Abstract

Autochthonous Dirofilaria repens infections have been found in dogs and other carnivores in most European countries. In the same countries, reports of human dirofilariasis are becoming increasingly prevalent. We present 18 cases of people infected with D. repens for a 10-year period traced in our hospital. The data was collected from the observations and tests of all 18 patients from the whole country treated at the Specialized Hospital for Infectious and Parasitic Diseases in Sofia in the period 2009 – 2018. We used a morphological method, serology test and Knott’s method for microfilariae. The patients were 11 to 74 years of age, 12 female and 6 male. In most cases, patients have subcutaneous nodules or face, eyelid and eyes localization. The trend of increasing incidence in Bulgaria continues, with age and sex distribution and localization of the larva being similar to those in other European countries.

Abstract

As the effectiveness of mebendazole and albendazole in patients with echinococcosis has been found to vary, we investigated some of the factors likely to be responsible. A total of 79 patients who were treated with mebendazole (44 patients) or albendazole (35 patients) were included in the study. Evaluation of the treatment results was based on the changes in cyst morphology, as evidenced by the results of X-ray radiography, sonography, and computed tomography, and on analysis of the findings in relation to parasitic and drug factors. The response of cyst according to their site did not vary much, with the exception of the poor response of bone cysts. A more important factor seems to be cyst size, since the treatment was more efficacious against smaller and younger cysts. The presence of daughter cysts should be regarded as an unfavourable factor for treatment response. Cyst multiplicity did not present insurmountable difficulties, provided the cysts were small and a prolonged course of therapy was undergone. The choice of drug used for the therapy was important, with the results supporting the advantage of albendazole. Abstract

6 páginas, 3 figuras, 1 tabla, Cystic echinococcosis (CE) is a zoonotic parasitic disease endemic in southern and eastern European countries. The true prevalence of CE is difficult to estimate due to the high proportion of asymptomatic carriers who never seek medical attention and to the underreporting of diagnosed cases, factors which contribute to its neglected status. In an attempt to improve this situation, the European Register of Cystic Echinococcosis (ERCE), was launched in October 2014 in the context of the HERACLES project. ERCE is a prospective, observational, multicentre register of patients with probable or confirmed CE. The first ERCE meeting was held in November 2015 at the Italian National Institute of Health (Istituto Superiore di Sanita, ISS) in Rome, to bring together CE experts currently involved in the Register activities, to share and discuss experiences, and future developments. Although the Register is still in its infancy, data collected at the time of writing this report, had outnumbered the total of national cases reported by the European endemic countries and published by the European Centre for Disease Prevention and Control in 2015. This confirms the need for an improved reporting system of CE at the European level. The collection of standardized clinical data and samples is expected to support a more rational, stage-specific approach to clinical management, and to help public authorities harmonize reporting of CE.
