SEX factors in disease ,CORONARY artery bypass ,WOMEN'S mortality ,MORTALITY risk factors ,SAPHENOUS vein - Abstract
There are controversies regarding the impact of sex on mortality and postoperative complications in patients undergoing on-pump coronary artery bypass grafting (CABG), although some studies demonstrate comparable outcomes. This study sought to evaluate sex differences regarding risk factors associated with hospital mortality and postoperative clinical outcomes among patients undergoing isolated on-pump CABG. We conducted a retrospective observational cohort study of patients who underwent isolated on-pump CABG from January 1996 to January 2020. Patients were divided into two groups (male and female) and compared regarding preoperative characteristics, surgical technical variables, and in-hospital outcomes. All-cause mortality between groups was compared using logistic regression. Risk factors for mortality, along with their respective odds ratios (OR), were separately assessed using a logistic regression model with p-values for interaction. We analyzed 4,882 patients, of whom 31.6% were female. Women exhibited a higher prevalence of age >75 years (12.2% vs 8.3%, p<0.001), obesity (22.6% vs 11.5%, p<0.001), diabetes (41.6% vs 32.2%, p<0.001), hypertension (85.2% vs 73.5%, p<0.001), and NYHA functional classes 3 and 4 (16.2% vs 11.2%, p<0.001) compared to men. Use of the mammary artery for revascularization was less frequent among women (73.8% vs 79.9%, p<0.001), who also received fewer saphenous vein grafts (2.17 vs 2.27, p = 0.002). A history of previous or recent myocardial infarction (MI) had an impact on women's mortality, unlike in men (OR 1.61 vs 0.94, p = 0.014; OR 1.86 vs 0.99, p = 0.015, respectively). After adjusting for several risk factors, mortality was found to be comparable between men and women, with an OR of 1.20 (95% CI 0.94–1.53, p = 0.129). In conclusion, female patients undergoing isolated on-pump CABG presented with a higher number of comorbidities. Previous and recent MI were associated with higher mortality only in women. In this cohort analysis, female gender was not identified as an independent risk factor for outcome after CABG. [ABSTRACT FROM AUTHOR]

The use of botanical insecticides is a tactic that can be highly useful for the management of Helicoverpa armigera populations in the field. However, most botanical insecticides are produced using primarily macerated leaves. The problem with this process is that all components of the leaves are present in the extract, and not just the plant defence chemicals. Therefore, the objective of this research was to evaluate the feeding deterrence of a tagitinin C-rich leaf rinse extract (TagCE) obtained from Tithonia diversifolia (which concentrates mainly the plant defence substances) towards H. armigera larvae. The RP-HPLC-PDA analytical method allowed the identification and quantification of the major constituent TagCE, showing that this compound is suitable to be considered as its chemical marker. Regarding biological activity, we observed high insect mortality, with up to 80%, and high antifeedant activity for H. armigera larvae for all host plant species treated with T. diversifolia extract. These results open up the possibility to use T. diversifolia extract as a potential botanical insecticide, mainly against plant-defoliating Lepidoptera.

BackgroundSchool cafeterias can promote poor eating habits, as these retail outlets have a variety of foods considered to be nonnutritive and unhealthy. However, despite the need for effective preventive strategies, there is still disagreement on the best approach due to the lack of evidence on interventions to prevent and treat obesity in the school settings. ObjectiveWe aim to verify the efficacy of an educational intervention program to improve the hygienic conditions and the composition of the menu offered in school cafeterias in the state of Rio Grande do Sul, Brazil. MethodsWe will conduct a randomized, parallel, two-arm, community-based controlled study. Elementary and high schools, both public and private, in the State of Rio Grande do Sul, Brazil, that have a cafeteria will be eligible. Schools will be recruited and randomly assigned to the intervention (n=27) or control (n=27) group. The intervention group will receive an educational intervention program based on the guidelines issued by the Ministry of Health of Brazil, consisting of a 160-hour distance-learning qualification course, for 10 weeks, and using the Moodle platform and WhatsApp app. The intervention targets the owners and people in charge of the cafeterias, food handlers, principals, vice principals, teachers, pedagogical coordinators, dietitians, representatives of students' parents, and students over 16 years old. Meanwhile, the control group will receive only a printed copy of the book containing the guidelines used. The efficacy of the intervention will be determined by the hygienic conditions of the cafeteria and the composition of the menu offered, also considering the levels of processing of food sold. All outcomes will be analyzed as intention-to-treat and per-protocol. We will use covariance analysis or a generalized linear model for continuous data and ordinal logistic regression for ordinal categorical data. The level of statistical significance considered will be P

Background. Zabrotes subfasciatus (Boh., 1833) (Coleoptera: Chrysomelidae: Bruchinae), is considered one of the most important pest of stored beans. Objective. This study reports the possible toxicity and repellence of powders prepared from eight plant species against the Mexican bean weevil in two formulations (dry powder and sachets). Methodology. A 10 ˟ 2 factorial design (10 species ˟ 2 formulations) with 8 repetitions in a completely randomized design was employed. Pots with no powder were used as a negative control, and a pyrethroid insecticide [K-Obiol® 2 DP (deltamethrin, 0.5 g a.i. ton-1)] was applied as a positive control. Results. A mixture of powdered Chenopodium ambrosioides L., Ruta graveolens L. and Mentha pulegium L. added to bean grains was confirmed to be toxic to Z. subfasciatus adults with promising grain protector properties. C. ambrosioides powder had the same effect when in a sachet. A mixture of powdered R. graveolens, M. pulegium and C. ambrosioides with the beans inhibited weevil oviposition. The same effect was achieved for M. pulegium and C. ambrosioides in sachets. A mixture of powdered C. ambrosioides, M. pulegium, R. officinalis and R. graveolens repelled Z. subfasciatus adults from bean grains. Implications. This is the first report of using botanical derivatives by means of sachets or dry formulations, a pre-commercial purpose for aromatic plants with insecticidal/repellent activities. Conclusions. Sachets containing powdered C. ambrosioides and M. pulegium efficiently controlled the Mexican bean weevil in stored beans and constitute an useful tools for domestic grain stock or post-harvest management of organic grains.

Abstract Background There are limited real-world data on the clinical course of untreated coronary lesions according to their functional severity. Objective To evaluate the 5-year clinical outcomes of patients with revascularized lesions with fractional flow reserve (FFR) ≤ 0.8 and patients with non-revascularized lesions with FFR > 0.8. Methods The FFR assessment was performed in 218 patients followed for up to 5 years. Participants were classified based on FFR into ischemia group (≤ 0.8, intervention group, n = 55), low-normal FFR group (> 0.8-0.9, n = 91), and high-normal FFR group (> 0.9, n = 72). The primary endpoint was major adverse cardiac events (MACEs), a composite of death, myocardial infarction, and need for repeat revascularization. The significance level was set at 0.05; therefore, results with a p-value < 0.05 were considered statistically significant. Results Most patients were male (62.8%) with a mean age of 64.1 years. Diabetes was present in 27%. On coronary angiography, the severity of stenosis was 62% in the ischemia group, 56.4% in the low-normal FFR group, and 54.3% in the high-normal FFR group (p

- Abstract
Objective: Compare a questionnaire based on the HEADSS approach (QBH-16) and the Child Behavior Checklist (CBCL) in the screening of mental disorder in adolescents with behavioral problems. Methods: Adolescents from both genders 12-17 years-old presenting behavioral problems without a previous diagnosis of mental disorder were referred from primary services to a specialized outpatient program for adolescent behavioral problems and evaluated with the QBH-16, CBCL, and the Wechsler Intelligence Scale for Children. This program is located in a secondary and tertiary university hospital, Hospital de Clinicas de Porto Alegre, in the capital city of the southernmost state of Brazil. Results: 98 adolescents and their families were interviewed. Scores greater than or equal to 9 in the QBH-16 had a likelihood ratio (LR) greater than 5.5 and scores under 6 had a LR of 0.13, identifying adequately 62 patients (71%) according to the CBCL. Cognitive performance was similar among all patients. Discussion: Our findings suggest the QBH-16 has a good accuracy for screening mental disorders and may help prioritize or choose which patients will benefit from psychiatric services. Introduction: Due to Brazilian population aging, prevalence of aortic stenosis, and limited number of scores in literature, it is essential to develop risk scores adapted to our reality and created in the specific context of this disease. Methods: This is an observational historical cohort study with analysis of 802 aortic stenosis patients who underwent valve replacement at Hospital São Lucas, Pontifícia Universidade Católica do Rio Grande do Sul, from 1996 to 2018. With the aid of logistic regression, a weighted risk score was constructed based on the magnitude of the coefficients β of the logistic equation. Two performance statistics were obtained: area under the receiver operating characteristic curve and the chi-square (χ2) of Hosmer-Lemeshow goodness-of-fit with Pearson's correlation coefficient between the observed events and predicted as a model calibration estimate. Results: The risk predictors that composed the score were valve replacement surgery combined with coronary artery bypass grafting, prior renal failure, New York Heart Association class III/IV heart failure, age > 70 years, and ejection fraction < 50%. The receiver operating characteristic curve area was 0.77 (95% confidence interval: 0.72-0.82); regarding the model calibration estimated between observed/predicted mortality, Hosmer-Lemeshow test χ2 = 3,70 (P=0.594) and Pearson's coefficient r = 0.98 (P<0.001). Conclusion: We propose the creation of a simple score, adapted to the Brazilian reality, with good performance and which can be validated in other institutions. [ABSTRACT FROM AUTHOR]

This cohort study evaluated the fate of sound surfaces and inactive non-cavitated (INC) and active non-cavitated (ANC) caries lesions in a population-based sample of South Brazilian adolescents, in answer to the question: "Is lesion activity assessment a reliable criterion to diagnose a patient's caries activity?" A total of 801 schoolchildren were examined at baseline (aged 12 years) and after a mean time interval of 2.5 years. Data collection included a questionnaire and clinical examination. Patients were classified as caries-free (patients without any lesion), caries-inactive (patients with only inactive lesions), and caries-active (patients with at least one active lesion). The primary outcome was caries progression (presence of cavity, underlying dentin shadow, filling, or extraction at the follow-up exam). Negative binomial regression models were used to estimate the risk for caries progression. The main predictor variable was status of the surface at baseline: sound, INC, or ANC. Progression rates of 1.0, 9.0, and 12.6% were found for sound surfaces, INC, and ANC, respectively. INC (incidence risk ratio [IRR] 5.37, 95% CI 4.22-6.83) and ANC (IRR 4.96, 95% CI 3.43-7.17) had greater risk for caries progression than sound surfaces. Similar risks for progression were found for ANC and INC (IRR 0.92, 95% CI 0.64-1.32). Progression rates were 0.6, 1.1, and 2.2% for caries-free, caries-inactive, and caries-active individuals, respectively (p < 0.05). The risk for caries progression of sound surfaces was higher among caries-active adolescents (caries-free: IRR 2.78, 95% CI 1.63-4.72; caries-inactive: IRR 2.19, 95% CI 1.65-2.90). Caries-inactive patients behaved similarly to caries-free individuals (IRR 1.27, 95% CI 0.73-2.20). This study demonstrated the possibility of defining a patient's caries activity profile based on lesion features. [ABSTRACT FROM AUTHOR]

Objectives: Describe the results of the red reflex test in full-term newborns, as well as identifyfactors associated with red reflex test outcome and compare hospital length of stay betweenpatients with inconclusive and normal red reflex test results. Methods: Descriptive cross-sectional study of the results of the red reflex test performed in atertiary hospital maternity unit between 2014 and 2018. A nested case-control study was alsoperformed to search for anthropometric, gestational, and neonatal variables associated withthe outcome of the red reflex test. Results: There were121 identified cases of inconclusive red reflex test in 11,833 newborns.Sixteen alterations were confirmed, four considered severe: two cases of congenital glaucoma, one of cataract, and one of coloboma. Mean birth weight (p = 0.04), length (p = 0.03), and headcircumference (p = 0.02) were lower in patients with inconclusive red reflex test; however with-out a relevant effect size (d = −0.21, −0.22, and −0.25, respectively). The proportion of white,mixed-race, and black patients was significantly different between the groups (p < 0.001), witha higher chance of inconclusive results in mixed-race (OR = 2.22) and black (OR = 3.37) patientswhen compared to whites. An inconclusive red reflex test led to an increase in hospital lengthof stay from 62 to 82 hours (p < 0.001). Conclusions: The red reflex test was able to identify four severe alterations in 11,833 newborns(0.03%). In the 121 newborns in which the red reflex test was classified as inconclusive, therewas a 20-hour increase in the hospital length of stay, but a severe alteration was confirmedin only 3.3% of them. Differences in red reflex between white, mixed-race, and black patientsshould be considered.

This thesis addresses the issue of sustainability in the urban development policies proposed by the municipal government of Porto Alegre, Brazil, for the area known as Lower 4th District (Baixo 4º Distrito). In order to verify the degree of sustainability contained in these policies, this research analyses the urban revitalization and economic reconversion plan proposed in the 2016 Masterplan do 4º Distrito de Porto Alegre. The thesis has two parts: a theoreticalconceptual part, in which the contemporary process of urban restructuring and the precepts of sustainable development are discussed; and another, the case study, which analyses the inclusion of the conceptual dimensions of sustainable development in national public policies and local plans for the Lower 4th District area. In the last three decades, a number of plans and projects have been introduced by the local government with the aim of restructuring its former industrial district. By examining the circumstances that led to these proposals, it is possible to identify three distinct periods: the Technological period 1995 to 2004; the World Cup period 2006 to 2009; and the Consortium period 2013 to 2016. Each of these reflects a distinct political ideology and real estate marketing position, driven by a new economic and spatial dynamic, based on public-private partnership and sustainability narratives. In order to assess the degree of real sustainability contained in these policies, the Emelianoff-Acselrad Verification Model (MVEA) was introduced, which uses prioritised techno-material and socio-environmental set of criteria. This research made it possible to identify the use of an environmentalized narrative, where the term sustainability is now used as a government and business marketing tool to lend credibility to any urban project. The MVEA is intended to serve as a benchmark for both the formulation and evaluation of plans, projects and public policies aiming for genuine sustainable development.

Abstract Objectives To evaluate the contribution of ultra-processed food (UPF) on the dietary consumption of children treated at a Basic Health Unit and the associated factors. Methodology Cross-sectional study carried out with a convenience sample of 204 children, aged 2-10 years old, in Southern Brazil. Children's food intake was assessed using a 24-h recall questionnaire. Food items were classified as minimally processed, processed for culinary use, and ultra-processed. A semi-structured questionnaire was applied to collect socio-demographic and anthropometric variables. Overweight in children was classified using a Z score >2 for children younger than 5 and Z score >+1 for those aged between 5 and 10 years, using the body mass index for age. Results Overweight frequency was 34% (95% CI: 28-41%). Mean energy consumption was 1672.3 kcal/day, with 47% (95% CI: 45-49%) coming from ultra-processed food. In the multiple linear regression model, maternal education (r = 0.23; p = 0.001) and child age (r = 0.40; p < 0.001) were factors associated with a greater percentage of UPF in the diet (r = 0.42; p < 0.001). Additionally, a statistically significant trend for higher UPF consumption was observed when data were stratified by child age and maternal educational level (p < 0.001). Conclusions The contribution of UPF is significant in children's diets and age appears to be an important factor for the consumption of such products.

The Mexican bean weevil Zabrotes subfasciatus (Boheman) (Coleoptera: Bruchidae) is one of the main pests affecting stored beans: it causes extensive qualitative and quantitative grain loss. We investigated the bioactivity of the essential oils extracted from Chenopodium ambrosioides L (CA-EO), Ocimum gratissimum L (OG-EO), and Schinus terebinthifolius Raddi (ST-EO) against Z. subfasciatus. After 12-h treatment, CA-EO and OG-EO at 20.0 mu L/L of air killed 100% Z subfasciatus, whereas ST-EO at 100.0 mu L/L of air afforded 100% 1 subfasciatus mortality after 24 h. CA-EO provided the lowest 24 h LD50 (0.8 mu L/L of air) and displayed efficient repellent activity against Z subfasciatus. Our results demonstrate that CA-EO is a potentially economical and environmentally friendly alternative to manage Z. subfasciatus in stored beans. (C) 2018 Elsevier Ltd. All rights reserved. - Abstract
OBJECTIVE: To assess the clinical behavior of inactive caries lesion on the occlusal sites of permanent molars over 4-5 years and to estimate the risk for progression of caries-inactive sites compared with sound ones. METHODS: Clinical examinations were conducted at baseline (n = 258) and after 4-5 years and included the recording of dental plaque and dental caries at the occlusal surfaces and the eruption stage of each permanent molar. RESULTS: One hudred ninety-three schoolchildren were followed (response rate of 74.8%), totalizing 1152 teeth. Of the children, 30.6% (n = 59) presented at least one molar containing an active lesion, filling, or that had been extracted; according to the activity criterion, inactive lesions presented around a twofold increased risk for caries progression than sound surfaces (OR = 2.34 95%CI = 1.51-3.62). Thirteen percent (n = 25) of the children presented at least one molar progressing to dentine cavity, filling, or extraction; according to the severity criterion, inactive caries lesions presented a significantly higher risk for progression when compared with sound surfaces (OR = 2.69, 95% CI = 1.50-4.83). CONCLUSION: The vast majority of lesions (85-90%) identified as inactive enamel caries at baseline did not progress over 4-5 years. Despite this fact, it was possible to detect an increased risk for caries progression in caries-inactive occlusal sites compared with the sound ones. CLINICAL RELEVANCE: Considering the low progression rates, inactive caries lesions do not need a specific caries-controlling treatment and should be monitored longitudinally in the same manner as sound surfaces.

Despite several pharmacological applications of Rosmarinus officinalis L. (Lamiaceae), studies on its analgesic and anti-inflammatory properties have been scarce. The aim of this work was to use in vivo models to evaluate the analgesic and anti-inflammatory activities of the aqueous extracts obtained from leaves (AEL) and stems (AES) of Rosmarinus officinalis, as well as its isolated compound--rosmarinic acid (RA). We also prepared and assessed the acetyl ester derivative of RA.The analgesic activity was evaluated using abdominal constriction and formalin tests. For the evaluation of the anti-inflammatory effects, carrageenin-induced paw edema in rats were used. The extracts were used at doses of 100, 200 and 400 mg kg⁻¹ compounds were tested at 10, 20 and 40 mg kg⁻¹.Orally administered AEL, AES and RA were not significantly active at any of the doses tested during the abdominal constriction test; the acetyl ester derivative of RA displayed significant analgesic activity. In the carrageenin-induced paw edema assay, the acetyl derivative of RA at all the tested doses produced significant anti-inflammatory effects and reduced the number of paw licks in the second phase of the formalin test. The results suggest that the analgesic effects of the acetyl derivative of RA operate via a peripheral-mediated mechanism. The acetyl ester derivative of RA is potentially applicable as a new lead compound for the management of pain and inflammation.

The essential oil of Rosmarinus officinalis L. (rosemary) was obtained by hydro-distillation and analysed by gas chromatography-mass spectrometry. Sixty-two constituents were identified, representing 98.06% of the total oil content. Oxygenated monoterpenes were the predominant components. The rosemary oil was characterized as having prominent (> 5%) contents of camphor (18.9%), verbenone (11.3%), α-pinene (9.6%), β-myrcene (8.6%), 1,8-cineole (8.0%), and β-caryophyllene (5.1%). The antimicrobial activity of the oil as well as of its major constituents was tested against the following microorganisms: Streptococcus mutans, Streptococcus mitis, Streptococcus sanguinis, Streptococcus salivarius, Streptococcus sobrinus, and Enterococcus faecalis, which are potentially responsible for the formation of dental caries in humans. The microdilution method was used for determination of the minimum inhibitory concentration (MIC) during evaluation of the antibacterial activity. The essential oil displayed low activity against the selected microorganisms. In the present study, the pure major compounds were more active than the essential oil. Among all the microorganisms tested, the pathogen S. mitis was the most susceptible and E. faecalis was the most resistant to the evaluated samples. This is the first report on antimicrobial activity of the major components of rosemary oil against oral pathogens.

The in vitro inhibitory activity of crude EtOH/H2O extracts from the leaves and stems of Rosmarinus officinalis L. was evaluated against the following microorganisms responsible for initiating dental caries: Streptococcus mutans, S. salivarius, S. sobrinus, S. mitis, S. sanguinis, and Enterococcus faecalis. Minimum inhibitory concentrations (MIC) were determined with the broth microdilution method. The bioassay-guided fractionation of the leaf extract, which displayed the higher antibacterial activity than the stem extract, led to the identification of carnosic acid (2) and carnosol (3) as the major compounds in the fraction displaying the highest activity, as identified by HPLC analysis. Rosmarinic acid (1), detected in another fraction, did not display any activity against the selected microorganisms. HPLC Analysis revealed the presence of low amounts of ursolic acid (4) and oleanolic acid (5) in the obtained fractions. The results suggest that the antimicrobial activity of the extract from the leaves of R. officinalis may be ascribed mainly to the action of 2 and 3.

Introduction: Contrast induced nephropathy is the third most prevalent preventable cause of acute kidney injury in hospitalized patients. It defined as an absolute increase in serum creatinine ≥ 0.5 mg/dL and relative ≥ 25% increase. Objective: We studied the risk factors to intravenous injection contrast nephropathy after computed tomography. Methods: We studied 400 patients prospectively. Results: The incidence of contrast induced nephropathy, with an absolute or a relative increase were 4.0% and 13.9%, respectively. Diabetes and cardiac failure were independent risk factors for CIN a relative increase de serum creatinine (O.R.: 3.5 [95% CI: 1.92-6.36], p < 0.01, 2.61 [95% CI: 1.14-6.03%], p < 0.05, respectively). Conclusions: We showed association between uses of intravenous injection contrast after computed tomography with acute injury renal, notably with diabetes and heart failure.

- Abstract
The National Examination of Student Performance (ENADE) integrates the National System of Higher Education Assessment (SINAES) as a mandatory curriculum component in Brazilian undergraduate courses. The objective of this work was to analyze, in a critical-reflexive way, the questions of the ENADE, applied to Pharmacy academics in 2010, in order to understand if the profile of the test and the evaluative intention are consistent with the recommendations by the National Curriculum Guidelines for pharmacy courses (DCNF). Through Qualtrics Survey Software were prepared four questionnaires sent by digital means teachers allocated to 18 Pharmacy courses, public and private institutions, from the Rio Grande do Sul State (RS). A total of 131 teachers evaluatedof the ENADE 2010 test. Teachers’ opinion for suitability of the Specific issues reached an average score 7.78 ± 2.22, most assessed exercise capacity in drugs and medicines (63%) they demanded critical-reflective ability (45.5%) and interdisciplinary approach (45.3%). The General Training questions, average score was 5.16 ± 2.87 seconds teachers, helped in understanding the social, cultural and economic environment in which health professionals be inserted (42.8%) and, to a lesser acuity assessed the critical-reflective character (32.2%), humanist (12.2%) and ethical (10.6%). It was through the opinion of teachers, that Pharmacy ENADE 2010 test contemplates the principles defined by NDCF, however presented an asymmetric distribution of the training aspects required to the formation of the general pharmacist., O Exame Nacional de Desempenho de Estudantes (ENADE) integra o Sistema Nacional de Avaliação da Educação Superior (SINAES) como um componente curricular obrigatório nos cursos de graduação brasileiros. No total, 131 docentes avaliaram a prova do ENADE 2010.
