BackgroundChildren who are high priority candidates for early intervention need to be identified to reduce their risk for experiencing problems in development. Those exposed to multiple risk factors are more likely to exhibit problems in development than those exposed to a single or no risk factor. We examined the longitudinal associations between persistence and timing of exposure to cumulative risk (CR) on three occasions by age 2 and problems in development at age 4.5 in health, behavior, and education-related domains.MethodsData are from Growing Up in New Zealand (NZ), a prospective longitudinal study of a birth cohort first assessed during their last trimester in 2009-10 and followed at ages 9 months and 2 and 4.5 years. All women with an expected delivery date in a 12-month period who resided within a defined region were invited to participate, with no additional eligibility criteria. Exposure was measured for 12 sociodemographic and maternal health risk factors at third trimester and ages 9 months and 2 years, from which developmental trajectories were constructed capturing persistence and timing of CR exposure. Ten developmental outcomes were measured at age 4.5 to classify problems in overall health status, obesity, and injuries; internalizing and externalizing behavior problems; and letter naming, counting forward and backward, and expectations for starting school and completing education.ResultsAnalyses of data from 6156 children (49% female, 33% Non-European ethnicity) who participated in the 4.5-age assessment uniformly showed associations between exposure to more than consistently zero CR across early development and higher prevalence of being classified with problems for 9 of 10 outcomes. Persistent exposure to a CR ≥ 4 was generally associated with a higher prevalence of problems for 7 of 10 outcomes, whereas the timing of first exposure to CR ≥ 4 showed a less consistent association with problem outcomes.ConclusionsThese findings are concerning because over 50% of NZ children are exposed to at least one of these risk factors at some point in early development. Routine screening of most of these risk factors during pregnancy is feasible and can identify priority candidates for intervention.

Exposure to cumulative risk (CR) has important implications for child development, yet little is known about how frequency, persistence, and timing of CR exposure during early childhood predict behavioral problems already before school start. We examine prospective longitudinal associations between patterns of CR exposure from third trimester through 2 years and subsequent behavior problems at 4.5 years. In 6156 diverse children in the Growing Up in New Zealand longitudinal study, the presence of 12 risk factors, spanning maternal health, social status, and home and neighborhood environment, defined CR and were assessed at last trimester and 9 months and 2 years of age. At child age 4.5 years, mothers completed the Strengths and Difficulties Questionnaire, where a score ≥ 16 indicated an abnormal level of problem behaviors (ALPB). Children exposed to a CR ≥ 1 at least once in early development, compared to those with consistent CR = 0, showed a significantly higher likelihood of ALPB at 4.5 years. Consistent high exposure to CR ≥ 4 across all three assessments had the highest prevalence (44%) of ALPB at age 4.5. Children with high CR exposure on two of three, compared to on all three, time points in early development did not evidence a significantly reduced prevalence (32%-41%) of ALPB. The common co-occurrence of risk factors and their significant developmental impact when accumulated early in life underscore the need for systematic multisector intervention and policy implementation during pregnancy and shortly after birth to improve outcomes for vulnerable children.

A variety of antenatal risk factors have been established as being detrimental to a child's developing language ability. Our aim was to examine the relationship between exposure to cumulative risk (CR), including antenatal maternal, perinatal, and maternal health characteristics, and children's preschool language ability. Analyses were comprised of interviews and observational data from 5,721 children and their mothers enrolled in the longitudinal "Growing up in New Zealand" cohort study. Language ability status was measured using the PPVT-III and DIBELS letter naming task (LNF) task at age 4.5 years. Results showed that CR was significantly associated with language status on both measures after controlling for multiple covariates. Improving maternal awareness and support during the perinatal period may reduce the number of risks a fetus is exposed to, which may aid early childhood language development.

Although there is a growing body of literature examining the positive aspects of being a parent, very few studies have considered the highlights mothers experience in parenting their infants. In particular, neither mothers' own perceptions of the highlights of parenting infants nor the factors that increase the odds of mothers experiencing these highlights have been identified within a representative, contemporary sample. To address this gap, the current study used mixed methods to explore the highlights reported by New Zealand mothers of infants in a large (N = 6383), demographically diverse cohort taking part in the Growing Up in New Zealand longitudinal study. Our sample included primiparous (42%) and multiparous (58%) mothers aged 15-47 years, of European (55%), Maori (13%), Pacific Peoples (14%), Asian (15%), Middle Eastern, Latin American or African (2%), and New Zealand (1%) ethnicities. We used thematic analysis to identify eight broad highlights that fitted within the PERMA[TM] model of wellbeing, and then identified the variables that make it more likely for mothers to experience these highlights using logistic regression. We found that parity, ethnicity, parental satisfaction, external support, family support, maternal education, and the child exceeding the mother's expectations for social development were each predictive of at least one highlight, with parity and ethnicity being the two strongest predictors. The 'terrible twos' are often associated with increased temper tantrums, noncompliance and aggression. Although some expression of these behaviors is normal, whether early individual factors can predict which children are most at risk of frequent or prolonged emotional and behavioral problems is of increasing interest. The current study of 6,067 toddlers found that their 9 month scores on a new brief measure of temperament--the Infant Behavior Questionnaire- Revised Very Short Form (IBQ-R-VSF)--were associated with both difficult behaviors and prosocial behaviors at age 2, measured using a preschool version of the Strengths and Difficulties Questionnaire. Controlling for a range of demographic, maternal characteristics and contextual factors, we found that infants higher on the trait Negative Emotionality and/ or lower on Orienting Capacity and Fear were more likely to develop hyperactivity, emotion, peer and conduct problems and less likely to demonstrate pro-social behaviors at two years of age. Temperament aged 9 months explained an additional 1-6 % of the variance in toddler behavior. These findings suggest that infant temperament is associated with the behavior strengths and difficulties of 2-year-olds and provide the first longitudinal validation of the new 5-factor structure of the new IBQ-R-VSF.

Background: The Aggressive Student Culture Scale (ASCS) is a survey designed to measure the extent to which New Zealand (NZ) students experience aggressive behaviours within the school environment. The aim of this study is to assess the psychometric properties of the ASCS in the multidisciplinary Growing Up in NZ longitudinal study. Methods: We used data from 4938 children from the Growing Up in NZ study to examine the psychometric properties of ASCS for 8-year-old children. Confirmatory factor analysis was conducted, and measurement invariance was tested across sex, ethnicity, and deprivation levels. Results: The ASCS tool comprises a single latent factor: aggressive student behaviour. The ASCS provides an adequate and satisfactory measure for student aggression experiences. Full measurement invariance was supported for child's sex, but only configural invariance was confirmed across ethnicity and area-level deprivation. Males reported higher levels of aggressive experiences than females. Conclusions: The one-factor model structure offers an excellent fit to our data with good internal consistency. Comparisons across sex are valid; however, direct comparisons across ethnicity and deprivation levels should be approached with caution. We recommend replication studies and encourage further research involving participants from different age groups to better understand the factor structure across diverse demographic variables. Objectives Antenatal and postnatal depression can lead to poor outcomes for women and their children. The aim of this study was to explore whether risk factors differ for depression symptoms that are present during pregnancy and/or after childbirth. Methods An ethnically and socioeconomically diverse sample of 5301 women completed interviews during the third trimester of pregnancy and 9 months after childbirth. Depression symptoms were measured using the Edinburgh Postnatal Depression Scale (EPDS). Depression symptoms (defined as EPDS >12) among participants and associations with pre-pregnancy and pregnancy maternal characteristics were explored using logistic regression. Results The rate of antenatal depression symptoms (ADS) only was 8.5, 5% of women had depression symptoms at 9 months postpartum (PDS) only and 3% experienced depression symptoms at both time points. Perceived stress and Pacific or Asian ethnicity were risk factors for ADS and PDS. Anxiety during and before pregnancy was a risk factor for ADS only while having a pre-pregnancy diagnosis of depression was a risk factor for PDS only. Having ADS increased the odds ratio of PDS by 1.5 (95% CI 1.01-2.30). Conclusions The results supported evidence from previous longitudinal studies that depression symptoms appear to be higher during pregnancy than in the first year following childbirth. Approximately half the world's population can now speak more than one language. Understanding the neural basis of language organisation in bilinguals, and whether the cortical networks involved during language processing differ from that of monolinguals, is therefore an important area of research. A main issue concerns whether L2 (second language) is processed using the same neural mechanisms that mediate L1 (first language) processing. Moderating factors include the age of L2 acquisition and the level of proficiency. Here we used a lexical decision task with five conditions during functional magnetic resonance imaging (fMRI) to investigate language processing in eight late proficient bilinguals when using Macedonian (L1) and English (L2). Bilinguals had greater bilateral activation during both L1 and L2 processing, and therefore weaker language lateralisation, compared to matched control English monolinguals. A greater amount of overall activation was also seen in bilinguals, especially during L2 conditions. Late proficient bilinguals living in their L2 environment employ a more extensive neural network than monolinguals when processing their second language. (Contains 3 figures and 2 tables.)
