16. Influence of the trunk position on knee kinematics during the single-leg landing: implications for injury prevention.

Abstract

The purpose of the present study was to investigate the influence of the trunk position during the single-leg landing on the knee angle and muscle activity. Forty healthy university students (20 men and 20 women) performed right single-leg landings from a 40 cm-high platform with the trunk in neutral, flexion, extension, and right and left lateral flexion. Knee flexion and valgus angles were determined by two-dimensional video analysis, and rectus femoris (RF) and biceps femoris (BF) muscle activities were assessed. The knee flexion angle was significantly higher in the trunk-flexion position than in the other trunk positions. The knee valgus angle was significantly lower in the trunk-neutral and trunk-flexion positions than in the trunk-extension, trunk-right-lateral-flexion, and trunk-left-lateral flexion positions. Muscle activity of the RF was significantly lower in the trunk-flexion position than in the trunk-extension position and that of the BF was significantly higher in the trunk-flexion position than in the trunk-neutral, trunk-extension, and trunk-right-lateral-flexion positions. Single (right)-leg landing with the trunk in flexion may confer a low risk of anterior cruciate ligament injury compared to that with the trunk in extension or right lateral flexion. Abstract

Aims: The objective of this study was to introduce mobile telerehabilitation with tablet computers and to verify its practical effectiveness in improving physical function and health status. Methods: In this study, nine adults aged 76.3 ± 3.3 years old participated in 13 exercise sessions led by a physical therapist each lasting 60 min every 2 weeks for a total of 6 months. A physical therapist performed exercises in front of a digital camera which was connected to a lab computer. In a community center, participants performed the exercises while watching the exercise performance on a screen. Intervention was delivered by video conferencing over high-speed mobile Internet. Results: All participants were satisfied with telerehabilitation. We found that patients experienced significant improvement in physical function following participation. Conclusion: Mobile telerehabilitation provides an alternative option for delivering physical exercise and supervision, especially for those with geographical restrictions and limited local professional resources. Abstract

敏捷性は日常生活の中でも重要な運動機能の一つであり, 加齢による運動機能低下の一つとしても考えられている.本研究の目的は, 地域居住高齢者に対してWayne Dite らにより考案されたFour Square Step Test (以下FSST)を用いて敏捷性を評価し, また他の身体運動機能評価項目とどのような関係があるかを明らかにすることである. 対象は地域に居住する健常高齢者63名(平均年齢73.5歳) とした. 評価項目はFour Square Step Test (FSST) の他,その他の身体運動機能評価項目として, 膝伸展筋群筋力(LES), 長座位体前屈距離(FFD), 片脚立位保持時間(SLS), Timed Up and Go test (TUG), 10ｍ最大歩行速度(10MWS) を計測した. 結果, Four Square Step Test の平均値は5.8±1.1sec となり, またFSST とその他の評価項目間においてFSST とSLS (ｒ＝－.597, P＜.0001), TUG (ｒ＝.717, P＜.0001), 10MWS (ｒ＝.589, P＜.0001) と, それぞれに相関関係が認められた. SLS・TUG・10MWS の評価項目は運動能力, 特にバランス能力を示す評価項目として一般的に確立されているため, 今回の結果は, 敏捷性の評価も身体運動機能を表す重要な評価指標と考えられ, また日常生活に結びつく重要な運動プログラムの一つと成りうることが示唆された., The purpose of this study was to assess the physical agility and to determine the relationship between Physical agility and Other Physical Motor Functions in healthy community-dwelling elderly people. Physical agility is the ability to change the direction of the body in an efficient and effective manner. Elderly people tend to fall easily due to the loss of this physical ability. Therefore to prevent falling, it is important to assess physical agility in elderly people. Sixty three functionally independent community dwelling elderly people volunteered to participate in the study. Four Square Step Test (FSST) was used as a test of physical agility. The other physical motor function assessments were composed of 10m walking time (10MWT), Timed Up and Go test (TUG), Single leg standing (SLS), lower extremity strength (LES), and Finger floor distance (FFD). Pearson’s correlation was used to investigate the relationship between physical agility and the other physical assessments. The level of statistical significance was set at p＜0.05. This study shows significant correlation between FSST and SLS (r＝－.597, P＜.0001), TUG (r＝.717, P＜.0001), and 10MWS (r＝.589, P＜.0001) respectively. To prevent falling, it is very important to approach muscle strength, balance, and flexibility. However, the assessment of physical agility is also very important for elderly people. Since there were significant correlations between FSST and SLS, TUG, and 10MWS, physical agility needs to be assessed.

Abstract

The purpose of this study is to investigate whether often used hypnotics are safe or not to elderlypeople by evaluating the early morning effects and residual effects on the physical and cognitivefunctions after hypnotic administration.Fourteen healthy elders (3 male, 11 female ; mean age 64 years) took part in the study. Triazolam0.125 mg, rilmazafone hydrochloride 1 mg or placebo was orally given to each subjectbefore going to bed (11 pm). Objective evaluations including 1. Total Sway Path test (openedand closed eye) 2. Functional Reach test 3. Timed Up and Go test 4. Simply DiscriminatoryReaction test 5. Critical Flicker Fusion test 6. Short-Term Memory test were done at 10 pm(an hour before subjects went to bed), 4 am, 6 am, 10 am and 2 pm.Among all evaluations for the physical and cognitive functions, main effects between medicineswere only seen in the Total Sway Path test, which is the indicator for static balance ability. Wheneyes were opened, trembling was significantly small in the rilmazafon group compared to the placebogroup (p=0.04).In this study, the hypnotics did not affect physical and cognitive functions for healthy elders. Furtherstudies using an interdisciplinary approach with different professionals such nurses, physicaltherapists, pharmacists and physicians are needed for evaluating the safety use of hypnotics in theelderly patients with insomnia.

Abstract

Aim: To clarify the age‐related changes in muscle elasticity or thickness of the lower extremities in older women, and to analyze their relationship with physical functions. Methods: The muscle elasticity and thickness of the rectus femoris and medial head of the gastrocnemius of 102 young women (young group) and 221 elderly women (elderly group) were measured using ultrasonography. In the elderly group, physical functions, including knee extension strength, Timed Up and Go test, single‐leg standing, maximum gait speed and Four Square Step Test were also measured. Results: The elasticity of the rectus femoris and medial head of the gastrocnemius were significantly higher, but the muscle thickness of both muscles was significantly lower in the elderly group than in the young group (both P < 0.001). In the elderly group, the elasticity of the rectus femoris was significantly correlated with Timed Up and Go test, maximum gait speed and Four Square Step Test (ρ = 0.481, r = −0.387 and ρ = 0.401, respectively, all P < 0.001), and the medial head of the gastrocnemius was similar (ρ = 0.471, r = −0.489 and ρ = 0.422, respectively, all P < 0.001). The muscle thickness of the rectus femoris was significantly associated with knee extension strength (r = 0.444, P < 0.001). Conclusions: Older women showed age‐related changes in high elasticity and atrophy of the rectus femoris and medial head of the gastrocnemius.
