Abstract

Context Genome-wide association studies have identified numerous single-nucleotide variations (SNVs, formerly single-nucleotide polymorphisms) linked to type 2 diabetes (T2D), thus improving the accuracy of genetic risk scores (GRS) in predicting T2D. Objective This study aimed to investigate the association between the novel GRS and the prevalence of T2D and clarify the characteristics that differentiate individuals with and without T2D with similar genetic risk. Methods This cross-sectional study analyzed 1610 Japanese individuals aged 65 to 84 years. GRS were calculated using 110 SNVs associated with T2D in Japanese, and GRS classified individuals as having low, average, or high risk for T2D. The characteristics of participants with or without diabetes were compared by sex at each risk level. Results The prevalences of T2D were 7.8%, 14.7%, and 16.7% at low-, average-, and high-risk levels, respectively. The odds ratios at the high- and average-risk levels were significantly higher than those at the low-risk level, even after adjusting for confounding factors. The diabetes group had a higher visceral fat area (VFA) and Homeostasis Model Assessment of Insulin Resistance (HOMA-IR) value, but a lower insulinogenic index, than the nondiabetes group across all risk levels. In the nondiabetes group, the II decreased significantly as GRS increased, but the HOMA-IR and Matsuda index values showed no association. In men with diabetes, VFA tended to decrease with higher GRS. Conclusion A higher GRS was significantly associated with increased T2D prevalence in older Japanese individuals. Abstract

Aim: To explore the efficacy and safety of once‐weekly insulin icodec (icodec) in Japanese adults (≥20 years old) with type 2 diabetes from the global ONWARDS 1, 2 and 4 trials. Materials and Methods: Insulin‐naive (ONWARDS 1) and insulin‐experienced (ONWARDS 2 and 4) individuals were randomized to icodec or a once‐daily insulin comparator: insulin glargine U100 [ONWARDS 1 (basal insulin only) and 4 (basal–bolus regimen)] or insulin degludec [ONWARDS 2 (basal insulin only)]. The primary outcome was change in glycated haemoglobin from baseline to end of treatment (EOT) (ONWARDS 1: Week 52; ONWARDS 2 and 4: Week 26). Here, we present the Japanese subgroup results. Results: Similar reductions in glycated haemoglobin from baseline to EOT were observed in each trial for icodec and comparators. The proportion of time in range (blood glucose 3.9–10.0 mmol/L) at EOT was also comparable across treatment groups (time in range: 58%–68%), as was time spent with blood glucose below 3.0 mmol/L (<1.0%). Combined clinically significant (blood glucose <3.0 mmol/L) or severe (requiring external assistance for recovery) hypoglycaemia rates were low, with no severe events (ONWARDS 1 and 2) or a single severe event (ONWARDS 4; icodec group) reported. These results generally aligned with findings from the respective global populations. No new safety issues were identified. Conclusions: Icodec improved glycaemic control to a similar degree as once‐daily basal insulin comparators while maintaining low levels of clinically significant or severe hypoglycaemia. Abstract

Background: Exercise in adolescence and old age improves cognitive function in older adults, but the combined effect of exercise habits in both periods is controversial. This study aimed to clarify the relationship between exercise habits in adolescence and old age and mild cognitive impairment (MCI) and to compare regional brain volumes and blood biochemical markers associated with cognitive function in older adults. Methods: Baseline data of 1615 participants aged 65–84 years from the Bunkyo Health Study were analyzed. MCI was diagnosed using the Japanese version of the Montreal Cognitive Assessment. Participants were divided into four groups based on their exercise habits in adolescence (13–18 years) and old age: no exercise in either period (None-None), exercise in adolescence only (Active-None), exercise in old age only (None-Active), and exercise in both periods (Active-Active). Logistic regression models estimated the odds ratios (ORs) of MCI prevalence. Regional brain volumes, such as the prefrontal cortex, temporal lobe, parietal cortex, and hippocampus, and blood biochemical markers, such as BDNF, IGF-1, and homocysteine, were compared between groups. Results: The OR for MCI was significantly lower in the Active-Active group than in the None-None group (OR, 0.62; 95% CI, 0.41–0.94). However, there were no significant differences in regional brain volumes and blood biochemical markers between the Active-Active and None-None groups. Conclusions: Older adults with exercise habits in both adolescence and old age have a lower risk of MCI. Abstract

Aim: To explore differences in imeglimin response among type 2 diabetes (T2D) patient clusters using data‐driven cluster analysis. Methods: Data‐driven cluster analysis (non‐hierarchical k‐means clustering) was performed on randomized, double‐blind, imeglimin monotherapy and adjunctive (to insulin) therapy trials based on four baseline variables: (1) disease duration; (2) body mass index (BMI); (3) HbA1c; and (4a) homeostatic model assessment of β‐cell function (HOMA‐β) (monotherapy trials) or (4b) insulin total daily dose (adjunctive trial). Results: Four clusters were identified with distinct clinical characteristics in both monotherapy (1‐4) and adjunctive therapy (I‐IV) trials; clusters 1 and I had lower values across all four indices versus the overall population, clusters 2 and II had a longer diabetes duration, cluster 3 had higher baseline BMI and HOMA‐β, and cluster III had higher baseline BMI and insulin total daily dose, while clusters 4 and IV had higher baseline HbA1c. Between‐group differences in HbA1c change (95% confidence interval) and effect size (ES) at week 24 varied considerably by cluster (cluster 1: −0.82 [−1.00, −0.63], ES = 1.47; cluster 2: −0.64 [−0.89, −0.39], ES = 1.18; cluster 3: −0.86 [−1.38, −0.33], ES = 0.84; cluster 4: −1.27 [−1.73, −0.82], ES = 1.44). For imeglimin adjunctive therapy, HbA1c improvements were significant versus placebo at week 16, excluding cluster III (cluster I: −0.63 [−0.95, −0.31], ES = 0.88; cluster II: −0.66 [−1.02, −0.30], ES = 1.13; cluster III: −0.31 [−0.73, 0.11], ES = 0.46; cluster IV: −0.82 [−1.29, −0.35], ES = 0.99). Conclusions: Differences in imeglimin response were observed among T2D patient clusters. Patient stratification may help with selection of those most probable to respond to imeglimin. Abstract

Aims: The utilization of long‐term effect of internet of things (IoT) on glycemic control is controversial. This trial aimed to examine the effect of an IoT‐based approach for type 2 diabetes. Materials and Methods: This randomized controlled trial enrolled 1,159 adults aged 20–74 years with type 2 diabetes with a HbA1c of 6.0–8.9% (42–74 mmol/mol), who were using a smartphone on a daily basis were randomly assigned to either the IoT‐based approach group (ITG) or the control group (CTG). The ITG were supervised to utilize an IoT automated system that demonstrates a summary of lifelogging data (weight, blood pressure, and physical activities) and provides feedback messages that promote behavioral changes in both diet and exercise. The primary end point was a HbA1c change over 52 weeks. Results: Among the patients, 581 were assigned to the ITG and 578 were in the CTG. The changes in HbA1c from baseline to the final measurement at 52 weeks [mean (standard deviation)] were −0.000 (0.6225)% in ITG and − 0.006 (0.6449)% in CTG, respectively (P = 0.8766). In the per protocol set, including ITG using the IoT system almost daily and CTG, excluding those using the application almost daily, the difference in HbA1c from baseline to 52 weeks were −0.098 (0.579)% and 0.027 (0.571)%, respectively (P = 0.0201). We observed no significant difference in the adverse event profile between the groups. Conclusions: The IoT‐based approach did not reduce HbA1c in patients with type 2 diabetes. IoT‐based intervention using data on the daily glycemic control and HbA1c level may be required to improve glycemic control. [ABSTRACT FROM AUTHOR]
