Solar fruit drying is a technology that is successfully applied on both domestic and commercial scale among smallholder farmers in Uganda. However, existing solar drying technologies are marred with multiple deficiencies such as inefficient conversion of trapped solar radiation to meet required enthalpy, low throughput, long drying times, and inherent difficulty to achieve acceptable hygiene among others. This review critically examines existing solar drying technologies in Uganda, highlighting design constraints and plausible solutions for supporting the growing fruit drying industry. The common types of solar dryers in Uganda are the static-bed box type solar dryer model, the PPI tunnel solar dryer model, the NRI Kawanda cabinet solar dryer, the hybrid tunnel solar dryer and the UNIDO solar hybrid dryer model. Findings reveal that the challenges characterizing existing dryers in perspective of design are attributed to; poor material selection, poor mass and energy transfers, total dependence on solar energy, lack of capacity by local craftsmen to replicate new and improved models, difficulty to clean the dryers caused by inapt model configurations, and high cost of installation to mention a few. Therefore, a need exists to develop efficient and affordable designs using scientifically proven methods such as Computer Fluid Dynamics to pre-test and optimize the dryer and incorporating alternative energy sources in the design to ensure an all-weather dryer. Additionally, disseminate such innovations to farmers, retool local artisans with quality fabrication skill sets, and develop simple manual with standards and fabrication procedures for the fruit dryers. Developing countries are grappling with numerous challenges including feeding rapidly growing populations, alleviating poverty, protecting the environment, and mitigating adverse impacts of climate change. For the coming years, one of the main agricultural development agenda for Sub-Saharan Africa (SSA) should be to increase agricultural productivity and production to achieve food security through agricultural mechanization with Agricultural Engineering Technologies (AETs) as major inputs. However, there is paucity of information on the status of existing AETs and their contribution to mechanizing smallholder agriculture which is crucial to effective planning and strategy formulation. Therefore, the overarching objective of this paper is to review the past and present status, the constraints to adoption and future of AETs in Uganda in the context of mechanizing smallholder agriculture. Several proven AETs developed through research institutions and universities have been profiled in different areas of farm power and mechanization systems, agro-processing for value addition; renewable energy systems; and water harnessing and utilization. Availability and prudent use of these AETs along the value chain has the potential to enhance labor use and efficiency, provide greater precision and timeliness in farm operations, reduce postharvest loses, contributing to adding value to products and profitability of farming through proper handling, drying, cleaning, grading, processing, preservation, packaging and storage. The future of AETs in Uganda is hinged on addressing the aspects on appropriateness of the AETs in the smallholder agriculture context and standardizing of AET. Furthermore, human resource capacity development through enhancement of technical skills in AE, increased private sector engagement, economic incentives and innovation protection should be an integral part of the future strategies for development and increased adoption of AETs. In this study cross-section data was used to analyze the effect of farmers' demographic, socioeconomic and institutional setting, market access and physical attributes on the probability and intensity of tissue culture banana (TCB) adoption. The study was carried out between July 2011 and November 2011. Both descriptive (mean, variance, promotions) and regression analysis were used in the analysis. A double hurdle regression model was fitted on the data. Using multistage sampling technique, four counties and eight sub-locations were randomly selected. Using random sampling technique, three hundred and thirty farmers were selected from a list of banana households in the selected sub-locations. The adoption level of tissue culture banana (TCB) was about 32%. The results also revealed that the likelihood of TCB adoption was significantly influenced by: availability of TCB planting material, proportion of banana income to the total farm income, per capita household expenditure and the location of the farmer in Kisii County; while those that significantly influenced the intensity of TCB adoption were: occupation of farmers, family size, labour source, farm size, soil fertility, availability/access of TCB plantlets to farmers, distance to banana market, use of manure in planting banana, access to agricultural extension services and index of TCB/non-TCB banana cultivar attributes which were scored by farmers. Compared to West Pokot County, farmers located in Bungoma County are more significantly and likely to adopt TCB technology. Therefore, the results of the study suggest that the probability of adoption and intensity of the use of TCB should be enhanced. This can be done by taking cognizance of these variables in order to meet the priority needs of the smallholder farmers who were the target group. This would lead to alleviating banana shortage in the region for enhanced food security. Subsequently, actors along the banana value chain are encouraged to target the intervention strategies based on the identified farmer, farm and institutional characteristics for enhanced impact on food provision. Sustainable land use systems planning and management requires a wider understanding of the spatial extent and detailed human-ecosystem interactions astride any landscape. This study assessed the extent of historical, current, and future land use systems in Uganda. The specific objectives were to (i) characterize and assess the extent of historical and current land use systems, and (ii) project future land use systems. The land use systems were defined and classified using spatially explicit land use/cover layers for the years 1990 and 2015, while the future prediction (for the year 2040) was determined using land use systems datasets for both years through a Markov chain model. This study reveals a total of 29 classes of land use systems that can be broadly categorized as follows: three of the land use systems are agricultural, five are under bushland, four under forest, five under grasslands, two under impediments, three under wetlands, five under woodland, one under open water and urban settlement respectively. The highest gains in the land amongst the land use systems were experienced in subsistence agricultural land and grasslands protected, while the highest losses were seen in grasslands unprotected and woodland/forest with low livestock densities. By 2040, subsistence agricultural land is likely to increase by about 1% while tropical high forest with livestock activities is expected to decrease by 0.2%, and woodland/forest unprotected by 0.07%. High demand for agricultural and settlement land are mainly responsible for land use systems patchiness. This study envisages more land degradation and disasters such as landslides, floods, droughts, and so forth to occur in the country, causing more deaths and loss of property, if the rate at which land use systems are expanding is not closely monitored and regulated in the near future.

The issue of postharvest losses (PHL) is of high importance in the efforts to combat hunger, raise income and improve food security in the world's poorest countries. PHL of horticultural products in developing countries sometimes may reach up to 45%. PHL have an impact on food security for poor people, food quality and safety, economic development and the environment. The exact causes of PHL vary throughout the world and are very much dependent on the specific conditions and local situation in a given country. Irrespective of the level of economic development and maturity of systems in a country, PHL should be kept to a minimum. The failure to harness the physical and chemical characteristics of horticultural products using technologies has been very costly to farmers, processors and marketers in many parts of the world. A number of researchers and undocumented indigenous African Knowledge custodians in rural areas have innovated and constructed different technologies using locally available materials for the purpose of extending the shelf life of horticultural produce like vegetables. This review article seeks to critically examine some of the selected agricultural technologies and equipment used in African rural settings with view of assessing why they fail. Irrigation schemes across sub-Saharan Africa are constructed with the intention of increasing agricultural production to increase food security, reduce poverty and improve economic growth. However, most of these schemes are not performing as expected. This study therefore, diagnosed performance gaps in the pilot irrigation schemes of Mubuku and Doho in Uganda and analysed sustainable improvement options. Data was collected through systematic review of literature and scheme data, direct measurements at the schemes, field surveys, inspections, and key informant interviews. For each scheme, data for climate, irrigation, flow measurements, crop yields and farm gate prices were collected. Comparative indicators of agricultural output, water supply, financial and physical sustainability were used to assess scheme performance using standard approaches by International Water Management Institute (IWMI). The findings showed that the schemes were not performing optimally with crop yields being far below the attainable potential. The major contributing factors to the low performance were low water use efficiency and low agricultural output. Poor flow control, poor water distribution, and poor on-farm water application contributed to low water use efficiency. Low agricultural production was attributed to poor crop yields resulting from poor agronomic practices, poor irrigation scheduling and low produce prices. The financial self-sufficiency indicator pointed to farmers' inability to operate and maintain irrigation schemes effectively. Soil erosion and sedimentation contribute to deteriorating water quality, adverse alterations in basin hydrology and overall ecosystem biogeochemistry. Thus, understanding soil erosion patterns in catchments is critical for conservation planning. This study was conducted in a peri-urban Inner Murchison Bay (IMB) catchment on the northern shores of Lake Victoria since most soil erosion studies in Sub-Saharan Africa have been focused on rural landscapes. The study sought to identify sediment sources by mapping erosion hotspots using the revised universal soil loss equation (RUSLE) model in appendage with field walks. RUSLE model was built in ArcGIS 10.5 software with factors including: rainfall erosivity, soil erodibility, slope length and steepness, land cover and support practices. The model was run, producing an erosion risk map and field assessments conducted to ground-truth findings and identify other hotspots. The percentage areas for RUSLE modelled erosion rates were: 66.8% for 0-2 t ha -1  year -1 ; 10.8% for 2-5 t ha -1  year -1 ; 10.1% for 5-10 t ha -1  year -1 ; 9% for 10-50 t ha -1  year -1 and 3.3% for 50-100 t ha -1  year -1 . Average erosion risk was 7 t ha -1  year -1 and the total watershed erosion risk was 197,400 t year -1 , with croplands and steep areas (slope factor > 20) as the major hotspots (> 5 t ha -1  year -1 ). Field walks revealed exposed soils, marrum (gravel) roads and unlined drainage channels as other sediment sources. This study provided the first assessment of erosion risk in this peri-urban catchment, to serve as a basis for identifying mitigation priorities. Background: Decades of war left the Republic of South Sudan with a fragile health system that has remained deprived of resources since the country's independence. We describe the coverage of interventions for women's and children's health in Upper Nile and Unity states, and explore factors that affected service provision during a protracted conflict., Methods: We conducted a case study using a desk review of publicly available literature since 2013 and a secondary analysis of intervention coverage and conflict-related events from 2010 to 2017. During June through September 2018, we conducted 26 qualitative interviews with technical leads and 9 focus groups among health workers working in women and children's health in Juba, Malakal, and Bentiu., Results: Coverage for antenatal care, institutional delivery, and childhood vaccines were low prior to the escalation of conflict in 2013, and the limited data indicate that coverage remained low through 2017. Key factors that determined the delivery of services for women and children in our study sites were government leadership, coordination of development and humanitarian efforts, and human resource capacity. Participants felt that national and local health officials had a limited role in the delivery of services, and financial tracking data showed that funding stagnated or declined for humanitarian health and development programming during 2013-2014. Although health services were concentrated in camp settings, the availability of healthcare providers was negatively impacted by the protracted nature of the conflict and insecurity in the region., Conclusions: Health care for women and children should be prioritized during acute and protracted periods of conflict by strengthening surveillance systems, coordinating short and long term activities among humanitarian and development organizations, and building the capacity of national and local government officials to ensure sustainability., Competing Interests: Competing interestsThe authors declare that they have no competing interests., (© The Author(s) 2020.)

Background: Short-medium term studies from sub-Saharan Africa show that, despite high early mortality, substantial loss to program, and high rates toxicity, patients on antiretroviral treatment have achieved outcomes comparable to those in developed settings. However, these studies were unable to account for long term outcomes of patients as they stayed longer on treatment., Objectives: We aim to describe ten years outcomes of one of the first cohort of HIV positive patients started on antiretroviral treatment (ART) in Sub-Saharan Africa., Methods: We report 10-years outcomes including mortality, retention, CD4-count response, virological outcomes, ART regimens change from a prospective cohort of 559 patients initiating ART and followed up for 10 years Uganda., Results: Of 559 patients, 69.1% were female, median age (IQR) was 38 (33-44) years, median CD4-count (IQR) 98 (21-163) cell/μL; 74% were started on stavudine, lamivudine and nevirapine, 26% on zidovudine, lamivudine and efavirenz. After 10 years 361 (65%) patients were still in the study; 127 (22.7%) had died; 30 (5%) were lost to follow-up; 27 (5%) transferred; 18 (3%) withdrew consent. The probability of death was high in the first year (0.15, 95%, CI 0.12-0.18). The median CD4 count increased from 98 to 589 cell/μL (IQR: 450-739 cell/μL) with a median increase of 357 cells/μL (IQR: 128-600 cells/μL); 7.4% never attained initial viral suppression and of those who did 31.7% experienced viral failure. Three hundred and two patients had at least one drug substitution while on first line after a median of 40 months; 66 (11.9%) of the patients were switched to a second line PI-based regimen due to confirmed treatment failure., Conclusions: Despite the high rate of early mortality due to advanced disease at presentation the outcomes from this cohort are encouraging, particularly the remarkable and incremental immune-recovery and a satisfactory rate of virologic suppression.

Background: HIV antiretroviral therapy (ART) is often managed without routine laboratory monitoring in Africa; however, the effect of this approach is unknown. This trial investigated whether routine toxicity and efficacy monitoring of HIV-infected patients receiving ART had an important long-term effect on clinical outcomes in Africa., Methods: In this open, non-inferiority trial in three centres in Uganda and one in Zimbabwe, 3321 symptomatic, ART-naive, HIV-infected adults with CD4 counts less than 200 cells per microL starting ART were randomly assigned to laboratory and clinical monitoring (LCM; n=1659) or clinically driven monitoring (CDM; n=1662) by a computer-generated list. Haematology, biochemistry, and CD4-cell counts were done every 12 weeks. In the LCM group, results were available to clinicians; in the CDM group, results (apart from CD4-cell count) could be requested if clinically indicated and grade 4 toxicities were available. Participants switched to second-line ART after new or recurrent WHO stage 4 events in both groups, or CD4 count less than 100 cells per microL (LCM only). Co-primary endpoints were new WHO stage 4 HIV events or death, and serious adverse events. Non-inferiority was defined as the upper 95% confidence limit for the hazard ratio (HR) for new WHO stage 4 events or death being no greater than 1.18. Analyses were by intention to treat. This study is registered, number ISRCTN13968779., Findings: Two participants assigned to CDM and three to LCM were excluded from analyses. 5-year survival was 87% (95% CI 85-88) in the CDM group and 90% (88-91) in the LCM group, and 122 (7%) and 112 (7%) participants, respectively, were lost to follow-up over median 4.9 years' follow-up. 459 (28%) participants receiving CDM versus 356 (21%) LCM had a new WHO stage 4 event or died (6.94 [95% CI 6.33-7.60] vs 5.24 [4.72-5.81] per 100 person-years; absolute difference 1.70 per 100 person-years [0.87-2.54]; HR 1.31 [1.14-1.51]; p=0.0001). Differences in disease progression occurred from the third year on ART, whereas higher rates of switch to second-line treatment occurred in LCM from the second year. 283 (17%) participants receiving CDM versus 260 (16%) LCM had a new serious adverse event (HR 1.12 [0.94-1.32]; p=0.19), with anaemia the most common (76 vs 61 cases)., Interpretation: ART can be delivered safely without routine laboratory monitoring for toxic effects, but differences in disease progression suggest a role for monitoring of CD4-cell count from the second year of ART to guide the switch to second-line treatment., Funding: UK Medical Research Council, the UK Department for International Development, the Rockefeller Foundation, GlaxoSmithKline, Gilead Sciences, Boehringer-Ingelheim, and Abbott Laboratories., (Copyright 2010 Elsevier Ltd. Although recent data suggest high levels of adherence to expanding antiretroviral therapy (ART) programmes in resource-limited settings, the culture-specific barriers to adherence are poorly understood. In a prospective observational study, we found that 1.2% of patients discontinued ART because of a belief in spiritual healing. Spiritual beliefs should be an important part of ART adherence counselling in resource-limited settings, requiring close collaboration between HIV care programmes and religious leaders to identify common goals and ensure successful treatment.
