Abstract

Hepatic encephalopathy (HE) is a major cause of morbidity in cirrhosis. However, its severity assessment is often subjective, which needs to be studied systematically. The aim was to determine how accurately trainee and nontrainee practitioners grade and manage HE patients throughout its severity. We performed a survey study using standardized simulated patient videos at 4 US and 3 Canadian centers. Participants were trainees (gastroenterology/hepatology fellows) and nontrainees (faculty, nurse practitioners, physician assistants). We determined the accuracy of HE severity identification and management options between grades 2. Serum ammonia levels were ordered in almost a third of grade ≥2 patients. For trainees and nontrainees, HE grades were identified similarly between groups. Trainees were less likely to order serum ammonia and low-protein diets, more likely to order rifaximin, and more likely to perform a more thorough workup for precipitating factors compared with nontrainee respondents. There was excellent concordance in the classification of grade ≥2 HE between nontrainees versus trainees, but lower grades showed discordance. Important differences were seen regarding blood ammonia, specialized testing, and nutritional management between trainees and nontrainees. 8. Looking the Part: Stereotypicality in Appearance Among White Professionals Predicts Leadership Attainment and Perceived Leadership Suitability.

Abstract

This project explores the effects of racial phenotypic stereotypicality, or the degree to which a person's appearance is perceived as typical of their racial group, on leadership outcomes. Building on research showing that people hold an image of the ideal leader as a White person, we propose that looking more typically White may facilitate leadership attainment. In Study 1, which used a sample of American college football coaches (N = 1,106), White (vs. Black) coaches were more likely to occupy leadership roles. Furthermore, within race, stereotypicality positively predicted occupying a leadership or head-coach role among White professionals (and negatively predicted occupying a head-coach role among Black professionals). Study 2 elucidated a possible mechanism by showing a causal effect of stereotypicality on perceived suitability for leadership among Whites. These findings advance theorizing on the White–leader link and have implications for the ability of people of color to access lucrative professional roles. [ABSTRACT FROM AUTHOR]

Abstract

Thiazide diuretics are used to treat hypertension; however, compensatory processes in the kidney can limit antihypertensive responses to this class of drugs. Here, we evaluated compensatory pathways in SPAK kinase-deficient mice, which are unable to activate the thiazide-sensitive sodium chloride cotransporter NCC (encoded by Slc12a3). Global transcriptional profiling, combined with biochemical, cell biological, and physiological phenotyping, identified the gene expression signature of the response and revealed how it establishes an adaptive physiology. Salt reabsorption pathways were created by the coordinate induction of a multigene transport system, involving solute carriers (encoded by Slc26a4, Slc4a8, and Slc4a9), carbonic anhydrase isoforms, and V-type [H.sup.+]-ATPase subunits in pendrin-positive intercalated cells (PP-ICs) and ENaC subunits in principal cells (PCs). A distal nephron remodeling process and induction of jagged 1/ NOTCH signaling, which expands the cortical connecting tubule with PCs and replaces acid-secreting [alpha]-ICs with PP- ICs, were partly responsible for the compensation. Salt reabsorption was also activated by induction of an [alpha]- ketoglutarate ([alpha]-KG) paracrine signaling system. Coordinate regulation of a multigene [alpha]-KG synthesis and transport pathway resulted in [alpha]-KG secretion into pro-urine, as the [alpha]-KG-activated GPCR (Oxgrl) increased on the PP-IC apical surface, allowing paracrine delivery of [alpha]-KG to stimulate salt transport. 44. Racial Disparity in Leadership: Evidence of Valuative Bias in the Promotions of National Football League Coaches1

Abstract

The authors propose that racial disparity in organizational leadership representation will persist until valuative bias favoring white men ceases to influence advancement from the lower-level positions where most careers begin. They consider how racial disparity results from the organizational matching of individuals to positions with different advancement prospects (i.e., allocative bias) and by the provision of differential rewards within those positions (i.e., valuative bias). Analyzing career history data for over 1,300 National Football League coaches from 1985 to 2015, the authors find that white assistant coaches were promoted at higher rates than Black coaches—holding constant many factors including unit and individual performance—both before and after a league-wide intervention explicitly implemented to close the racial gap in leadership representation. They further demonstrate that this white promotion advantage is specific to the position typically occupied before promotion to head coach. Simulations demonstrate how racial disparity persists even absent bias in positional allocations; eliminating valuative bias at early career stages is, thus, necessary to achieve racial parity in leadership representation.
