17. Characteristic clinicopathologic features of adult B-cell lymphoblastic lymphoma with special emphasis on differential diagnosis with an atypical form probably of blastic lymphocytic lymphoma of intermediate differentiation origin

Abstract

Background. Lymphoblastic lymphoma is typically of thymic T-cell phenotype. Lymphoblastic lymphoma of B-cell origin (B-lymphoblastic lymphoma) has been relatively poorly described. Whether B-lymphoblastic lymphoma should be managed like its T-cell counterpart remains to be clarified. Methods. From 1983 to 1991, 10 adult patients were diagnosed as having B-lymphoblastic lymphoma at National Taiwan University Hospital by using the histomorphologic criteria of international working formulation. B-cell phenotype was determined by the immunohistochemistry method. Clinicopathologic features of these 10 patients were reviewed. Results. Seven patients were grouped as typical type and were characterized by an aggressive clinical course with lymph node (7 of 7), bone marrow (6 of 7), liver (3 of 7), spleen (3 of 7), and central nervous system (2 of 7) involvement. The median survival time was 8 months. In contrast, three patients had an atypical clinical picture. They were older patients (64-73 years) and were characterized by a relatively less aggressive course with predominantly bulky nodal involvement. Two of these three patients are alive (31 and 49 months, respectively) and well at this report, with one of them being repeatedly experiencing disease remission with the use of simple salvage chemotherapeutic regimens. Further studies revealed that tumor tissues of these three atypical cases had strong expression of CD5 (Leu-1) marker. Conclusion. B-lymphoblastic lymphoma diagnosed by histomorphologic criteria should be further distinguished from a relatively favorable subtype, which probably represents a variant of blastic lymphocytic lymphoma of intermediate differentiation as described by Lardelli et al.[1] Clinical features of typical B-lymphoblastic lymphoma, except for the lack of mediastinal involvement, is similar to its T-cell counterpart.

Abstract

This work describes a novel, first-year graduate-level analog integrated circuit (IC) design course. The course teaches students analog circuit design; an external manufacturer then produces their designs in three different silicon chips. The students, working in pairs, then test these chips to verify their success. All work is completed within one semester, and the grading cycle in the most recent offering of the course extended from September 2007 to February 2008, when there were 10 students enrolled. The manufacturer's shuttle cycle is 3.5 months. Most students in the course have only a college-level electronics background. The manufacturing process is Taiwan Semiconductor Manufacturing Company's (TSMC) 0.35 ?m CMOS Mixed-Signal 2P4M Polycide 3.3/5 V. The three successful chips consist of a voltage controlled oscillator, a high-performance differential amplifier, and a temperature-independent voltage reference generator. Section VI describes assessment and student feedback as well as proposed course improvement.

Abstract

This study utilizes a new adaptive sense current controller to get an accurate power supply. The proposed controller effectively reduces output ripple voltage of converters operated over the load current range. This reduction is realized using an adaptive sense current circuit that automatically adjusts the inductor current according to operational conditions. The proposed boost converter is designed and fabricated with a standard TSMC 3.3/5V 0.35-µm 2P4M CMOS technology. The experimental results show that the power-conversion efficiency of the proposed boost converter is 2-5% higher than that of the conventional converter with a current-limited circuit. The proposed circuit greatly reduces (i.e. by 76%) output ripple voltage compared with the conventional circuit at a 10mA loading current.

Abstract

Major source for human exposure to cadmium nitrate (Cd(NO3)2) is from smoking and occupational hazard. Cd(NO3)2-induced neurotoxicity and related mechanism in neurons are still unknown. Here, Cd(NO3)2induced cytotoxicity in a concentration- and time-dependent manners on Neuro-2A (N2A) neurons. Genotoxicity, which included micronucleus formation and DNA damage, and caspases, which included caspase-3, -8 and -9, activities were increased by Cd(NO3)2in a concentration-dependent manner. The concentration trends of Cd(NO3)2: 1, 3 and 10 μM, were similar in the significant induction of cytotoxicity and genotoxicity and the up-regulation of the activities of caspase-3, -8, -9. In conclusion, cytotoxicity and genotoxicity were induced by Cd(NO3)2via caspases activities on neurons. [ABSTRACT FROM AUTHOR]

Abstract

Objective: We evaluated the analgesic efficacy and safety of tramadol 37.5 mg/acetaminophen 325 mg combination tablet, for the treatment of breakthrough pain in cancer patients., Methods: This study was conducted at Changhua Christian Hospital, Changhua, Taiwan from January 2006 to February 2007. The single-center and open-label study enrolled 59 opioid-treated cancer patients with at least moderate breakthrough pain (visual analog scale [VAS] score ≥40mm on a 100-mm scale). The efficacy measures included VAS scores and adverse effect assessment 10, 30, and 60 minutes after the administration of tramadol/acetaminophen. Visual analog scale score at time of pain relief was reported., Results: The mean VAS score when the breakthrough pain episode began (0 minute) was 77.8. Analysis showed significant better mean pain VAS scores at 10, 30, and 60 minutes after the administration of tramadol/acetaminophen (p≤0.001 versus 0 min for all 3 time points). The mean time to pain relief was 597.2 seconds and the mean VAS score at time of relief was 43.4. The effective rates, defined by more than 30% reduction of the VAS score, after 10 minutes of administration was 74.6%, 30 minutes 86.4%, and one hour 94.9% (p≤0.001 versus 0 minute for all 3 time points). Two cases of drowsiness were reported., Conclusion: Tramadol/acetaminophen might be efficacious and safe in the treatment of breakthrough pain in cancer.
