31. Diurnal Variation in Straylight in Patients With Fuchs Endothelial Corneal Dystrophy and Controls

Abstract

PURPOSE: The goal of this study was to investigate diurnal changes in intraocular straylight in relation to other corneal parameters and subjective complaints in patients with Fuchs endothelial dystrophy and healthy controls. METHODS: This is a prospective study conducted in 2 tertiary care hospitals in Germany and the Netherlands. Patients with Fuchs endothelial dystrophy (n = 71) and healthy controls (n = 34) were included. Patients with Fuchs dystrophy were grouped by the presence of subjective complaints and measured over multiple time points during the day. Measurements included intraocular straylight using the C-Quant and corneal thickness and backscatter using a Scheimpflug camera. A separate group of healthy controls was measured intensively with repeated straylight measurements directly after waking. An exponential decay model was used to model the diurnal change. RESULTS: Healthy controls showed an average straylight baseline of 1.17 log(s) with an increase in straylight after waking of 0.22 log(s). In the repeated measurements subgroup, the increase in morning straylight lasted for 22 minutes. Patients with Fuchs dystrophy showed a morning increase in straylight of 0.21 log(s) present up to 4 hours after waking before reaching an average baseline of 1.30 log(s). Straylight was positively correlated with anterior corneal backscatter, r = 0.21, P = 0.022, and corneal thickness, r = 0.46, P < 0.01. CONCLUSIONS: Healthy eyes experience a diurnal straylight increase similar to patients with Fuchs dystrophy in intensity. However, in Fuchs dystrophy, the resolution of increased straylight is prolonged over multiple hours compared with minutes in healthy eyes. This suggests pathological exacerbation of a physiological diurnal change. This mechanism can play a role in subjective complaints experienced by patients with Fuchs dystrophy.

34. Online-Untersuchungskurs statt Präsenzveranstaltung: Anpassung der studentischen Lehre im Medizinstudium während der COVID-19-Pandemie

Abstract

Hintergrund: Da Präsenzveranstaltungen im Sommersemester 2020 durch die Coronavirus-Disease-2019(COVID-19)-Pandemie nicht möglich waren, entwickelten wir einen Online-Untersuchungskurs. Ziel der Arbeit: Die Studie untersuchte die Zufriedenheit der Student*innen mit dem Online-Untersuchungskurs und die Auswirkungen auf die praktische „Objective structured clinical examination“(OSCE)-Prüfung. Material und Methoden: Der Online-Untersuchungskurs wurde als videobasiertes Tutorial nach dem SMART(Specific, Measurable, Activating, Reasonable, Time-bound)-Prinzip entwickelt und allen Student*innen des Blockpraktikums Augenheilkunde zur Verfügung gestellt. Die Student*innen benoteten am Ende des Semesters den Online-Untersuchungskurs mittels Online-Fragebogen auf einer Ordinalskala von 1 bis 6 und wurden um Freitextkommentare gebeten. Die relative Punktzahl der OSCE-Prüfung am Ende des Semesters wurde mit den Prüfungsergebnissen des vorangegangenen Semesters verglichen. Alle Auswertungen erfolgten anonymisiert. Ergebnisse: Es beteiligten sich 67 der 164 Student*innen des Blockpraktikums an der Online-Evaluation (41 %). Die Student*innen fühlten sich auf die praktische Prüfung gut vorbereitet (Mittelwert 2,0; SD 1,0); 70 % der Student*innen (47 von 67) wünschten sich auch zukünftig die Möglichkeit eines digitalen Angebots in Ergänzung zur Lehre vor Ort. Die Prüfungsergebnisse im OSCE waren im Mittelwert mit einer Note von 1,1 sehr gut (SD 0,2; n= 164) und ähnlich zum vorangegangenen Semester (Mittelwert 1,1; SD 0,2; n= 166, zweiseitiger t-Test p= 0,86). Diskussion: Der Online-Untersuchungskurs erlaubt es den Student*innen, sich auf die OSCE-Prüfung vorzubereiten. Über die COVID-19-Pandemie hinaus kann die Bereitstellung eines Online-Untersuchungskurses eine gewinnbringende Ergänzung zur Präsenzlehre darstellen.

Abstract

To discuss and evaluate new technologies for a better diagnosis of corneal diseases and limbal stem cell deficiency, the outcomes of a consensus process within the European Vision Institute (and of a workshop at the University of Cologne) are outlined. Various technologies are presented and analyzed for their potential clinical use also in defining new end points in clinical trials. The disease areas which are discussed comprise dry eye and ocular surface inflammation, imaging, and corneal neovascularization and corneal grafting/stem cell and cell transplantation. The unmet needs in the abovementioned disease areas are discussed, and realistically achievable new technologies for better diagnosis and use in clinical trials are outlined. To sum up, it can be said that there are several new technologies that can improve current diagnostics in the field of ophthalmology in the near future and will have impact on clinical trial end point design.
