASL) has abundant antimicrobial peptides whose potency increases as the salt concentration decreases. Xylitol is a 5-carbon sugar that has the ability to lower ASL salt concentration, potentially enhancing innate immunity. Xylitol was detected for 8 hours in the ASL after application in airway epithelium in vitro. We tested the airway retention time of aerosolized iso-osmotic xylitol in healthy volunteers. Methods After a screening spirometry, volunteers received 10 ml of nebulized 5% xylitol. Bronchoscopy was done at 20 minutes (n = 6), 90 minutes (n = 6), and 3 hours (n = 5) after nebulization and ASL was collected using microsampling probes, followed by bronchoalveolar lavage (BAL). Xylitol concentration was measured by nuclear magnetic resonance spectroscopy and corrected for dilution using urea concentration. Results All subjects tolerated nebulization and bronchoscopy well. Mean ASL volume recovered from the probes was 49 ± 23 μl. The mean ASL xylitol concentration at 20, 90, and 180 minutes was 1.6 ± 1.9 μg/μl, 0.6 ± 0.6 μg/μl, and 0.1 ± 0.1 μg/μl, respectively. Corresponding BAL concentration corrected for dilution was consistently lower at all time points. The terminal half-life of aerosolized xylitol obtained by the probes was 45 minutes with a mean residence time of 65 minutes in ASL. Corresponding BAL values were 36 and 50 minutes, respectively. Conclusion After a single dose nebulization, xylitol was detected in ASL for 3 hours, which was shorter than our in vitro measurement. The microsampling probe performed superior to BAL when sampling bronchial ASL.

Abstract

Studies of recent destructive megathrust earthquakes and tsunamis along subduction margins in Japan, Sumatra, and Chile have linked forearc morphology and structure to megathrust behavior. This connection is based on the idea that spatial variations in the frictional behavior of the megathrust influence the tectono-morphological evolution of the upper plate. Here we present a comprehensive examination of the tectonic geomorphology, outer wedge taper, and structural vergence along the marine forearc of the Cascadia subduction zone (offshore northwestern North America). The goal is to better understand geologic controls on outer wedge strength and segmentation at spatial scales equivalent to rupture lengths of large earthquakes (=M 6.7), and to examine potential linkages with shallow megathrust behavior. We use cross-margin profiles, spaced 25 km apart, to characterize along-strike variation in outer wedge width, steepness, and structural vergence (measured between the toe and the outer arc high). The width of the outer wedge varies between 17 and 93 km, and the steepness ranges from 0.9° to 6.5°. Hierarchical cluster analysis of outer wedge width and steepness reveals four distinct regions that also display unique patterns of structural vergence and shape of the wedge: Vancouver Island, British Columbia, Canada (average width, linear wedge, seaward and mixed vergence); Washington, USA (higher width, concave wedge, landward and mixed vergence); northern and central Oregon, USA (average width, linear and convex wedge, mixed and seaward vergence); and southern Oregon and northern California, USA (lower width, convex wedge, seaward and mixed vergence). Variability in outer wedge morphology and structure is broadly associated with along-strike megathrust segmentation inferred from differences in oceanic asthenospheric velocities, patterns of episodic tremor and slow slip, GPS models of plate locking, and the distribution of seismicity near the plate interface. In more detail, our results appear to delineate the extent, geometry, and lithology of dynamic and static backstops along the margin. Varying backstop configurations along the Cascadia margin are interpreted to represent material-strength contrasts within the wedge that appear to regulate the along- and across-strike taper and structural vergence in the outer wedge. We argue that the morphotectonic variability in the outer wedge may reflect spatial variations in shallow megathrust behavior occurring over roughly the last few million years. Comparing outer wedge taper along the Cascadia margin to a global compilation suggests that observations in the global catalog are not accurately representing the range of heterogeneity within individual margins and highlights the need for detailed margin-wide morphotectonic analyses of subduction zones worldwide. Abstract

Abstract Background Xylitol is a 5-carbon sugar that can lower the airway surface salt concentration, thus enhancing innate immunity. We tested the safety and tolerability of aerosolized iso-osmotic xylitol in mice and human volunteers. Methods This was a prospective cohort study of C57Bl/6 mice in an animal laboratory and healthy human volunteers at the clinical research center of a university hospital. Mice underwent a baseline methacholine challenge, exposure to either aerosolized saline or xylitol (5% solution) for 150 minutes and then a follow-up methacholine challenge. The saline and xylitol exposures were repeated after eosinophilic airway inflammation was induced by sensitization and inhalational challenge to ovalbumin. Normal human volunteers underwent exposures to aerosolized saline (10 ml) and xylitol, with spirometry performed at baseline and after inhalation of 1, 5, and 10 ml. Serum osmolarity and electrolytes were measured at baseline and after the last exposure. A respiratory symptom questionnaire was administered at baseline, after the last exposure, and five days after exposure. In another group of normal volunteers, bronchoalveolar lavage (BAL) was done 20 minutes and 3 hours after aerosolized xylitol exposure for levels of inflammatory markers. Results In naïve mice, methacholine responsiveness was unchanged after exposures to xylitol compared to inhaled saline (p = 0.49). There was no significant increase in Penh in antigen-challenged mice after xylitol exposure (p = 0.38). There was no change in airway cellular response after xylitol exposure in naïve and antigen-challenged mice. In normal volunteers, there was no change in FEV1 after xylitol exposures compared with baseline as well as normal saline exposure (p = 0.19). Safety laboratory values were also unchanged. The only adverse effect reported was stuffy nose by half of the subjects during the 10 ml xylitol exposure, which promptly resolved after exposure completion. BAL cytokine levels were below the detection limits after xylitol exposure in normal volunteers. Conclusions Inhalation of aerosolized iso-osmotic xylitol was well-tolerated by naïve and atopic mice, and by healthy human volunteers.

Abstract

Investigators have validated an abbreviated protocol for testing nonspecific bronchial reactivity with methacholine. We performed a similar validation study with histamine, another bronchoprovocative agent known to induce airflow obstruction. Histamine is pharmacologically distinct from methacholine and, under some circumstances, may provide specific clinical and investigative advantages to methacholine. Twenty-four patients with a clinical history of asthma underwent bronchoprovocative testing using the standard histamine airway protocol recommended by the American Academy of Allergy, Committee on Standardization of Bronchoprovocation. In addition, two abbreviated histamine challenge protocols were tested using the same administration and testing equipment. The abbreviated protocols involved fewer dilutions and dosages of histamine than the standard histamine protocol but covered the same range of cumulative doses. The two abbreviated protocols differed only in the intervals for determination of [FEV.sub.1] between doses of histamine (30 s vs 3 min). The sequence of these three protocols was randomized for each study subject and each airway challenge was separately by one week. The two abbreviated protocols took significantly less time to administer than the standard protocol - 18 min vs 30 min vs 44 min. Both the provocative dose to cause a 20 percent decline in the [FEV.sub.1] ([PD.sub.20 FEV.sub.1]) and the slope of the dose-response curve were not significantly different between the standard protocol and either of the two abbreviated protocols. Moreover, a high degree of agreement was observed between the two abbreviated protocols and the standard histamine protocol for both the [PD.sub.20 FEV.sub.1] and the slope of the dose-response curve. These findings indicate that similar estimates of bronchial reactivity are obtained from either of the abbreviated protocols when compared with the standard histamine protocol.
