14. Risk factors for gastrointestinal bleeding in patients with gastrointestinal cancer using edoxaban

Item does not contain fulltext, BACKGROUND: In the Hokusai VTE Cancer study, the risk of major bleeding was 2.9% higher in the edoxaban group compared with the dalteparin group, mainly due to more gastrointestinal bleedings in patients with gastrointestinal cancer. The identification of risk factors for gastrointestinal bleeding may help to guide the use of DOACs in these patients. OBJECTIVES: To evaluate risk factors for gastrointestinal bleeding in patients with gastrointestinal cancer receiving edoxaban. PATIENTS/METHODS: In this nested case-control study in patients with gastrointestinal cancer randomized to edoxaban in the Hokusai VTE Cancer study, cases (patients with clinically relevant gastrointestinal bleeding during treatment) were randomly matched to three controls (patients who had no gastrointestinal bleeding). Data for the 4-week period prior to bleeding were retrospectively collected. Odds ratios (ORs) were calculated in a crude conditional logistic regression model and a multivariable model adjusted for age, sex, and cancer type. RESULTS: Twenty-four cases and 64 matched controls were included. In the multivariable analysis, advanced cancer, defined as regionally advanced or metastatic cancer (OR 3.6, 95% CI 1.01-12.6) and low hemoglobin levels (OR 4.8, 95% CI 1.5-16.0) were significantly associated with bleeding. There was no significant difference in patients with resected tumors (OR 0.4, 95% CI 0.1-1.4), or in patients on chemotherapy (OR 1.3, 95% CI 0.5-3.5). CONCLUSION: Advanced cancer and low hemoglobin levels were associated with an increased risk of gastrointestinal bleeding in patients with gastrointestinal cancer receiving edoxaban. We were unable to identify other risk factors, mainly due to limited statistical power.

Abstract

The time-of-flight (TOF) mass spectrometry (MS), a high-resolution magnetic spectrometer equipped with a fast particle tracking system, is well recognized by its ability in weighing the most exotic nuclei. Currently such TOF-MS can achieve a mass resolution power of about 2×10−4. We show that the mass resolution can be further improved by one order of magnitude with augmented timing and position detectors. We report the progress in developing ultra-fast detectors to be used in TOF-MS.

Abstract

We have fielded a neutron imaging system at the National Ignition Facility to collect images of fusion neutrons produced in the implosion of inertial confinement fusion experiments and scattered neutrons from (n, n′) reactions of the source neutrons in the surrounding dense material. A description of the neutron imaging system is presented, including the pinhole array aperture, the line-of-sight collimation, the scintillator-based detection system and the alignment systems and methods. Discussion of the alignment and resolution of the system is presented. We also discuss future improvements to the system hardware.

Abstract

Serum pepsinogen I (PGI) levels were measured in 231 duodenal ulcer (DU) patients and 100 sex and age-comparable healthy controls. Significantly higher mean serum PGI levels were found in DU patients than in controls (124,7 ± 3,4 ng/ml v. 92,9 ± 2,3 ng/ml; P < 0,001) (mean ± SE). These levels were higher in male DU patients than in female DU patients (128,5 ± 3,9 ng/IDl v. 107,4 ± 6,4 ng/ml; P < 0,05). Smoking was associated with elevated serum PGI levels in DU patients (145,3 ± 5,1 ng/ml v. 109,0 ± 4,2 ngl/ml; P < 0,001). Healed DUs were associated with lower mean serum PGI levels than active ulcers (110,9 ± 7,6 ng/ml v. 129,4 ± 3,8 ng/ml, P < 0,05). Whether patients were positive or negative for Helicobacter pylori, infection did not affect mean serum PGI levels. All the risk factors for DU may not affect serum PGI levels and DU may therefore be considered a multifactorial disease.

Abstract

The astrophysically important reaction 14O(α,p)17F has been studied through a measurement of the time-inverse p(17F,α)14O reaction using a radioactive 17F beam. Resonance parameters for several states above an excitation energy of 7 MeV in 18Ne have been obtained. Through a measurement of the partial widths for elastic and inelastic proton scattering, it was determined that for these resonances the contribution of the 14O(α,p)17F* branch populating the first excited state in 17F is small. The results indicate that the contribution of resonances above Ex=7 MeV to the astrophysical (α,p) reaction rate is smaller than was previously assumed.

Abstract

A method has been developed that can provide beams of many short-lived nuclei of interest in nucleosynthesis along the rp process path. With a 17F beam (T1/2=64s) the excitation function of the 17F(p, α)14O reaction was measured to determine properties of excited states in 18Ne. These states influence the rate of the 14O(α, p)17F reaction which is important for understanding energy generation and nucleosynthesis in x-ray bursts. The present direct measurements yield a pattern of resonances and cross sections which differ substantially from previous estimates.

Abstract

We have studied via in-beam -ray spectroscopy Po196 and Po198, which are the first neutron-deficient Po isotopes to exhibit a collective low-lying structure. The ratios of yrast state energies and the E2 branching ratios of transitions from non-yrast to yrast states are indicative of a low-lying vibrational structure. The onset of collective motion in these isotopes can be attributed to the opening of the neutron i13/2 orbital at N112 and the resulting large overlap between the two valence protons in the h9/2 orbital and the valence neutrons in the i13/2 orbital.
