In this work, we estimate the effect of photoinduced screening breakdown of the Coulomb potential mediated by plasmons. In contrast to previous studies, we consider the contribution from the divergence of renormalized inverse electron permittivity, which enhances Coulomb interaction at longer ranges. Our work reveals that the Coulomb potential acquires oscillating terms, which for the charged plane case could lead to a spatial heterostructure. In the resonant case ($\omega_p = \Omega$), photoinduced Coulomb interaction becomes attractive for small values of momenta. This Coulomb interaction renormalization could potentially lead to instabilities in the electron system, such as superconductivity and/or charge density waves. We found possible divergence of the electron coupling constant and discuss herein a potential new mechanism of superconductivity based on electron-electron interaction renormalization via the plasmon oscillating electric field. 5. Trust in the data : external data use by the Scottish third sector

This thesis investigates how the Scottish third sector engages with external data resources. The research is broken into two main strands. The first strand determines factors which affect the level of data use by charities. The second concerns the role of trust in third sector data use, in particular, what effect third sector use of data has on other users of data and the role data plays in society more generally. While the first of these strands is more pertinent to charities themselves, the second is particularly important in the wake of 'Fake news' and a general decline in trust for experts driven by misinformation online. Findings from this research show that there is a relatively low ability to use data among Scottish charities and many are resorting to pre-analysed, aggregate findings. Factors related to a low ability to use data include being a small charity, an old charity and a charity with a narrow focus. Analysis of a series of barriers and enablers found that barriers tend to inhibit data use altogether, where enablers tend to determine level of use. Support to help facilitate data use was then considered, finding that Twitter acts as a forum where support relationships develop between charities and infrastructure bodies, who share widely aimed, one-to-many tweets to support data use. The importance of infrastructure organisation became even more apparent when issues of trust were considered and found that, while charities trust data, they are less trusting of the interpretation which is laid over data and therefore they invest their trust in infrastructure organisations. Infrastructure organisations were found to have a healthy distrust of government data and are invested in feedback mechanisms where they correct mistakes in data and help increase the quality of, and trust in, Scottish Government data more generally.

8. Acceptability of adding a non-contrast abdominal CT scan to screen for kidney cancer and other abdominal pathology within a community-based CT screening programme for lung cancer: A qualitative study.

Objectives: The Yorkshire Kidney Screening Trial (YKST) is a feasibility study of adding non-contrast abdominal CT scanning to screen for kidney cancer and other abdominal malignancies to community-based CT screening for lung cancer within the Yorkshire Lung Screening Trial (YLST). This study explored the acceptability of the combined screening approach to participants and healthcare professionals (HCPs) involved in the trial. Methods: We conducted semi-structured interviews with eight HCPs and 25 participants returning for the second round of scanning within YLST, 20 who had taken up the offer of the additional abdominal CT scan and five who had declined. Transcripts were analysed using thematic analysis, guided by the Theoretical Framework of Acceptability. Results: Overall, combining the offer of a non-contrast abdominal CT scan alongside the low-dose thoracic CT was considered acceptable to participants, including those who had declined the abdominal scan. The offer of the additional scan made sense and fitted well within the process, and participants could see benefits in terms of efficiency, cost and convenience both for themselves as individuals and also more widely for the NHS. Almost all participants made an instant decision at the point of initial invitation based more on trust and emotions than the information provided. Despite this, there was a clear desire for more time to decide whether to accept the scan or not. HCPs also raised concerns about the burden on the study team and wider healthcare system arising from additional workload both within the screening process and downstream following findings on the abdominal CT scan. Conclusions: Adding a non-contrast abdominal CT scan to community-based CT screening for lung cancer is acceptable to both participants and healthcare professionals. Giving potential participants prior notice and having clear pathways for downstream management of findings will be important if it is to be offered more widely. [ABSTRACT FROM AUTHOR]

16. Optimising outcomes in the treatment of superficial venous insufficiency

The traditional “gold-standard” treatment for symptomatic SVI affecting the GSV is conventional open surgery and stripping under general anaesthesia. Despite improved QoL and cost-effectiveness when compared to conservative management, conventional surgery is not without drawbacks. Endovenous ablative treatments have been developed, which seek to address some of these limitations. Randomised clinical trial (RCT) data has demonstrated the superiority of endovenous laser ablation (EVLA) over surgery in the short term. Attention is now focused on evaluating its mid- and long-term outcomes, and to further evolve the technique to improve patient outcomes. In this thesis, five studies were conceived to address two main objectives. Firstly, two-year follow-up of the HELP-1 RCT of EVLA versus conventional surgery was performed to assess clinical, QoL and duplex ultrasound (DUS) outcomes and identify potential for EVLA technique evolution. Four further studies were performed, aimed at improvement of patient outcomes by modification of the EVLA technique via i) pH buffering of tumescent anaesthesia, ii) concomitant treatment of varicosities, and iii) endovenous energy delivery via longer wavelength laser. Two-year outcomes from the HELP-1 RCT demonstrated continued superiority of EVLA over conventional surgery in terms of lower clinical recurrence rates, with maintained improvements in clinical and QoL outcomes. DUS outcomes identified patterns of clinical recurrence that can be addressed by simple modifications of the EVLA technique. Buffering of tumescent anaesthesia resulted in significantly reduced patient-reported periprocedural pain. Concomitant treatment of varicosities with ambulatory phlebectomy under tumescent anaesthesia demonstrated significant benefits in clinical severity and disease-specific QoL over foam sclerotherapy. Use of longer laser wavelength (1470nm) resulted in significantly reduced postprocedural pain in comparison to shorter (810nm) wavelength. EVLA is demonstrated to have significant short- and medium-term benefits over conventional surgery. Further evolution of the technique, including the modifications described, should provide additional benefit in terms of patient outcomes.

- Abstract
This article presents findings from the Housing and Ageing programme conducted in 2018 that investigated how the housing sector can effectively plan for an ageing population. The project took a transdisciplinary approach to focus on new, critical insights into the process of decision making concerning housing and ageing across Scotland, England and Wales. A ‘Serious Game’ methodology was developed that explored over 200 policy maker, practitioner and service user perspectives. This was used as a framework to capture priorities, decisions, negotiations and processes that indicate how a ‘sense of place’ and ‘place belonging’ can influence the development of suitable housing for older people. Key housing provision challenges identified were tackling inequality, preserving autonomy, in(ter)dependence, empowerment and accessibility. Such challenges need consideration when strategically planning for the future. The findings recommend placing housing at the heart of service integration to support the co-production of decisions that emphasise the importance of working together across boundaries within social policy, service and stakeholder groups. A place-based approach can support the perception that we are all stakeholders in ageing.

Background A reliable system for grading operative difficulty of laparoscopic cholecystectomy would standardise description of findings and reporting of outcomes. The aim of this study was to validate a difficulty grading system (Nassar scale), testing its applicability and consistency in two large prospective datasets. Methods Patient and disease-related variables and 30-day outcomes were identified in two prospective cholecystectomy databases: the multi-centre prospective cohort of 8820 patients from the recent CholeS Study and the single-surgeon series containing 4089 patients. Operative data and patient outcomes were correlated with Nassar operative difficultly scale, using Kendall’s tau for dichotomous variables, or Jonckheere–Terpstra tests for continuous variables. A ROC curve analysis was performed, to quantify the predictive accuracy of the scale for each outcome, with continuous outcomes dichotomised, prior to analysis. Results A higher operative difficulty grade was consistently associated with worse outcomes for the patients in both the reference and CholeS cohorts. The median length of stay increased from 0 to 4 days, and the 30-day complication rate from 7.6 to 24.4% as the difficulty grade increased from 1 to 4/5 (both p p Conclusion We have shown that an operative difficulty scale can standardise the description of operative findings by multiple grades of surgeons to facilitate audit, training assessment and research. It provides a tool for reporting operative findings, disease severity and technical difficulty and can be utilised in future research to reliably compare outcomes according to case mix and intra-operative difficulty.
