Abstract

Laser dacryocystorhinostomy (LDCR) is a surgical procedure designed to treat obstructions in the lacrimal duct system, which can cause excessive tearing, infections, and discomfort. This technique involves creating a new passage for tear drainage, thereby restoring normal tear flow and alleviating symptoms associated with duct obstruction. A retrospective study was conducted on 48 patients who underwent the LDCR procedure, amounting to 56 eyes. The primary outcome measured was the anatomical success rate, defined as the restoration of duct patency. Patients were examined postoperatively from 6 months to 3.5 years. The LDCR method demonstrated a 95% success rate, encompassing both anatomical and functional outcomes. The procedure's effectiveness was determined by achieving a patent osteotomy and resolving symptoms. Anatomical success was measured by the creation of a viable drainage pathway, while functional success pertained to the resolution of symptoms such as epiphora. The efficacy of the procedure was found to be independent of both age and gender. Among patients with successful anatomical outcomes, there was a statistically significant improvement in their Munk scores. The LDCR method is highly effective in treating lacrimal duct obstruction. Abstract

Abstract: The pharmacological management of glaucoma and ocular hypertension has significantly changed over the last 18 years with the introduction of PGF2α analogues, more specifically latanoprost (6), travoprost (8), bimatoprost (10) and tafluprost (12). Prostanoids are currently the first-line medicines among ocular antihypertensive drugs in terms of efficacy, safety, patient compliance and medical economy. Their ability to effectively reduce intraocular pressure with once-per-day dosing, ocular tolerability comparable to timolol and general lack of systemic adverse effects have made them the mainstay of pharmacological therapy for glaucoma and ocular hypertension all over the world. The present review reports a novel, convergent and highly diastereoselective method for the synthesis of PGF2α analogues from the structurally advanced prostaglandin phenylsulfone (5Z)-(+)-15 and new ω-chain synthons. The biochemistry, clinical efficacy and side effects of four commercially available PGF2α analogues, currently used as first-line agents for reducing intraocular pressure in patients with glaucoma or ocular hypertension, are also discussed. Abstract

Purpose: The purpose of our study is to evaluate retinal fiber layer thickness with scanning polarymetry laser (GDx), in patients suffering from SM with or without optic nerve symptoms. Multiple sclerosis proceeds to neurodegenerative changes and commonly appears with retrobulbar optic nerve damage. Examination of retinal nerves fiber layer examination with scanning laser polarymetry (GDx) enables to perform quantitative evaluation of retinal nerves fiber layer thickness within the area around the optic nerve disc. It finds application in diagnosis and monitoring of glaucoma, however its usefulness as a tool evaluating optic nerve in multiple sclerosis, has not been proved yet., Materials and Methods: Subjects diagnosed with multiple sclerosis (SM) were divided into 2 groups. First group was comprised of subjects, who suffered from at least one episode of retrobulbar neuritis, in at least one eye. Second group was made up of 8 subjects with no history of retrobulbar neuritis. Retinal nerves fiber layer thickness was measured with scanning polarymetry laser (GDx)., Results: Symptoms of retinal nerves fiber layer damage, were discovered with GDx significantly more common in first group (70% vs 18.75% accordingly)., Conclusions: Moreover, we stated that evaluation with scanning polarymetry laser might be precious method in discovering retinal nerves fiber layer damage in the course of multiple sclerosis. Presence of defects in retinal nerves fiber layer in patients suffering from multiple sclerosis with no history of retrobulbar neuritis may suggest subclinical damage of optic nerve.
