Abstract

Chronic kidney disease (CKD) is a major disease that seriously endangers human health. Traditional Chinese medicine (TCM) has certain characteristic effect in the prevention and treatment of CKD, especially in reducing urinary proteins, protecting renal function, delaying the progress of CKD and alleviating related complications. This study reviews the new understanding of TCM theory of CKD in recent years, and summarizes the evidence-based medical evidence of the efficacy of TCM in the treatment of CKD and the new progress related to the effect mechanism of TCM treatment of CKD, summarizing the current situation of clinical and basic research and achievements transformation of CKD, and proposes future research prospects, in order to contribute to the further innovation and development of TCM in the prevention and treatment of CKD.

Abstract

Diabetic kidney disease (DKD) is a severe complication of diabetes. Its incidence increases annually, posing a significant burden on public health. The strategy of symptomatic treatment based on pathogenesis differentiation, focusing on identifying pathogenesis, is particularly meaningful for managing complex and variable chronic diseases like DKD. Within this framework, the state of latent heat persists throughout DKD, with "latent heat causing accumulation" identified as the core pathogenesis affecting and promoting the development and progression of DKD. This paper is centered on the concept of "latent heat causing accumulation" and adopts symptomatic treatment based on pathogenesis differentiation as its guiding principle to explore the role of latent heat in DKD. It highlights that the onset of DKD involves environmental and constitutional pathogenesis associated with "the concealment of latent heat" and "stagnant-heat invading collaterals" as the initial pathogenesis," latent heat causing accumulation" as the core mechanism, and" secondary turbid heat" as the derivative pathogenesis. These pathogenesis factors collectively influence the symptoms, sequelae, and prognosis of DKD. Moreover, this paper provides commonly used prescriptions for different stages, syndrome types, and complications of the disease, aiming to offer a reference for clinical practice in flexibly addressing changes in disease conditions based on varied pathogenesis. This study aimed to identify gene signatures of COPD through bioinformatics analysis based on multiple gene sets and explore their immune characteristics and transcriptional regulation mechanisms.Methods: Data from four microarrays were downloaded from the Gene Expression Omnibus database to screen differentially expressed genes (DEGs) between COPD patients and controls. Weighted gene co-expression network analysis was applied to identify trait-related modules and then select key module-related DEGs. The optimized gene set of signatures was obtained using the least absolute shrinkage and selection operator (LASSO) regression analysis. The CIBERSORT algorithm and Pearson correlation test were used to analyze the relationship between gene signatures and immune cells. Finally, public databases were used to predict the transcription factors (TFs) and upstream miRNAs.Results: A total of 127 DEGs in COPD were identified from the combined dataset. By considering the intersection of DEGs and genes in two trait-related modules, 83 key module-related DEGs were identified, which were mainly enriched in interleukin-related pathways. Abstract

Diabetic kidney disease (DKD) is a global pandemic, and microinflammation has been reported as an important pathogenic factor of DKD. Traditional Chinese Medicine (TCM) has been used in the treatment of DKD for thousands of years, and modern Chinese medicine studies have found that herbal medicines with heat-clearing property have a curative anti-inflammation effect in DKD. This article reviews the new clinical and experimental progress made in herbal medicines with heat-clearing property, in the treatment of DKD, as well as their safety aspects.
