Abstract

BackgroundThe Prostate Cancer Prevention Trial (PCPT)-a randomized placebo-controlled study of the efficacy of finasteride in preventing prostate cancer-offered the opportunity to prospectively study effects of finasteride and other covariates on the health-related quality of life of participants in a multiyear trial.MethodsWe assessed three health-related quality-of-life domains (measured with the Health Survey Short Form-36: Physical Functioning, Mental Health, and Vitality scales) via questionnaires completed by PCPT participants at enrollment (3 months before randomization), at 6 months after randomization, and annually for 7 years. Covariate data obtained at enrollment from patient-completed questionnaires were included in our model. Mixed-effects model analyses and a cross-sectional presentation at three time points began at 6 months after randomization. All statistical tests were two-sided.ResultsFor the physical function outcome (n = 16 077), neither the finasteride main effect nor the finasteride interaction with time were statistically significant. The effects of finasteride on physical function were minor and accounted for less than a 1-point difference over time in Physical Functioning scores (mixed-effect estimate = 0.07, 95% confidence interval [CI] = -0.28 to 0.42, P = .71). Comorbidities such as congestive heart failure (estimate = -5.64, 95% CI = -7.96 to -3.32, P < .001), leg pain (estimate = -2.57, 95% CI = -3.04 to -2.10, P < .001), and diabetes (estimate = -1.31, 95% CI = -2.04 to -0.57, P < .001) had statistically significant negative effects on physical function, as did current smoking (estimate = -2.34, 95% CI = -2.97 to -1.71, P < .001) and time on study (estimate = -1.20, 95% CI = -1.36 to -1.03, P < .001). Finasteride did not have a statistically significant effect on the other two dependent variables, mental health and vitality, either in the mixed-effects analyses or in the cross-sectional analysis at any of the three time points.ConclusionFinasteride did not negatively affect SF-36 Physical Functioning, Mental Health, or Vitality scores.

44. Discriminant Analysis and Classification of Teaching Effectiveness Using Student Ratings: The Search for Doctor Fox.

Abstract

This study was designed to: (1) derive a set of weights for student-faculty rating item scores that maximize differences in mean rating scores for groups of students known to have experienced real differences in instructional effectiveness; and (2) attempt to cross validate the new scoring methods in a second study of students who rated lectures under the same conditions. Participants in the first study were 207 undergraduate and graduate students who were enrolled in general studies sections, while participants in the second study were 213 students enrolled in 12 sections of an undergraduate psychology course. The four types of faculty lectures were: (1) high information-high enthusiasm; (2) high information-low enthusiasm; (3) low information-high enthusiasm; (4) low information-low enthusiasm. In the first study, sections of the same class were randomly divided, while in the second study intact sections of the same class constituted the study group. Lecturers were randomly assigned to student groups. Students in all groups viewed one lecture presentation, rated the presentation using an 18-item questionnaire, and were tested on the material. The use of discrimination analysis to develop student-rating scales that are valid with respect to faculty enthusiasm is supported by this study. This study also indicated that empirically based selection and weighting of items does not improve the validity of student rating scores in detecting real differences in information giving. Current practices in the evaluation of teaching effectiveness are limited almost entirely to proxy measures, namely, student ratings. (SK)

45. The Factor Structure of Consumer Beliefs and Attitudes About Medical Care: Implications for the Academic Medical Community.

Abstract

This paper summarizes findings of a program of evaluation research. In this research the authors assumed that reliable and valid measurement of consumers' attitudes and beliefs about various aspects of medical care is possible and that more valid evaluation systems could be achieved if consumer perceptions are carefully considered during instrument development. The development and validation of seven experimental scales containing 134 variables (items) was described. The scales were administered to a representative sample of 903 heads of households in southernmost Illinois. These data were factor analyzed. Careful review of orthogonal and oblique rotations resulted in development of 20 perceptual constructs in sex areas (availability-accessibility, convenience, continuite, financial, humanness, quality). Reliabilities were computed and were high enough to warrant further study of nearly all of the constructs. Definitions of these constructs were offered. Multiple regression analyses predicting to behavioral criteria were used to obtain preliminary estimates of the validity of the constructs. The authors concluded that preliminary findings offer justification for the continued study and usage of the measurements in the evaluation of medical care facilities as well as in the evaluation of medical students and physicians at all levels of training.
