Cases of AIDS have been reported in the U.S.A. since 1981 and its aetiological agent, human immunodeficiency virus type 1, HIV-1, has been spreading in the population for longer. A testing system was established at the hepatitis Reference Laboratory, HRL, to identify those infected in the Glasgow and West of Scotland population. The performance of commercial screening tests was examined using sera locally available at the HRL. Confirmation of reactivity was confirmed by western blot analysis; other methods of confirmation were examined. The criteria of positivity on a western blot were established from observation of the antibody response in seroconversion and weakly reactive samples compared to false reactive or indeterminate samples. In this way genuine HIV-1 infection could be reliably diagnosed.

Background: The first signal of a new infection is often severe cases presenting at hospital. Enhanced surveillance of these cases is critical to learning more about disease epidemiology and patient outcomes, but nationallevel surveillance can lack power to draw conclusions. In response to the emergence of SARS-CoV-2, the Influenza-Monitoring Vaccine Effectiveness (I-MOVE) network, founded in 2007, expanded to establish the I-MOVE-COVID-19 Consortium in February 2020. The Consortium’s surveillance objectives included using pooled data to describe clinical and epidemiological characteristics of hospitalised COVID-19 patients across Europe, in order to contribute to the knowledge base, guide patient management, and inform public health response. Methods: Eleven study sites participated in the surveillance, including 23 hospitals across six EU Member States and Albania, and hospitals nationally in England and Scotland. A standardised protocol and dataset for collection was agreed by April 2020. In England and Scotland, data were generated by linkage of routine datasets; other sites used bespoke paper or electronic questionnaires. Data were submitted, pooled and analysed quarterly. Results: Data were received regarding 84,297 COVID-19 patients hospitalised between 1 February 2020 and 31 January 2021. Three surveillance bulletins were published between September 2020 and March 2021, providing key insights into severe COVID-19 at European level. However, the unexpected, overwhelming workload at participating sites, and difficulties securing data protection and ethics permissions, delayed data submissions and presented challenges for timely analysis. Conclusions: Building on an existing network facilitated a novel European multicentre hospital surveillance system to be implemented during a pandemic; however, timeliness was nonetheless problematic. In future, processes could be streamlined e.g. by developing pre-approved template protocols with information governance and ethical approvals in place during the inter- pandemic period. BACKGROUND: Global genetic diversity of HIV-1 is a major challenge to the development of HIV vaccines. We aimed to estimate the regional and global distribution of HIV-1 subtypes and recombinants during 1990-2015. METHODS: We searched PubMed, EMBASE (Ovid), CINAHL (Ebscohost), and Global Health (Ovid) for HIV-1 subtyping studies published between Jan 1, 1990, and Dec 31, 2015. We collected additional unpublished HIV-1 subtyping data through a global survey. We included prevalence studies with HIV-1 subtyping data collected during 1990-2015. We grouped countries into 14 regions and analysed data for four time periods (1990-99, 2000-04, 2005-09, and 2010-15). The distribution of HIV-1 subtypes, circulating recombinant forms (CRFs), and unique recombinant forms (URFs) in individual countries was weighted according to the UNAIDS estimates of the number of people living with HIV (PLHIV) in each country to generate regional and global estimates of HIV-1 diversity in each time period. The primary outcome was the number of samples designated as HIV-1 subtypes A, B, C, D, F, G, H, J, K, CRFs, and URFs. The systematic review is registered with PROSPERO, number CRD42017067164. FINDINGS: This systematic review and global survey yielded 2203 datasets with 383 519 samples from 116 countries in 1990-2015. Globally, subtype C accounted for 46·6% (16 280 897/34 921 639 of PLHIV) of all HIV-1 infections in 2010-15. Subtype B was responsible for 12·1% (4 235 299/34 921 639) of infections, followed by subtype A (10·3%; 3 587 003/34 921 639), CRF02_AG (7·7%; 2 705 110/34 921 639), CRF01_AE (5·3%; 1 840 982/34 921 639), subtype G (4·6%; 1 591 276/34 921 639), and subtype D (2·7%; 926 255/34 921 639). Subtypes F, H, J, and K combined accounted for 0·9% (311 332/34 921 639) of infections. Other CRFs accounted for 3·7% (1 309 082/34 921 639), bringing the proportion of all CRFs to 16·7% (5 844 113/34 921 639). URFs constituted 6·1% (2 134 405/34 921 639), resulting in recombinants accounting for 22·8% (7 978 517/34 921 639) of all global HIV-1 infections. The distribution of HIV-1 subtypes and recombinants changed over time in countries, regions, and globally. At a global level during 2005-15, subtype B increased, subtypes A and D were stable, and subtypes C and G and CRF02_AG decreased. CRF01_AE, other CRFs, and URFs increased, leading to a consistent increase in the global proportion of recombinants over time. INTERPRETATION: Global and regional HIV diversity is complex and evolving, and is a major challenge to HIV vaccine development. Surveillance of the global molecular epidemiology of HIV-1 remains crucial for the design, testing, and implementation of HIV vaccines. 31. Results from a national treatment database – does it matter which ART combination is prescribed in the real world?

Introduction: Clinical trials frequently show differences in viral load (VL) response between antiretroviral therapy (ART) regimes. Patterns of prescribing vary from country to country (Mocroft et al. Infection 2014 Jun 6 [epub ahead of print]), and are likely to vary between individual clinics. Scotland has a national database that records VL results and specific ART regimes for every patient under care, thus allowing different prescribing patterns between clinical centres to be monitored. Does this reveal any difference in achievement of undetectable VL? Materials and Methods: We interrogated the database held by Health Protection Scotland (HPS) that contains a record of every VL result matched against prescribed ART. Results were censored at the end of December 2013 and are based on the latest attendance of patients who have been under monitoring for at least six months. For simplicity, we have broken the results into class of drug rather than individual drugs for example, nucleoside reverse transcriptase inhibitor (NRTI) rather than lamivudine, abacavir etc. The data were analyzed using univariate Poisson regression. Results: The anonymized records of 3302 individual patients who attended in 11 separate regions were scrutinized. Sixty‐eight different combinations of antiretroviral regimes were identified. The prescribing patterns for the five most frequently prescribed regimes in the four largest clinics are shown in Table 1, along with the overall percentage of patients with undetectable VL. A higher proportion of patients in Scotland who are prescribed regimes of NRTI×2 or NRTI/NtRTI plus PI have detectable VL but this is not statistically significant. Although the percentage of patients with VL Conclusions: Overall, a high proportion of Scottish patients on ART have undetectable VL. Patterns of ART prescribing in Scotland do vary by region but there are no significant differences in outcome with regard to undetectable VL. Cytotoxic T lymphocytes (CTLs) control viral infections by recognizing viral peptides presented by major histocompatibility complex (MHC) class I molecules. Human leukocyte antigen (HLA)-A11-restricted CTLs that recognize peptide residues 416 to 424 of the Epstein-Barr virus (EBV) nuclear antigen-4 frequently dominate EBV-induced responses in [A11.sup.+] Caucasian donors. This epitope is conservved in type A EBV strains from Caucasians and central African populations, where A11 relatively infrequent. However, strains from highly [A11.sup.+] populations in New Guinea carry a lysine-to-threonine mutation at residue 24 that abrogates CTL recognition and binding of the peptide to nascent A11 molecules. The results suggest that evolution of a widespread and genetically stable virus such as EBV is influenced by pressure from MHC-restricted CTL responses., EBV-specific CTL memory can be demonstrated in virtually all healthy virus-carrying individuals [1, 2]. Objective\ud \ud To examine HIV prevalence, HIV testing behaviour, undiagnosed infection and risk factors for HIV positivity among a community sample of gay men in Scotland.\ud \ud Methods\ud \ud Cross-sectional survey of gay and bisexual men attending commercial gay venues in Glasgow and Edinburgh, Scotland with voluntary anonymous HIV testing of oral fluid samples in 2011. A response rate of 65.2% was achieved (1515 participants).\ud \ud Results\ud \ud HIV prevalence (4.8%, 95% confidence interval, CI 3.8% to 6.2%) remained stable compared to previous survey years (2005 and 2008) and the proportion of undiagnosed infection among HIV-positive men (25.4%) remained similar to that recorded in 2008. Half of the participants who provided an oral fluid sample stated that they had had an HIV test in the previous 12 months; this proportion is significantly higher when compared to previous study years (50.7% versus 33.8% in 2005, p25 years) was associated with HIV positivity (1.8% in those \ud \ud Conclusion\ud \ud HIV transmission continues to occur among gay and bisexual men in Scotland. Despite evidence of recent testing within the previous six months, suggesting a willingness to test, the current opt-out policy may have reached its limit with regards to maximising HIV test uptake. Novel strategies are required to improve regular testing opportunities and more frequent testing as there are implications for the use of other biomedical HIV interventions.

Curtin researcher Tom Fitzgerald amassed a wealth of evidence that John Curtin wrote under the pen name "Vigilant" in the early years of his editorship of the Westralian Worker. If true, the personal and literary columns penned by "Vigilant" provide new insights into the inner temperament of Australia's war time Prime Minister. Fitzgerald's evidence for Curtin as "Vigilant" is presented in this paper and the attribution is further explored by applying stylistic tests developed at the Centre for Literary and Linguistic Computing at the University of Newcastle (NSW).

This paper explores the journey Curtin University Library has taken in providing digital access to recently acquired collections. It discusses the collections, outlines important criteria and processes, and presents the challenges and benefits of digitising collections.Building on the expertise gained in establishing the digital archive of the John Curtin Prime Ministerial Library, Curtin University Library has since developed a number of digital collections with themes as varied as politics, literature, folklore and circumnavigating the world.In order to consider collections of vastly different subject matter, formats and information types for acceptance, the Library developed ?Archival Collections Principles? and a selection checklist for donations. These documents ensure decisions are made in a consistent and defensible manner, and take into account available resources. The range of formats (e.g. print, audio, video, images, objects) and information types (e.g. speeches, scrapbooks, journal articles, plays, sail plans, echo sound charts, questionnaires, interviews) across the collections has required an individual and creative approach to organising and providing digital access to each collection.The benefits resulting from these activities include utilising and developing staff expertise, making valuable collections widely available where they otherwise would have remained largely inaccessible, and enhancing the reputation of Curtin University.For libraries considering developing a digital collection, this paper outlines some useful principles and guidelines with which to proceed.
