9. Gait analysis and evaluation of fall risk in patients with neuromyelitis optica spectrum disorders

Abstract

Objective To investigate the time?distance parameters of gait, balance function and mobility performance in patients with neuromyelitis optica spectrum disorders (NMOSDs) compared with healthy subjects, so as to evaluate the fall risk in NMOSDs patients. Methods Twelve patients with NMOSDs and 9 healthy subjects with matched age and sex were enrolled in this study. The time-distance parameters of gait (including cadence, stride duration, stride length, gait velocity, symmetry of phases) were assessed using RehaWatch Gait Analysis System. The fall risk was evaluated by Berg Balance Scale (BBS), Timed Up and Go Test (TUGT), Chair Rising Test (CRT) and Tandem Gait Test (TGT). Results Compared with control group, patients in NMOSDs group had slower cadence (t = - 5.354, P = 0.000), prolonged stride duration (t = 4.045, P = 0.005), lower stride length (t = -7.243, P = 0.000), slower gait velocity (t = - 10.161, P = 0.000), decreased symmetry of stance phase (t = - 2.220, P = 0.050) and symmetry of single support phase (t = - 2.359, P = 0.020), spent more seconds in performing TUGT (t = 3.197, P = 0.024) and finished fewer numbers of steps on TGT (Z = - 1.544, P = 0.049). In BBS, items related to static balance (including nonsupported stand and sit) got higher scores than items related to dynamic balance (including picking up something on the floor when standing). Conclusions NMOSDs patients had abnormal ime-distance parameters in gait cycle, decreased balance function, especially dynamic balance, and higher fall risk. 11. Analysis of balance function, falling risk and gait in the early and middle stages of patients with Parkinson's disease

Abstract

Objective To analyze the balance function, falling risk and gait in the early and middle stages of patients with Parkinson's disease (PD) for provide clinical basis for patients' rehabilitation treatment. Methods There were 30 PD patients in the early and middle stages and 15 healthy subjects matched in gender, age and degree of education. Berg Balance Scale (BBS) was used to evaluate balance function. Timed Up and Go Test (TUGT), Chair Rising Test (CRT) and Tandem Gait Test (TGT) were used to evaluate falling risk. The gait analysis system was used to evaluate gait. Results Compared with healthy subjects, PD patients obtained lower scores on BBS (P = 0.001). In the falling risk, PD patients spent more seconds in performing TUGT (P = 0.003) and CRT (P = 0.002) and finished fewer numbers of steps on TGT (P = 0.041). In 10 - Meter Walk Test (10MWT), PD patients had shorter step length (P =0.020), decreased step speed (P = 0.038), increased ratio of toe touches (P = 0.000) and decreased left and right ankle dorsiflexion in swing phase (P = 0.005, 0.006). Conclusions In the early and middle stages, PD patients have decreased balance function, increased falling risk and unusual gait. The rehabilitation treatment should be given as soon as possible. Abstract

This study was aimed at investigating the carrier rate of, and molecular variation in, α- and β-globin gene mutations in Hunan Province. We recruited 25,946 individuals attending premarital screening from 42 districts and counties in all 14 cities of Hunan Province. Hematological screening was performed, and molecular parameters were assessed. The overall carrier rate of thalassemia was 7.1%, including 4.83% for α-thalassemia, 2.15% for β-thalassemia, and 0.12% for both α- and β-thalassemia. The highest carrier rate of thalassemia was in Yongzhou (14.57%). The most abundant genotype of α-thalassemia and β-thalassemia was -α3.7/αα (50.23%) and βIVS-II-654/βN (28.23%), respectively. Four α-globin mutations [CD108 (ACC>AAC), CAP +29 (G>C), Hb Agrinio and Hb Cervantes] and six β-globin mutations [CAP +8 (C>T), IVS-II-848 (C>T), -56 (G>C), beta nt-77 (G>C), codon 20/21 (-TGGA) and Hb Knossos] had not previously been identified in China. Furthermore, this study provides the first report of the carrier rates of abnormal hemoglobin variants and α-globin triplication in Hunan Province, which were 0.49% and 1.99%, respectively. Our study demonstrates the high complexity and diversity of thalassemia gene mutations in the Hunan population. Abstract

In view of the limitation of the traditional PID controller in the processing of nonlinear time-varying systems, a neuron adaptive PID control algorithm is proposed. Based on the traditional PMSM speed control, the neuron adaptive PID control method is adopted, using the Hebb learning algorithm, the error square is used as the performance index to adjust the parameters online in order to realize the fast tracking of motor speed. The simulation results show that compared with traditional PID control mode, the neuron adaptive PID control method can achieve fast response and high precision on permanent magnet synchronous motor.

Abstract

Autosomal dominant polycystic kidney disease (ADPKD) is one of the most common life-threatening monogenetic diseases characterized by progressive enlargement of fluid-filled renal cysts. Our previous study has shown that Ganodermatriterpenes (GT) retards PKD renal cyst development. In the present study we identified the effective ingredient of GT in suppression of kidney cyst development. Using an in vitro MDCK cystogenesis model, we identified ganoderic acid A (GA-A) as the most promising candidate among the 12 ganoderic acid (GA) monomers. We further showed that GA-A (6.25−100 μM) significantly inhibited cyst growth in MDCK cyst model and embryonic kidney cyst model in vitro, and the inhibitory effect was reversible. In kidney-specific Pkd1knockout (kPKD) mice displaying severe cystic kidney disease, administration of GA-A (50 mg· kg−1·d−1, sc) significantly attenuated renal cyst development. In both MDCK cells and kidney of kPKD mice, we revealed that GA-A dose-dependently downregulated the Ras/MAPK signaling pathway. The expression of proliferating cell nuclear antigen (PCNA) was also suppressed, suggesting a possible effect of GA-A on cell proliferation. These experimental data suggest that GA-A may be the main ingredient of GT as a potential therapeutic reagent for treating ADPKD.
