The reduced coenzyme nicotinamide adenine dinucleotide (NADH) has been used as medication in 885 parkinsonian patients in an open label trial. About half of the patients received NADH by intravenous infusion, the other part orally by capsules. In about 80% of the patients a beneficial clinical effect was observed: 19.3% of the patients showed a very good (30-50%) improvement of disability, 58.8% a moderate (10-30%) improvement. 21.8% did not respond to NADH. Statistical analysis of the improvement in correlation with the disability prior to treatment, the duration of the disease and the age of the patients revealed the following results: All these 3 parameters have a significant although weak influence on the improvement. The disability before the treatment has a positive regression coefficient (t value < 0.01). The duration of the disease has a negative regression coefficient (< 0.01) and so has the age a negative regression coefficient (t value < 0.05). In other words younger patients and patients with a shorter duration of disease have a better chance to gain a marked improvement than older patients and patients with longer duration of the disease. The orally applied form of NADH yielded an overall improvement in the disability which was comparable to that of the parenterally applied form.

Future phased array antennas will have stringent requirements in terms of performance, mass and power, which can be efficiently met by optoelectronic techniques. By employing coherent optical processing in guided-wave optical components, and with extensive use of optoelectronic integrated circuits, substantial reductions in mass and power are feasible in comparison with conventional beamformers. In a major study for European Space Agency, reductions of 30% and power reductions of 60% were seen to be feasible in a complex beamformer for a space-borne communications antenna, relative to a MMIC-based system with the same performance requirements. In order to verify the system concept, we designed and constructed a complete proof of concept model (PCM) offering both transmit and receive mode operation in the 12 GHz band. Detailed performance evaluation has been carried out, resulting in the first practical demonstration of the application of coherent optical techniques in an antenna beamformer. In this paper we discuss the optoelectronic beamforming concept, the application of this concept to a specific antenna and the design and construction of the proof of concept model.

Mit der Entwicklung optischer Fernmesverfahren eroffnen sich fur Umweltforschung und praktischen Umweltschutz neue Moglichkeiten, die in der Praxis bisher erst wenig genutzt werden. Ursache hierfur ist neben dem Fehlen von Vorschriften zu ihrer Anwendung vor allem die mangelnde Verfugbarkeit leistungsfahiger, einfach zu bedienender Messysteme. Mit ARGOS liegt ein Lidarsystem vor, das diesem Mangel abhilft.

For hundreds and even thousands of years drugs have been used to alter psychic behaviour, and alcohol is only one example of a drug that is still being used today. Since the beginning of our century, however, researchers have been interested in the mechanism(s) of drug action. It became clear by the early observations of Potzl and Hess at the beginning of this century that glucose loading during melancholic phases led to changes in the response of depressed patients. Clinical, behavioural and biochemical studies after drug administration - including Freud’s first trials with cocaine — led to a break-through in biochemically oriented brain research. It became evident that rauwolfia serpentina improved psychotic behaviour and decreased blood pressure by releasing biogenic amines and emptying their specific storage sites (Carlsson 1959; Brodie and Shore 1957). This loss of biogenic amines caused depressive mood in about 10% of patients with hypertension. Generation of hypotheses that depression is caused by the loss of serotonin, noradrenaline and/or dopamine gave a first biochemical approach to the pathophysiology of psychiatric disturbances. On the other hand, research into the mechanism of action of reserpine showed its cataleptogenic effects in animal studies. This motor deficiency could be antagonized by the precursor amino acid of dopamine, L-dopa (Carlsson 1959). Later a loss of dopamine could be demonstrated in the nigrostriatal system of Parkinson’s disease (Ehringer and Hornykiewicz 1960). As a consequence of all these studies, L-dopa therapy was introduced by Birkmayer and Hornykiewicz (1961) and is nowadays a therapeutic standard for treatment of Parkinson disease.

Deprenyl combined with levodopa and a peripheral decarboxylase inhibitor is of therapeutic value in the treatment of Parkinson's disease. This conclusion is substantiated by the improvement of akinesia, on-off phases, fluctuations of disability and rigidity. Levodopa doses can be reduced. The onset of adverse reactions is later and side-effects are even milder when compared to those with combined levodopa treatment. The most significant effect of deprenyl, however, is its ability to prolong the life expectancy of parkinsonian patients.

With the 430 MHz incoherent scatter radar facility at the Arecibo Observatory, the plasma line spectrum has been measured with a linearly frequency modulated or ‘chirped’ pulse. On reception the signal is demodulated (‘dechirped’). The paper describes the experimental set-up and the parameter estimation for day-time ionosphere observations. Using a non-linear least squares fit it is possible to determine the linear and quadratic coefficients of a locally parabolic plasma line frequency versus height profile. The strength of the plasma line and the point of tangency of the locally parabolic plasma line frequency profile can be determined very accurately. In measurements of the day-time ionosphere the plasma line was found to be enhanced by about 70 times over the thermal equilibrium level.

The antidepressive efficacy of 1-deprenyl (5-10 mg daily) plus 1-phenylalanine (250 mg/day) has been evaluated in 155 unipolar depressed patients. Both oral and intravenous administration showed beneficial effects in 90% of outpatients and 80.5% of inpatients. It is concluded that this combined treatment has a potent antidepressive action based on the accumulation of 1-phenylethylamine in the brain.

Parkinsonian patients with orthostatic hypotension and dizziness due to usual antiparkinson therapy have been treated with the precursor amino-acid of noradrenaline, DL-3,4-threo-dihydroxyphenylserine (DL-3,4-threo-DOPS). Oral and intravenous administration improved these side effects significantly. A combined treatment of L-dopa, peripheral decarboxylase inhibitor and DL-3,4-threo-DOPS seems to be of benefit with respect to akinesia and orthostatic hypotension.

1. A Dopa-medication leads sometimes in Parkinsonian patients to a psychosis. 2. The diminished capacity of striatal dopaminergic neurons to store DA leads to a storage of DA in serotoninergic or noradrenergic neurons. 3. Extrastriatal noradrenergic and/or serotoninergic neurons are involved. 4. Dopaminergic receptors of the striatium are not involved because of a lack of kinetic response after L-Dopa application in Parkinsonian patients with akinetic crises. 5. Extrastriatal dopaminergic receptors of DA as "false transmitter" at serotoninergic receptors seem to be responsible for the production of psychotic symptoms, whereas noradrenaline is not responsible. Psychotic symptoms can be imagined to be triggered by a contact of a transmitter to a nonspecific receptor.

Biochemical human post-mortem studies on depressed patients indicate an unspecific deficiency of neurotransmitters in several brain areas. The loss of drive of these patients could be correlated with a decrease of striatal dopamine concentration. Noradrenaline was significantly diminished in red nucleus, a fact which points to the characteristic posture of depressed patients. Serotonin was diminished in all brain areas. During remission all values trended to be normal. There also exists a circadian disrhythm in depressed patients resulting in lowered VMA- and HVA-levels in urines during the morning and a remission to normal values in the evening. This agrees with the findings of lowered blood tyrosine levels in the morning. The ratio of blood tyrosine and tryptophan is disturbed during depression and recovers during remission. Central and peripheral biochemical mechanisms seems to be involved in depression syndrom.

Clinical data demonstrate that there is a significant difference between at least two types of idiopathic Parkinsonian patients; a benign type of Parkinson's disease and a malignant type. They can be distinguished by the time course of a disability-score, independent of the duration of the disease. The benign cases respond very well to L-Dopa (Madopar) treatment and show a long lasting (7 years) drop in the disability, whereas the improvement of disability in the malignant cases is much smaller and lasts for about one year, followed by an increase of disability. The duration of the disease is 12,5 +/- 0,4 years in the benign type but only 4,0 +/- 0,3 years in the malignant type. Akinetic crises, off-phases, hyperkinetic crises and toxic deliria after Madopar treatment can be observed later in the benign than in the malignant cases. These results suggest that the degeneration of the dopaminergic nigrostriatal system occurs much more rapidly in malignant cases than in benign cases of Parkinson's disease.

It has been shown that 5 or 10 mg (-)deprenyl after i.v. application inhibited platelet MAO within 30 min. This effect correlated with the improvement of parkinsonian patients disability. Platelet MAO is purely of type B, thus resembling the human brain enzyme, which is 80% of type B. In other organs of the human MAO-A is of higher activity, thus it can oxidatively deaminate 5-HT, noradrenaline and tyramine in the periphery. The rather low peripheral side effects of (-)deprenyl can be explained by this fact. In vitro studies demonstrated that (-)deprenyl in comparison to d, l-tranylcypromine, clorgyline and harmaline is by far the most potent inhibitor of human brain MAO. Post-mortem studies in different human brain areas showed that there are differences in the behaviour of (-)deprenyl (10 mg) between short- and long-term treatment. Both show sufficient inhibition of DA-sensitive MAO (85–90%). However, when 5-HT is used as a substrate short-term treatment inhibits by about 40–50% whereas long-term treatment inhibits by about 65% which is higher than that mentioned before but not sufficient to increase brain 5-HT or decrease 5-HIAA. Therefore, long-term treatment with more than 1 mg/10 kg body weight could result in an accumulation of (-)deprenyl in the brain. Evidence for this derives from one parkinsonian patient, who was treated with 100 mg (-)deprenyl in which case both forms of the enzyme were inhibited sufficiently to increase DA and 5-HT in several brain regions.

Disorders of neurotransmitter balance are observed in Parkinson's disease, pharmacotoxic psychosis and depression. The dopamine-serotonin ratio is reduced to about 20% in Parkinson and pharmacotoxic patients in the caudate nucleus and in the substantia nigra. The serotonin content in these brain areas is lowered only to about 50% in comparison to that of the control, whereas the dopamine level is reduced to 85% in Parkinson patients. This dopamine deficiency has been substituted by exogenous supply of L-dopa in combination with decarboxylase and monoaminooxydase inhibitors. First evidence is presented that L-dopa can be replaced, at least partially, by iron in form of a ferriascorbate complex. This iron compound improves the symptoms of Parkinson's disease to almost the same extent as L-dopa.

There were determined free plasma tyrosine and tryptophan spectral-photofluorometrically at 22 healthy controls and 81 depressed patients (13 of them were controlled in a long-time-study; mean time: 11 months, and 68 patients were controlled ambulant). The biochemical data were demonstrated in a tyrosine-tryptophan diagram and were correlated with the clinical behaviour of the patients. This correlation only was possible with the postulation of a balance between catecholamine-and indolamine metabolism. Three biochemical parameters are necessary. The values of 1. tyrosine that of 2. tryptophan and not only 3. the resulting quotient. With help of such demonstration we showed this tyrosine-tryptophan balance graphically. We think, that both catecholamine-and the indolamine-hypothesis of depression seem to have their availability.

An accumulation of norepinephrin, serotonin and dopamin exists in the brainstem. A decrease of biogenic amines in the brains of Parkinson's cases has been demonstrated. A substitution by the precursor L-DOPA produces an increase of dopamin in the brain and a decrease of serotonin. The high level of dopamin improves the akinesia and the depletion of serotonin causes a toxic delir. A substitution by l-tryptophan restores the balance and the Dopa-psychosis disappears.

In zwei Versuchsreihen konnte festgestellt werden 1. das beim Skifahren durch Kopfdrehungen Tonusveranderungen auftreten, die ein Abweichen nach der Gegenseite der Drehung bewirken. Bei Kopfneigung kommt es zum Abweichen nach der gleichen Seite der Neigung. 2. konnte mit Hilfe einer Rohrenbrille das periphere Gesichtsfeld ausgeschaltet werden und dadurch eine vollige Insuffizienz der Versuchspersonen beobachtet werden, beim Befahren von Bodenkanten. Es wurde daraus der Schlus abgeleitet, das die geordnete Vorwegnahme motorischer Entscheidungen an Sinneseindrucke, die von der Netzhautperipherie geliefert werden, gebunden ist.

1. Beim Auftreffen einer Gewalt am Schadel kommt es zur Zerteilung der Energie. Diese durchlauft in Form potentieller Stoswellen den Schadelinhalt und wird an den Grenzflachen physikalisch differenter Strukturen in kinetische Energie umgewandelt bzw. zu sekundaren Stoswellen reflektiert. 2. Als unmittelbares Ereignis stellt sich eine thixotrope Veranderung des Parenchyms ein, die zur Volumszunahme des Gehirns und gleichzeitiger Funktionsbeeintrachtigung fuhrt. Im Gegensatz zu dieser generellen Schadigung treten klinisch nur Erscheinungen des Hirnstammes auf. 3. Die vegetativen Fehlsteuerungen nach einem Schadeltrauma bleiben fallweise langere Zeit bestehen. Mit Hilfe vegetativer Belastungsproben gelingt es, auch langere Zeit nach dem Trauma diencephal bedingte Koordinationsstorungen der vegetativen Regulationen aufzuzeigen (vegetative Ataxie), die damit eigentlich der Contusio cerebri zuzurechnen sind. Obwohl pathogenetisch zwischen Commotio cerebri und Contusio cerebri fliesende Ubergange bestehen, ist fur den Begutachter eine klare Trennung notwendig. Die Dauer der Bewustlosigkeit ist fur den praktischen Begutachter das wesentlichste Kriterium. 4. Fur Falle mit starken subjektiven Beschwerden ohne objektiven Befund wird eine spezielle Hirntraumaabteilung empfohlen, in der mit Hilfe vegetativer Belastungsproben, mit EEG.-Untersuchungen und psychologischen Arbeitsbelastungstests zwischen organischer Schadigung und psychogener Reaktion mit groser Wahrscheinlichkeit unterschieden werden kann.

Mit einem Kleinstroboskop wurden drei Bewegungsformen (Fuhrung —Schwung —Stos) beim Parkinson, Chorea und cerebellaren Syndrom registriert. Aus den Geschwindigkeitund Beschleunigungskurven wurden charakteristische mathematische Kriterien abgeleitet. Das Beschleunigungsmaximum des Normalen ist bei der Fuhrungsbewegung am geringsten, beim Schwung groser, beim Stos am grosten. Das kinetische Energiepotential ist bei der Fuhrungsbewegung klein, beim Schwung 6 mal und beim Stos 3 mal so gros. Als gemeinsames mathematisches Kriterium der verschiedenen extrapyramidalen Syndrome wurde die geringe Variabilitat des Energieeinsatzes aufgezeigt. Wahrend fur die motorische Schablone des Parkinson das geringe Beschleunigungsund Verzogerungsmaximum und die geringe kinetische Energie aller Bewegungsablaufe spezifisch ist, scheint beim choreatischen und beim cerebellaren Syndrom ein ubernormales kinetisches Energiepotential auf. Der Cerebellare zeigt ferner bei der Fuhrungsbewegung das groste Beschleunigungsmaximum und beim Stos das groste Verzogerungsmaximum. Das choreatische Syndrom ist durch das hohe Beschleunigungs und Verzogerungsmaximum und durch eine dem Normalen eingenaherte Form der kinetischen Energieproduktion charakterisiert.

Das unbestreitbare Verdienst der psychosomatischen Medizin ist, die psychosomatische Einheit des kranken Menschen wieder hergestellt zu haben. Langdauernde vegetative Funktionsstorungen konnen zu Strukturveranderungen fuhren. Die durch die psychoanalytische Methode aufgezeigten emotionalen Konflikte stellen unserer Meinung nach beim Einzelindividuum einen differenten Stellenwert als Kausalfaktor dar.

Es wurde der Versuch unternommen, im Insulinshock Bewegungsphanomene, wisZingerle sie als Automatosen beschrieben hat, einer systematischen Kontrolle zu unterziehen. Wir konnten feststellen, das in gewissen Phasen des Shocks eine mehr oder minder grose Bereitschaft besteht, solche Bewegungsfolgen auszulosen. Fur das Verhalten unserer Falle ist charakteristisch, das diese Erscheinungen nach passiv erteilter Lageveranderung nicht reflexartig, sondern erst nach einer schwankenden Tonusadaptation in Erscheinung treten. Hatte diese tonische Anpassung aber einmal stattgefunden, so folgte zwangslaufig eine weitere Tonusverteilung auf fast samtliche synergische Muskelgruppen in jeweils bestimmter Reihenfolge nach. Auf diese Besonderheiten Bezug nehmend halten wir den Ausdruck „adaptive Zwangskoordinationen” fur unsere Falle am zutreffendsten. Praktisch fur die Methode des Shocks konnten wir zeigen, das eine allgemeine Streckerstarre (Enthirnungsstarre), die durch die verschiedensten Bewegungsversuche nicht mehr durchbrechbar war, eine Indikation zur Shockunterbrechung darstellt. Auserdem konnte das Auftreten solcher adaptiver Zwangskoordinationen im Insulinshock einen Beitrag zur Frage der gegenseitigen Beeinflussung von Bewustseinslage und motorischer Enthemmungsreaktion bringen.

Mittels Filmaufnahmen wurden Ganganalysen von Kindern verschiedenen Alters unternommen. Das Ziel war, die Entwicklung des Gehens in verschiedenen Altersstufen aufzuzeigen. Aus der Analyse der bisherigen Befunde kann gesagt werden: 1. Die einzelnen Phasen des Ganges sind beim Gehanfanger viel unregelmasiger. 2. Die Standdauer ist langer, die Unterstutzungsflache ist groser, der Schwerpunkt fallt fast stets innerhalb der Unterstutzungsflache auf, der Kopf und Rumpf befinden sich in Vorlage. 3. Im Lauf der Entwicklung wird Schrittdauer, Schrittlange und der Schrittindex groser. Die Schwungdauer nimmt zu. Die Unterstutzungsflache wird relativ kleiner, der Schwerpunkt fallt in mehreren Phasen rhythmisch auserhalb der Unterstutzungsflache. 4. Die statische Ordnung geht in eine dynamische uber.

Mit J131 markiertes Humanserumalbumin wurde i.v. injiziert. Mit einem Szintillationszahler uber dem Confluens sinuum wurde die Zahlrate der das Gehirn passierenden Blutmenge gemessen und auf einem Linienschreiber registriert. Bei jugendlichen Gesunden ist die Durchblutungszeit kurz und die Amplitude hoch. Die rasche Zeit spricht fur einen intakten Antrieb, die hohe Amplitude fur eine elastische Erweiterung der Hirngefase beim Passieren der Blutwelle. Beim alten Patienten mit dem Syndrom der cerebralen Mangeldurchblutung ist die Durchblutungszeit verlangsamt und die Amplitude erniedrigt. Das heist: der dynamische Antrieb ist vermindert und die starren Gefase lassen in der Zeiteinheit nur eine geringe Blutmenge passieren. Nach Papaverin sieht man eine Erhohung der Amplitude als Zeichen der Gefaserweiterung. Dieser Effekt steht in keiner Korrelation zur Gewebsernahrung. Nach Strophantin kommt es zu einer Verlangerung der cerebralen Verweildauer des Blutes. Dieses Phanomen wurde als positiver Nutritionseffekt bezeichnet und fur die bessere Parenchymernahrung verantwortlich gemacht.

Es wurde an Hand eigenartiger und teilweise schon bekannter Haltungsanomalien beim postencephalitischen Parkonsonismus der Versuch unternommen, diese Phanomene im Sinne der „Schablonenlehre“ zu interpretieren. Sie wurde als „Schwimmschablone“ bezeichnet und als spezifische Haltungsschablone beim Parkinsonismus als phanomenologischer Ausdruck einer speziellen Desintegration gedeutet.
