The integrated medical and nursing ward rounds contributes greatly to quality improvement of clinical diagnosis and treatment, better meets the needs of patients, and promotes the harmonious development of the doctor-patient relationship. With the continuous development of modern medicine, medical science is becoming increasingly specialized, and increasing attention has being paid to the inter-professional cooperation. The implementation of medical and nursing integrated ward rounds is an important measure to promote medical and nursing cooperation. In order to promote the in-depth development, sustainability and standardization, this article briefly discussed the practice and quality improvement of the integrated medical and nursing ward rounds, and provided a reference basis for its clinical application. Traditional Chinese medicine (TCM) nursing technology has a history of more than one thousand years, and it has rich content and wide practicability. In ancient times, Medical scientists combined medical knowledge and experience of medicine, medicine and nursing, and gained rich experience in TCM nursing technology in practice. With the development of TCM, the operation of TCM nursing technology is more scientific and standardized. According to the clue of development of TCM, this paper summarized the development history of TCM nursing technology, and provided ideas for the inheritance and innovation of TCM nursing technology. In order to promote the inheritance innovation and development of Traditional Chinese Medicine nursing, the establishment of Traditional Chinese Medicine nursing veteran expert experience inheritance system and sharing platform, enhance the confidence of Traditional Chinese Medicine nursing culture. This paper summarized the nursing management of a case of cervical spondylotic radiculopathy treated with Traditional Chinese Medicine ointment. Based on nursing evaluation and syndrome differentiation, Traditional Chinese Medicine ointment and rubbing technique was used to relieve the symptoms of cervical spondylotic radiculopathy, and comprehensive nursing interventions such as daily living guidance and health education were carried to improve the quality of life during the treatment. (本文总结1例中药膏摩治疗项痹病(神经根型颈椎病)的护理体会。护理人员科学评估, 明确患者证型, 开展辨证施护, 采用中药膏摩技术治疗项痹病, 同时配合生活指导、健康教育等常规护理措施, 明显改善患者颈部疼痛的症状, 值得临床参考应用。)

Objective To systematically evaluate the clinical effect of acupoint application in the treatment of postoperative pain of mixed. Methods The CNKI, VIP, Wanfang Data, CBM and PubMed databases were searched by computer. Randomized controlled trials (RCTs) on the efficacy of acupoint application in the treatment of postoperative pain of mixed were included. The retrieval time was from the establishment of each database to December 2021. A literature screening, quality evaluation and data extraction were independently conducted by two reviewers in accordance with the inclusion and exclusion criteria. A Meta analysis was then carried out using RevMan 5. 3 software. Results In total, 6 RCTs involving 532 patients were included in the meta-analysis. The Meta analysis showed that effective rate of pain control in the study group was higher than that in the control group (RR = 1. 03, 95%CI［1. 02, 1. 15］, P = 0. 0001). The Visual Analogue Scale of pain in the in the study group was lower than that in the control group at 1 day(MD =-1. 32, 95%CI［-2. 33, -0. 31］, P = 0. 01), 3 days (MD = -1. 64, 93% CI［-2. 51, -0. 76］, P=0. 002) and 5 days (MD = -1. 32, 95%CI［-1. 89, -0. 75］, P < 0. 00001) after surgery. Conclusion The implementation of acupoint application could relieve the postoperative pain of patients undergoing surgical treatment for mixed hemorrhoids. However, given the low quality of included research methodology and small sample size, more efforts should be paid on large sample size and multi-centered clinical randomized control trails, especially studies concerning the long-term intervention efficacy. (目的 系统评价穴位贴敷治疗混合痔术后疼痛的干预效果。方法 计算机检索中国知网(CNKI)、维普(VIP)、万方(Wanfang Data)、中国生物医学文献数据库(CBM) 、PubMed数据库。检索年限为各数据库建库至2021年12月, 检索关于穴位贴敷对混合痔术后疼痛效果的随机对照试验, 由两名研究员对纳入的文献进行筛选、质量评价和资料提取, 采用Revman 5. 3软件进行Meta分析。结果 筛选出符合标准的文献6篇, 共计532例患者。Meta分析结果显示, 混合痔术后镇痛有效率试验组优于对照组［RR = 1. 03, 95%CI(1. 02, 1. 15), P = 0. 0001］。; 采用疼痛视觉模拟评分(VAS)评估术后疼痛, 试验组术后1 d［MD =-1. 32, 95%CI(-2. 33, -0. 31), P = 0. 01］、3 d［MD = -1. 64, 93%CI(-2. 51, -0. 76), P=0. 002］和5 d ［MD = -1. 32, 95%CI(-1. 89, -0. 75), P < 0. 00001］的VAS评分低于对照组。结论 穴位贴敷能有效缓解混合痔术后患者疼痛, 提高临床有效率, 但鉴于纳入的研究方法学质量均较低, 样本量少, 因此下一步仍需要依赖于大样本、多中心的临床随机对照验证, 尤其是长期干预疗效研究加以试验。)

This paper summarized the nursing experience of a case of lower extremity arteriosclerosis obliterans treated with Chinese herbal ointment and massage combined with intradermal acupuncture. Based on the nursing evaluation on pain, skin temperature, intermittent claudication, arteriopalmus of dorsalis pedis artery and other indicators, the application of Chinese herbal ointment and massage combined with intradermal acupuncture is effective to relieve the symptom of the lower-limb ischemia. (本文总结中药膏摩联合皮内针辅助治疗1例下肢动脉硬化闭塞症住院患者的护理体会。在完善疼痛、皮肤温度、间歇性跛行、足背动脉搏动等指标的评估的基础上, 运用中药膏摩联合皮内针辅助治疗, 能有效改善患者下肢缺血症状和体征。)

Objective To investigate the effect of Traditional Chinese Medicine (TCM) five-element cupping therapy on postoperative wound pain of patients undergoing Milligan-Morgan surgery for mixed hemorrhoids of damp-heat type. Methods Totally 60 patients undergoing Milligan-Morgan surgery for mixed hemorrhoids of damp-heat type were randomly divided into the observation group and control group, with 30 cases in each group. Anti-infection, hemostasis and other symptomatic treatment were carried out after surgery. The control group received routine wound cleansing and dressing change, and the observation group received TCM five-element cupping therapy on the basis of routine care. The Visual Analogue Scale (VAS) was used to assess the wound pain degree at 24 h、48 h、72 h after treatment. Results The VAS score in the treatment group was significantly lower than that in the control group at 24 h、48 h and 72 h after treatment, respectively (P＜0. 01). Conclusion The TCM five-element cupping therapy is effective to relieve the postoperative pain of patients with mixed hemorrhoids. It is an easy-to-operate and safe technique of TCM nursing, which is worthy of promoting in pain control after surgical treatment for anorectal diseases. (目的 观察五行能量罐治疗混合痔(湿热下注型)术后伤口疼痛的效果。方法 选取2021年5月—12月行手术治疗的混合痔(湿热下注型)患者共60例, 采用随机数字表法分为观察组和对照组, 各30例。两组患者均在麻醉下行混合痔外剥内扎术, 术后常规给予抗感染、止血等对症治疗。对照组接受术后常规换药护理, 观察组在对照组基础上增加五行能量罐中医护理干预。采用疼痛视觉模拟评分(VAS)分别对治疗后24 h、48 h、72 h伤口疼痛程度进行评价。结果 观察组拔罐治疗后24 h、48 h、72 h的疼痛评分低于对照组, 差异有统计学意义(P＜0. 01)。结论 五行能量罐中医护理干预能减轻混合痔术后患者伤口疼痛程度, 且操作简便, 患者依从性好, 可以广泛应用于包含混合痔在内的多种肛肠疾病术后疼痛护理中。)

This paper summarized the observation and nursing of a patient with constipation of damp-heat type treatment by acupoint massage. Under the guidance of Traditional Chinese Medicine (TCM) syndrome differentiation and nursing theory, palm massage, one-finger Zen massage, kneading, massage and other meridian massage interventions were carried out. The Traditional Chinese Medicine nursing technology is potentially effective to relieve the constipation nd improve the patient’s constitution and their satisfaction with TCM nursing. Objective To investigate the practice effect of continuing care with Traditional Chinese Medicine nursing for patients after surgical treatment of mixed hemorrhoids in the specialized outpatient clinic. Methods Totally 80 patients undergoing surgical treatment of mixed hemorrhoids of Damp-Heat type were randomly divided into the observation group and the control group, with 40 cases in each group. All patients received routine perioperative nursing during hospitalization. Patients in the control group received routing telephone follow-up and health education after discharged form hospital, and those in the observation group were given continuing care with Traditional Chinese Medicine nursing in the specialized outpatient clinic, and a three-step integrated management of follow-up was carried out. The occurrence of postoperative complications was assessed, and patient satisfaction was evaluated by using a self-made questionnaire. Results The score of postoperative complication evaluation in the observation group was lower than that in the control group (P＜0. 01). The patient satisfaction rate was 97. 50%(39/40), which was higher than 82. 50%(33/40) in the control group (P＜0. 05). Conclusion The practice of continuing care with Traditional Chinese Medicine nursing in the specialized outpatient clinic is potentially effective to reduce the risk of postoperative complications after surgical treatment of mixed hemorrhoids and improve the patient satisfaction. (目的 探讨基于专病门诊的延续性中医护理在混合痔(湿热下注型)术后出院患者中的应用效果。方法 选取2020年4月—12月科室收治的80例混合痔(湿热下注型)术后出院患者为研究对象, 随机分为观察组和对照组, 各40例。两组患者住院期间均接受常规围手术期护理。对照组患者出院后给予电话回访、健康宣教; 观察组患者出院后, 实施基于专病门诊的延续性中医护理方案, 采取“三步护理一体化”的管理模式进行随访管理。对术后两周内并发症情况进行评分, 采用自制问卷调查患者满意度。结果 观察组患者并发症评分低于对照组, 差异有统计学意义(P＜0. 01)。观察组患者满意率97. 50%(39/40), 高于对照组的82. 50%(33/40), 差异有统计学意义(P＜0. 05)。结论 基于专病门诊的延续性中医护理方案能降低混合痔术后患者并发症发生风险, 提高患者满意度。)

This paper reviewed the nursing measures of Traditional Chinese medicine fumigation in a patient with pain and edema after operation of mixed hemorrhoids. Under the guidance of dialectical nursing theory of traditional Chinese medicine, the fumigation technology of Traditional Chinese medicine granules was adopted. After treatment, the pain of patients was significantly reduced and the edema was significantly improved. (本文总结1例混合痔术后伴疼痛及水肿的患者行中药熏蒸技术治疗的护理经验, 包括中药熏蒸的药方组成、操作方法、注意事项以及饮食、生活等常规护理, 在中医辨证施护理论指导下, 采取针对性措施, 患者疼痛和水肿症状明显改善。)

This paper summarized the application of surrounding acupuncture combined with suspended moxibustion in treating one case of herpes zoster and related nursing management. The surrounding acupuncture combined with suspended moxibustion was carried out at regions affected by herpes zoster. The Numeral Rating Scale (NRS) was used to evaluate the degree of pain caused by herpes zoster. A decreased NRS score was reported after intervention, indicating a positive effect of surrounding acupuncture combined with suspended moxibustion in relieving neuropathic pain caused by herpes zoster. (本文观察围刺联合悬灸治疗1例带状疱疹患者急性期神经痛的应用效果, 总结护理经验。针对患者的带状疱疹区域应用围刺联合悬灸方法进行治疗, 根据NRS疼痛评分进行效果评估。干预后患者NRS疼痛评分降低, 提示围刺联合悬灸可有效降低带状疱疹的神经性疼痛。)

Objective To compare the effect of pulse-type and direct push-type flushing method after intravenous injection of non-irritating drugs with peripheral venous catheter (PVC). Methods Totally 60 patients with mixed hemorrhoids were randomly divided into the observation group and the control group, 30 cases in each group. The observation group was injected with 5 mL 0. 9% sodium chloride injection by slow direct push-type flushing method, while the control group was injected with pulse-type flushing method. The indwelling time of venous indwelling catheter, the incidence of blood return, tube blockage and phlebitis were compared between the two groups. Results The incidence of blood return and tube blockage in the observation group was lower than that in the control group(P＜0. 05). The phlebitis occurred in neither of the two groups. The indwelling time in the observation group was significantly longer than that in the control group(P＜0. 05). Conclusion During intravenous injection of non-irritating drugs, the direct push-type flushing method is potentially effective to reduce the incidence of blood return and tube blockage, reduce the pain of patients, and improve the clinical nurse work efficiency and nursing quality. (目的 比较经外周静脉留置针输注非刺激性药物中采用脉冲式和直接推注式冲管的效果观察。方法 选取混合痔术后采用静脉留置针输液的患者60例, 随机分为观察组和对照组, 各30例。观察组采用注射器将0. 9%氯化钠注射液5 mL以缓慢直接推注式冲管法推注, 对照组采用脉冲式冲管法推注。比较2组静脉留置针留置时间、回血、堵管及静脉炎的发生率。结果 观察组患者回血、堵管发生率均低于对照组, 差异有统计学意义(P＜0. 05)。2组均无静脉炎发生。观察组留置时间长于对照组, 差异有统计意义(P＜0. 05)。结论 输注非刺激性药物时采用直接推注式冲管会降低患者回血、堵管的发生风险, 减少患者痛苦, 对提高护士临床工作效率和护理质量具有积极意义。)

This paper reviewed the clinical data of 1 patient undergoing five-element cupping therapy for iliotibial band syndrome. Under the concept of Traditional Chinese Medicine syndrome differentiation, the five-element cupping therapy was carried out on Governor Vessel (GV), bladder meridian of Foot-taiyang, Yang-wei meridian and Yang-qiao meridian. This paper summarized the etiology and pathogenesis of chronic pelvic inflammatory disease in Traditional Chinese Medicine (TCM), TCM formula, diet and nutrition guidance and oral care. The targeted TCM nursing interventions by syndrome differentiation were carried out, helping to relieve disease-associated symptoms and pain, and achieving better patient compliance with treatment. Pelvic inflammatory disease is an infection of a woman’s reproductive organs, which can lead to a series of sequelae if not properly treated. Sequelae of pelvic inflammatory disease may seriously threaten the health of women and increase financial burden to the family and society. The syndrome of sequelae of pelvic inflammatory disease can be differentiated into four types: dampness-heat attacking lower energizer, Qi stagnation and blood stasis syndrome, cold-dampness stagnation syndrome and Qi deficiency and blood stasis syndrome. The main Traditional Chinese Medicine nursing techniques have been applied are Intermediate frequency electrotherapy, ultrasound drug penetration, acupoint application, Traditional Chinese Medicine soaking, Traditional Chinese Medicine enema, auricular point sticking, Traditional Chinese Medicine cupping therapy and so on. Based on the syndrome differentiation, a set of nursing intervention including body posture nursing, emotional nursing, medication service, symptom assessment, dietary instruction, exercise instruction, defecation instruction, etc, is required to improve the rehabilitation of patients. This paper summarized the factors causing peripheral venipuncture failure in the clinical setting and principles of vascular visualization technology, briefly introduce the application process of infrared vein finder in peripheral venipuncture, and discuss related standardized management procedure. Objective To evaluate the establishment of a two-part three-level model of quality control management in improving writing quality of nursing documents. Methods A two-part three-level model of quality control management has been carried out to improve the writing quality of nursing documents since 2016. The quality assessment outcomes of writing nursing documents in 2015-2017 were analyzed respectively. Results The score of quality assessment had increased from 97. 70 in 2015 to 98. 53 in 2016, and had continued to climb in 2017 (98. 88). Conclusion The establishment of a two-part three-level model of quality control management is effective to improve the writing quality of nursing documents. (目的 探讨科室建立并实施全面质量管理(TQC)模式下“两部三级”质量控制提高护理文件书写质量中效果。方法 2016年起, 科室开展TQC模式下“两部三级”质量控制, 细化质量控制流程, 加强环节和终末质量控制, 旨在提高护理文书书写质量。统计并比较2015、2016和2017年度科室护理文件书写考核情况。结果 护理文件书写考核评分由2015年度的97. 70分上升至2016年度的98. 53分和2017年度的98. 88分, 护理文件书写质量得到提升。结论 科室在TQC模式下建立并实施“两部三级”质量控制, 对提高护理文件书写质量具有积极意义。)
