19. Effects of the genetic variants of alcohol-metabolizing enzymes on lipid levels in Asian populations: a systematic review and meta-analysis.

Abstract

Context Emerging evidence indicates that variants of alcohol-metabolizing enzymes may influence lipid metabolism. Objective This study aimed to investigate whether the rs671 and rs1229984 variants affect lipid levels in East Asian individuals. Data Sources PubMed, Foreign Medical Journal Service, Embase, Cochrane Library, Scopus, MEDLINE, Web of Science, Web of Knowledge, Wanfang, and Chinese Biomedical Literature databases were searched until December 31, 2021. Data Extraction Meta-analyses of studies that examined the effects of alcohol-metabolizing enzyme variants on lipid levels, as well as the interaction with alcohol intake, were selected. Data extraction was conducted independently by two investigators and confirmed by the third. Data Analysis In total, 86 studies (179 640 individuals) were analyzed. The A allele of rs671 (a functional variant in the ALDH2 gene) was linked to higher levels of low-density lipoprotein cholesterol (LDL-C) and lower levels of triglycerides and high-density lipoprotein cholesterol. In contrast, the A allele of the rs1229984 (a functional variant in the ADH2 gene) was associated only with lower levels of LDL-C. The effects of rs671 and rs1229984 on lipid levels were much stronger in Japanese than in Chinese individuals and in males than in females. Regression analysis indicated that the effects of rs671 on lipid levels were independent of alcohol intake in an integrated East Asian population (ie, Japanese, Chinese, and Korean individuals). Intriguingly, alcohol intake had a statistical influence on lipid levels when the sample analyzed was restricted to Japanese individuals or to males. Conclusions The rs671 and rs1229984 variants of alcohol-metabolizing enzymes have significant effects on lipid levels and may serve as genetic markers for lipid dyslipidemia in East Asian populations. Abstract

Mechanical stretch and oxidative stress have been shown to prolong action potential duration (APD) and produce early afterdepolarizations (EADs). Here, we developed a simulation model to study the role of stretch-activated channel (SAC) currents in triggering EADs in ventricular myocytes under oxidative stress. We adapted our coupling clamp circuit so that a model ionic current representing the actual SAC current was injected into ventricular myocytes and added as a real-time current. This current was calculated as [I.sub.SAC] = [G.sub.SAC] * ([V.sub.m] - [E.sub.SAC]), where [G.sub.SAC] is the stretch-activated conductance, [V.sub.m] is the membrane potential, and [E.sub.SAC] is the reversal potential. In rat ventricular myocytes, application of [G.sub.SAC] did not produce sustained automaticity or EADs, although turn-on of [G.sub.SAC] did produce some transient automaticity at high levels of [G.sub.SAC]. Exposure of myocytes to 100 [micro]M [H.sub.2][O.sub.2] induced significant APD prolongation and increase in intracellular [Ca.sup.2+] load and transient, but no EAD or sustained automaticity was generated in the absence of GsAc. However, the combination of [G.sub.SAC] and [H.sup.2][O.sub.2] consistently produced EADs at lower levels of [G.sub.SAC] (2.6 [+ or -] 0.4 nS, n = 14, P < 0.05). Pacing myocytes at a faster rate further prolonged APD and promoted the development of EADs. SAC activation plays an important role in facilitating the development of EADs in ventricular myocytes under acute oxidative stress. Abstract

Background In December 2019, the outbreak of coronavirus disease 2019 (COVID-19) started in Wuhan, and is now causing a worldwide pandemic. However, the experience in very elderly patients is very limited, which has important implications for the investigation of hospital infection in medical and health institutions. Methods Seven patients with confirmed COVID-19 infection in the Department of Geriatrics at Zhongnan Hospital of Wuhan University were included. Clinical data were retrospectively collected and analyzed. Laboratory tests and chest computed tomography (CT) images from the patients before and after the COVID-19 infection were compared. Results The median age of patients was 91 years old (87-96). Six patients had pneumonia in the last 6 months. Dyspnea occurred in one patient 64 h after the onset of the disease. In the other six patients, minor fatigue with low fever were the only other manifestations of the disease. Lymphopenia and a significant reduction in plasma globulin level was observed compared with levels before the onset of the disease. None had typical chest CT phenotypes during the early stage, except the critically ill patient mentioned who had developed "white lung" and then died. One patient even showed absorption of inflammation compared with previous hypostatic pneumonia. Conclusions The ratio of fatal cases in very elderly patients with COVID-19 is no higher than that reported in non-elderly patients was, and probably due to a low immune response. However, the elderly patients manifested minor clinical symptoms and atypical changes in chest CT images, which usually lead to misdiagnosis or delayed diagnosis. Abstract

We investigated differences in L-type C[a.sup.2+] current (I[sub.ca]) between infant (INF, 1-12 mo old), young adult (YAD, 14-18 yr old), and older adult (AD) myocytes from biopsies of right atrial appendages. Basal I[sub.Ca] was smaller in INF myocytes (1.2 [+ or -] 0.1 pA/pF, n = 29, 6 [+ or -] 1 mo old, 11 patients) than in YAD (2.5 [+ or -] 0.2 pA/pF, n = 20, 16 [+ or -] 1 yr old, 5 patients) or AD (2.6 [+ or -] 0.3 pA/pF, n = 19, 66 [+ or -] 3 yr old, 9 patients) myocytes (P < 0.05). Maximal I[sub.Ca] produced by isoproterenol (Iso) was similar in INF, YAD, and AD ceils: 8.4 [+ or -] 1.1, 9.6 [+ or -] 1.0, and 9.2 [+ or -] 1.3 pA/pF, respectively. Efficacy ([E.sub.max]) was larger in INF (607 [+ or -] 50%) than for YAD (371 [+ or -] 29%) or AD (455 [+ or -] 12%) myocytes. Potency (E[C.sub.50]) was 8- to 10-fold higher in AD (0.82 [+ or -] 0.09 nM) or YAD (0.41 [+ or -] 0.14 nM) than in INF (7.6 [+ or -] 3.5 nM) myocytes. Protein levels were similar for [G.sub.i][[alpha].sub.2] but much greater for [G.sub.i][[alpha].sub.3] in INF than in AD or YAD atrial tissue. When [G.sub.i][[alpha].sub.3] activity was inhibited by inclusion of a [G.sub.i[alpha]3] COOH-terminal decapeptide in the pipette, basal [I.sub.Ca] and the response to 10 nM Iso were increased in INF, but not in YAD, cells. We propose that basal [I.sub.Ca] and the response to low-dose [beta]-adrenergic stimulation are inhibited in INF (but not YAD or AD) cells as a result of constitutive inhibitory effects of [G.sub.i][[alpha].sub.3].
