2. Adipose tissue production of hepatocyte growth factor contributes to elevated serum HGF in obesity

Abstract

Serum HGF is elevated in obese individuals. This study examined the contribution of excess adipose tissue to increased circulating HGF levels in obesity. Serum HGF was measured by ELISA before and after weight loss due to bariatric surgery or a 24-h fast. At 6.1 [+ or -] 0.1 mo following surgery, BMI (50.6 [+ or -] 1.6 vs. 35.1 [+ or -] 1.3 kg/[m.sup.2]; P < 0.0001) and serum HGF were significantly decreased (1,164 [+ or -] 116 vs. 529 [+ or -] 39 pg/ml, P < 0.001). A 24-h fast did not change serum HGF, but serum leptin was significantly reduced (67.7 [+ or -] 7.1 vs. 50.3 2 8.3 ng/ml, P = 0.02). HGF secretion in vitro from adipocytes of obese (BMI 40.3 [+ or -] 2.8 kg/[m.sup.2]) subjects was significantly greater (80.9 [+ or -] 10.4 vs. 21.5 [+ or -] 4.0 pg/[10.sup.5] cells, P = 0.008) than release from adipocytes of lean (BMI 23.3 [+ or -] 1.4 kg/[m.sup.2]) subjects. HGF mRNA levels determined by real-time RT-PCR were not different in adipocytes from lean (BMI 24.0 [+ or -] 0.8 kg/[m.sup.2]) and obese (45.7 [+ or -] 3.0 kg/[m.sup.2]) subjects, but serum HGF was significantly elevated in the obese individuals studied (787 [+ or -] 61 vs. 489 [+ or -] 49 pg/ml, P = 0.001). TNF-[alpha] (24 h treatment) significantly increased HGF release from subcutaneous adipocytes 23.6 [+ or -] 8.3% over control (P = 0.02). These data suggest that elevated serum HGF in obesity is in part attributable to excess adipose tissue and that this effect can be reversed by reducing adipose tissue mass through weight loss. Increased HGF secretion from adipocytes of obese subjects may be due to posttranscriptional events possibly related to adipocyte size and stimulation by elevated TNF-[alpha] in the adipose tissue of obese individuals. adipocytes; obesity; tumor necrosis factor-s; adiponectin; leptin; weight loss; hepatocyte growth factor

Abstract

This report contains the workflow analysis artifacts derived from a front-end analysis (FEA) of the Air Force Flight and Operational Medicine Clinic (FOMC). The primary methods of the FEA included workflow analysis, process mapping, and process redesign. The FEA methodology was executed using a 3-phased approach. The first phase consisted of a series of technical meetings with FOMC subject matter experts to develop baseline workflows. The second phase comprised site visits to a representative sample of FOMCs to identify and document the as is workflows and elicit user feedback regarding their tasks, processes, and interaction with current health IT systems. The third phase utilized a qualitative analysis of the FOMC baseline and as is workflows to reengineer optimized to be workflows to improve clinic operations and identity requirements for an enhanced Electronic Health Record system.

Abstract

A front-end analysis was accomplished of the medical system comprised of the personnel, health information technology systems, and policies that are the functional instantiation of an Air Force Flight and Operational Medicine Clinic (FOMC). The purpose of the FEA was to understand the clinic staff (i.e., the users), their needs in accomplishing the clinic's mission, and the demands of the work situation. The primary methods of the FEA included workflow analysis, process mapping, and process redesign. Each FOMC-unique workflow was observed and analyzed to identify points of waste (i.e., inefficiency or decreased effectiveness). Types of waste were categorized based on lean production principles. Using the DoD Human Factors Analysis and Classification System (HFACS), observed waste in the workflows was considered an active failure and the related latent failures at the preconditions, supervisory, and organizational levels were subsequently identified. These latent failures were then addressed for remediation in terms of the DoD DOTMLPF-P framework. Accordingly, the overall workflow analysis flowed from observed waste to underlying latent system failures to DOTMLPF-P findings and recommendations.

The article reports on planning and implementation of WorldCat Local, a discovery and delivery platform built by the University of Washington (UW) Libraries in collaboration with Online Computer Library Center (OCLC). WorldCat Local integrates a number of data sources and library services and because of this the importance of functional experts in all of these areas involved in planning and implementation is stressed. Details of how UW Libraries worked with OCLC are noted. Steps involved once a library signs a contract for WorldCat Local are highlighted. Also mentioned is the need for the lines between circulation and interlibrary loan to be blurred to bring about seamless integration of delivery for users.

The article reports on usage and impact of WorldCat Local, a discovery and delivery platform built by the University of Washington (UW) Libraries in collaboration with Online Computer Library Center (OCLC). When WorldCat Local was released, the WorldCat Local search box was made the default on the UW Libraries main home page. The platform recorded over 31,000 requests in the first quarter of 2008. The article reports on a discovery and delivery platform called WorldCat Local that resulted from a pilot project began by the University of Washington (UW) Libraries, in collaboration with Online Computer Library Center (OCLC). UW Libraries was inspired by topics such as the future of the catalog, discovery to delivery, working at the network level and syndication of information. It cites the 2005 OCLC report "Perceptions of Libraries and Information Resources" which found that 84% of users start their search with a search engine and 2% start at a library page. The strategic direction of the UW Libraries Vision 2010 Strategic Plan is to enhance user services.

Abstract

We report the occurrence of the Asiatic clam, Corbicula sp. in Lake Superior. Although Corbicula sp. has been recorded earlier from the Great Lakes, this is the first report of the clam from Lake Superior. Individuals were distributed in the thermal effluent at the Upper Peninsula Power Company, with the greatest density at 22 deg.C. Density averaged 96-m{sup}2{end}.
