In JT-60U, simultaneous excitation of n = 1 helical cores (HCs) and m/n = 2/1 Tearing Modes (TMs) was observed [T. Bando et al., Plasma Phys. Control. Fusion 61 115014 (2019)]. In this paper, we have investigated the excitation mechanism of n = 1 HCs with m/n = 2/1 TMs based on the experimental observations and a simple quasi-linear MHD model. In the previous study, it was reported that a "coupling" on the phase of the MHD mode is observed between n = 1 HCs and m/n = 2/1 TMs. In this study, it is found that the coupling is observed with the mode frequency from several Hz to 6 kHz. This indicates that the resistive wall and the plasma control system do not induce the coupling because the both time scales are different from the mode frequency. In addition, n = 1 HCs appear to be the non-resonant mode from the two observations: n = 1 HCs do not rotate with the plasma around the q = 1 surface in the core and the coupling is also observed even when qmin > 1. It is also observed that the electron fluctuation due to an n = 1 HC in the core region disappears with the stabilization of an m/n = 2/1 neoclassical tearing mode by electron cyclotron current drive, implying that n = 1 HCs are driven by m/n = 2/1 TMs. This perspective, n = 1 HCs are driven by m/n = 2/1 TMs, is supported by the observation that the saturated amplitude of the m/n = 1/1 component of the radial displacement in the core is smaller than that of the m/n = 2/1 component. Finally, we revisit a quasi-linear MHD model where the m/n = 1/1 HC is induced directly by the sideband of the current for the m/n = 2/1 TM, which allows to excite the non-resonant m/n = 1/1 mode. The model also describes the characteristic of the coupling, fm/n=1/1(HC) = 2fm/n=2/1(TM).

- Abstract
A real-time processing system for the two-color CO
2 laser interferometer on the JT-60SA has been developed for density feedback control. The system has a novel feature that can detect fringe jumps due to off-normal events, such as loss detection due to displacement of the beam axis and changes in the laser wavelengths. Because a phase change due to the JT-60SA plasma is smaller than π/2, corresponding to the line-integral electron density NL of ∼ 6 × 1 0 19 m−2 in a short interval of 500 ns, the threshold of the fringe jump detection is decided to be π/2. Hence, off-normal events can be detected from a fringe jump, leading to the abort of the real-time feedback control. In the density feedback control of the JT-60SA plasma, the system is employed as a density monitor, with NL being successfully controlled at 16.8% ± 6.6% lower than the reference. The clinical factors that may affect adherence to regorafenib remain unclear. The aim of this study was to evaluate adherence to regorafenib with mCRC and to identify factors that might affect adherence to regorafenib. Methods: A total of 108 consecutively enrolled Japanese patients with mCRC received regorafenib. Adherence was measured by pharmacists using pill counts and a self-reported treatment diary for patients at a pharmaceutical outpatient clinic. The median relative dose intensities of regorafenib and the factors adversely affecting adherence were retrospectively surveyed. Logistic regression analysis was then performed using patient socio-demographic factors and clinical factors. Results: A total of 96 patients were included in the analysis. The median adherence rate was 61.7% in the first cycle. The median relative dose intensity was 57.1%. The most common reason for non-adherence was a hand-foot-skin reaction (35.6%). On multivariate analysis, increased non-adherence to regorafenib was significantly associated with sex (female) [odds ratio (OR) = 4.36; 95% confidence interval (CI): 1.43–13.22, p = 0.01].Discussion: Hand-foot-skin reactions and female sex were associated with lower adherence to regorafenib. Since these factors could be associated with lower adherence to regorafenib, it would be useful to consider these factors when assessing adherence. Keywords: adherence, regorafenib, colorectal cancer, hand-foot-skin reactions

Although platinum-based combination chemotherapy is commonly used as first-line treatment, the benefit of amrubicin (AMR) and salvage chemotherapy in those who develop platinum-refractory GI-NEC remains unknown. This study aimed to evaluate the efficacy and safety of AMR monotherapy in patients with platinum-refractory GI-NEC.Patients and methods: Platinum-refractory GI-NEC patients who received AMR monotherapy between April 2012 and September 2017 were retrospectively analyzed. The overall response rate (ORR), progression-free survival (PFS), overall survival (OS), and adverse events were evaluated. PFS and OS were estimated using Kaplan-Meier methods and compared using log-rank tests. Results: In total, 16 patients were enrolled. Of them, 13 (81.3%), 1 (6.2%), and 2 (12.5%) received cisplatin plus irinotecan, cisplatin plus etoposide, and fluoropyrimidine plus platinum, respectively, before AMR monotherapy. The primary sites of NEC included the esophagus (N=3, 18.8%), stomach (N=10, 62.5%), duodenum (N=1, 6.2%) and colorectum (N=2, 12.5%). Patients were administered a median of 3 (range, 1–15) cycles of AMR. The ORR was 6.3%, and the median PFS and OS were 2.9 months (95% CI: 1.7–7.4) and 13.8 months (95% CI: 7.9–23.5), respectively. Neutropenia was the most serious adverse event. Grade 3 or higher neutropenia and febrile neutropenia occurred in 50.0% and 6.2% of patients, respectively. Other nonhematological toxicities were not severe, and no treatment-related deaths occurred. The 10 patients who received subsequent chemotherapy after AMR had significantly longer OS than those who did not (17.3 months vs 8.9 months; p=0.018). The median PFS of those who received organ-specific subsequent chemotherapy after AMR was 3.8 months, which was longer than that of those who received prior AMR.Conclusion: AMR is feasible with minimal side effects for platinum-refractory GI-NEC. Organ-specific subsequent chemotherapy after AMR may improve patient survival. Keywords: amrubicin monotherapy, platinum refractory, gastrointestinal neuroendocrine carcinoma

Patients who receive insufficient doses of antihypertensive agents or who are poorly adherent to multidrug treatment regimens often fail to achieve adequate blood pressure (BP) control. The aim of this study was to determine the efficacy of an angiotensin II receptor blocker (ARB) and calcium channel blocker (CCB) combination tablet containing a regular dose of irbesartan (100 mg) and a high dose of amlodipine (10 mg) with regard to lowering BP and other risk factors for cardiovascular disease. Methods: We retrospectively evaluated data from 68 patients with essential hypertension whose treatment regimen was changed either from combination treatment with an independent ARB and a low-dose or regular-dose CCB or from a combination tablet of ARB and a low-dose or regular-dose CCB to a combination tablet containing amlodipine 10 mg and irbesartan 100 mg, because of incomplete BP control. Previous treatments did not include irbesartan as the ARB.Results: The combination tablet decreased systolic and diastolic BP. In addition, it significantly decreased serum uric acid, low-density lipoprotein cholesterol, and increased high-density lipoprotein cholesterol levels, independent of the BP-lowering effect. Treatment with the combination tablet did not affect serum triglycerides, plasma glucose, glycated hemoglobin, serum potassium or creatinine levels, or the urinary albumin excretion rate. Conclusion: The combination tablet containing amlodipine 10 mg and irbesartan 100 mg had a greater BP-lowering effect than an ARB and a low-dose or regular-dose CCB. In addition, the combination tablet had more favorable effects on serum uric acid, low-density lipoprotein cholesterol, and high-density lipoprotein cholesterol levels in patients with hypertension. Keywords: blood pressure, combination tablet, uric acid, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol

Sole mineral fertilizers use by poorly resourced farmers for rice production in the lowlands is usually low and unsustainable. Field experiments were therefore conducted within two contrasting environments (Forest and Savannah), using two common organic amendments (Poultry manure and Cattle manure) to establish an effective and integrated soil nutrient management system for improved lowland soil productivity and increased rice yields. The study was also partly intended to encourage and promote the effective and sustainable use of locally available organic amendments for nutrient management in lowland rice production. Results showed that organic amendments {cattle manure (CM) and poultry manure (PM)} contributed significantly to grain yield increases and total productivity with PM having a significantly greater and positive effect on grain yield than CM. In addition, the application of organic amendments in combination with mineral fertilizer significantly contributed to increased grain yield over the application of sole mineral fertilizer. Within the savannah agro- cological zone, there was a 130% (CM) and 203% (PM) grain yield increase over the control due to the application of organic amendments . When organic amendments were applied in combination with mineral fertilizer (MF), grain yield increased by 21% and 43% over sole MF for CM and PM respectively. However, sole CM contributed 12% increase in grain yield over the control while PM gave a 35% increase within the forest agro- cological zone. The combined application of MF and CM resulted in an 11% increase in grain yield while MF and PM combinations produced a 30% yield increase within the ecology. The non-addition of N, P, K as mineral fertilizer resulted in a yield reduction of about 84% at both sites. Resource poor farmers within the West African sub-region should therefore be encouraged to use organic amendments, which are not only available locally but also affordable. Proper storage and handling of these organic materials is very important to minimize nutrient losses.

- Abstract
DIII-D physics research addresses critical challenges for the operation of ITER and the next generation of fusion energy devices. This is done through a focus on innovations to provide solutions for high performance long pulse operation, coupled with fundamental plasma physics understanding and model validation, to drive scenario development by integrating high performance core and boundary plasmas. Substantial increases in off-axis current drive efficiency from an innovative top launch system for EC power, and in pressure broadening for Alfven eigenmode control from a co-/counter-I p steerable off-axis neutral beam, all improve the prospects for optimization of future long pulse/steady state high performance tokamak operation. Fundamental studies into the modes that drive the evolution of the pedestal pressure profile and electron vs ion heat flux validate predictive models of pedestal recovery after ELMs. Understanding the physics mechanisms of ELM control and density pumpout by 3D magnetic perturbation fields leads to confident predictions for ITER and future devices. Validated modeling of high-Z shattered pellet injection for disruption mitigation, runaway electron dissipation, and techniques for disruption prediction and avoidance including machine learning, give confidence in handling disruptivity for future devices. For the non-nuclear phase of ITER, two actuators are identified to lower the L-H threshold power in hydrogen plasmas. With this physics understanding and suite of capabilities, a high poloidal beta optimized-core scenario with an internal transport barrier that projects nearly to Q = 10 in ITER at ∼8 MA was coupled to a detached divertor, and a near super H-mode optimized-pedestal scenario with co-I p beam injection was coupled to a radiative divertor. The hybrid core scenario was achieved directly, without the need for anomalous current diffusion, using off-axis current drive actuators. Also, a controller to assess proximity to stability limit

- Abstract
A large superconducting machine, JT-60SA has been constructed to provide major contributions to the ITER program and DEMO design. For the success of the ITER project and fusion reactor, understanding and development of plasma controllability in ITER and DEMO relevant higher beta regimes are essential. JT-60SA has focused the program on the plasma controllability for scenario development and risk mitigation in ITER as well as on investigating DEMO relevant regimes. This paper summarizes the high research priorities and strategy for the JT-60SA project. Recent works on simulation studies to prepare the plasma physics and control experiments are presented, such as plasma breakdown and equilibrium controls, hybrid and steady-state scenario development, and risk mitigation techniques. Contributions of JT-60SA to ITER and DEMO have been clarified through those studies.

Substantial increases in off-axis current drive efficiency from an innovative top launch system for EC power, and in pressure broadening for Alfven eigenmode control from a co-/counter-I (p) steerable off-axis neutral beam, all improve the prospects for optimization of future long pulse/steady state high performance tokamak operation. With this physics understanding and suite of capabilities, a high poloidal beta optimized-core scenario with an internal transport barrier that projects nearly to Q = 10 in ITER at similar to 8 MA was coupled to a detached divertor, and a near super H-mode optimized-pedestal scenario with co-I (p) beam injection was coupled to a radiative divertor. The hybrid core scenario was achieved directly, without the need for anomalous current diffusion, using off-axis current drive actuators. Also, a controller to assess proximity to stab

Construction of the JT-60SA tokamak was completed on schedule in March 2020. Manufacture and assembly of all the main tokamak components satisfied technical requirements, including dimensional accuracy and functional performances. Development of the plasma heating systems and diagnostics have also progressed, including the demonstration of the favourable electron cyclotron range of frequency (ECRF) transmission at multiple frequencies and the achievement of long sustainment of a high-energy intense negative ion beam. Development of all the tokamak operation control systems has been completed, together with an improved plasma equilibrium control scheme suitable for superconducting tokamaks including ITER. For preparation of the tokamak operation, plasma discharge scenarios have been established using this advanced equilibrium controller. Individual commissioning of the cryogenic system and the power supply system confirmed that these systems satisfy design requirements including operational schemes contributing directly to ITER, such as active control of heat load fluctuation of the cryoplant, which is essential for dynamic operation in superconducting tokamaks. The integrated commissioning (IC) is started by vacuum pumping of the vacuum vessel and cryostat, and then moved to cool-down of the tokamak and coil excitation tests. Transition to the super-conducting state was confirmed for all the TF, EF and CS coils. The TF coil current successfully reached 25.7 kA, which is the nominal operating current of the TF coil. For this nominal toroidal field of 2.25 T, ECRF was applied and an ECRF plasma was created. The IC was, however, suspended by an incident of over current of one of the superconducting equilibrium field coil and He leakage caused by insufficient voltage holding capability at a terminal joint of the coil. The unique importance of JT-60SA for H-mode and high-β steady-state plasma research has been confirmed using advanced integrated modellings. These experiences of assembly, IC and plasma operation of JT-60SA contribute to ITER risk mitigation and efficient implementation of ITER operation.

The plasma startup with ITER-relevant sub-mPa neutral pressures in the JT-60SA superconducting tokamak is studied using a zero-dimensional (0D) power balance model for the plasma burn-through phase. In the ohmic startup scenario assumed here, the lower limit of the operational neutral pressure is determined by the condition for the Townsend avalanche at a high E / p limit, whereas the upper limit depends on the available ohmic heating power for the burn-through with the base power supply (PS) of the superconducting coils, corresponding to 0.2–0.3 V m−1. The operational conditions that lead to RE generation are evaluated to mitigate the risk of RE discharges. The simulation shows that startup REs are generated during the temperature rise immediately after the burn-through under relatively low-density conditions, and then the RE current value is gradually increased in balance with the RE loss processes on the timescale of I p ramp-up. Therefore, it is expected that REs can be avoided or mitigated if the electron density during the early build-up phase is tuned by a combination of prefill gas and additional fuelling. However, a startup with an excessive prefill gas will lose the margin of the density control, and a full RE discharge can be triggered when the plasma burn-through fails owing to a high concentration of impurities such as oxygen, if the operational point is marginal to the burn-through limit. The present modelling work implies that preparing the operation with sufficient margins against the failure of impurity burn-through is important for mitigating the risk of unintended plasma termination. Although the carbon wall environment may have a lower risk of RE generation, to alleviate the operational uncertainties of the new device, strategies for expanding the operational window are also discussed in support of JT-60SA initial operation. Quantitative data on carbon stock (C stock) in and beyond the topsoil (0-30 cm) under natural terrestrial ecosystems in West African savanna could provide information about their relative potential, and management options, for C sequestration, but these data are still scanty in the region. In selected locations (Nsukka, Obimo, and Ibagwa-aka) in the derived savanna zone of south-eastern Nigeria, secondary forest (SFT), grassland fallow (GLF), and bare footpath (BFP) were sampled from the topsoils (0-30 cm) and subsoils (30-60 cm) in triplicate. The soils are generally sandy, with low (1.4-13.8%) mean silt content. Mean bulk density ranged from 1.30 to 1.83 Mg/[m.sup.3]. The soils were acidic ([pH.sub.water] 4.0-4.8) and low in organic C (0.10-1.14%). There was a consistent trend in C stock (SFT > GLF > BFP) in the topsoil, whereas only higher values in SFT than BFP were consistent in the subsoil. In both soil layers, the scale of the differences among the land-cover types was location-specific. Values of C stock were higher in the topsoil than subsoil, except for GLF and BFP at Obimo due to recent bush burning. Irrespective of location, the mean topsoil-subsoil values under SFT, GLF, and BFP were 45.7-30.6, 27.7-25.8, and 19.0-18.8 Mg/ha, respectively. Soil structural stability, indexed as the ratio of organic matter to silt + clay, explained roughly 61 and 89% of the variability in C stock of topsoils and subsoils, respectively. These results should benefit the planning of C sequestration projects in savanna agroecosystems of West Africa. Additional keywords: derived savanna ecosystem, highly weathered soils, natural terrestrial vegetation, organic carbon pool, structurally fragile soils, topsoil and subsoil., Introduction It has long been recognised that the soil provides the largest terrestrial pool for carbon (C). This, coupled with the relatively long residence time of organic C in the [...]
