Lipid-lowering medication is effective in reducing the risk of cardiovascular disease in several clinical scenarios. However, the evidence in patients with familial hypercholesterolemia (FH) and severe primary hypercholesterolemia is less robust.The main objective of the present systematic review was to analyze the association between lipid-lowering medication and cardiovascular risk reduction in patients with FH or severe primary hypercholesterolemia.This systematic review was performed according to PRISMA guidelines. A literature search was performed to detect studies that evaluated the association between lipid-lowering medication and cardiovascular events in FH patients. The diagnosis of FH varied in the studies analyzed. Genetic and clinical criteria or a combination of both were used. Likewise, we considered patients with severe primary hypercholesterolemia.Fourteen studies including 21059 patients were considered eligible for this research. This systematic review showed that the vast majority of the studies with statins reported a significant cardiovascular risk reduction. Statin use was associated with a lower risk of major adverse cardiovascular events (3 studies), coronary heart disease (2 studies), cardiovascular death (4 studies), all-cause mortality (4 studies) and combined endpoint of coronary heart disease and mortality (1 study). When analyzing the association between non-statin lipid-lowering medications and the incidence of cardiovascular events, the results were conflicting.Despite the low level of evidence, this systematic review showed that statins reduce cardiovascular events in patients with HeFH. Evidence for other lipid-lowering drugs is not conclusive.

La asociación entre el tejido adiposo visceral y la enfermedad cardiovascular ha sido claramente establecida. Asimismo, se ha determinado que la adiposidad ectópica se asocia con un mayor riesgo cardiovascular en comparación a la adiposidad subcutánea. En este contexto, múltiples investigaciones han evaluado el rol del tejido adiposo epicárdico (TAE) en la enfermedad cardiovascular. El TAE se localiza entre la superficie miocárdica y la hoja visceral del pericardio, y puede cuantificarse mediante técnicas no invasivas como ser el ecocardiograma, la tomografía computada o la resonancia nuclear magnética. El TAE no es simplemente un órgano de depósito. Actualmente, se considera que es un tejido metabólicamente activo capaz de secretar múltiples adipoquinas que actúan mediante diferentes vías de señalización parácrina, endócrina, vasócrina y/o autócrina. La evidencia actual sugiere que el TAE puede ser un factor contribuyente en la patogénesis de la enfermedad coronaria, asociándose además con su gravedad y progresión. En ese sentido, algunos autores han postulado al TAE como un nuevo factor de riesgo cardiovascular y como un potencial blanco terapéutico. El objetivo de esta revisión es analizar la relación del TAE con la enfermedad cardiovascular, principalmente con la enfermedad coronaria.

The association between visceral adipose tissue and cardiovascular disease has been clearly established. Likewise, it has been determined that ectopic adiposity is associated with a higher cardiovascular risk compared to subcutaneous adiposity. In this context, multiple investigations have evaluated the role of epicardial adipose tissue (EAT) in cardiovascular disease. EAT is located between the myocardial surface and the visceral layer of the pericardium, and can be quantified by noninvasive techniques such as echocardiography, computed tomography, or magnetic resonance imaging. The EAT is not simply a storage organ. Currently, it is considered to be a metabolically active tissue capable of secreting multiple adipokines that act through different paracrine, endocrine, vasocrine and/or autocrine signaling pathways. Current evidence suggests that EAT may be a contributing factor in the pathogenesis of coronary heart disease, as well as being associated with its severity and progression. In this sense, some authors have postulated EAT as a new cardiovascular risk factor and as a potential therapeutic target. The aim of this review is to analyze the association between EAT and cardiovascular disease, mainly coronary artery disease.

Introducción: Varios estudios han evaluado la asociación entre los niveles plasmáticos de lipoproteína (a) [Lp(a)] y la aparición de eventos relacionados con la estenosis valvular aórtica, aunque los resultados fueron contradictorios. Objetivo: El objetivo de esta revisión fue analizar la capacidad predictiva de los niveles elevados de Lp(a) sobre los eventos clínicos relacionados con la estenosis valvular aórtica. Material y métodos: Esta revisión sistemática se realizó de acuerdo con las recomendaciones PRISMA y STROBE. Se realizó una búsqueda en diferentes bases de datos con el objetivo de identificar estudios de cohorte que evaluaran la asociación entre los niveles de Lp(a) y los eventos de interés. El punto final primario fue la incidencia de eventos clínicos relacionados con la estenosis aórtica (reemplazo valvular aórtico, muerte u hospitalización). Esta revisión fue registrada en PROSPERO. Resultados: Se consideraron elegibles para el análisis siete estudios observacionales con un total de 58 783 pacientes. Los valores elevados de Lp(a) se asociaron con un mayor riesgo de eventos relacionados con la estenosis valvular aórtica en la mayoría de los estudios evaluados (entre un 70% y aproximadamente 3 veces más riesgo), a pesar de ajustar por otros factores de riesgo. Conclusión: Esta revisión sugiere que los niveles elevados de Lp(a) se asocian con una mayor incidencia de eventos clínicos relacionados con la estenosis valvular aórtica. Sin embargo, y considerando las limitaciones de este estudio, la utilidad clínica de la Lp(a) como marcador pronóstico en la enfermedad valvular aórtica deberá confirmarse en futuras investigaciones.

Different strategies were proposed to stratify cardiovascular risk and assess the appropriate use of statins in patients with chronic kidney disease (CKD).(1) To apply two strategies on the management of lipids in patients with CKD, analyzing what proportion of patients received lipid-lowering treatment and how many patients without statin therapy would be candidates for receiving them; (2) to identify how many patients achieve the lipid goals.A cross-sectional study was performed. Patients aged between 18 to 70 years and CKD with an estimated glomerular filtration rate (eGFR)60 ml/min/1.73 mA total of 300 patients were included. According to ESC/EAS guidelines, 62.3 and 37.7% of the population was classified at high or very high cardiovascular risk. In total, 52% of patients received statins. Applying the 2013 KDIGO and the 2019 ESC/EAS guidelines, 92.4 and 95.8% of the population without lipid-lowering treatment were eligible for statin therapy, respectively. Globally, only 9.1 and 10.6% of the patients with high or very high risk achieved the suggested lipid goals.A large proportion of patients with CKD showed considerable cardiovascular risk and were eligible for statin therapy according to the two strategies evaluated. However, observed statin use was deficient and current lipid goals were not achieved in most cases.

Background: Several studies have evaluated the association between lipoprotein(a) plasma levels [Lp(a)] and the occurrence of aortic valve stenosis related events, with contradictory results. Objective: The main objective of this systematic review was to analyze the predictive capacity of elevated Lp(a) levels on major clinical events associated with aortic valve stenosis. Methods: This systematic review was conducted in accordance with PRISMA and STROBE recommendations. A search was carried out in order to identify studies with a cohort design evaluating the association between Lp(a) levels and the events of interest. The primary endpoint was the incidence of clinical events related with aortic valve stenosis (aortic valve replacement, death or hospitalization). This review was registered in PROSPERO. Results: Seven observational studies with a total of 58 783 patients were eligible for analysis. Our findings showed that the presence of elevated Lp(a) levels was associated with an increased risk of events related with aortic valve stenosis in most of the studies evaluated (between 70% and approximately 3-fold higher risk), despite adjusting for other risk factors. Conclusion: This review suggests that elevated Lp(a) levels are associated with a higher incidence of aortic valve stenosis related clinical events. However, considering the limitations of this study, the clinical usefulness of Lp(a) as a prognostic marker in aortic valve disease should be confirmed in future investigations.

Introduction: Several studies have evaluated the relation between variables of cardiopulmonary exercise testing (CPET) and major clinical events in pulmonary hypertension (PH) patients, although the results were conflicting. The main objective of this study was to investigate the prognostic value of the CPET derived parameters on all-cause mortality or urgent transplantation in PH patients. Material and Methods: A meta-analysis of time-to-event outcomes were performed from observational studies that evaluated the predictive value of CEPT-related variables [peak oxygen uptake (VO₂) and the ventilation to CO₂ production slope (VE/VCO₂)] in PH patients, reporting data from mortality or urgent transplantation, after searching the PubMed/MEDLINE, Embase, Science Direct, Scopus, Google Scholar, and Cochrane databases. A random-effects meta-analysis model was then applied. Results: Nine eligible studies, including 986 patients, were identified and considered eligible for the quantitative analyses. This meta-analysis showed that high peak VO₂ was associated with a lower mortality or transplant occurrence (HR: 0.81; 95% CI: 0.78–0.85, I2 = 29%). In addition, high VE/VCO₂ slope was associated with a higher incidence of the primary endpoint (HR: 1.04; 95% CI: 1.02-1.06, I2 = 78%). The sensitivity analysis showed that the results were robust. Conclusions: Our data suggest that in a population with PH the CPET-related variables have predictive capacity regarding mortality and the risk of transplantation. Future studies should establish the best cut-off points for these CPET-related variables.

Sodium Glucose Co-transporter 2 inhibitors and glucagon-like peptide 1 receptor agonists (GLP-1RAs) were associated with a reduction in cardiovascular disease events in cardiovascular outcomes trials (CVOTs) in type 2 diabetes. Most of the patients included in these trials received metformin as background therapy. To evaluate the effect of glucagon-like peptide 1 receptor agonists on major cardiovascular events (MACE) and mortality in metformin-naive patients with type 2 diabetes. A systematic review and meta-analysis of randomized controlled clinical trials of GLP-1RAs on type 2 diabetes population was performed, after searching the PubMed/MEDLINE, Embase, Scielo, Google Scholar and Cochrane Controlled Trials databases. The primary endpoint was MACE. The secondary endpoints were cardiovascular death and all-cause mortality. A meta-analysis of time-to-event outcomes was performed. This meta-analysis was registered in PROSPERO (CRD42021260040) Seven trials, including 11510 patients, were identified and considered eligible for the analyses. GLP-1RAs were associated with a significant reduction in MACE incidence (HR: 0.86, 95% confidence interval: 0.79–0.94; I2: 0%). The secondary endpoints analysis showed a non-significant reduction in all-cause mortality (HR: 0.86, 95% confidence interval: 0.73–1.00 I2: 0%) and cardiovascular mortality (HR: 0.81, 95% confidence interval: 0.63–1.05; I2: 0%). In this meta-analysis, GLP-1RAs reduced the incidence of MACE in patients with type 2 diabetes without metformin at baseline, without significant reduction in all-cause mortality and cardiovascular mortality. These results support the fact that when a GLP-1RAs is administered, the benefit on cardiovascular outcomes is independent of the use of metformin.

International audience; Introduction Obesity and its co-morbidities, including type 2 diabetes (T2DM) and dyslipidemia, are accompanied by excess cardiovascular morbi-mortality. Aside from excess low density lipoprotein-cholesterol (LDL-C), atherogenic dyslipidemia (AD), mainly characterized by elevated triglycerides and decreased high density lipoprotein-cholesterol (HDL-C) levels, is often present in T2DM obese patients. Bariatric surgery, such as Roux-en-Y gastric bypass (RYGB) and sleeve gastrectomy (SG), has become a reference treatment in that population. However, the respective effects of RYGB vs SG on lipid metabolism in T2DM patients have been rarely studied.Methods A meta-analysis of randomized controlled trials, comparing the effects of RGYBG vs SG on lipid metabolism 12 months after surgery in T2DM patients, was performed.Results Four studies including a total of 298 patients (151 patients in the RYGB and 147 patients in the SG group) were examined. The relationship between physical activity and coronary artery calcification (CAC) was evaluated in different studies during the last years, although the results were conflicting.The main objective of the present systematic review was to assess the association between different levels of physical activity and CAC score estimated by computed tomography (CT).This systematic review was performed according to PRISMA guidelines. A literature search was performed to detect studies that evaluated the association between physical activity and CAC score. The levels of physical activity evaluated were those reported by the original publications. The CAC score was estimated by CT and was reported in Agatston units.Twenty six studies including 89,405 subjects were considered eligible for this research. The studies developed in the general population showed different results regarding the association between physical activity and CAC score: no association (7 studies), a positive association (4 studies), an inverse relationship (6 studies), a U-shaped relationship (2 studies), or different results depending on the subgroup evaluated (2 studies). In the largest studies, a positive association was observed. When we analyzed the studies that evaluated athletes, four studies showed a positive association between exercise intensity and CAC.This systematic review showed disparate results regarding the association between physical activity and CAC score. The largest studies and most studies developed in athletes suggest that intense physical activity could be associated with high CAC score, although this hypothesis should be confirmed in future research.

Objective: The association between the use of colchicine and the incidence of acute myocardial infarction (AMI) is inconsistent. The main objective of this study was to evaluate the effect of colchicine on the incidence of AMI. Assessment of the incidence of stroke and cardiovascular mortality were secondary endpoints. Methods: A meta-analysis of randomized studies that evaluated the use of colchicine in patients with atherosclerotic disease and reported cardiovascular events was performed, after searching the PubMed/MEDLINE, Embase, Scielo and Cochrane Controlled Trials databases. A fixed or random effects model were used depending on the heterogeneity observed. Results: Seven studies were selected for the analysis of the primary end point (5966 subjects in the colchicine arm and 5948 patients in the control arm). This meta-analysis demonstrated that colchicine therapy was associated with a lower risk of AMI (OR: 0.76, 95% CI: 0.62-0.92; I2 = 15%). Likewise, a significant reduction in the incidence of stroke was observed without a significant effect on cardiovascular mortality with pharmacological intervention. Conclusion: The use of colchicine in patients with atherosclerotic cardiovascular disease was associated with a significant reduction in the incidence of AMI. The incorporation of colchicine into the therapeutic arsenal of cardiovascular disease should be considered by future clinical practice guidelines.

Introducción y objetivos: La asociación entre el uso de colchicina y la incidencia de infarto agudo de miocardio (IAM) es heterogénea. El objetivo principal del presente estudio fue evaluar el efecto de la colchicina sobre la incidencia de IAM. La evaluación de la incidencia de accidente cerebrovascular (ACV) y la mortalidad cardiovascular fueron los objetivos secundarios. Material y métodos: Se realizó un metaanálisis de estudios aleatorizados que evaluaron el uso de colchicina en pacientes con enfermedad aterosclerótica y que reportaron los eventos cardiovasculares, luego de una búsqueda en las bases de datos PubMed/MEDLINE, Embase, Scielo y Cochrane Controlled Trials. Se utilizó un modelo de efectos fijos o aleatorios según la heterogeneidad observada. Resultados: Se seleccionaron para el análisis del punto final primario 7 estudios (con un total de 5966 sujetos en la rama colchicina y 5948 pacientes en la rama control). Este metaanálisis demostró que la terapia con colchicina se asoció a un menor riesgo de IAM (OR: 0,76, IC 95%: 0,62-0,92; I2 =15%). Asimismo, se observó una reducción significativa en la incidencia de ACV, sin un efecto significativo en la mortalidad cardiovascular, con la intervención farmacológica. Conclusión: El uso de colchicina en pacientes con enfermedad cardiovascular aterosclerótica se asoció a una reducción significativa en la incidencia de IAM. La incorporación de la colchicina dentro del arsenal terapéutico de la enfermedad cardiovascular deberá ser considerada por las futuras guías de práctica clínica.

Elevated lipoprotein(a) [Lp(a)] levels are independently associated with atherosclerotic cardiovascular disease, although this association is less explored in postmenopausal women. The main objective of this systematic review was to analyze the association between elevated Lp(a) levels and cardiovascular outcomes in posmenopausal women. Studies that evaluated this association were searched in the current literature. Ten studies including 157.690 women were considered eligible for this study. In total, 4 prospective cohorts, 3 cross-sectional studies, 2 nested case-control studies, and one post-hoc analysis from a randomized clinical trial were analyzed. The included studies showed different results regarding the association between Lp(a) levels and cardiovascular outcomes: a positive association (4 studies), no association (2 studies), or different results depending on the subgroups or outcomes evaluated (4 studies). The results were robust when evaluating coronary events. The reduction in coronary events attributed to a hormone replacement therapy-associated decrease in Lp(a) levels was controversial.

Patients with coronary heart disease and their families often face numerous changes in their lives. Recommendations on sexual activity (SA) should be included in the management of these patients. The objective of this study was to evaluate the degree of knowledge and professional attitude regarding the patient's SA.A descriptive and analytical cross-sectional study was carried out. The data were obtained from a structured, virtual and anonymous survey that was carried out among cardiologists.Three hundred forty-five surveys were analyzed. In total, 63.8% considered sexual dysfunction as a cardiovascular risk marker. Likewise, 68.1% considered it relevant or very relevant to ask patients about SA. Regarding counseling, it is believed that it should be initiated primarily by the cardiologist. A more active attitude regarding the re-initiation of SA after a cardiovascular event, a greater interest in SA training, more testosterone orders and more indications of phosphodiesterase inhibitors were observed in professionals older than 60 years and male compared to younger or female physicians, respectively.This study showed that the degree of knowledge of the patients about the aspects related to SA was poor. Given the relevance of the topic, we consider it important to strengthen medical education in all areas.Los pacientes con enfermedad coronaria y sus familias, a menudo enfrentan numerosos cambios en sus vidas. Las recomendaciones sobre la actividad sexual (AS) deben incluirse en el manejo de estos pacientes. El objetivo de este estudio fue evaluar el grado de conocimiento y la actitud profesional con respecto a la AS del paciente.Se realizó un estudio descriptivo y analítico de corte transversal. Los datos se obtuvieron a partir de una encuesta estructurada, virtual y anónima, realizada entre médicos cardiólogos/as.Se analizaron 345 encuestas. El 63.8% consideró la disfunción sexual como un marcador de riesgo cardiovascular. Asimismo, el 68.1% consideró relevante o muy relevante interrogar acerca de la AS. En relación con el asesoramiento, se cree que fundamentalmente lo debería comenzar el cardiólogo/a. Se observó una actitud más activa, respecto al abordaje del reinicio de la AS luego de un evento cardiovascular, un mayor interés por capacitarse en temas relacionados con la AS y una mayor solicitud de dosaje de testosterona e indicación de inhibidores de la fosfodiesterasa tipo 5 en los profesionales60 años y de sexo masculino en comparación con los médicos más jóvenes o de sexo femenino, respectivamente.Este estudio mostró que el grado de conocimiento sobre los aspectos relacionados con la AS de los pacientes fue deficiente. Dada la relevancia del tema, consideramos importante fortalecer la educación médica en todos los ámbitos.

Background: Despite strong association of elevated lipoprotein (a) (Lp(a)) levels with incident coronary and cerebrovascular disease, data for incident peripheral artery disease (PAD) are less robust. The main objective of the present systematic review was to analyze the association between elevated Lp(a) levels and PAD outcomes. Methods: This systematic review was performed according to PRISMA guidelines. A literature search was performed to detect randomized clinical trials or observational studies with a cohort design that evaluated the association between Lp(a) levels and PAD outcomes. Results: Fifteen studies including 493,650 subjects were identified and considered eligible for this systematic review. This systematic review showed that the vast majority of the studies reported a significant association between elevated Lp(a) levels and the risk of PAD outcomes. The elevated Lp(a) levels were associated with a higher risk of incident claudication (RR: 1.20), PAD progression (HR: 1.41), restenosis (HR: 6.10), death and hospitalization related to PAD (HR: 1.37), limb amputation (HR: 22.75), and lower limb revascularization (HR: 1.29 and 2.90). In addition, the presence of elevated Lp(a) values were associated with a higher risk of combined PAD outcomes, with HRs in a range between 1.14 and 2.80, despite adjusting for traditional risk factors. Heterogeneity of results can be explained by different patient populations studied and varying Lp(a) cut-off points of Lp(a) analyzed. Conclusion: This systematic review suggests that evidence is available to support an independent positive association between Lp(a) levels and the risk of future PAD outcomes. The COVID-19 pandemic elicited important changes in community habits and behaviors, including a distancing of people from the healthcare system. The objective of this work was to understand the causes that gave rise to changes in behavior from an individual perspective in the Argentine Republic. We performed a cross-sectional, web-based survey using an online questionnaire. The survey was distributed via the WhatsApp application for smartphones among subjects across the Argentine territory using a combination of convenience and snowball sampling. We received 6176 responses. Almost 70% of respondents manifested fear of visiting a physician. One third of respondents manifested having a desire or need in at least one occasion of visiting a physician but, of these, 48% avoided it. The main reasons for avoiding visits were: 1) a recommendation of staying home (40%); 2) lack of access to the physician (35%); and 3) fear of contagion (26%). The most common form of consultation was through unconventional means (e-mail, telephone, or WhatsApp). One of 5 respondents had difficulties to obtain a medication prescription and 5% stopped the use of at least one medication. Regarding healthy habits, almost 2/3 of those surveyed stated that they became more sedentary; 11% of hypertensive patients increased their consumption of salt and 15% saw their blood pressure values increase, while 16% of dyslipidemic patients showed increased consumption of fats.La pandemia de COVID-19 determinó un importante cambio de los hábitos y comportamientos comunitarios, entre ellos se observó un distanciamiento de la gente del sistema de salud y el abandono de hábitos saludables. El objetivo de este trabajo fue comprender las causas que dieron lugar a dichos cambios de comportamiento desde una perspectiva individual y evaluar el impacto en el control de los factores de riesgo cardiovasculares. Realizamos una encuesta utilizando un cuestionario en línea y distribuida mediante la aplicación WhatsApp entre personas de la Argentina utilizando una combinación de muestreo por conveniencia y en "bola de nieve". Recibimos 6176 respuestas, casi el 70% de los encuestados manifestaron temor de realizar una consulta médica. Un tercio de los encuestados manifestó haber tenido necesidad de ir al médico en al menos una oportunidad desde el inicio de la pandemia, pero de éstos, el 48% evitó la visita. Las principales razones para evitarla: 1) la recomendación de quedarse en casa (40%); 2) difi cultad para acceder al sistema de salud (35%); y 3) miedo al contagio (26%). La forma de consulta más común fue a través de medios no convencionales (correo electrónico, teléfono o WhatsApp). Uno de cada 5 encuestados tuvo dificultades para obtener una receta y el 5% dejó de tomar al menos un medicamento. En cuanto a los hábitos saludables, casi 2/3 de los encuestados afirmaron volverse más sedentarios, el 11% de los hipertensos aumentó su consumo de sal y el 15% aumentó sus valores de presión arterial, mientras que el 16% de los dislipidémicos mostró un mayor consumo de grasas.
